| 1 | 负压创伤治疗的循证推荐(压力水平、创面填充物、接触材料)——迈向达成国际共识的一步显示文摘负压创面引流正日渐成为临床上常用的治疗方法,也出现了许多相关的新设备与敷料,但仍有许多关于负压创伤治疗不确定的问题。为了回应这些争议,一个全球专家顾问小组专门被召集来达成负压创伤治疗的循证共识。本文中,通过对关于负压创伤治疗变量:不同的创面填充物(主要是泡沫和纱布)、何时应用接触层、不同压力水平的设定、对细菌生物符合的影响,进行证据评价。循证推荐由对文献的系统评价获得,由专家顾问小组对证据分级的起草共识。证据与推荐根据苏格兰校际指南协作组分级系统分级。总的来说,共有14项推荐,5项与创面敷料和接触层有关,4项和压力水平设定有关,5项和负压创伤治疗与创面感染有关。在细菌负担方面,现有证据不支持负压创伤治疗是创面细菌感染的主要治疗方法。 | 方亮(编译) Birke-Sorensen H Malmsjo M Rome P | 2012 | 中华普通外科学文献(电子版)2012，6，4： | 9 |

| 4 | MicroRNAs as emerging biomarkers and therapeutic targets for pancreatic cancer显示文摘Despite tremendous efforts from scientists and clinicians worldwide, pancreatic adenocarcinoma(PDAC) remains a deadly disease due to the lack of early diagnostic tools and reliable therapeutic approaches. Consequently, a majority of patients(80%) display an advanced disease that results in a low resection rate leading to an overall median survival of less than 6 months. Accordingly, robust markers for the early diagnosis and prognosis of pancreatic cancer, or markers indicative of survival and/or metastatic disease are des-perately needed to help alleviate the dismal prognosis of this cancer. In addition, the discovery of new therapeutic targets is mandatory to design effective treatments. | 5 | Epistaxis in end stage liver disease masquerading as severe upper gastrointestinal hemorrhage显示文摘AIM:To describe the prevalence,diagnosis,treatment,and outcomes of end stage liver disease(ESLD) patients with severe epistaxis thought to be severe upper gastrointestinal hemorrhage(UGIH).METHODS:This observational single center study included all consecutive patients with ESLD and epistaxis identified from consecutive subjects hospitalized with suspected UGIH and prospectively enrolled in our databases of severe UGIH between 1998 and 2011.RESULTS:A total of 1249 patients were registered for severe UGIH in the data basis,461(36.9%) were cirrhotics. Epistaxis rather than UGIH was the bleeding source in 20 patients. All patients had severe coagulopathy. Epistaxis was initially controlled in all cases. Fifteen(75%) subjects required posterior nasal packing and 2(10%) embolization in addition to correction of coagulopathy. Five(25%) patients died in the hospital,12(60%) received orthotopic liver transplantation(OLT),and 3(15%) were discharged without OLT. In particular, curves providing CRvalues as a function of amplitude and phase channel mismatching, radar bandwidth, and jammer direction of arrival (JDOA) are provided for the cases of one and two auxiliary antennas. Subsequently, a time -space processor for performance restoration is analyzed in detail. In addition to the above mentioned quantities, the attainable CR value is expressed as a function of the space-time processor parameters. The contribution of the paper is related to the derivation of a number of mathematical equations of CR for several cases of practical interest for the radar engineer. Al-though pancreaticoduodenectomy has been considered the standard surgical procedure because of its favorable results on pain control, its high postoperative complica-tion and pancreatic exocrine or/and endocrine dysfunc-tion rates have led to a growing enthusiasm for duodenal preserving pancreatic head resection. The aim of this review is to better understand the rationale underlying of the Frey procedure in chronic pancreatitis and to ana-lyze its outcome. Because of its hybrid nature, combin-ing both resection and drainage, the Frey procedure has been conceptualized based on the pathophysiology of chronic pancreatitis.
