| 1 | Anterior resection for rectal carcinoma-risk factors for anastomotic leaks and strictures显示文摘AIM:To determine the incidence and factors responsible for anastomotic leaks and stricture following anterior resection(AR)and its subsequent management.METHODS:Retrospective analysis of data from 108 patients with rectal carcinoma who underwent AR or low anterior resection(LAR)to identify the various preoperative,operative,and post operative factors that might have influence on anastomotic leaks and strictures.RESULTS:There were 68 males and 40 females with an average of 47 years(range 21-75 years).The median distance of the tumor from the anal verge was 8 cm(range 3-15 cm).Sixty(55.6%)patients underwent handsewn anastomosis and 48(44.4%)were stapled.The median operating time was 3.5 h(range2.0-7.5 h).Sixteen(14.6%)patients had an anastomotic leak.Among these,11 patients required reexploration and five were managed expectantly.The anastomotic leak rate was similar in patients with and without diverting stoma(8/60,13.4%with stoma and 8/48;16.7%without stoma).In 15(13.9%)patients,resection margins were positive for malignancy.Ninteen(17.6%)patients developed anastomotic strictures at a median duration of 8 mo(range 3-20 mo).Among these,15 patients were successfully managed with per-anal dilatation.On multivariate analysis,advance age(>60 years)was the only risk factor for anastomotic leak(P=0.004).On the other hand,anastomotic leak(P=0.00),mucin positive tumor(P =0.021),and lower rectal growth(P=0.011)were found as risk factors for the development of an anastomotic stricture.CONCLUSION:Advance age is a risk factor for an anastomotic leak.An anastomotic leak,a mucin-secreting tumor,and lower rectal growth predispose patients to develop anastomotic strictures. | Ashok Kumar Ram Daga Paari Vijayaragavan Anand Prakash Rajneesh Kumar Singh Anu Behari Vinay K Kapoor Rajan Saxena | 2011 | World Journal of Gastroenterology2011,17,11: | 28 |
| 2 | Targeted therapy of gastrointestinal stromal tumours显示文摘Gastrointestinal stromal tumours(GISTs) are mesen-chymal neoplasms originating in the gastrointestinal tract, usually in the stomach or the small intestine, and rarely elsewhere in the abdomen. The malignant potential of GISTs is variable ranging from small lesions with a benign behaviour to fatal sarcomas. The majo-rity of the tumours stain positively for the CD-117(KIT) and discovered on GIST-1(DOG-1 or anoctamin 1) expression, and they are characterized by the presence of a driver kinase-activating mutation in either KIT or platelet-derived growth factor receptor α. Although surgery is the primary modality of treatment, almost half of the patients have disease recurrence following surgery, which highlights the need for an effective adjuvant therapy. Traditionally, GISTs are considered chemotherapy and radiotherapy resistant. With the advent of targeted therapy(tyrosine kinase inhibitors), there has been a paradigm shift in the management of GISTs in the last decade. | 5 | Proximate Composition,Mineral Content and Fatty Acids Analyses of Aromatic and Non-Aromatic Indian Rice显示文摘Awareness on nutritive value and health benefits of rice is of vital importance in order to increase the consumption of rice in daily diet of the human beings. In this study, a total of six aromatic and two non-aromatic rice accessions grown in India were analysed for their nutritional quality attributes including proximate composition, mineral contents and fatty acids. Data with three replications were used to measure Pearson's simple correlation co-efficient in order to establish the relationship among various nutritional quality attributes. The result on proximate composition showed that Govind Bhog had the highest moisture(13.57%) and fat(0.92%) content, which signifies its tasty attribute. Badshah Bhog exhibited the highest fibre content(0.85%), carbohydrate content(82.70%) and food energy(365.23 k Cal per 100 g). Among the minerals, the higher Ca(98.75 mg/kg), Zn(17.00 mg/kg) and Fe(31.50 mg/kg) were in Gopal Bhog, whereas the highest Na(68.85 mg/kg) was in Badshah Bhog, the highest K(500.00 mg/kg) was in Swetganga, Khushboo and Sarbati. The highest contents of unsaturated fatty acids viz. oleic acid(49.14%), linoleic acid(46.99%) and linolenic acid(1.27%) were found in Sarbati, whereas the highest content of saturated fatty acids viz. myristic acid(4.60%) and palmitic acid(31.91%) were found in Govind Bhog and stearic acid(6.47%) in Todal. The identified aromatic rice accessions Gopal Bhog, Govind Bhog and Badshah Bhog and non-aromatic rice accession Sarbati were found nutritionally superior among all eight tested accessions. | 6 | Remediation of nutrient-rich waters using the terrestrial plant, Pandanus amaryllifolius Roxb.