| 1 | Relationship between intestinal microbiota and ulcerative colitis:Mechanisms and clinical application of probiotics and fecal microbiota transplantation显示文摘Ulcerative colitis(UC) is an inflammatory disease that mainly affects the colon and rectum. It is believed that genetic factors, host immune system disorders, intestinal microbiota dysbiosis, and environmental factors contribute to the pathogenesis of UC. however, studies on the role of intestinal microbiota in the pathogenesis of UC have been inconclusive. Studies have shown that probiotics improve intestinal mucosa barrier function and immune system function and promote secretion of anti-inflammatory factors, thereby inhibiting the growth of harmful bacteria in the intestine. Fecal microbiota transplantation(FMT) can reduce bowel permeability and thus the severity of disease by increasing the production of short-chain fatty acids, especially butyrate, which help maintain the integrity of the epithelial barrier. FMT can also restore immune dysbiosis by inhibiting Th1 differentiation, activity of T cells, leukocyte adhesion, and production of inflammatory factors. Probiotics and FMT are being increasingly used to treat UC, but their use is controversial because of uncertain efficacy. | 4 | Randomized controlled trial of uncut Roux-en-Y vs Billroth Ⅱ reconstruction after distal gastrectomy for gastric cancer: Which technique is better for avoiding biliary reflux and gastritis?显示文摘AIM To identify which technique is better for avoiding biliary reflux and gastritis between uncut Roux-en-Y and Billroth Ⅱ reconstruction. METHODS A total of 158 patients who underwent laparoscopyassisted distal gastrectomy for gastric cancer at the First Hospital of Jilin University(Changchun, China) between February 2015 and February 2016 were randomized into two groups: uncut Roux-en-Y(group U) and Billroth II group(group B). Postoperative complications and relevant clinical data were compared between the two groups. RESULTS According to the randomization table, each group included 79 patients. There was no significant difference in postoperative complications between groups U and B(7.6% vs 10.1%, P = 0.576). During the postoperative period, group U stomach p H values were lower than 7 and group B p H values were higher than 7. After 1 year of follow-up, group B presented a higher incidence of biliary reflux and alkaline gastritis. However, histopathology did not show a significant difference in gastritis diagnosis(P = 0.278), and the amount of residual food and gain of weight between the groups were also not significantly different. At 3 mo there was no evidence of partial recanalization of uncut staple line, but at 1 year the incidence was 13%. CONCLUSION Compared with Billroth II reconstruction, uncut Rouxen-Y reconstruction is secure and feasible, and can effectively reduce the incidence of alkaline reflux, residual gastritis, and heartburn. | 5 | Spatial-Temporal Pattern and Driving Forces of Land Use Changes in Xiamen显示文摘Using Landsat TM data of 1988, 1998 and 2001, the dynamic process of the spatial-temporal characteristics of land use changes during 13 years from 1988 to 2001 in the special economic zone of Xiamen, China was analyzed to improve understanding and to find the driving forces of land use change so that sustainable land utilization could be practiced. During the 13 years cropland decreased remarkably by nearly 11304.95 ha. The areas of rural-urban construction and water body increased by 10 152.24 ha and 848.94 ha, respectively. From 1988 to 2001, 52.5% of the lost cropland was converted into rural-urban industrial land. Rapid urbanization contributed to a great change in the rate of cropland land use during these years. Land-reclamation also contributed to a decrease in water body area as well as marine ecological and environmental destruction. In the study area 1) urbanization and industrialization, 2) infrastructure and agricultural intensification, 3) increased affluence of the farming community, and 4) policy factors have driven the land use changes. | 6 | Anisotropic coding metamaterials and their powerful manipulation of differently polarized terahertz waves显示文摘Metamaterials based on effective media can be used to produce a number of unusual physical properties(for example,negative refraction and invisibility cloaking)because they can be tailored with effective medium parameters that do not occur in nature.Recently,the use of coding metamaterials has been suggested for