| 2 | Telbivudine vs tenofovir in hepatitis B e antigen-negative chronic hepatitis B patients: OPTIMA roadmap study显示文摘AIM To make efficacy and safety comparison of telbivudineraodmap and tenofovir-roadmap in hepatitis B e antigen(HBe Ag)-negative chronic hepatitis B(CHB) patients.METHODS This was the first prospective, randomised, two-arm, open-label, non-inferiority study in HBe Ag-negative CHB patients that compared telbivudine and tenofovir administered as per roadmap concept. Patients were treated up to 24 wk and, depending on virologic response, continued the same therapy or received addon therapy up to 104 wk. Eligible patients received an additional 52 wk of treatment in the extension period(i.e., up to 156 wk). Patients who developed virologic breakthrough(VB) while on monotherapy also received add-on therapy. The primary efficacy endpoint was the rate of patients achieving hepatitis B virus(HBV) DNA < 300 copies/m L at week 52. Secondary efficacy endpoints included the rates of HBV DNA < 300 and < 169 copies/m L, HBV DNA change from baseline, alanine aminotransferase normalisation, hepatitis B surface antigen(HBs Ag) loss, HBs Ag seroconversion, VB, and emergence of resistance at various timepoints throughout the study. Safety and estimated glomerular filtration rate(e GFR) were also analysed.RESULTS A total of 241 patients were randomised. Non-inferiority of telbivudine arm to tenofovir arm was demonstrated at week 52(± 7 d window), with over 91% of patients in each treatment arm achieving HBV DNA level < 300 copies/m L. Both arms were similar in terms of key secondary efficacy variables at weeks 104 and 156. The percentage of patients achieving HBV DNA < 300 copies/m L remained high and was similar in the telbivudine and tenofovir arms at both weeks 104 and 156. Over 82% of patients in both arms achieved alanine aminotransferase normalisation at week 52, and this percentage remained high at weeks 104 and 156. Telbivudine treatment progressively reduced serum HBs Ag levels from baseline while no change was reported in quantitative HBs Ag during therapy with tenofovir. Both treaments showed acceptable safety profiles. The telbivudine arm showed e GFR improvement unlike the tenofovir arm.CONCLUSION Efficacy was shown for both telbivudine-roadmap and tenofovir-roadmap regimens in HBe Ag-negative CHB patients over 156 wk. | 3 | A systematic review on the aboveground biomass and carbon stocks of Indian forest ecosystems显示文摘Background:Tropical forests play a crucial role as source and sink in global carbon cycle.Development and other anthropogenic activities have led to degradation of forest land,and ultimately,it results in loss of biodiversity and increases concentration of CO_(2) in atmospheres.Therefore,there is urgent need to estimate regional and national level carbon stock for making forest-based policies and strategies for mitigation of CO_(2).Patchy and sporadic information is available on biomass and carbon stock of Indian forests.The paper presents a systematic review and comprehensive account of studies conducted in different forest types in India.Result:There are six major forest types found in India consisting of 15 groups and other subgroups with peculiar characteristics.Methodologies used by researchers for biomass/carbon stock estimation are destructive,nondestructive,tree inventories data,species-specific biomass estimation,and remote sensing.Majority of estimates are based on nondestructive allometric equation approach.Studies showed positive correlation between tree species,diameter at breast height,and biomass/carbon stock.Small-and medium-sized growing trees,invasive species,mixed forest,Agroforestry,and Agrosilviculture also play an important role in atmospheric carbon assimilation.The results of diverse forest carbon stock studies are broadly categorized in North,Central,and Southern India.Present review will be helpful for developing conservation policies and decision to increase carbon stock and also REDD+program for particular forest ecosystem.Conclusion:The systematic literature review was carried out to gather and summarize information from different studies conducted on forest ecosystems and quantification methods used for biomass estimation and carbon stock in different forests types and states of India.In general,great variability occurs in aboveground biomass and carbon stock on account of climatic and geographic differences.To obtain good and accurate estimations,following nondestructive approach,species-specific density-based equations are required from different habitats and also in relation to degradation status of forests.As such regional volume equations would increase error of estimations.The comprehensive account of data would be helpful to formulate strategies based on carbon sequestration in Indian forests for CO_(2) mitigation. | Onkar Salunkhe P.K.Khare Richa Kumari M.L.Khan | 2018 | Ecological Processes2018,7,1: | 4 |