显示文摘Effective control of eutrophication is generally established through the reduction of nutrient loading into waterways and water bodies. An economically viable and ecologically sustainable approach to nutrient pollution control could involve the integration of retention ponds, wetlands and greenways into water management systems. Plants not only play an invaluable role in the assimilation and removal of nutrients, but they also support fauna richness and can be aesthetically pleasing. Pandanus amaryllifolius, a tropical terrestrial plant, was found to establish well in hydrophytic conditions and was highly effective in remediating high nutrient levels in an aquatic environment showing 100% removal of NO-3-N up to 200 mg/L in 14 days. Phosphate uptake by the plant was less efficient with 64% of the PO-4-P removed at the maximum concentration of 100 mg/L at the end of 6 weeks. With its high NO-3-N and PO3-4-P removal efficiency, P. amaryllifolius depleted the nutrient-rich media and markedly contained the natural colonization of algae. The impediment of algal growth led to improvements in the water quality with significant decreases in turbidity, pH and electrical conductivity. In addition, the plants did not show stress symptoms when grown in high nutrient levels as shown by the changes in their biomass, total soluble proteins and chlorophyll accumulation as well as photochemical efficiency. | 7 | A Modified Plate Assay for Rapid Screening of Potassium-Solubilizing Bacteria显示文摘The utility of microorganisms for solubilizing the unavailable forms of potassium(K) from soil has led to renewed interest in fabrication of rapid and sensitive plate assays for their isolation and screening. The present study developed a modified plate assay and compared it with previously reported methods for the isolation and screening of K-solubilizing bacteria. The newly developed plate assay is based on improved visualization of halo zone formation around the colonies on agar plates, through inclusion of an acid-base indicator dye, bromothymol blue(BTB), to modify the previously reported Aleksandrov medium. The halo zone exhibited a significant correlation(R = 0.939) with K released in liquid medium. The visualization of potential K solubilizers was improved using this method, which would help in detection of weak/non-acid producers based on secretion of organic acids in the medium. Organic acids in plate diffuse radially and form halo zones in response to reaction with the acid-base indicator dye BTB. Furthermore, K solubilization on plates with this method can be observed within 48–72 h, against the incubation time of 4–5 d needed in the earlier method. | 8 | Co-gasification of coal and biomass an emerging clean energy technology: Status and prospects of development in Indian context显示文摘Co-gasification of coal and biomass is emerging as potential clean fuel technology to achieve high thermodynamic efficiency with relatively low CO2 emission. The coal and biomass have been exclusively gasified more than a century to obtain gas–liquid fuels and the production of chemicals. Co-gasification has higher efficiency than the solitary coal gasification because the cellulose, hemicellulose and lignin content of biomass help to ignite and enhance the rate of gasification. It is suggested that the extensive research on carbon reactivity pattern, heat release, reaction kinetics, etc. may support to reduce the uncertainties in the co-gasification performance of coal and biomass blends, particularly in India. The prospects of co-gasification technology in Indian context have been discussed considering the abundance of varieties of coal and biomass. The suitability of existing gasifier procedures and their limitations with operating parameters like temperature, residence time, density optimisation, feed rate, agglomeration intensity, the tar formation and techno-economics involved are described. | 9 | Assessment of tumor markers CA 19-9,CEA,CA 125,and CA 242 for the early diagnosis and prognosis prediction of gallbladder cancer显示文摘BACKGROUND Gallbladder cancer(GBC)is one of the leading and aggressive cancers in this region of India.It is very difficult to diagnose in the early stage,as it lacks typical early signs and symptoms;thus,the diagnosis is often in the advanced stage,which ultimately leads to a poor 5-year survival outcome.Tumor markers including carbohydrate antigen 19-9(CA 19-9),carcinoembryonic antigen(CEA),CA 125,CA 242,and alpha fetoprotein