the control of electromagnetic waves through the design of coding sequences using digital elements‘0’and‘1,'which possess opposite phase responses.Here we propose the concept of an anisotropic coding metamaterial in which the coding behaviors in different directions are dependent on the polarization status of the electromagnetic waves.We experimentally demonstrate an ultrathin and flexible polarization-controlled anisotropic coding metasurface that functions in the terahertz regime using specially designed coding elements.By encoding the elements with elaborately designed coding sequences(both 1-bit and 2-bit sequences),the x-and y-polarized waves can be anomalously reflected or independently diffused in three dimensions.The simulated far-field scattering patterns and near-field distributions are presented to illustrate the dual-functional performance of the encoded metasurface,and the results are consistent with the measured results.We further demonstrate the ability of the anisotropic coding metasurfaces to generate a beam splitter and realize simultaneous anomalous reflections and polarization conversions,thus providing powerful control of differently polarized electromagnetic waves.The proposed method enables versatile beam behaviors under orthogonal polarizations using a single metasurface and has the potential for use in the development of interesting terahertz devices. | Shuo Liu Tie Jun Cui Quan Xu Di Bao Liangliang Du Xiang Wan Wen Xuan Tang Chunmei Ouyang Xiao Yang Zhou Hao Yuan Hui Feng Ma Wei Xiang Jiang Jiaguang Han Weili Zhang Qiang Cheng | 2016 | Light(Science & Applications)2016,5,1: | 36 |
| 7 | Routine lymph node dissection may be not suitable for all intrahepatic cholangiocarcinoma patients: Results of a monocentric series显示文摘AIM:To investigate the indications for lymph node dissection(LND)in intrahepatic cholangiocarcinoma patients.METHODS:A retrospective analysis was conducted on 124 intrahepatic cholangiocarcinoma(ICC)patients who had undergone surgical resection of ICC from January 2006 to December 2007.Curative resection was attempted for all patients unless there were metastases to lymph nodes(LNs)beyond the hepatoduodenal ligament.Prophylactic LND was performed in patients in whom any enlarged LNs had been suspicious for metastases.The patients were classified according to the LND and LN metastases.Clinicopathologic,operative,and long-term survival data were collected retrospectively.The impact on survival of LND during primary resection was analyzed.RESULTS:Of 53 patients who had undergone hepatic resection with curative intent combined with regional LND,11 had lymph nodes metastases.Whether or not patients without lymph node involvement had undergone LND made no significant difference to their survival(P=0.822).Five patients with multiple tumors and involvement of lymph nodes underwent hepatic resection with LND;their survival curve did not differ significantly from that of the palliative resection group(P=0.744).However,there were significant differences in survival between patients with lymph node involvement and a solitary tumor who underwent hepatic resection with LND and the palliative resection group(median survival time 12 mo vs 6.0 mo,P=0.013).CONCLUSION:ICC patients without lymph node involvement and patients with multiple tumors and lymph node metastases may not benefit from aggressive lymphadenectomy.Routine LND should be considered with discretion. | Dong-Yu Li Hai-Bin Zhang Ning Yang Yuan Quan Guang-Shun Yang | 2013 | World Journal of Gastroenterology2013,19,47: | 31 |
| 8 | Combined therapy with transcatheter arterial chemoembolization and percutaneous microwave coagulation for small hepatocellular carcinoma显示文摘AIM:To assess the efficacy of combined transcatheter arterial chemoembolization(TACE)and percutaneous microwave coagulation therapy(PMCT)for small hepatocellular carcinoma(HCC). METHODS:Thirty-five patients with a total of 41 HCC nodules(≤3 cm in diameter)were treated with TACE followed by computed tomograghy(CT)-guided percutaneous microwave coagulation therapy(PMCT) within 1-3 wk. RESULTS:By biopsies and enhanced CT scans, complete necrosis of the tumor and 3-5 mm of the surrounding non-cancerous area were observed in 34 foci.In seven foci,incomplete necrosis of the surrounding parenchyma was observed.Serum alpha- fetoprotein(AFP)levels returned to normal 10 d after treatment in 25 patients who originally had high serum AFP levels.The follow-up period was 6-31 mo,and all patients