| 2 | Tibetan Plateau climate dynamics: recent research progress and outlook显示文摘This paper reviews progress in the study of Tibetan Plateau(TP) climate dynamics over the past decade.Several theoretical frameworks, including thermal adaptation and the TP sensible heat(SH) driving air-pump, have been developed to identify the mechanisms responsible for the circulation anomaly produced by thermal forcing of the TP. Numerical simulations demonstrate that the thermal efects of large-scale orography, including the Tibetan and Iranian Plateaus(TIP), are crucial for the formation of the East Asian and South Asian summer monsoons(SASM) because the surface SH of the TIP is the major driver of the water vapor transport required for the genesis of the north branch of the SASM. he large-scale orography of the TP afects the Asian climate through thermal forcing in spring and summer, and mechanical forcing in winter. he TP forcing can also inluence the Asian summer monsoon(ASM) onset over the Bay of Bengal(BOB) by enhancing the BOB warm pool at the surface and by modulating the South Asian High(SAH) in the upper troposphere. On intra-seasonal timescales, the TP thermal forcing signiicantly modulates spring rainfall in southern China and generates the biweekly oscillation of the SAH in summer. Despite climate warming, the atmospheric heat source over the TP, particularly the spring SH,exhibits a clear weakening trend from the 1980 s to 2000 s. his weakening of the spring SH contributed to the anomalous ‘dry in the north' and ‘wet in the south' rainfall patern observed over East China. | 3 | The Association of Maternal Body Composition and Dietary Intake with the Risk of Gestational Diabetes Mellitus during the Second Trimester in a Cohort of Chinese Pregnant Women显示文摘Objective To investigate the association of maternal body composition and dietary intake with the risk of gestational diabetes mellitus(GDM). Methods A total 154 GDM subjects and 981 controls were enrolled in a prospective cohort study in 11 hospitals from May 20, 2012 to December 31, 2013. Bioelectrical impedance analysis and dietary surveys were used to determine body composition and to evaluate the intake of nutrients in subjects at 21-24 weeks' gestation(WG). Logistic regression analysis was applied to explore the relationships of maternal body composition and dietary intake with the risk of GDM morbidity. Results Age, pre-pregnant body weight(BW), and body mass index(BMI) were associated with increased risk of GDM. Fat mass(FM), fat mass percentage(FMP), extracellular water(ECW), BMI, BW, energy, protein, fat, and carbohydrates at 21-24 WG were associated with an increased risk of GDM. In contrast, fat free mass(FFM), muscular mass(MM), and intracellular water(ICW) were associated with a decreased risk of GDM. | 6 | Continuous regional arterial infusion for the treatment of severe acute pancreatitis: a meta-analysis显示文摘BACKGROUND: Continuous regional arterial infusion(CRAI) is a drug delivery system, which dramatically increases the drug concentration in the pancreas. Previous clinical and basic studies have demonstrated the possible therapeutic efficacy of CRAI for severe acute pancreatitis(SAP). This meta-analysis of all published randomized controlled trials(RCTs) was conducted to assess the efficacy and safety of CRAI for the treatment of SAP. DATA SOURCES: Up to August 10, 2014, RCTs comparing CRAI with intravenous infusion for SAP in PubM ed, Embase, EBSCO, MEDLINE, Science Citation Index Expanded, Cochrane Library, China Academic Journals Full-Text Database, Chinese Biomedical Literature Database, and Chinese Scientific Journals Database were selected by two independent reviewers. The relative risk(RR) and their 95% confidence intervals(CI) for duration of elevated serum amylase and urine amylase, duration of abdominal pain, infection rate, incidence of complication, overall mortality, curative rate, hospital stay and details of subgroup analysis were extracted. Meta-analyses were made using the software Review Manager(RevM an version 5.10).RESULTS: Six RCTs with 390 patients meeting the inclusion criteria were included in the final analysis. Compared with intravenous infusion route, CRAI significantly shortened the duration of elevated urine amylase(MD=-2.40, 95% CI=-3.20,-1.60; P<0.00001) and the duration of abdominal pain(MD=-1.46, 95% CI=-1.94,-0.98; P<0.00001), decreased