| 1 | Chemotherapy initiation with single-course methotrexate alone or combined with dactinomycin versus multi-course methotrexate for low-risk gestational trophoblastic neoplasia: a multi-centric randomized clinical trial显示文摘We aimed to evaluate the effectiveness and safety of single-course initial regimens in patients with low-risk gestational trophoblastic neoplasia(GTN).In this trial (NCT01823315),276 patients were analyzed.Patients were allocated to three initiated regimens:single-course methotrexate(MTX),single-course MTX+dactinomycin(ACTD),and multi-course MTX(control arm).The primary endpoint was the complete remission(CR)rate by initial drug(s).The primary CR rate was 64.4%with multi-course MTX in the control arm.For the single-course MTX arm,the CR rate was 35.8%by one course;it increased to 59.3%after subsequent multi-course MTX,with non-inferiority to the control(difference-5.1%,95%confidence interval(CI)-19.4%to 9.2%,P=0.014).After further treatment with multi-course ACTD,the CR rate(93.3%)was similar to that of the control(95.2%,P=0.577).For the single-course MTX+ACTD arm,the CR rate was 46.7%by one course,which increased to 89.1%after subsequent multi-course,with non-inferiority(difference 24.7%,95%CI 12.8%-36.6%,P<0.001)to the control.It was similar to the CR rate by MTX and further ACTD in the control arm(89.1%vs.95.2%,P=0.135).Four patients experienced recurrence,with no death,during the 2-year follow-up.We demonstrated that chemotherapy initiation with single-course MTX may be an alternative regimen for patients with low-risk GTN. | Lili Chen Ling Xi Jie Jiang Rutie Yin Pengpeng Qu Xiuqin Li Xiaoyun Wan Yaxia Chen Dongxiao Hu Yuyan Mao Zimin Pan Xiaodong Cheng Xinyu Wang Qingli Li Danhui Weng Xi Zhang Hong Zhang Quanhong Ping Xiaomei Liu Xing Xie Beihua Kong Ding Ma Weiguo Lu | 2022 | Frontiers of Medicine2022，16，2： | 2 |
| 2 | Epiretinal membrane following pars plana vitrectomy for rhegmatogenous retinal detachment repair显示文摘AIM: To determine the rate and possible contributors for post-pars plana vitrectomy(PPV) epiretinal membrane(ERM) in patients treated for rhegmatogenous retinal detachment(RRD). METHODS: This prospective, nonrandomized study comprised 47 consecutive patients(47 eyes) with acute RRD treated with 23 G post-PPV. All participants were followed prospectively for 6 mo for the development of ERM using spectral domain optical coherence tomography. Preoperative and intraoperative data were collected by questionnaires to surgeons. Main outcome measure was the percentage of the ERM formation following post-PPV for RRD. RESULTS: ERM developed postoperatively in 23 eyes(48.9%), none necessitated surgical removal. There was a statistically significant difference between patients with and without ERM postoperatively in preoperative best corrected visual acuity(median log MAR 1.9 vs 0.3, respectively;P=0.003) rate of macula-off(69.6% vs 37.5%, respectively, P=0.028), and rate of ≥5 cryo-applications(55.6% and 18.8%, respectively, P=0.039). ERM developed mainly between the 1st and 3rd months of follow-up. Macula-off status increased the risk of ERM, with the odds ratio of 3.81(P=0.031). | 20 | Increasing the value of weather-related warnings显示文摘Despite advances in forecasting and emergency preparedness, weather related disasters continue to cost many lives, to displace populations and to cause wide-spread damage. Therefore, High Impact Weather Project (HIWeather), a 10-year research project (http://gffzz72c37a797ca44d97s9poc5vnb0qxp6065.ffgz.tsg.suse.edu.cn/en/resources/buHetin/hiweather-10-yearresearch- project), was established in 2016 by WM0 (World Meteorology Organization) WWRP (World Weather Research Program: http://gffzz74e393fd09b145d8s9poc5vnb0qxp6065.ffgz.tsg.suse.edu.cn/pages/prog/arep/wwrp/new/wwrp_new_en. html). HIWeather aimed at achieving dramatic improvements in the effectiveness of weather-related hazard warnings, following recent advancement in numerical weather prediction at km-scale and in disaster risk reduction. The implementation plan was developed under the concept of warning chain, which comprises all components and the connections between to a successful weather-hazards warning: observations, weather forecast, hazard forecast, impact forecast, the generation of warnings and decision making (Fig. 1). A successful warning relies on information produced by the meteorological and related physical sciences, thus its effectiveness of delivery depends on applications of social, behavioral and economic sciences.
