1在良性的胆汁的阻塞的 choledochoduodenostomy 的长期的结果显示文摘 AIM:To determine the long-term results of choledochodudenostomy in patients with benign billiary obstruction.METHODS:This prospective study was conducted at Sheri Kashmir Institute of Medical Sciences Srinagar Kashmir,India over a period of 10 years from January 1997 to December 2007.The total number of patients who underwent choledochoduodenostomy during this period was 270.On the basis of etiology of biliary tract obstruction,patients were divided into a calculus group,an oriental cholangiohepatitis group,a benign biliary stricture group and others.Patients were followed for a variable period of 13 mo to 15 years.RESULTS:Choledochoduodenostomy(CDD) with duodenotomy was performend in four patients.CDD with removal of T-tube,CDD with left hepatic lobectomy and CDD with removal of intra biliary ruptured hydatid was performed in three patients each.In the remaining patients only CDD was performed.Immediate post operative complications were seen in 63(23%) patients,while long-term complications were seen in 28(11%) patients,which were statistically significant.Three patients died during hospitalization while four patients died in the late post-operative period.CONCLUSION:Our conclusion is that CDD is safe and produces good long term results when a permanent biliary drainage procedure is required.Ajaz A Malik Shiraz A Rather Shams UL Bari Khursheed Alam Wani 2012World Journal of Gastrointestinal Surgery2012,4,2:7
2Detection and sequencing of plasmid encoded tetracycline resistance determinants(tetA and tetB) from food-borne Bacillus cereus isolates显示文摘Objective:To investigate the detection and sequencing of plasmid encoded tetracycline resistance genes(tet A and tet B) from food-borne and standard strains of Bacillus cereus(B. cereus).Methods:PCR was carried out to detect the tetracycline resistance genes(tet A and tet B) in food-borne B.cereus strains and the amplified products were sequenced.Results:The phenotypic resistance against tetracycline was observed in 39 of the 118 food-borne isolates and two reference strains(MTCC 430 and MTCC 1307) of B.cereus.Among the phenotypically resistant isolates,tet A was detected in 36 food-borne isolates and two reference strains(MTCC 430 and MTCC 1307).whereas,tel B was detected in 12 food-bome isolates and MTCC 1307 strain. Conclusions:A close association was therefore found between phenotypic resistance against tetracycline and presence of tetracycline resistance genes.The tet A and tet B gene fragments were amplified,purified and sequenced.The gene sequences of the isolates studied herein were found similar to tetA and tetB gene sequences of other bacteria available in NCBI.The occurrence of tetA and tetB genes in B.cereus indicate the horizontal transfer of antibiotic resistance determinants from other bacteria into B.cereus.The transfer of these resistant determinants to other potentially pathogenic bacteria may be a matter of great concern.Mudasir Ali Rather Rabinder Singh Aulakh Jatinder Paul Singh Gill Abdul Qayoom Mir Mir Nadeem Hassan 2012Asian Pacific Journal of Tropical Medicine2012,5,9:6
3Portal biliopathy显示文摘Portal biliopathy refers to cholangiographic abnormalities which occur in patients with portal cavernoma. These changes occur as a result of pressure on bile ducts from bridging tortuous paracholedochal, epicholedochal and cholecystic veins. Bile duct ischemia may occur due prolonged venous pressure effect or result from insufficient blood supply. In addition, encasement of ducts may occur due fibrotic cavernoma. Majority of patients are asymptomatic. Portal biliopathy is a progressive disease and patients who have long standing disease and more severe bile duct abnormalities present with recurrent episodes of biliary pain, cholangitis and cholestasis. Serum chemistry, ultrasound with color Doppler imaging, magnetic resonance imaging with magnetic resonance cholangiopancreatography and magnetic resonance portovenography are modalities of choice for evaluation of portal biliopathy. Endoscopic retrograde cholangiography being an invasive procedure is indicated for endotherapy only. Management of portal biliopathy is done in a stepwise manner. First, endotherapy is done for dilation of biliary strictures, placement of biliary stents to facilitate drainage and removal of bile duct calculi. Next portal venous pressure is reduced by formation of surgical porto-systemic shunt or transjugular intrahepatic portosystemic shunt. This causes significant resolution of biliary changes. 