the incidence of complication(RR=0.35, 95% CI=0.15, 0.81; P=0.01) and overall mortality(RR=0.25, 95% CI=0.08, 0.78;P=0.02), shortened the duration of hospital stay(MD=-10.36, 95% CI=-17.05,-3.68; P=0.002), and increased the curative rate(RR=1.66, 95% CI=1.13, 2.46; P=0.01). No mortality and catheter-related infections due to CRAI administration was reported in these studies. | 7 | Amplitude-Integrated Electroencephalography Predicts Outcome in Patients with Coma After Acute Brain Injury显示文摘Prognostication of coma patients after brain injury is important, yet challenging. In this study, we evaluated the predictive value of amplitude-integrated electroencephalography(a EEG) for neurological outcomes in coma patients. From January 2013 to January 2016, 128 coma patients after acute brain injury were prospectively enrolled and monitored with a EEG. The 6-month neurological outcome was evaluated using the Cerebral Performance Category Scale. a EEG monitoring commenced at a median of 7.5 days after coma onset. Continuous normal voltage predicted a good 6-month neurological outcome with a sensitivity of 93.6% and specificity of 85.2%. In contrast, continuous extremely low voltage, burst-suppression, or a flat tracing was correlated with poor 6-month neurological outcome with a sensitivity of 76.5% and specificity of 100%. | 11 | Trabecular-like Ti-6Al-4V scaffolds for orthopedic: fabrication by selective laser melting and in vitro biocompatibility显示文摘Porous metal scaffolds play an important role in the orthopedic field, due to their wide applications in prostheses implantation. Some previous studies showed that the scaffolds with trabecular bone structure reconstructed via computed tomography had satisfactory biocompatibility. However, the reverse modeling scaffolds were inflexible for customized design. Therefore, a top-down designing biomimetic bone scaffold with favorable mechanical performances and cytocompatibility is urgently demanded for orthopedic implants. An emerging additive manufacturing technique, selective laser melting, was employed to fabricate the trabecular-like porous Ti-6Al-4 V scaffolds with varying irregularities(0.05-0.5) and porosities(48.83%–74.28%) designed through a novel Voronoi-Tessellation based method. Micro-computed tomography and scanning electron microscopy were used to characterize the scaffolds’ morphology.Quasi-static compression tests were performed to evaluate the scaffolds’ mechanical properties. The MG63 cells culture in vitro experiments, including adhesion, proliferation, and differentiation, were conducted to study the cytocompatibility of scaffolds. Compressive tests of scaffolds revealed an apparent elastic modulus range of 1.93–5.24 GPa and an ultimate strength ranging within 44.9–237.5 MPa, which were influenced by irregularity and porosity, and improved by heat treatment. Furthermore, the in vitro assay suggested that the original surface of the SLM-fabricated scaffolds was favorable for osteoblasts adhesion and migration because of micro scale pores and ravines. The trabecular-like porous scaffolds with full irregularity and higher porosity exhibited enhanced cells proliferation and osteoblast differentiation at earlier time, due to their preferable combination of small and large pores with various shapes. | 12 | Generalized mixed finite element method for 3D elasticity problems显示文摘Without applying any stable element techniques in the mixed methods, two simple generalized mixed element(GME) formulations were derived by combining the minimum potential energy principle and Hellinger–Reissner(H–R) variational principle. The main features of the GME formulations are that the common C0-continuous polynomial shape functions for displacement methods are used to express both displacement and stress variables, and the coefficient matrix of these formulations is not only automatically symmetric but also invertible. Hence, the numerical results of the generalized mixed methods based on the GME formulations are stable. Displacement as well as stress results can be obtained directly from the algebraic system for finite element analysis after introducing stress and displacement boundary conditions simultaneously. Numerical examples show that displacement and stress results retain the same accuracy. The results of the noncompatible generalized mixed method proposed herein are more accurate than those of