2Percutaneous nephrolithotomy for staghorn calculi: Troubleshooting and managing complications显示文摘Staghorn calculi comprise a unique subset of complex kidney stone disease. Percutaneousnephrolithotomy (PCNL) is the gold standard treatment for staghorn stones. Despitecontinuous refinements to the technique and instrumentation of PCNL, these stones remaina troublesome challenge for endourologists and are associated with a higher rate of perioperativecomplications than that for non-staghorn stones. Common and notable intraoperativecomplications include bleeding, renal collecting system injury, injury of visceral organs, pulmonarycomplications, thromboembolic complications, extrarenal stone migration, andmisplacement of the nephrostomy tube. Postoperative complications include infection and urosepsis,bleeding, persistent nephrocutaneous urine leakage, infundibular stenosis, and death.In this review, we report recommendations regarding troubleshooting measures that can beused to identify and characterize these complications. 3Infected pancreatic necrosis: Not necessarily a late event in acute pancreatitis显示文摘It is widely believed that infection of pancreatic necrosis is a late event in the natural course of acute pancreatitis. This paper discusses the available data on the timing of pancreatic infection. It appears that infected pancreatic necrosis occurs early in almost a quarter of patients. This has practical implications for the type, timing and duration of preventive strategies used in these patients. There are also implications for the classification of severity in patients with acute pancreatitis. Given that the main determinants of severity are both local and systemic complications and that they can occur both early and late in the course of acute pancreatitis, the classification of severity should be based on their presence or absence rather than on when they occur. 5Relationship between the exocrine and endocrine pancreas after acute pancreatitis显示文摘AIM:To determine the prevalence and time course of pancreatic exocrine insufficiency in individuals with newly diagnosed prediabetes or diabetes mellitus after acute pancreatitis.METHODS:Relevant literature cited in three major biomedical journal databases(EMBASE,MEDLINE,and Scopus)was reviewed independently by two authors.There were no language constraints but the search was limited to human studies.Studies included were cohort studies of adult patients who were discharged after an attack of acute pancreatitis.Patients were excluded if they were under 18 years of age or had a previous diagnosis of prediabetes or diabetes mellitus,pancreatic exocrine insufficiency,or chronic pancreatitis.The main outcome measure was the prevalence of concomitant pancreatic exocrine insufficiency in patients who were diagnosed with prediabetes and diabetes mellitus after an attack of acute pancreatitis.Subgroup analysis was conducted for patients who were diagnosed with prediabetes only and those who were diagnosed withdiabetes mellitus only.Subgroup analysis looking at the time course of concomitant pancreatic exocrine and endocrine insufficiency was also conducted.Pooled prevalence and corresponding 95%confidence intervals were calculated for all outcome measures and P-values<0.05 were deemed statistically significant.RESULTS:Eight clinical studies comprising of 234patients met all eligibility criteria.The pooled prevalence of newly diagnosed prediabetes or diabetes in individuals after acute pancreatitis was 43%(95%CI:30%-56%).The pooled prevalence of pancreatic exocrine insufficiency in individuals after acute pancreatitis was 29%(95%CI:19%-39%).The prevalence of concomitant pancreatic exocrine insufficiency in individuals with newly diagnosed prediabetes or diabetes was 40%(95%CI:25%-55%).The prevalence of concomitant pancreatic exocrine insufficiency among individuals with prediabetes alone and diabetes mellitus alone was 41%(95%CI:12%-75%)and 39%(95%CI:28%-51%),respectively.Further analysis showed that the prevalence of concomitant pancreatic exocrine insufficiency in individuals with prediabetes or diabetes decreases over time after an attack of acute pancreatitis.CONCLUSION:Pancreatic exocrine insufficiency occurs in 40%of individuals with newly diagnosed prediabetes or diabetes mellitus after acute pancreatitis.Further studies are needed to investigate the pathogenesis of diabetes in this setting.Stephanie L M Das James I C Kennedy Rinki Murphy Anthony R J Phillips John A Windsor Maxim S Petrov 2014World Journal of Gastroenterology2014,20,45:9