are used as indicators in the diagnosis and prognosis of GBC.AIM To compare tumor marker levels between GBC and benign GB diseases(GBDs)and to assess the combined use of tumor markers to increase the diagnostic accuracy for GBC.METHODS Patients of either sex aged≥18 years,with suspected GBC(GB polyp,irregular thick GB wall,GB mass,porcelain GB)on the basis of radiological imaging were included in this study.GB wall thickness using ultrasonography and tumor markers CEA,CA 125,CA 19-9,and CA 242 in all patients were recorded.All cases after surgical intervention were divided into two groups,GBC and benign GBD,according to histopathological examination findings.The cases were followed up and clinical findings,radiological findings,and levels of tumor markers were assessed.RESULTS A total of 200 patients were included in this study,of whom 80 patients had GBC and 120 patients had benign GBD.The median(interquartile range)age was 52.0(41.0-60.0)years and the majority of patients(132,66.0%)were women.Tumor markers including CA 19-9,CA 125,CEA,and CA 242 were significantly elevated in patients with GBC(P<0.001).There was a significant reduction in tumor markers at 3 and 6 mo from baseline(P<0.001).The mean survival of patients with normal and elevated levels of tumor markers CA 125,CA 19-9,and CEA was comparable;however lymph node metastasis and CA 242 expression level were independent prognostic factors.CONCLUSION Serum levels of tumor markers including CA 19-9,CA 125,CEA,and CA 242 were significantly associated with GBC.However,no significant association was observed between the presence of elevated levels of any tumor marker with respect to survival.Tumor marker assessment during follow-up may represent a treatment response. | Seema Rani Sinha Prem Prakash Rakesh Kumar Singh Dinesh Kumar Sinha | 2022 | World Journal of Gastrointestinal Surgery2022,14,11: | 5 |

| 12 | Role of Joshi's external stabilization system with percutaneous screw fixation in high-energy tibial condylar fractures associated with severe soft tissue injuries显示文摘Purpose: The treatment of high-energy tibial condylar fractures which are associated with severe soft tissue injuries remains contentious and challenging. In this study, we assessed the results of Joshi's external stabilization system (JESS) by using the principle of ligamentotaxis and percutaneous screw fixation for managing high-energy tibial condylar fractures associated with severe soft tissue injuries. Methods: Between June 2008 and June 2010, 25 consecutive patients who were 17-71 years (mean, 39.7), underwent the JESS fixation for high-energy tibial condylar fractures associated with severe soft tissue injuries. Out of 25 patients, 2 were lost during follow-up and in 1 case early removal of frame was done, leaving 22 cases for final follow-up. Among them, 11 had poor skin condition with abrasions and blisters and 2 were open injuries (Gustilo-Anderson grade I & 11). The injury mechanisms were motor vehicle accidents (n - 19), fall from a height (n = 2) and assault (n - 1). The fractures were classified according to Schatzker classification system. Results: There were 7 type-V, 14 type-Vl and 1 type-IV Schatzker's tibial plateau fractures. The average interval between the injury and surgery was 6.8 days (range 2-13). The average hospital stay was 13 days (range, 7-22). The average interval between the surgery and full weight bearing was 13.6 weeks (range 11-20). The average range of knee flexion was 121° (range 105°-135°). The normal extension of the knee was observed in 20 patients, and an extensor lag of 5°-8° was noted in 2 patients. The complications included superficial pin tract infections (n 4) with no knee stiffness. Conclusion: JESS with lag screw fixation combines the benefit of traction, external fixation, and limited internal fixation, at the same time as allowing the ease of access to the soft tissue for wound checks, pin care, dressing changes, measurement of compartment pressure, and the monitoring of the neurovascular status. | 13 | Role of angiotensin converting enzyme and angiotensinogen gene polymorphisms in angiotensin converting enzyme inhibitor-mediated antiproteinuric action in type 2 diabetic nephropathy patients显示文摘AIM To investigate the role of genetic variants of angiotensin converting enzyme(ACE) and angiotensinogen(AGT) genes in the antiproteinuric efficacy of ACE inhibitor therapy in diabetic nephropathy(DN) patients.METHODS In the present study, 270 type 2 diabetes mellitus patients with nephropathy were enrolled and treated with ACE inhibitor(ramipril) and followed at 6 mo for renal function and