| 2 | Takotsubo cardiomyopathy:Pathophysiology and role of cardiac biomarkers in differential diagnosis显示文摘Takotsubo cardiomyopathy(TC) is characterized by reversible ventricular dysfunction, not limited to the distribution of an epicardial coronary artery. A disease primarily afflicting post-menopausal women, it is frequently mistaken for acute anterior wall myocardial infarction. Alternatively called Stress Cardiomyopathy, physical or emotional triggers are identified in only three fourths of TC patients. Long considered a benign condition, recent findings suggest poor short term prognosis similar to acute coronary syndrome(ACS). Despite the widely recognized pathophysiological role of catecholamine excess, its diagnostic role is uncertain. TC is suspected based on typical wall motion abnormalities in ventriculogram or echocardiogram. Several additional electrocardiographic, laboratory and imaging parameters have been studied with the goal of clinical diagnosis of TC. While several clinical clues differentiate it from ACS, a clinical diagnosis is often elusive leading to avoidable cardiac catheterizations. Natriuretic peptides(NPs), a family of peptide hormones released primarily in response to myocardial stretch, play a significant role in pathophysiology, diagnosis as well as treatment of congestive heart failure. TC with its prominent ventricular dysfunction is associated with a significant elevation of NPs. NPs are elevated in ACS as well but the degree of elevation is typically lesser than in TC. Markers of myocardial injury such as troponin are usually elevated to a higher degree in ACS than in TC. | 4 | Is current training in basic and advanced cardiac life support(BLS & ACLS) effective? A study of BLS & ACLS knowledge amongst healthcare professionals of North-Kerala显示文摘BACKGROUND:Healthcare professionals are expected to have knowledge of current basic and advanced cardiac life support(BLS/ACLS) guidelines to revive unresponsive patients.METHODS:Across-sectional study was conducted to evaluate the current practices and knowledge of BLS/ACLS principles among healthcare professionals of North-Kerala using pretested self-administered structured questionnaire.Answers were validated in accordance with American Heart Association's BLS/ACLS teaching manual and the results were analysed.RESULTS:Among 461 healthcare professionals,141(30.6%) were practicing physicians,268(58.1%) were nurses and 52(11.3%) supporting staff.The maximum achievable score was 20(BLS15/ACLS 5).The mean score amongst all healthcare professionals was 8.9±4.7.The mean score among physicians,nurses and support staff were 8.6±3.4,9±3.6 and 9±3.3 respectively.The majority of healthcare professionals scored <50%(237,51.4%);204(44.3%) scored 51%-80%and 20(4.34%)scored >80%.Mean scores decreased with age,male sex and across occupation.Nurses who underwent BLS/ACLS training previously had significantly higher mean scores(10.2±3.4) than untrained(8.2±3.6,P=0.001).Physicians with <5 years experience(P=0.002) and nurses in the private sector(P=0.003)had significantly higher scores.One hundred and sixty three(35.3%) healthcare professionals knew the correct airway opening manoeuvres like head tilt,chin lift and jaw thrust.Only 54(11.7%) respondents were aware that atropine is not used in ACLS for cardiac arrest resuscitation and 79(17.1%) correctly opted ventricular fibrillation and pulseless ventricular tachycardia as shockable rhythms.The majority of healthcare professionals(356,77.2%) suggested that BLS/ACLS be included in academic curriculum.CONCLUSION:Inadequate knowledge of BLS/ACLS principles amongst healthcare professionals,especially physicians,illuminate lacunae in existing training systems and merit urgent redressal. | Madavan Nambiar Nisanth Menon Nedungalaparambil Ottapura Prabhakaran Aslesh | 2016 | World Journal of Emergency Medicine2016,7,4: | 7 |

| 8 | An innovative approach to minimize excess sludge production in sewage treatment using integrated bioreactors显示文摘The present investigation deals with an application of integrated sequential oxic and anoxic bioreactor(SOABR) and fluidized immobilized cell carbon oxidation(FICCO) reactor for the treatment of domestic wastewater with minimum sludge generation. The performance of integrated SOABR-FICCO system was evaluated on treating the domestic wastewater at hydraulic retention time(HRT) of 3 hr and 6 hr for 120 days at organic loading rate(OLR)of 191 ± 31 mg/(L·hr). The influent wastewater was characterized by chemical oxygen demand(COD) 573 ± 93 mg/L; biochemical oxygen demand(BOD5) 197 ± 35 mg/L and total suspended solids(TSS) 450 ± 136 mg/L. The integrated SOABR-FICCO reactors have established a significant removal of COD by 94% ± 1%, BOD5 by 95% ± 0.6% and TSS by 95% ± 4% with treated domestic wastewater characteristics COD 33 ± 5 mg/L; BOD59 ± 0.8 mg/L and TSS 17 ± 9 mg/L under continuous mode of operation for 120 days. The mass of dry sludge generated from SOABR-FICCO system was 22.9 g/m^3. The sludge volume index of sludge formed in the SOABR reactor was 32 mL/g and in FICCO reactor it was 46 mL/g. The sludge formed in SOABR and FICCO reactor was characterized by TGA, DSC and SEM analysis. | 9 | Characterization and optimization of EPSproducing and diesel oil-degrading Ochrobactrum anthropi MP3 isolated from refinery wastewater显示文摘Petroleum refinery wastewater(PRW) containing hydrocarbon is highly toxic to the environment and the surrounding ecosystem. Proper treatment of the PRW effluent is necessary to remove the pollutants before discharge. Bioremediation is considered to be a promising approach as it is ecofriendly and efficient. The exopolysaccharide(EPS) produced by the O. anthropi acts as a bioemulsifier and showed the highest emulsification activity of 60% on diesel. An EPS yield of about 0.42 g/L was obtained under optimized conditions. The carbohydrate and protein content of the EPS was found to be 71.1% and 19.7% respectively, showing the glycoprotein nature. The structural properties of EPS were analyzed by FT-IR and 1H NMR. | 10 | White matter hyperintensity progression is associated with incident probable dementia or mild cognitive impairment显示文摘Background White matter hyperintensity(WMH)on brain MRI is associated with developing dementia or mild cognitive impairment(MCI),but WMH progression over time has not been fully investigated as an independent risk factor.Methods We performed a post hoc analysis of the Systolic Blood Pressure Intervention Trial-Memory and Cognition in Decreased Hypertension(SPRINT MIND)trial.The primary outcome was incident probable dementia or MCI(dementia/MCI)before the follow-up MRI at 48 months from enrolment.The primary predictor was WMH progression,defined as the Z score difference between the follow-up and baseline WMH volumes.The secondary predictor was a binary WMH progression threshold(≥1.4 mL vs<1.4 mL).Results Among the 433 included patients,33(7.6%)developed dementia/MCI.There were 156(36.0%)patients who met the WMH progression threshold of≥1.4 mL,in whom the rate of dementia/MCI was 12.8%(20/156)vs 4.7%(13/277)of patients with<1.4 mL WMH progression(p=0.002).In multivariable logistic regression,the Z score of WMH progression was associated with dementia/MCI(OR 1.51,95%CI 1.12 to 2.04,p=0.007)as was the WMH progression threshold of≥1.4 mL(OR 2.89,95%CI 1.23 to 6.81,p=0.015).Conclusions In this post hoc analysis of SPRINT MIND,WMH progression over 48 months was associated with the development of probable dementia or MCI. | Adam de Havenon Kevin N Sheth Sharon D Yeatts Tanya N Turan Shyam Prabhakaran | 2022 | Stroke & Vascular Neurology2022,7,4: | 2 |
