| 1 | Acoustic radiation force imaging sonoelastography for noninvasive staging of liver fibrosis显示文摘AIM:To investigate the diagnostic accuracy of acoustic radiation force impulse (ARFI) imaging as a noninvasive method for the assessment of liver fibrosis in chronic hepatitis C (CHC) patients.METHODS:We performed a prospective blind com-parison of ARFI elastography,APRI index and FibroMax in a consecutive series of patients who underwent liver biopsy for CHC in University Hospital Bucharest. His-topathological staging of liver fibrosis according to the METAVIR scoring system served as the reference. A to-tal of 74 patients underwent ARFI elastography,APRI index,FibroMax and successful liver biopsy. RESULTS:The noninvasive tests had a good correlation with the liver biopsy results. The most powerful test in predicting fibrosis was ARFI elastography. The diagnostic accuracy of ARFI elastography,expressedas area under receiver operating characteristic curve (AUROC) had a validity of 90.2% (95% CI AUROC = 0.831-0.972,P < 0.001) for the diagnosis of significant f ibrosis (F ≥ 2). | 5 | New targeted therapies in pancreatic cancer显示文摘Patients with pancreatic cancer have a poor prognosis with a median survival of 4-6 mo and a 5-year survival of less than 5%. Despite therapy with gemcitabine, patient survival does not exceed 6 mo, likely due to natural resistance to gemcitabine. Therefore, it is hoped that more favorable results can be obtained by using guided immunotherapy against molecular targets. This review summarizes the new leading targeted therapies in pancreatic cancers, focusing on passive and specific immunotherapies. Passive immunotherapy may have a role for treatment in combination with radiochemotherapy, which otherwise destroys the immune system along with tumor cells. It includes mainly therapies targeting against kinases, including epidermal growth factor receptor, Ras/Raf/mitogenactivated protein kinase cascade, human epidermal growth factor receptor 2, insulin growth factor-1 receptor, phosphoinositide 3-kinase/Akt/m TOR and hepatocyte growth factor receptor. Therapies against DNA repair genes, histone deacetylases, micro RNA, and pancreatic tumor tissue stromal elements(stromal extracellular matric and stromal pathways) are also discussed. Specific immunotherapies, such as vaccines(whole cell recombinant, peptide, and dendritic cell vaccines), adoptive cell therapy and immunotherapy targeting tumor stem cells, have the role of activating antitumor immune responses. | 6 | A Comparative Analysis of Binary Logistic Regression and Analytical Hierarchy Process for Landslide Susceptibility Assessment in the Dobrovǎt River Basin,Romania显示文摘A correct assessment of the landslide susceptibility component is extremely useful for the diminution of associated potential risks to local economic development, particularly in regard to land use planning and soil conservation. The purpose of the present study was to compare the usefulness of two methods, i.e., binary logistic regression(BLR) and analytical hierarchy process(AHP), for the assessment of landslide susceptibility over a 130-km^2 area in the Moldavian Plateau(eastern Romania) region, where landslides affect large areas and render them unsuitable for agriculture. A large scale inventory mapping of all types of landslides(covering 13.7% of the total area) was performed using orthophoto images, topographical maps, and field surveys. A geographic information system database was created, comprising the nine potential factors considered as most relevant for the landsliding process. Five factors(altitude, slope angle, slope aspect, surface lithology, and land use) were further selected for analysis through the application of a tolerance test and the stepwise filtering procedure of BLR. For each predictor, a corresponding raster layer was built and a dense grid of equally spaced points was generated, with an approximately equal number of points inside and outside the landslide area, in order to extract the values of the predictors from raster layers. Approximately half of the total number of points was used for model computation, while the other half was used for validation. Analytical hierarchy process was employed to derive factor weights, with several pair-wise comparison matrices being tested for this purpose. The class weights, on a scale of 0 to 1, were taken as normalized landslide densities. A comparison of results achieved through these two approaches showed that BLR was better suited for mapping landslide susceptibility, with 82.8% of the landslide area falling into the high and very high susceptibility classes. The susceptibility class separation using standard deviation was superior to either the equal interval or the natural break method. | 7 | Is it better to use two elastographic methods for liver fibrosis assessment?