albumin excretion by estimating serum creatinine, end stage renal disease, and albumin/creatinine ratio(ACR) in urine. Genotyping of ACE I/D and AGT M235 T polymorphisms were performed by using primer specific polymerase chain reaction(PCR) and PCR-RFLP techniques, respectively. RESULTS Forty-eight percent of DN patients(responders) benefited with respect to proteinuria from ACE inhibitor therapy at 6 mo follow-up. A significant reduction in ACR was observed after 6 mo treatment with ACE inhibitor irrespective of whether DN patients were micro-albuminuric(≥ 30 and < 300 mg/g creatinine) or macro-albuminuric(≥ 300 mg/g creatinine) at the time of enrollment. However, macro-albuminuric patients(55%) showed better response to therapy. A reduction in urinary ACR was found independent of genotypes of ACE I/D and AGT M235 T polymorphisms although macro-albuminuric patients having TT genotype showed statistically insignificant increased response(72%). | 15 | Antidiabetic,hypolipidemic and histopathological analysis of Dillenia indica (L.) leaves extract on alloxan induced diabetic rats显示文摘Objective:To investigate antidiabetic,hypolipidemic histopathological analysis of Dillenia indica(D.indica) methanolic leaves(DIME) extract in alloxan induced diabetic rat by administering oral doses(250 and 500 mg/kg body weight).Methods:Blood glucose levels were measured using blood glucose test strips with elegance glucometer on weekly intervals till the end of study(i.e.3 weeks).Other parameters e.g.liver profile,renal profile and total lipid levels were determined in normal and alloxan induced diabetic rats after oral administration of the extract for 21 days.Histopathological changes in diabetic rat organs(pancreas,liver and kidney) were also observed after extract treatment Results:Daily oral administration DIME(250 and 500 mg/kg body weight) and glibenclamide(10 mg/kg) showed beneficial effects on blood glucose level (P<0.001) as well as improving kidney,liver functions and hyperlipidaemia due to diabetes. The extract treatment also showed to enhanced serum insulin level and body weight of diabetic rats as compared to diabetic control group.Furthermore,the extract has a favorable effect on the histopathological changes of the pancreas,liver and kidney in alloxan induced diabetes. | 17 | Antidiabetic and hypolipidemic activities of Kigelia pinnata flowers extract in streptozotocin induced diabetic rats显示文摘Objective:To evaluate antidiabetic and hypolipidemic activities of Kigelia pinnata rnethanolic(lowers extract in slreplozolocin(STZ) induced diabetic W islar rat.Methods.Rats were made diabetic by a single dose of STZ at 60 mg/kg body weight i.p.The blood glucose level was checked before and 72 h after STZ injection to confirm the development ot diabetes.The flower extract and glibenclamide were administered orally at the doses of 250 and 500 mg/kg body weight for 21 days.Results:Daily oral treatment with the extract and standard drug for 21 days significantly reduced blood glucose,serum cholesterol and triglycerides levels.High density lipoprotein-cholesterol level was found to be improved(P<0.01) as compared to diabetic control group.Conclusions:It is concluded that Kigellia pinnata flowers extract have significant antidiabetic and hypolipidemic effect. | Kumar S Kumar V Prakash OM | 2012 | Asian Pacific Journal of Tropical Biomedicine2012,2,7: | 4 |
| 18 | Antidiabetic and anti-lipemic effects of Cassia siamea leaves extract in streptozotocin induced diabetic rats显示文摘Objective:To investigate the antidiabetic and anti-lipemic effects of Cassia siamea methanolic leaves extract.Methods:The antidiabetic study was performed by measuring blood glucose level with elegance glucometer at weekly intervals i.e.0,7,14 and 21 in normal and streptozotocin induced diabetic rats.Total cholesterol,triglyceride and HDL-cholesterol were determined in normal and streptozotocin induces diabetic rats by autoanalyser.Glibenclamide was used as a reference drug at a dose of 10 mg/kg.Results:After the oral administration of extracts at doses of 250 mg/kg and 500 mg/kg for three weeks,blood glucose levels and body weights were significantly improved(P<0.01).Daily oral treatment with the extract also resulted in significantly reduction of serum cholesterol and triglycerides.HDL-cholesterol level was found to be improved to(P<0.01).Conclusions:The Cassia siamea leaf extract is useful in controlling blood glucose level as well as improving lipid metabolism and body weight in rats with induced diabetes. | S Kumar V Kumar Om Prakash | 2010 | Asian Pacific Journal of Tropical Medicine2010,3,11: | 3 |
