1Euglycemic diabetic ketoacidosis:A missed diagnosis显示文摘Euglycemic diabetic ketoacidosis(DKA)is an acute life-threatening metabolic emergency characterized by ketoacidosis and relatively lower blood glucose(less than 11 mmol/L).The absence of hyperglycemia is a conundrum for physicians in the emergency department and intensive care units;it may delay diagnosis and treatment causing worse outcomes.Euglycemic DKA is an uncommon diagnosis but can occur in patients with type 1 or type 2 diabetes mellitus.With the addition of sodium/glucose cotransporter-2 inhibitors in diabetes mellitus management,euglycemic DKA incidence has increased.The other causes of euglycemic DKA include pregnancy,fasting,bariatric surgery,gastroparesis,insulin pump failure,cocaine intoxication,chronic liver disease and glycogen storage disease.The pathophysiology of euglycemic DKA involves a relative or absolute carbohydrate deficit,milder degree of insulin deficiency or resistance and increased glucagon/insulin ratio.Euglycemic DKA is a diagnosis of exclusion and should be considered in the differential diagnosis of a sick patient with a history of diabetes mellitus despite lower blood glucose or absent urine ketones.The diagnostic workup includes arterial blood gas for metabolic acidosis,serum ketones and exclusion of other causes of high anion gap metabolic acidosis.Euglycemic DKA treatment is on the same principles as for DKA with correction of dehydration,electrolytes deficit and insulin replacement.The dextrosecontaining fluids should accompany intravenous insulin to correct metabolic acidosis,ketonemia and to avoid hypoglycemia.Prashant Nasa Sandeep Chaudhary Pavan Kumar Shrivastava Aanchal Singh 2021World Journal of Diabetes2021,12,5:11
2Development and validation of a stability indicating RP-HPLC method for the determination of Rufinamide显示文摘A stability-indicating RP-HPLC method was developed and validated for the determination of Rufinamide in tablet dosage forms using C 18 column (250 mm 4.6 mm, 5 mm) with mobile phase consisting of water-acetonitrile (40:60, v/v) with a flow rate of 0.8 mL/min (UV detection 215 nm). Linearity was observed over the concentration range 1.0-200 mg/mL (R 2 0.9997) with regression equation y 113190 xt63053. Rufinamide was subjected to stress conditions including acidic, alkaline, oxidation, photolysis and thermal degradation. Rufinamide is more sensitive towards acidic degradation. 4Neuroprotective effects of Alda-1 mitigate spinal cord injury in mice:involvement of Alda-1-induced ALDH2 activation-mediated suppression of reactive aldehyde mechanisms显示文摘Spinal cord injury(SCI)is associated with high production and excessive accumulation of pathological 4-hydroxy-trans-2-nonenal(4-HNE),a reactive aldehyde,formed by SCI-induced metabolic dysregulation of membrane lipids.Reactive aldehyde load causes redox alteration,neuroinflammation,neurodegeneration,pain-like behaviors,and locomotion deficits.Pharmacological scavenging of reactive aldehydes results in limited improved motor and sensory functions.In this study,we targeted the activity of mitochondrial enzyme aldehyde dehydrogenase 2(ALDH2)to detoxify 4-HNE for accelerated functional recovery and improved pain-like behavior in a male mouse model of contusion SCI.N-(1,3-benzodioxol-5-ylmethyl)-2,6-dichlorobenzamide(Alda-1),a selective activator of ALDH2,was used as a therapeutic tool to suppress the 4-HNE load.SCI was induced by an impactor at the T9–10 vertebral level.Injured animals were initially treated with Alda-1 at 2 hours after injury,followed by once-daily treatment with Alda-1 for 30 consecutive days.Locomotor function was evaluated by the Basso Mouse Scale,and pain-like behaviors were assessed by mechanical allodynia and thermal algesia.ALDH2 activity was measured by enzymatic assay.4-HNE protein adducts and enzyme/protein expression levels were determined by western blot analysis and histology/immunohistochemistry.SCI resulted in a sustained and prolonged overload of 4-HNE,which parallels with the decreased activity of ALDH2 and low functional recovery.Alda-1 treatment of SCI decreased 4-HNE load and enhanced the activity of ALDH2 in both the acute and the chronic phases of SCI.Furthermore,the treatment with Alda-1 reduced neuroinflammation,oxidative stress,and neuronal loss and increased adenosine 5′-triphosphate levels stimulated the neurorepair process and improved locomotor and sensory functions.Conclusively,the results provide evidence that enhancing the ALDH2 activity by Alda-1 treatment of SCI mice suppresses the 4-HNE load that attenuates neuroinflammation and neurodegeneration,promotes the neurorepair process,and improves functional outcomes.Consequently,we suggest that Alda-1 may have therapeutic potential for the treatment of human SCI.Animal procedures were approved by the Institutional Animal Care and Use Committee(IACUC)of MUSC(IACUC-2019-00864)on December 21,2019.Mushfiquddin Khan Fei Qiao Pavan Kumar S.M.Touhidul Islam Avtar K.Singh Jeseong Won Inderjit Singh 2022Neural Regeneration Research2022,17,1:3