remained alive.One patient had a recurrence in the subsegments of the liver,and another patient had a recurrence near the original lesion. | 9 | Effects of Nitrogen Application Rate and Ratio on Lodging Resistance of Super Rice with Different Genotypes显示文摘The objective of this study was to determine the morphology mechanism of nitrogen(N) fertilizer rates and ratio on lodging resistance through analying its effects among lodging index(LI),lodging-related morphological traits and physical strength in basal internodes by comparing japonica and indica super rice cultivars.Field experiments,with three nitrogen levels(0,150 and 300 kg ha-1) and two ratios of basal to topdressing(8:2 and 5:5) with two super rice cultivars(Yliangyou 2 and Wuyunjing 23),were conducted in the Baolin Farm,Danyang Country,Jiangsu Province,China,in 2011 and 2012.Effects of N fertilizer rates and ratios on morphology of whole plant,morphology traits in basal internodes and culm's physical strength parameters were investigated at 20 d after full heading stage.LI of Yliangyou 2 was significant greater than that of Wuyunjing 23 due to larger bending moment by whole plant(WP) with higher plant height and gravity center height.With higher volume of N fertilizer,LI of two super rice cultivars was increased conspicuously.However,no significant effect was detected with increase of panicle fertilizer ratio.The size of breaking strength(M) in basal internodes was the key factor determining LI among N fertilizer treatments.Correlation analysis revealed that M value was positively related bending stress(BS) of Wuyunjing 23 and section modulus(Z) of Yliangyou 2,respectively.The higher N fertilizer levels induced reduction of BS of Wuyunjing 23 due to weak culm and leaf sheath plumpness status and reduced Z of Yliangyou 2 owning to small diameter and culm wall thickness,consequently,influencing their M indirectly.These results suggested that breaking strength was the key factor influencing LI with increase of N fertilizer levels.However,the lodging-related morphology mechanism was different with genotypes.Culm wall thickness and diameter in basal internodes of indica super rice and culm and leaf sheath plumpness status of japonica super rice influenced breaking strength,as well as lodging index,respectively. | ZHANG Wu-jun LI Gang-hua YANG Yi-ming LI Quan ZHANG Jun LIU Jin-you WANG Shao-hua TANG She DING Yan-feng | 2014 | Journal of Integrative Agriculture2014,13,1: | 32 |

| 11 | Combined detection of serum tumor markers for differential diagnosis of solid lesions located at the pancreatic head显示文摘BACKGROUND: The differential diagnosis of solid lesions located at the pancreatic head is very important for choosing therapies and setting up surgical tactics. This study was designed to evaluate the clinical significance of combined measurement of multiple serum tumor markers and the application of the receiver-operating characteristic (ROC) curves in the differential diagnosis of solid lesions located at the pancreatic head. METHODS: The serum levels of CA19-9, CA242, CA50 and carcinoembryonic antigen (CEA) in 112 patients with carcinoma of the pancreatic head and 38 patients with focal chronic pancreatitis in the pancreatic head were measured with ELISA. The sensitivity, specificity, positive likelihood ratio (PLR) and negative likelihood ratio (NLR) of the four serum tumor markers were calculated. The ROC curves for the four serum tumor markers were constructed and the area under the curve (AUC) was calculated. RESULTS: The AUCs of CA19-9, CA242, CA50 and CEA were 0.805, 0.749, 0.738 and 0.705; the PLRs were 1.91, 3.43, 5.09 and 5.46; and the NLRs were 0.41, 0.56, 0.59 and 0.71, respectively. Combined measurements increased the diagnostic specificity, and parallel combined testing increased the diagnostic sensitivity. CONCLUSIONS: Combined measurement of serum tumor markers CA19-9, CA242, CA50 and CEA is valuable in differential diagnosis of solid lesions located at the pancreatic head, and CA19-9 has the best diagnostic ability. Combined measurements can increase the specificity ofdiagnosis. | 12 | Single-cell RNA-seq highlights intra-tumoral heterogeneity and malignant progression in pancreatic ductal adenocarcinoma显示文摘Pancreatic ductal adenocarcinoma (PDAC) is the most common type of pancreatic cancer featured with high intra-tumoral heterogeneity and poor prognosis. To comprehensively delineate the PDAC intra-tumoral heterogeneity and the underlying mechanism for PDAC