1Pancreatitis in pregnancy: etiology, diagnosis, treatment, and outcomes显示文摘BACKGROUND:Acute pancreatitis in pregnancy is a rare and dangerous disease.This study aimed to examine the etiology,treatment,and outcomes of pancreatitis in pregnancy.METHOD:A total of 25 pregnant patients diagnosed with pancreatitis during the period of 1994 and 2014 was analyzed retrospectively.RESULTS:The pregnant patients were diagnosed with pancreatitis during a period of 21 years.Most(60%)of the patients were diagnosed with pancreatitis in the third trimester.The mean age of the patients at presentation was 25.7 years,with a mean gestational age of 24.4 weeks.Abdominal pain occurred in most patients and vomiting in one patient was associated hyperemesis gravidarum.The common cause of the disease was gallstone-related(56%),followed by alcohol-related(16%),post-ERCP(4%),hereditary(4%)and undetermined conditions(20%).The level of triglycerides was minimally high in three patients.ERCP and wire-guided sphincterotomy were performed in 6(43%)of 14 patients with gallstone-related pancreatitis and elevated liver enzymes with no complications.Most(84%)of the patients underwent a full-term,vaginal delivery.There was no difference in either maternal or fetal outcomes after ERCP.CONCLUSIONS:Acute pancreatitis is rare in pregnancy,occurring most commonly in the third trimester,and gallstones are the most common cause.When laparoscopic cholecystectomy is not feasible and a common bile duct stone is highly suspected on imaging,endoscopic sphincterotomy or stenting may help to prevent recurrence and postpone cholecystectomy until after delivery.Padmavathi Mali 2016Hepatobiliary & Pancreatic Diseases International2016,15,4:23
2Gastric cancer risk in relation to tobacco use and alcohol drinking in Kerala, India-Karunagappally cohort study显示文摘AIM: To assess the risk of gastric cancer(GC) in relation to tobacco use and alcohol drinking in the Karunagappally cohort in Kerala, South India.METHODS: This study examined the association of tobacco use and alcohol drinking with GC incidence among 65553 men aged 30-84 in the Karunagappally cohort. During the period from 1990-2009, 116 GC cases in the cohort were identified as incident cancers. These cases were identified from the populationbased cancer registry. Information regarding risk factors such as socioeconomic factors and tobacco and alcohol habits of cohort members were collected from the database of the baseline survey conducted during 1990-1997. The relative risks(RRs) and the corresponding 95% confidence intervals(95%CIs) fortobacco use were obtained from Poisson regression analysis of grouped survival data, considering age, follow-up period, occupation and education.RESULTS: Bidi smoking was associated with GC risk(P = 0.042). The RR comparing current versus never smokers was 1.6(95%CI: 1.0-2.5). GC risk was associated with the number of bidis smoked daily(P = 0.012) and with the duration of bidi smoking(P = 0.036). Those who started bidi smoking at younger ages were at an elevated GC risk; the RRs for those starting bidi smoking under the age of 18 and ages 18-22 were 2.0(95%CI: 1.0-3.9) and 1.8(95%CI: 1.1-2.9), respectively, when their risks were compared with lifetime non-smokers of bidis. Bidi smoking increased the risk of GC among never cigarette smokers more evidently(RR = 2.2; 95%CI: 1.3-4.0). GC risk increased with the cumulative amount of bidi smoking, which was calculated as the number of bidis smoked per day x years of smoking(bidi-year; P = 0.017). Cigarette smoking, tobacco chewing or alcohol drinking was not significantly associated with GC risk. 3Bone mineral density and disorders of mineral metabolism in chronic liver disease显示文摘AIM:To estimate the prevalence and identify the risk factors for metabolic bone disease in patients with cirrhosis.METHODS:The study was performed on 72 Indian patients with cirrhosis(63 male, 9 female;aged < 50 years).Etiology of cirrhosis was alcoholism(n = 37), hepatitis B(n = 25) and hepatitis C(n = 10).Twenty-three patients belonged to Child class A, while 39 were in class B and 10 in class C.Secondary causes for metabolic bone disease and osteoporosis were ruled out.Sunlight exposure, physical activity and dietary constituents were calculated.Complete metabolic profiles were derived, and bone mineral density(BMD) was measured using dual energy X ray absorptiometry.Low BMD was defined as a Z score below-2.RESULTS:Low BMD was found in 68% of patients.Lumbar spine was the most frequently and severely affected site.Risk factors for low BMD included low physical activity, decreased sunlight exposure, and low lean body mass.Calcium intake was adequate, with unfavorable calcium:protein ratio and calcium:phosphorus ratio.Vitamin D deficiency was highly prevalent(92%).There was a high incidence of hypogonadism(41%).Serum estradiol level was elevated significantly in patients with normal BMD.Insulin-like growth factor(IGF) 1 and IGF binding protein 3 levels were below the age-related normal range in both groups.IGF-1 was signiflcantly lower in patients with low BMD.Serum osteocalcin level was low(68%) and urinary deoxypyridinoline to creatinine ratio was high(79%), which demonstrated low bone formation with high resorption.CONCLUSION:Patients with cirrhosis have low BMD.Contributory factors are reduced physical activity, low lean body mass, vitamin D def iciency and hypogonadism and low IGF-1 level.Joe George Hosahithlu K Ganesh Shrikrishna Acharya Tushar R Bandgar Vyankatesh Shivane Anjana Karvat Shobna J Bhatia Samir Shah Padmavathy S Menon Nalini Shah 2009World Journal of Gastroenterology2009,15,28:15
4PCR analysis of Yq microdeletions in infertile males, a study from South India显示文摘Aim:To estimate the frequency of microdeletions in the long arm of Y-chromosome of 20 infertile males from South India.Methods:Polymerase chain reaction (PCR) amplification using Y-specific STS of azoospermia factor (AZF) regions i.e.,SY 84 for AZFa,SY 127 for AZFb and SY 254 for AZFc.Results:Of the 20 infertile subjects 3(15%),one azoospermic and two oligozoospermic,showed microdeletions in the AZF region of Y-chromosome.Conclusion:The frequency of deletions involving AZF region of the Y-chromosome is 15% in azoospermic and severely oligozoospermic infertile men.PCR amplification of AZF locus is useful for the diagnosis of microdeletions in the Y-chromosome.S. 6Physiological Age Status of Female Adults and Off-Season Survival of Rice Leaffolder Cnaphalocrocis medinalis in India显示文摘Rice leaffolder, Cnaphalocrocis medinalis, is one of the major foliage feeders found in the rice growing regions in India. When the crop was at maturity, numerous adults of rice leaffolder were found in the rice fields though the larval population gradually decreased, and no eggs were found on rice leaves. The population characteristics of C. medinalis were assessed based on the physiological age status of adults at different crop growth stages. Based on egg development within ovarioles, ovariole appearance, number and colour of fat bodies, and characteristics of bursa copulatrix, physiological age status of the adults was described, which served as a basis for the determination of age composition. C. medinalis adults were found during the first week of August on rice plants, of which 44% were in Age 0 with immature ovaries, indicating immigrants. However, 28% adults each were at Ages 1–2 with developing ovaries, indicating local breeding population. The carryover and off-season survival of C. medinalis were also studied to determine the contribution of the alternative hosts in the population growth that helped in devising efficient management strategies. Rice was the most preferred host followed by Triticum aestivum, Echinochloa crusgulli and Brachiaria plantaginea.