5Preliminary study on modelling, fabrication by photo-chemical etching and in vivo testing of biodegradable magnesium AZ31 stents显示文摘Magnesium metal(Mg)is a promising material for stent applications due to its biocompatibility and ability to be resorbed by the body.Manufacturing of stents by laser cutting has become an industry standard.Our alternative approach uses photo-chemical etching to transfer a pattern of the stent onto a Mg sheet.In this study,we present three stages of creating and validating a stent prototype,which includes design and simulation using finite element analysis(FEA),followed by fabrication based on AZ31 alloy and,finally,in vivo testing in peripheral arteries of domestic pigs.Due to the preliminary character of this study,only six stents were implanted in two domestic farm pigs weighing 25-28 kg and they were evaluated after 28 days,with an interim follow-up on day 14.The left and right superficial femoral,the left iliac,and the right renal artery were selected for this study.The diameters of the stented artery segments were evaluated at the time of implantation,on day 14 and then,finally,on day 28,by quantitative vessel analysis(QVA)using fluoroscopic imaging.Optical Coherence Tomography(OCT)imaging displayed some malposition,breaks,stacking,and protrusion into the lumen at the proximal,distal,and mid-sections of the stented arteries.The stents degraded with time,but simultaneously became embedded in the intima.After 28 days,the animals were euthanized,and explanted vessels were fixed for micro-CT imaging and histology studies.Micro-CT imaging revealed stent morphological and volumetric changes due to the in-body degradation.An in vivo corrosion rate of 0.75 mm/year was obtained by the CT evaluation.The histology suggested no-life threatening effects,although moderate injury,inflammation,and endothelialization scores were observed.Bala Subramanya Pavan Kumar Kandala Guangqi Zhang Capucine LCorriveau Mark Paquin Madeleine Chagnon Dana Begun Vesselin Shanov 2021Bioactive Materials2021,6,6:2
6Systematic review and meta-analysis of the impact of deviations from a clinical pathway on outcomes following pancreatoduodenectomy显示文摘BACKGROUND Enhanced recovery after surgery is steadily gaining importance in patients undergoing pancreatic surgery,including pancreatoduodenectomy(PD).While clinical pathways targeting enhanced-recovery can achieve their intended outcome in reducing length of stay,compliance to these pathways,and their relevance is poorly understood.The aim of this systematic review was to assess the impact of deviations from/non-compliance to a clinical pathway on post-PD outcomes.AIM To assess the impact of deviations from/non-compliance to a clinical pathway on post-PD outcomes.METHODS A systematic review of major reference databases was undertaken,according to preferred reporting items for systematic reviews and meta-analysis guidelines,between January 2000 and November 2020 relating to compliance with clinical pathways and its impact on outcomes in patients undergoing PD.A meta-analysis was performed using fixed-effects or random-effects models.RESULTS Eleven studies including 1852 patients were identified.Median overall compliance to all components of the clinical pathway was 65.7%[interquartile range(IQR):62.7%-72.3%]with median compliance to post-operative parameters of the clinical pathway being 44%(IQR:34.5%-52.25%).Meta-analysis using a fixed-effects model showed that≥50%compliance to a clinical pathway predicted significantly fewer post-operative complications[pooled odds ratio(OR):9.46,95%confidence interval(CI):5.00-17.90;P<0.00001]and a significantly shorter length of hospital stay[pooled mean difference(MD):4.32,95%CI:-3.88 to-4.75;P<0.0001].At 100%compliance which was associated with significantly fewer post-operative complications(pooled OR:11.25,95%CI:4.71-26.84;P<0.00001)and shorter hospital stay(pooled MD of 4.66,95%CI:2.81-6.51;P<0.00001).CONCLUSION Compliance to post-PD clinical pathways remains low.Deviations are associated with an increased risk of complications and length of hospital stay.Understanding the relevance of deviations to clinical pathways post-PD presents pancreatic surgeons with opportunities to actively pursue an enhanced-recovery of their patients.Monish Karunakaran Pavan Kumar Jonnada Savio George Barreto 2021World Journal of Clinical Cases2021,9,13:2
7Metallurgical Behaviour and Carbon Diffusion in Buttering Deposits Prepared With and Without Buffer Layers显示文摘Use of a buttering deposit on ferritic steel in dissimilar metal weld(DMW) joint is a common practice in nuclear plants to connect pressure vessel components(ferritic steel) to pipelines(austenitic stainless steel).Carbon migration and metallurgical changes near fusion interface(ferritic steel–austenitic stainless steel) lead to a steeper gradient in material properties,and minimizing this gradient is the major challenge in the manufacturing of DMW joints.Inconel 82 is often deposited on ferritic steel material as buttering to reducing the gradient of physical and attaining material compatibility.Inconel 82/182 fillers are used to minimize the carbon migration,but the results are not truly adequate.In this paper,Ni–Fe alloy(chromium-free) has been used as the intermediate buffer layer in the weld buttering deposit to address the issue of carbon migration and subsequent metallurgical deterioration.The weld pads with and without buffer layers of Ni–Fe alloy have been investigated and compared in detail for metallurgical properties and carbon diffusivities.Ni–Fe buffer layer can significantly control the carbon migration which resists the metallurgical deterioration.It showed the better results in postweld heat treatment and thermally aged conditions.The buttering deposit with Ni–Fe buffer layer could be the better choice for DMW joints requirements.Dinesh W.Rathod Sunil Pandey Sivanandam Aravindan Pavan Kumar Singh 2017Acta Metallurgica Sinica(English Letters)2017,30,2:2