progression, we employed single-cell RNA-seq (scRNA-seq) to acquire the transcriptomic atlas of 57,530 individual pancreatic cells from primary PDAC tumors and control pancreases, and identified diverse malignant and stromal cell types, including two ductal subtypes with abnormal and malignant gene expression profiles respectively, in PDAC. We found that the heterogenous malignant subtype was composed of several subpopulations with differential proliferative and migratory potentials. Cell trajectory analysis revealed that components of multiple tumor-related pathways and transcription factors (TFs) were differentially expressed along PDAC progression. Furthermore, we found a subset of ductal cells with unique proliferative features were associated with an inactivation state in tumor-infiltrating T cells, providing novel markers for the prediction of antitumor immune response. | 14 | Upper-gastrointestinal bleeding secondary to peptic ulcer disease:Incidence and outcomes显示文摘AIM:To evaluate the incidence,surgery,mortality,and readmission of upper gastrointestinal bleeding(UGIB)secondary to peptic ulcer disease(PUD).METHODS:Administrative databases identified all hospitalizations for UGIB secondary to PUD in Alberta,Canada from 2004 to 2010(n=7079)using the International Classification of Diseases Codes(ICD-10).A subset of the data was validated using endoscopy reports.Positive predictive value and sensitivity with 95%confidence intervals(CI)were calculated.Incidence of UGIB secondary to PUD was calculated.Logistic regression was used to evaluate surgery,in-hospital mortality,and 30-d readmission to hospital with recurrent UGIB secondary to PUD.Co-variants accounted for in our logistic regression model included:age,sex,area of residence(i.e.,urban vs rural),number of Charlson comorbidities,presence of perforated PUD,undergoing upper endoscopy,year of admission,and interventional radiological attempt at controlling bleeding.A subgroup analysis(n=6356)compared outcomes of patients with gastric ulcers to those with duodenal ulcers.Adjusted estimates are presented as odds ratios(OR)with95%CI.RESULTS:The positive predictive value and sensitivity of ICD-10 coding for UGIB secondary to PUD were85.2%(95%CI:80.2%-90.2%)and 77.1%(95%CI:69.1%-85.2%),respectively.The annual incidence between 2004 and 2010 ranged from 35.4 to 41.2 per100000.Overall risk of surgery,in-hospital mortality,and 30-d readmission to hospital for UGIB secondary to PUD were 4.3%,8.5%,and 4.7%,respectively.Interventional radiology to control bleeding was performed in 0.6%of patients and 76%of these patients avoided surgical intervention.Thirty-day readmission significantly increased from 3.1%in 2004 to 5.2%in 2010(OR=1.07;95%CI:1.01-1.14).Rural residents(OR rural vs urban:2.35;95%CI:1.83-3.01)and older individuals(OR≥65 vs<65:1.57;95%CI:1.21-2.04)were at higher odds of being readmitted to hospital.Patients with duodenal ulcers had higher odds of dying(OR=1.27;95%CI:1.05-1.53),requiring surgery(OR=1.73;95%CI:1.34-2.23),and being readmitted to hospital(OR=1.54;95%CI:1.19-1.99)when compared to gastric ulcers.CONCLUSION:UGIB secondary to PUD,particularly duodenal ulcers,was associated with significant morbidity and mortality.Early readmissions increased over time and occurred more commonly in rural areas. | Samuel Quan Alexandra Frolkis Kaylee Milne Natalie Molodecky Hong Yang Elijah Dixon Chad G Ball Robert P Myers Subrata Ghosh Robert Hilsden Sander Veldhuyzen van Zanten Gilaad G Kaplan | 2014 | World Journal of Gastroenterology2014,20,46: | 21 |
| 15 | The Dihedral Angle and Intersection Processes of a Conjugate Strike-Slip Fault System in the Tarim Basin, NW China显示文摘Recent studies, focused on dihedral angles and intersection processes, have increased understandings of conjugate fault mechanisms. We present new 3-D seismic data and microstructural core analysis in a case study of a large conjugate strike-slip fault system from the intracratonic Tarim Basin, NW China. Within our study area, 'X' type NE and NW trending faults occur within CambrianOrdovician carbonates. The dihedral angles of these conjugate faults have narrow ranges, 19° to 62° in the Cambrian and 26° to 51° in the Ordovician, and their modes are 42° and 44° respectively. These data are significantly different from the ~60° predicted by the Coulomb fracture criterion. It is concluded that:(1) The dihedral angles of the conjugate faults were not controlled by confining pressure, which was low and associated with