| 6 | ICESat/GLAS-derived changes in the water level of Hulun Lake, Inner Mongolia, from 2003 to 2009显示文摘Hulun Lake is the largest freshwater lake innorthern Inner Mongolia and even minor changes in itslevel may have major effects on the ecology of the lake andthe surrounding area. In this study, we used high-precisionelevation data for the interval from 2003-2009 measuredby the Geoscience Laser Altimetry System (GLAS) onboard the Ice, Cloud, and land Elevation Satellite (ICESat)to assess annual and seasonal water level variations ofHulun Lake. The altimetry data of 32 satellite tracks wereprocessed using the RANdom SAmple Consensus algo-rithm (RANSAC) to eliminate elevation outliers, andsubsequently the Normalized Difference Water Index(NDWI) was used to delineate the area of the lake. From2003-2009, the shoreline of Hulun Lake retreatedwestwards, which was especially notable in the southernpart of the lake. There was only a small decrease in waterlevel, from 530.72 m to 529.22 m during 2003-2009, anaverage rate of 0.08 m/yr. The area of the lake decreased ata rate of 49.52 km^2/yr, which was mainly the result of theshallow bathymetry in the southern part of the basin. Thedecrease in area was initially rapid, then much slower, andfinally rapid again. Generally, the lake extent and waterlevel decreased due to higher temperatures, intenseevaporation, low precipitation, and decreasing runoff.And their fluctuations were caused by a decrease in intra-annual temperature, evaporation, and a slight increase inprecipitation. Overall, a combination of factors related toclimate change were responsible for the variations of thewater level of Hulun Lake during the study interval. | 9 | Immunophenotypic signature of primary glioblastoma multiforme: A case of extended progression free survival显示文摘Glioblastoma-multiforme(GBM), the most aggressive glial tumor, has a worldwide age-adjusted incidence ranging from 0.59-3.69/100000 persons. Despite current multimodal-treatment approach, median-survival time and progression-free survival(PFS) remains short. Glioblastomas display a variety of molecular alterations, which necessitates determining which of these have a prognostic significance. This is a case of a 45-yearold patient who presented with progressive slurring of speech and features of raised intracranial pressure. Computed tomography(CT) scan revealed a large heterogeneously enhancing lesion in the left front-temporalperisylvian region with solid, cystic areas, suggestive of malignant glioma. Partial tumor-excision was followed by concurrent chemo-radiotherapy. Histopathologically, the tumor was astrocytoma grade-IV. Patient had an extended PFS of 12 mo, with an overall survival of 26 mo. Primary-GBM was confirmed using molecular markers and the immunophenotypic signature was defined by evaluating systemic expression of human telomerase reverse transcriptase, interleukin-6, neutrophil-lymphocyte ratio, tissue inhibitor of metalloproteinases-1, human chitinase-3-like-protein-1(YKL-40) and high mobility group-A1. | 10 | Unique case of oligoastrocytoma with recurrence and grade progression:Exhibiting differential expression of high mobility group-A1 and human telomerase reverse transcriptase显示文摘Mixed gliomas, primarily oligoastrocytomas, account for about 5%-10% of all gliomas. Distinguishing oligoastrocytoma based on histological features alone has limitations in predicting the exact biological behavior, necessitating ancillary markers for greater specificity. In this case report, human telomerase reverse transcriptase(hT ERT) and high mobility group-A1(HMGA1); markers of proliferation and stemness, have been quantitatively analyzed in formalin-fixed paraffin-embedded tissue samples of a 34 years old patient with oligoastrocytoma. Customized florescence-based immunohistochemistry protocol with enhanced sensitivity and specificity is used in the study. The patient presented with a history of generalized seizures and his magnetic resonance imaging scans revealed infiltrative ill-defined mass lesion with calcified foci within the left frontal white matter, suggestive of glioma. He was surgically treated at our center for four consecutive clinical events. Histopathologically, the tumor was identified as oligoastrocytoma-grade Ⅱ followed by two recurrence events and final progression to grade Ⅲ. Overall survival of the patient without adjuvant therapy was more than 9 years. Glial fibrillary acidic protein, p53, Ki-67, nuclear atypia index, pre-operative neutrophillymphocyte ratio, are the other parameters assessed. Findings suggest that hT ERT and HMGA1 are linked to tumor recurrence and progression. | 11 | Viral Genotypes and Associated Risk Factors of Hepatocellular Carcinoma in India显示文摘Objective This study aims to investigate the etiological relationship among hepatitis B virus(HBV),hepatitis C virus(HCV),and alcohol as risk factors in a cohort of hepatocellular carcinoma(HCC) patients from India.The clinical and biochemical profiles and tumor characteristics in the HCC cases were also evaluated. Methods A total of 357 consecutive cases of HCC fulfilling the diagnostic criteria from the Barcelona-2000 EASL conference were included in the study.The blood samples were evaluated for serological evidence of HBV and HCV infection,viral load,and genotypes using serological tests,reverse transcription-polymerase chain reaction,and restriction fragment length polymorphism. Results The male/female ratio for the HCC cases was 5.87:1.Majority of the HCC patients(33.9% ) were 50 to 59 years of age,with a mean age of 4±13.23 years.More than half the cases(60.8% ) had underlying cirrhosis at presentation.Among the HCC patients,68.9% were HBV related,21.3% were HCV related,18.8% were alcoholic,and 18.2% were of cryptogenic origin.The presence of any marker positive for HBV increased the risk for developing HCC by almost 27 times[OR:27.33;(12.87-60.0)].An increased risk of 10.6 times was observed for HCC development for cases positive for any HCV marker[OR:10.55;(3.13-42.73)].Heavy