| 1 | Pentoxifylline versus prednisolone for severe alcoholic hepatitis:A randomized controlled trial显示文摘AIM:To compare the efficacy of pentoxifylline and prednisolone in the treatment of severe alcoholic hepatitis,and to evaluate the role of different liver function scores in predicting prognosis.METHODS:Sixty-eight patients with severe alcoholic hepatitis(Maddrey score≥32)received pentoxifylline(n=34,groupⅠ)or prednisolone(n=34,groupⅡ) for 28 d in a randomized double-blind controlled study,and subsequently in an open study(with a tapering dose of prednisolone)for a total of 3 mo,and were followed up over a period of 12 mo.RESULTS:Twelve patients in groupⅡdied at the end of 3 mo in contrast to five patients in groupⅠ.The probability of dying at the end of 3 mo was higher in groupⅡas compared to groupⅠ(35.29%vs 14.71%,P=0.04;log rank test).Six patients in groupⅡ developed hepatorenal syndrome as compared to none in groupⅠ.Pentoxifylline was associated with a significantly lower model for end-stage liver disease(MELD)score at the end of 28 d of therapy(15.53±3.63 vs 17.78±4.56,P=0.04).Higher baseline Maddrey score was associated with increased mortality.CONCLUSION:Reduced mortality,improved risk-benefit profile and renoprotective effects of pentoxifylline compared with prednisolone suggest that pentoxifylline is superior to prednisolone for treatment of severe alcoholic hepatitis. | Binay Krishna De Subhabrata Gangopadhyay Deep Dutta Sumanta Das Baksi Adyapad Pani Pramit Ghosh | 2009 | World Journal of Gastroenterology2009，15，13： | 26 |
| 2 | Computational Identification of Sweet Wormwood (Artemisia annua) microRNA and Their mRNA Targets显示文摘Despite its efficacy against malaria, the relatively low yield (0.01%-0.8%) of artemisinin in Artemisia annua is a serious limitation to the commercialization of the drug. A better understanding of the biosynthetic pathway of artemisinin and its regulation by both exogenous and endogenous factors is essential to improve artemisinin yield. Increasing evidence has shown that microRNAs (miRNAs) play multiple roles in various biological processes. In this study, we used previously known miRNAs from Arabidopsis and rice against expressed sequence tag (EST) database of A. annua to search for potential miRNAs and their targets in A. annua. A total of six potential miRNAs were predicted, which belong to the miR414 and miR1310 families. Furthermore, eight potential target genes were identified in this species. Among them, seven genes encode proteins that play important roles in ar- temisinin biosynthesis, including HMG-CoA reductase (HMGR), amorpha-4,11-diene synthase (ADS), farnesyl pyrophosphate synthase (FPS) and cytochrome P450. In addition, a gene coding for putative AINTEGUMENTA, which is involved in signal transduction and development, was also predicted as one of the targets. | 4 | Genetic Relationship and Structure Analysis of Root Growth Angle for Improvement of Drought Avoidance in Early and Mid-Early Maturing Rice Genotypes显示文摘Deeper rooting 1(Dro1)and Deeper rooting 2(Dro2)are the QTLs that contribute considerably to root growth angle assisting in deeper rooting of rice plant.In the present study,a set of 348 genotypes were shortlisted from rice germplasm based on root angle study.Screening results of the germplasm lines under drought stress identified 25 drought tolerant donor lines based on leaf rolling,leaf drying,spikelet fertility and single plant yield.A panel containing 101 genotypes was constituted based on screening results and genotyped using Dro1 and Dro2 markers.Structure software categorized the genotypes into four sub-populations with different fixation index values for root growth angle.The clustering analysis and principal coordinate analysis could differentiate the genotypes with or without deeper rooting trait.The dendrogram constructed based on the molecular screening for deep rooting QTLs showed clear distinction between the rainfed upland cultivars and irrigated genotypes.Eleven genotypes,namely Dular,Tepiboro,Surjamukhi,Bamawpyan,N22,Dinorado,Karni,Kusuma,Bowdel,Lalsankari and Laxmikajal,possessed both the QTLs,whereas 67 genotypes possessed only Dro1.The average angle of Dro positive genotypes ranged from 82.7°to 89.7°.These genotypes possessing the deeper rooting QTLs can be taken as donor lines to be used in marker-assisted breeding programs. | Elssa PANDIT Rajendra Kumar PANDA Auromeera SAHOO Dipti Ranjan PANI Sharat Kumar PRADHAN | 2020 | Rice science2020，27，2： | 4 |