显示文摘AIM:To find out if by combining 2 ultrasound based elastographic methods:acoustic radiation force impulse(ARFI)elastography and transient elastography(TE),we can improve the prediction of fibrosis in patients with chronic hepatitis C.METHODS:Our study included 197 patients with chronic hepatitis C.In each patient,we performed,in the same session,liver stiffness(LS)measurements by means of TE and ARFI,respectively,and liver biopsy(LB),assessed according to the Metavir score.10 LS measurements were performed both by TE and ARFI;median values were calculated and expressed in kilopascals(kPa)and meters/second(m/s),respectively.Only TE and ARFI measurements with IQR<30%andSR≥60%were considered reliable.RESULTS:On LB 13(6.6%)patients had F0,32(16.2%) had F1,52(26.4%)had F2,47(23.9%)had F3,and 53(26.9%)had F4.A direct,strong correlation was found between TE measurements and fibrosis(r=0.741),between ARFI and fibrosis(r=0.730)and also between TE and ARFI(r=0.675).For predicting significant fibrosis(F≥2),for a cutoff of 6.7 kPa,TE had 77.5% sensitivity(Se)and 86.5%specificity(Sp)[area under the receiver operating characteristic curve(AUROC)0.87] and for a cutoff of 1.2 m/s,ARFI had 76.9%Se and 86.7%Sp(AUROC 0.84).For predicting cirrhosis(F=4),for a cutoff of 12.2 kPa,TE had 96.2%Se and 89.6% Sp(AUROC 0.97)and for a cutoff of 1.8 m/s,ARFI had 90.4%Se and 85.6%Sp(AUROC 0.91).When both elastographic methods were taken into consideration,for predicting significant fibrosis(F≥2),(TE≥6.7 kPa and ARFI≥1.2 m/s)we obtained 60.5%Se,93.3% Sp,96.8%positive predictive value(PPV),41.4%negative predictive value(NPV)and 68%accuracy,while for predicting cirrhosis(TE≥12.2 kPa and ARFI≥1.8 m/s) we obtained 84.9%Se,94.4%Sp,84.9%PPV,94.4% NPV and 91.8%accuracy.CONCLUSION:TE used in combination with ARFI is highly specific for predicting significant fibrosis;therefore when the two methods are concordant,liver biopsy can be avoided. | Ioan Sporea Roxana Sirli Alina Popescu Simona Bota Radu Badea Monica Lupsor Mircea Focsa Mirela Danila | 2011 | World Journal of Gastroenterology2011,17,33: | 11 |

| 10 | Maximizing the endosonography: The role of contrast harmonics, elastography and confocal endomicroscopy显示文摘New technologies in endoscopic ultrasound(EUS) evaluation have been developed because of the need to improve the EUS and EUS-fine needle aspiration(EUS- FNA) diagnostic rate. This paper reviews the principle, indications, main literature results, limitations and future expectations for each of the methods presented. Contrast-enhanced harmonic EUS uses a low mechanical index and highlights slowflow vascularization. This technique is useful for differentiating solid and cystic pancreatic lesions and assessing biliary neoplasms, submucosal neoplasms and lymph nodes. It is also useful for the discrimination of pancreatic masses based on their qualitative patterns; however, the quantitative assessment needs to be improved. The detection of small solid lesions is better, and the EUS-FNA guidance needs further research. The differentiation of cystic lesions of the pancreas and the identification of the associated malignancy features represent the main indications. Elastography is used to assess tissue hardness based on the measurement of elasticity. Despite its low negative predictive value, elastography might rule out the diagnosis of malignancy for pancreatic masses. | 12 | Cancer stem cells: Involvement in pancreatic cancer pathogenesis and perspectives on cancer therapeutics显示文摘Pancreatic cancer is one of the most aggressive and lethal malignancies. Despite remarkable progress in understanding pancreatic carcinogenesis at the molecular level, as well as progress in new therapeutic approaches, pancreatic cancer remains a disease with a dismal prognosis. Among the mechanisms responsible for drug resistance, the most relevant are changes in individual genes or signaling pathways and the presence of highly resistant cancer stem cells(CSCs). In pancreatic cancer, CSCs represent 0.2%-0.8% of pancreatic cancer cells and are considered to be responsible for tumor growth, invasion, metastasis and recurrence. CSCs have been extensively studied as of late to identify specific surface markers to ensure reliable sorting and for signaling pathways identified to play a pivotal role in CSC self-renewal. Involvement of CSCs in pancreatic cancer pathogenesis has also highlighted these cells as the preferential targets for therapy. | 20 | Evaluation of the seismic hazard for 20 cities in Romania using Monte Carlo based simulations显示文摘This work focuses on the evaluation of the seismic hazard for Romania using earthquake catalogues generated by a Monte Carlo approach. The seismicity of Romania can be attributed to the Vrancea intermediate-depth seismic source and to 13 other crustal seismic sources. The recurrence times of large magnitude seismic events(both crustal and subcrustal), as well as the moment release rates are computed using simulated earthquake catalogues. The results show that the largest contribution to the overall moment release for the crustal seismic sources is from the seismic regions in Bulgaria, while the seismic regions in Romania contribute less than 5% of the overall moment release. In addition, the computations show that the moment release rate for the Vrancea subcrustal seismic source is about ten times larger than that of all the crustal seismic sources. Finally, the Monte Carlo approach is used to evaluate the seismic hazard for 20 cities in Romania with populations larger than 100,000 inhabitants.