shallow burial;(2) As dihedral angles were not controlled by pressure they can be used to determine the shortening direction during faulting;(3) Sequential slip may have played an important role in forming conjugate fault intersections;(4) The conjugate fault system of the Tarim basin initiated as rhombic joints; these subsequently developed into sequentially active 'X' type conjugate faults; followed by preferential development of the NW-trending faults; then reactivation of the NE trending faults. Methods:The effects of regenerated silk fibroin film on cytotoxicity, adhesion, cell cycle, and apoptosis of L929 cells, growth and vascular endothelial growth factor (VEGF) expression of ECV304 cells, and VEGF, angiopoietin-1 (Ang-1), platelet-derived growth factor (PDGF) and fibroblast growth factor 2 (FGF2) expression of WI-38 cells were assessed by 3-(4,5)-dimethylthiahiazo (-z-y1)-3,5-di-phenytetrazoliumromide (MTT) assay, viable cell counting, flow cytometry (FCM), and enzyme-linked immunosorbant assay (ELISA). Results: We showed that the regenerated silk fibroin film was not cytotoxic to L929 cells and had no adverse influence on their adhesion, cell cycle or apoptosis; it had no adverse influence on the growth and VEGF secretion of ECV304 cells and no effect on the secretion of VEGF, Ang-1, PDGF and FGF2 by WI-38 cells. | 19 | Impact of Heat Wave in 2005 on Mortality in Guangzhou, China显示文摘Objective To assess the impact of the heat wave in 2005 on mortality among the residents in Guangzhou and to identify susceptible subpopulations in Guangzhou, China. Methods The data of daily number of deaths and meteorological measures from 2003 to 2006 in Guangzhou were used in this study. Heat wave was defined as ≥7 consecutive days with daily maximum temperature above 35.0 ℃ and daily mean temperature above the 97th percentile during the study period. The excess deaths and rate ratio (RR) of mortality in the case period compared with the reference period in the same summer were calculated. Results During the study period, only one heat wave in 2005 was identified and the total number of excess deaths was 145 with an average of 12 deaths per day. The effect of the heat wave on non-accidental mortality (RR=1.23, 95% CI: 1.11-1.37) was found with statistically significant difference. Also, greater effects were observed for cardiovascular mortality (RR=1.34, 95% CI: 1.13-1.59) and respiratory mortality (RR=1.31, 95% CI: 1.02-1.69). Females, the elderly and people with lower socioeconomic status were at significantly higher risk of heat wave-associatedmortality. Conclusion The 2005 heat wave had a substantial impact on mortality among the residents in Guangzhou, particularly among some susceptible subpopulations. | 20 | Excavator Energy-saving Efficiency Based on Diesel Engine Cylinder Deactivation Technology显示文摘The hydraulic excavator energy-saving research mainly embodies the following three measures: to improve the performance of diesel engine and hydraulic component, to improve the hydraulic system, and to improve the power matching of diesel-hydraulic system-actuator. Although the above measures have certain energy-saving effect, but because the hydraulic excavator load changes frequently and fluctuates dramatically, so the diesel engine often works in high-speed and light load condition, and the fuel consumption is higher. Therefore, in order to improve the economy of diesel engine in light load, and reduce the fuel consumption of hydraulic excavator, energy management concept is proposed based on diesel engine cylinder deactivation technology. By comparing the universal characteristic under diesel normal and deactivated cylinder condition, the mechanism that fuel consumption can be reduced significantly by adopting cylinder deactivation technology under part of loads condition can be clarified. The simulation models for hydraulic system and diesel engine are established by using AMESim software, and fuel combustion consumption by using cylinder-deactivation-technology is studied through digital simulation approach. In this way, the zone of cylinder deactivation is specified. The testing system for the excavator with this technology is set up based on simulated results, and the results show that the diesel engine can still work at high efficiency with part of loads after adopting this technology; fuel consumption is dropped down to 11% and 13% under economic and heavy-load mode respectively under the condition of driving requirements.