alcohol consumption along with HCV RNA positivity in cirrhotic patients was found to be a risk for developing HCC by 3 folds[OR:3.17;(0.37-70.71)]. Conclusions Patients of chronic HBV infection followed by chronic HCV infection were at higher risk of developing HCC in India. Chronic alcohol consumption was found to be a risk factor in cirrhotic cases only when it was associated with HCV RNA positivity. | 12 | Cowpea NAC1/NAC2 transcription factors improve growth and tolerance to drought and heat in transgenic cowpea through combined activation of photosynthetic and antioxidant mechanisms显示文摘NAC(NAM/ATAF1/2/CUC2)transcription factors are central switches of growth and stress responses in plants.However,unpredictable interspecies conservation of function and regulatory targets makes the well-studied NAC orthologs inapt for pulse engineering.The knowledge of suitable NAC candidates in hardy pulses like cowpea(Vigna unguiculata(L.)Walp.)is still in infancy,hence warrants immediate biotechnological intervention.Here,we showed that overexpression of two native NAC genes(VuNAC1and VuNAC2)promoted germinative,vegetative,and reproductive growth and conferred multiple abiotic stress tolerance in a commercial cowpea variety.The transgenic lines displayed increased leaf area,thicker stem,nodule-rich denser root system,early flowering,higher pod production(~3.2-fold and~2.1-fold),and greater seed weight(10.3%and 6.0%).In contrast,transient suppression of VuNAC1/2 caused severe growth retardation and flower inhibition.The overexpressor lines showed remarkable tolerance to major yielddeclining terminal stresses,such as drought,salinity,heat,and cold,and recovered growth and seed production by boosting photosynthetic activity,water use efficiency,membrane integrity,Na^(+)/K^(+)homeostasis,and antioxidant activity.The comparative transcriptome study indicated consolidated activation of genes involved in chloroplast development,photosynthetic complexes,cell division and expansion,cell wall biogenesis,nutrient uptake and metabolism,stress response,abscisic acid,and auxin signaling.Unlike their orthologs,VuNAC1/2 direct synergistic transcriptional tuning of stress and developmental signaling to avoid unwanted trade-offs.Their overexpression governs the favorable interplay of photosynthesis and reactive oxygen species regulation to improve stress recovery,nutritional sufficiency,biomass,and production.This unconventional balance of strong stress tolerance and agronomic quality is useful for translational crop research and molecular breeding of pulses. | Richa Srivastava Yuriko Kobayashi Hiroyuki Koyama Lingaraj Sahoo | 2023 | Journal of Integrative Plant Biology2023,65,1: | 2 |
| 13 | Novel molecular panel for evaluating systemic inflammation and survival in therapy naïve glioma patients显示文摘BACKGROUND Inflammation is crucial to tumor progression.A traumatic event at a specific site in the brain activates the signaling molecules,which triggers inflammation as the initial response within the tumor and its surroundings.The educated immune cells and secreted proteins then initiate the inflammatory cascade leading to persistent chronic inflammation.Therefore,estimation of the circulating inflammatory indicators kynurenine(KYN),interleukin-6(IL-6),tissue-inhibitor of matrix-metalloproteinase-1 and human telomerase reverse transcriptase(hTERT)along with neutrophil-lymphocyte ratio(NLR)has prognostic value.AIM To assess the utility of chosen inflammatory marker panel in estimating systemic inflammation.METHODS The chosen markers were quantitatively evaluated in 90 naive,molecularly subtyped plasma samples of glioma.A correlation between the markers and confounders was assessed to establish their prognostication power.Follow-up on the levels of the indicators was done 3-mo post-surgery.To establish the validity of circulating KYN,it was also screened qualitatively by dot-immune-assay and by immunofluorescence-immunohistochemistry in tumor tissues.RESULTS Median values of circulating KYN,IL-6,hTERT,tissue-inhibitor of matrixmetalloproteinase-1 and NLR in isocitrate-dehydrogenase-mutant/wildtype and within the astrocytic sub-groups were estimated,which differed from controls,reaching statistical significance(P<0.0001).All markers negatively correlated with mortality(P<0.0001).Applying combination-statistics,the panel of KYN,IL-6,hTERT and NLR achieved higher sensitivity and specificity(>90%)than standalone markers,to define survival.The inflammatory panel could discriminate between WHO grades,and isocitrate-dehydrogenase-mutant/wildtype and define differential survival between astrocytic isocitrate-dehydrogenase-mutant/wildtype.Therefore,its assessment for precise disease prognosis is indicated.Association of KYN with NLR,IL-6 and hTERT was significant.Cox-regression described KYN,IL-6,NLR,and hTERT as good prognostic markers,independent of confounders.Multivariate linear-regression analysis confirmed the association of KYN and hTERT with inflammation marker IL-6.There was a concomitant significant decrease in their levels in a 3-mo follow-up.CONCLUSION The first evidence-based study of circulating-KYN in molecularly defined gliomas,wherein the tissue expression was found to be concomitant with plasma levels.A non-invasive model for assessing indicators of chronic systemic inflammation is proposed. | Puneet Gandhi Richa Shrivastava Nitin Garg Sandeep K Sorte | 2021 | World Journal of Clinical Oncology2021,12,10: | 2 |
