| 1 | Role of CD56-expressing immature biliary epithelial cells in biliary atresia显示文摘AIM: To analyze the clinical and pathological parameters and expression of the neural cell adhesion molecule(CD56) in patients with biliary atresia(BA).METHODS: Established clinical laboratory markers of hepatic function, including enzyme activity, protein synthesis, and bilirubin metabolism, were evaluated in patients with BA and compared with those in patients with choledochal cysts and neonatal hepatitis. Pathological changes in tissue morphology and fibrosis were examined by histological and tissue collagen staining. Immunohistochemical staining for the biliary epithelial cell markers CD56 and CK19 together with the Notch signaling related molecules Notch1 and Notch2 was performed in the context of alterations in the structure of intrahepatic biliary ducts.RESULTS: Differences in some clinical laboratoryparameters among the three diseases examined were observed, but they did not correlate with the pathological classification of fibrosis in BA. Immunohistochemical staining showed the presence of CD56-positive immature bile ducts in most patients(74.5%) with BA but not in patients with choledochal cysts or neonatal hepatitis. The number of CD56-expressing cells correlated with disease severity, with more positive cells present in the later stages of liver damage(81.8% vs 18.2%). Furthermore, bile plugs were mainly found in CD56-positive immature biliary ducts. Notch signaling was a key regulatory pathway in biliary duct formation and played a role in tissue fibrosis. Notch1 was co-expressed in CD56-positive cells, whereas Notch2 was found exclusively in blood vessels in the portal area of patients with BA. | 2 | Split liver transplantation: a reliable approach to expand donor pool显示文摘Orthotopic liver transplantation as a successful treatment of end-stage liver disease is hampered by a persistent lack of cadaveric organs. Split liver transplantation, which was first successfully performed by Medical School of Hannover in 1988, has become a mature surgical technique to expand the donor pool. Between 1993 and 1999, split liver transplantation activities have increased in Europe from 1. 2% to 10. 4% in all performed liver transplantations. Current data have strongly supported that the survival rate of patients after split liver transplantation is not significantly different from that of patients after whole-size orthotopic liver transplantation. The most important step of donor graft selection is surgeon’ s observation judged by the experience of individual transplant center. The paper aims to provide the guideline of donor selection, hepatic graft splitting, and recipient management as well. DATA SOURCES: Medical School of Hannover has accumulated plentiful experience of split liver transplantation for more than 10 cases ever since 1998. Besides that, we also reviewed a variety of literatures from other famous European and American centers specialized in this field for many years. RESULTS:According to our experience combined with the view points of others, the donor should meet the following criteria as well: (1) age less than 50 years; (2) hemodynamics stable; (3) ICU less than 5 days; (4) Na less than 170 mmol/L or better if less than 150 mmol/L. In 1996 and 1997, the Hamburg group and the UCLA group separately introduced a breakthrough technique performing split liver transplantation in situ. Evidently, the in situ technique has been limited by prolonged time of donor organ procurement , coordination with other organ procurement teams, and even extra burden on donor hospital. Some groups, therefore, have restored the ex situ or bench splitting technique , and fortunately the transplant outcomes of the ex situ technique are equivalent to those of the in situ one. Recently some new techniques have been introduced to split the liver for two adult patients, including the split-cava technique. CONCLUSIONS:It is clear that the most important factor for determining the prognosis of the patient is the time of receiving liver transplantation, not the type of liver transplantation. | 3 | Design and Simulation of Flow Field for Bone Tissue Engineering Sca old Based on Triply Periodic Minimal Surface显示文摘A novel method was proposed to design the structure of a bone tissue engineering scafold based on triply periodic minimal surface.In this method,reverse engineering software was used to reconstruct the surface from point cloud data.This method overcomes the limitations of commercially available software packages that prevent them from generating models with complex surfaces used for bone tissue engineering scafolds.Additionally,the fluid feld of the scafolds was simulated through a numerical method based on fnite volume and the cell proliferation performance was evaluated via an in vitro experiment.The cell proliferation and the mass flow evaluated in a bioreactor further verifed the flow feld simulated using computational fluid dynamics.The result of this study illustrates that the pressure value drops rapidly from 0.103 Pa to 0.011 Pa in the y-axis direction and the mass flow is unevenly distributed in the outlets.The mass flow in the side outlets is observed to be approximately 24.3 times higher thanthe bottom.Importantly,although the mean value of wall shear stress is signifcantly more than 0.05 Pa,there is stil a large area with a suitable shear stress below 0.05 Pa where most cells can proliferate well.The result shows that th inlet velocity 0.0075 m/s is suitable for cell proliferation in the scafold.This study provides an insight into the design analysis,and in vitro experiment of a bone tissue engineering scafold. | Zhen Wang Chuanzhen Huang Jun Wang Peng Wang Shisheng Bi Ch Asad Abbas | 2019 | Chinese Journal of Mechanical Engineering2019，32，2： | 2 |
