| 1 | PEComa of the colon resistant to sirolimus but responsive to doxorubicin/ifosfamide显示文摘A 23-year-old male presented with a three-week-history of crampy abdominal pain and melaena.Colonoscopy revealed a friable mass filling the entire lumen of the cecum;histologically,it was classified as perivascular epithelioid cell tumor(PEComa).An magnetic resonance imaging scan showed,in addition to the primary tumor,two large mesenteric lymph node metastases and four metastatic lesions in the liver.The patient underwent right hemicolectomy and left hemihepatectomy combined with wedge resections of metastases in the right lobe of the liver,the resection status was R0.Subsequently,the patient was treated with sirolimus.After 4 mo of adjuvant mammalian target of rapamycin inhibition he developed two new liver metastases and a local pelvic recurrence.The visible tumor formations were again excised surgically,this time the resection status was R2 with regard to the pelvic recurrence.The patient was treated with 12 cycles of doxorubicin and ifosfamide under which the disease was stable for 9 mo.The clinical course was then determined by rapid tumor growth in the pelvic cavity.Second line chemotherapy with gemcitabine and docetaxel was ineffective,and the patient died 23 mo after the onset of disease.This case report adds evidence that,in malignant PEComa,the mainstay of treatment is curative surgery.If not achievable,the effects of adjuvant or palliative chemotherapy are unpredictable. | Wolfgang Scheppach Nikolaus Reissmann Thomas Sprinz Ekkehard Schippers Bjoern Schoettker Justus G Mueller | 2013 | World Journal of Gastroenterology2013，19，10： | 8 |

| 3 | hsa-mir-183 is frequently methylated and related to poor survival in human hepatocellular carcinoma显示文摘AIM To screen clinically relevant micro RNAs (mi RNAs) silenced by DNA methylation in human hepatocellular carcinoma(HCC).METHODS Knockdown of DNA methyltransferases (DNMTs) using si RNAs and mi RNA profiling in HCC cell lines were performed to identify DNA hypermethylation-mediated mi RNA downregulation. Confirmation using individual quantitative real-time PCR (qR T-PCR) assays was thenperformed followed by DNA methylation quantification at the promoter of the mi RNA genes. Quantification of DNA methylation and mi RNA expression was then performed in primary HCC tumor samples and related with clinicopathological variables.RESULTS mi RNA profiling after DNMT knockdown in HCC cell lines revealed upregulation of mi R-23, mi R-25 and mi R-183. After q RT-PCR confirmation and Cp G island methylation quantification of these miR NAs in cell lines, further analysis in primary HCC specimens showed that hsa-mi R-183 is hypermethylated in 30% of HCC (n = 40). Expression of mature miR-183 showed an inverse correlation with DNA methylation levels. In HCC cells, DNMT knockdown and 5-aza-2'-deoxycytidine treatment reduced methylation and stimulated expression of mi R-183. In HCC patients, hypermethylation at hsami R-183 promoter significantly correlates with poor survival (log-rank test P = 0.03). | 4 | Beta-7 integrin controls enterocyte migration in the small intestine显示文摘AIM:To hypothesize that beta-7 integrin affects cellularmigration of both,lymphocytes and enterocytes.METHODS:The nucleoside analog Brd U was ip injected in beta-7-deficient mice(C57BL/6-Itgbtmlcgn/J)of male gender and age-matched male C57BL/J J mice(wild type)4,20,or 40 h before analysis.The total small intestine was isolated,dissected,and used for morphometrical studies.Brd U-positive epithelial cells were numbered in at least 15 hemi-crypts per duodenum,jejunum,and ileum of each animal.The outer most Brd U-positive cell(cellmax)was determined per hemi-crypt,numerically documented,and statistically analysed.RESULTS:Integrins containing the beta-7-chain were exclusively expressed on leukocytes.In the small intestinal mucosa of beta-7 integrin-deficient mice the number of intraepithelial lymphocytes was drastically decreased.Moreover,the Peyer’s patches of beta-7integrin-deficient mice appeared hypoplastic.In beta-7integrin-deficient mice the location of cellmax was found in a higher position than it was the case for the controls.The difference was already detected at 4 h after Brd U application,but significantly increased with time(40 h after Brd U injection)in all small intestinal segments investigated,i.e.,duodenum,jejunum,and ileum.Migration of small intestinal enterocytes was different between the experimental groups measured by cellmax locations.CONCLUSION:The E-cadherin beta-7 integrin pathway probably controls migration of enterocytes within the small intestinal surface lining epithelial layer. | Elke Kaemmerer Paula Kuhn Ursula Schneider Thomas Clahsen Min Kyung Jeon Christina Klaus Julia Andruszkow Michael Hrer Sabine Ernst Angela Schippers Norbert Wagner Nikolaus Gassler | 2015 | World Journal of Gastroenterology2015，21，6： | 3 |