5Hepatobiliary and pancreatic ascariasis显示文摘Hepatobiliary and pancreatic ascariasis(HPA) was described as a clinical entity from Kashmir,India in 1985. HPA is caused by invasion and migration of nematode,Ascaris lumbricoides,in to the biliary tract and pancreatic duct. Patients present with biliary colic,cholangitis,cholecystitis,hepatic abscesses and acute pancreatitis. Ascarides traverse the ducts repeatedly,get trapped and die,leading to formation of hepatolithiasis. HPA is ubiquitous in endemic regions and in Kashmir,one such region,HPA is the etiological factor for 36.7%,23%,14.5% and 12.5% of all biliary diseases,acute pancreatitis,liver abscesses and biliary lithiasis respectively. Ultrasonography is an excellent diagnostic tool in visualizing worms in gut lumen and ductal system. The rational treatment for HPA is to give appropriate treatment for clinical syndromes along with effective anthelmintic therapy. Endotherapy in HPA is indicated if patients continue to have symptoms on medical therapy or when worms do not move out of ductal lumen by 3 wk or die within the ducts. 6Biomimetic polycaprolactone-chitosan nanofibrous substrate influenced cell cycle and ECM secretion affect cellular uptake of nanoclusters显示文摘Biomimetic cell culture substrates are developed as an alternative to the conventional substrates.They provide necessary biochemical and biophysical cues to the cells from their surrounding environment for their optimal growth,behaviour and physiology.Changes in physiology of cells growing on biomimetic substrate can essentially affect results of in vitro biological experiments such as drug cytotoxicity,nanoparticle internalization or signalling pathways.As majority of ECM proteins are fibrous in nature,nanofibrous scaffolds have more biomimicking properties.Therefore,in this study,we developed ECM mimicking polycaprolactone-chitosan nanofiber substrate and evaluated its effect on cell morphology,proliferation,cell cycle and ECM production.Further,cellular uptake of BSA-AuNCs has been assessed on conventional and biomimetic substrate in order to demonstrate the effect of these events on cellular properties.It was observed that the cells that were grown for 15 days on the nanofibers,had majority of cells in the proliferative phase of cell cycle compared to TCPS.Moreover,these cells showed extensive collagen and fibronectin production.Due to these conditions C3H10T1/2 cells displayed higher cell internalization of BSA-AuNCs.Overall,this study indicates that the nano-topographical and biochemical environment could alter the cell proliferative behaviour and ECM production,which affects the cell internalization of BSA-AuNCs.Also,PCL-chitosan nanofibrous substrate could be a better alternative to TCPS for cell culture studies.Dhwani Jhala Hilal Rather Dhaval Kedaria Juhi Shah Sanjay Singh Rajesh Vasita 2019Bioactive Materials2019,4,1:4
7Antioxidative study of Cerium Oxide nanoparticle functionalised PCL-Gelatin electrospun fibers for wound healing application显示文摘Skin wound healing involves a coordinated cellular response to achieve complete reepithelialisation.Elevated levels of reactive oxygen species(ROS)in the wound environment often pose a hindrance in wound healing resulting in impaired wound healing process.Cerium oxide nanoparticles(CeNPs)have the ability to protect the cells from oxidative damage by actively scavenging the ROS.Furthermore,matrices like nanofibers have also been explored for enhancing wound healing.In the current study CeNP functionalised polycaprolactone(PCL)-gelatin nanofiber(PGNPNF)mesh was fabricated by electrospinning and evaluated for its antioxidative potential.Wide angle XRD analysis of randomly oriented nanofibers revealed^2.6 times reduced crystallinity than pristine PCL which aided in rapid degradation of nanofibers and release of CeNP.However,bioactive composite made between nanoparticles and PCLgelatin maintained the fibrous morphology of PGNPNF upto 14 days.The PGNPNF mesh exhibited a superoxide dismutase(SOD)mimetic activity due to the incorporated CeNPs.The PGNPNF mesh enhanced proliferation of 3T3-L1 cells by^48%as confirmed by alamar blue assay and SEM micrographs of cells grown on the nanofibrous mesh.Furthermore,the PGNPNF mesh scavenged ROS,which was measured by relative DCF intensity and fluorescence microscopy;and subsequently increased the viability and proliferation of cells by three folds as it alleviated the oxidative stress.Overall,the results of this study suggest the potential of CeNP functionalised PCL-gelatin nanofibrous mesh for wound healing applications.Hilal Ahmad Rather Ria Thakore Ragini Singh Dhwani Jhala Sanjay Singh Rajesh Vasita 2018Bioactive Materials2018,3,2:4