the standard noncompatible displacement method. | 17 | Geochronology and Geochemical Characteristics of the Early Mesozoic Tangquan Pluton in Southwestern Fujian and Its Tectonic Implications显示文摘The Tangquan granodioritic pluton in Datian County, southwestern Fujian, China, which extends in a NE direction with an exposed area of about 130 km2, used to be considered a product of Early Cretaceous magmatism. The present study suggests for the first time that the pluton was formed in the Early Jurassic by using multiple methods for isotopic dating, which give zircon U-Pb ages of 186.8 Ma and 179.0 Ma, Rb-Sr isochron age of 162.02(4.5 Ma, and biotite 40Ar/39Ar plateau age of 158.1(0.7 Ma. The cooling rate for the pluton was relatively low (4.76°C/Ma) during the early stage (183(162 Ma) because of the compressional environment. It was emplaced in a higher cooling rate (50°C/Ma) in an extensional environment during the later stage (162(158 Ma). The granodiorites are metaluminous(peraluminous, relatively enriched in Na2O and depleted in K2O, and characteristic of I-type granites of crust-mantle mixed sources. They are moderately enriched in Rb, Th, Hf and LREE, and depleted in Ti, Nb, Ta and Sr, and have geochemical features of cal-alkaline rocks of an island arc or active continental margin. Their (87Sr/86Sr)i ratio varies from 0.70769 to 0.70822, εNd(t) from -9.68 to -10.07 and TDM from 1.52 to 1.55 Ga. | 19 | Clinical characteristics and current management of hepatitis B and C in China显示文摘AIM: To describe a population of outpatients in Chinainfected by hepatitis B virus(HBV) and/or hepatitis C virus(HCV), and assess their current management status.METHODS: A multicenter, cross-sectional study of HBV- and/or HCV-infected patients was conducted from August to November, 2011 in western China. Patients ≥ 18 years of age with HBV and/or HCV infections who visited outpatient departments at 10 hospitals were evaluated, whether treated or not. Data were collected on the day of visit from medical records and patient interviews.RESULTS: A total 4010 outpatients were analyzed, including 2562 HBV-infected and 1406 HCV-infected and 42 HBV/HCV co-infected patients. The median duration of documented infection was 7.5 years in HBV-infected and 1.8 years in HCV-infected patients. Cirrhosis was the most frequent hepatic complication(12.2%), appearing in one-third of patients within 3 years prior to or at diagnosis. The HCV genotype was determined in only 10% of HCV-infected patients. Biopsy data were only available for 54 patients(1.3%). Antiviral medications had been received by 58.2% of patients with HBV infection and 66.6% with HCV infection. Nucleos(t)ide analogs were the major antiviral medications prescribed for HBV-infected patients(most commonly adefovir dipivoxil and lamivudine). Ribavirin + pegylated interferon was prescribed for two-thirds of HCV-infected patients. In the previous 12 mo, around one-fifth patients had been hospitalized due to HBV or HCV infection.CONCLUSION: This observational, real-life study has identified some gaps between clinical practice and guideline recommendations in China. | 20 | One hour of pilocarpine-induced status epilepticus is sufficient to develop chronic epilepsy in mice, and is associated with mossy fiber sprouting but not neuronal death显示文摘Determining the minimal duration of status epilepticus (SE) that leads to the development of subsequent spontaneous seizures (i.e., epilepsy) is important, because it provides a critical time-window for seizure intervention and epilepsy prevention. In the present study, male ICR (Imprinting Control Region) mice were injected with pilocarpine to induce acute seizures. SE was terminated by diazepam at 10 min, 30 min, 1 h, 2 h and 4 h after seizure onset. Spontaneous seizures occurred in the 1, 2 and 4 h SE groups, and the seizure frequency increased with the prolongation of SE. Similarly, the Morris water maze revealed that the escape latency was significantly increased and the number of target quadrant crossings was markedly decreased in the 1, 2 and 4 h SE groups. Robust mossy fiber sprouting was observed in these groups, but not in the 10 or 30 min group. In contrast, Fluoro-Jade B staining revealed significant cell death only in the 4 h SE group. The incidence and frequency of spontaneous seizures were correlated with Timm score (P = 0.004) and escape latency (P = 0.004).