| 5 | New trends in colorectal surgery: Single port and natural orifice techniques显示文摘Single-incision laparoscopic surgery(SILS)and natural orifice transluminal endoscopic surgery(NOTES)have rapidly gained pace worldwide,potentially replacing conventional laparoscopic surgery(CLS)as the preferred colorectal surgery technique.Currently available data mainly consist of retrospective series analyzed in four meta-analyses.Despite conflicting results and lack of an objective comparison,SILS appears to offer cosmetic advantages over CLS.However,due to conflicting results and marked heterogeneity,present data fail to show significant differences in terms of operative time,postoperative morbidity profiles,portsite complications rates,oncological appropriateness,duration of hospitalization or cost when comparing SILS with conventional laparoscopy for colorectal procedures.The application of'pure'NOTES in humans remains limited to case reports because of unresolved issues concerning the ideal access site,distantorgan reach,spatial orientation and viscera closure.Alternatively,minilaparoscopy-assisted natural orifice surgery techniques are being developed.The transanal'down-to-up'total mesorectum excision has been derived for transanal endoscopic microsurgery(TEM)and represents the most encouraging NOTES-derived technique.Preliminary experiences demonstrate good oncological and functional short-term outcomes.Largescale randomized controlled trials are now mandatory to confirm the long-term SILS results and validate transanal TEM for the application of NOTES in humans. | Ronald Daher Elie Chouillard Yves Panis | 2014 | World Journal of Gastroenterology2014，20，48： | 4 |

| 14 | FINITE ELEMENT ANALYSIS OF A VERTICAL RECTANGULAR PLATE COUPLED WITH AN UNBOUNDED FLUID DOMAIN ON ONE SIDE USING A TRUNCATED FAR BOUNDARY显示文摘The dynamic pressure distribution on a rectangular plate attached to a rigid wall and supporting an infinitely large extent of fluid subjected to a harmonic ground excitation is evaluated in the time domain. Governing equations for the fluid domain are set considering the compressibility of the fluid with negligibly small change in density and a linearized free surface. A far boundary condition for the three-dimensional fluid domain is developed so that the far boundary is truncated at a closer proximity to the structure. The coupled problem is solved independently for the structure and the fluid domain by transferring the acceleration of the plate to the fluid and pressure of the fluid to the plate in sequence. Helmholtz equation for the three-dimensional fluid domain and Mindlin's theory for the two-dimensional plate are used for the solution of the interacting domains. Finite element technique is adopted for the solution of this problem with pressure as nodal variable for the fluid domain and displacement for the plate. The time dependent equations are solved in each of the interacting domain using Newmark-β method. The effectiveness of the technique is demonstrated and the influences of surface wave, exciting frequency and flexibility of the plate on dynamic pressure are investigated. | PANI P. K. | 15 | Short-term outcomes after laparoscopic colorectal surgery in patients with previous abdominal surgery: A systematic review显示文摘AIM: To perform a systematic review focusing on shortterm outcomes after colorectal surgery in patients with previous abdominal open surgery(PAOS).METHODS: A broad literature search was performed with the terms 'colorectal', 'colectomy', 'PAOS', 'previous surgery' and 'PAOS'. Studies were included if their topic was laparoscopic colorectal surgery in patients with PAOS, whether descriptive or comparative. Endpoints of interest were conversion rates, inadvertent enterotomy and morbidity. Analysis of articles was made according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses.RESULTS: From a total of 394 citations, 13 full-texts achieved selection criteria to be included in the study. Twelve of them compared patients with and without PAOS. All studies were retrospective and comparative and two were case-matched. The selected studies comprised a total of 5005 patients, 1865 with PAOS. Among the later, only 294(16%) had history of a midline incision for previous gastrointestinal surgery. Conversion rates were significantly higher in 3 of 12 studies and inadvertent enterotomy during laparoscopywas more prevalent in 3 of 5 studies that disclosed this event. Morbidity was similar in the majority of studies. A quantitative analysis(meta-analysis) could not be performed due to heterogeneity of the studies. CONCLUSION: Conversion rates were slightly higher in PAOS groups, although not statistical significant in most studies.