7Liver blood supply after a modified Appleby procedure in classical and aberrant arterial anatomy显示文摘Reported here are two cases of a modified Appleby operation for borderline resectable ductal adenocarcinoma of the pancreatic body, in one of which a R0 distal resection was attended to by excision, not only of the celiac axis, but also of the common and left hepatic arteries in the presence of arterial anatomic variation Michels, type Ⅷb. The possibility and avenues of the maintenance of the blood supply to the left hepatic lobe after surgical aggression of this kind are demonstrated employing computed tomography (CT) and 3-D CT angiography. 11Pancreatic enzymes and abdominal adipose tissue distribution in new-onset prediabetes/diabetes after acute pancreatitis显示文摘BACKGROUND New-onset prediabetes/diabetes after acute pancreatitis(NODAP)is the most common sequela of pancreatitis,and it differs from type 2 prediabetes/diabetes mellitus(T2DM).AIM To study the associations between circulating levels of pancreatic amylase,pancreatic lipase,chymotrypsin and fat phenotypes in NODAP,T2DM,and health.METHODS Individuals with NODAP(n=30),T2DM(n=30),and sex-matched healthy individuals(n=30)were included.Five fat phenotypes(intra-pancreatic fat,liver fat,skeletal muscle fat,visceral fat,and subcutaneous fat)were determined using the same magnetic resonance imaging protocol and scanner magnet strength for all participants.One-way analysis of covariance,linear regression analysis,and relative importance analysis were conducted.RESULTS Intra-pancreatic fat deposition(IPFD)was higher in NODAP(9.4%±1.8%)and T2DM(9.8%±1.1%)compared with healthy controls(7.8%±1.9%)after adjusting for covariates(P=0.003).Similar findings were observed in regards to visceral fat volume(P=0.005),but not subcutaneous fat volume,liver fat,or skeletal muscle fat.Both IPFD(β=-2.201,P=0.023)and visceral fat volume(β=-0.004,P=0.028)were significantly associated with circulating levels of pancreatic amylase in NODAP,but not in T2DM or healthy individuals.Of the five fat phenotypes,IPFD explained the highest amount of variance in pancreatic amylase concentration(R2=15.3%out of 41.2%).None of the phenotypes contributed meaningfully to the variance in pancreatic lipase or chymotrypsin.CONCLUSION Both NODAP and T2DM are characterized by increased IPFD and visceral fat volume.However,only NODAP is characterized by significant inverse associations between the two fat phenotypes and pancreatic amylase.Juyeon Ko Loren Skudder-Hill Jaelim Cho Sakina H Bharmal Maxim S Petrov 2021World Journal of Gastroenterology2021,27,23:5
12Generation of 84-fs pulses from a mode-locked Tm:CNNGG disordered garnet crystal laser显示文摘A mode-locked laser based on a Tm:CNNGG disordered crystal as an active medium and a single-walled carbon nanotube saturable absorber is demonstrated, operating at a central wavelength of 2018 nm. 13Computed tomography-based diagnostics might be insufficient in the determination of pancreatic cancer unresectability显示文摘AIM: To inquire into a question of an overestimation of arterial involvement in patients with pancreatic cancer (PC). METHODS: Radiology data were compared with the findings from 51 standard, 58 extended and 17 total pancreaticoduodenectomies; 9 distal resections with celiac artery (CA) excision; and 28 palliations for PC. The survival of 11 patients with controversial computed tomography (CT) and endoscopic ultrasound data with regard to arterial invasion, after R0/R1 procedures (false-positive CT results, Group A), was compared to survival after eight R2 resections (false-negative CT results, Group B) and after 12 bypass procedures for locally advanced cancer (true-positive CT results, Group C).RESULTS: In all of the cases in group A, operative exploration revealed no arterial invasion, which was predicted by CT. The one-year survival in Group A was 88.9%, and the two-year survival was 26.7%, with a median follow-up of 22 mo. One-year survival was not attained in groups B and C, with a significant difference in survival (P a-b = 0.0029, P b-c = 0.003).CONCLUSION: Arterial encasement on CT does not necessarily indicate arterial invasion. Whenever PC is considered unresectable, endoUS should be used. 14主动失谐叶盘振动特性及鲁棒性研究显示文摘由于随机失谐的存在,实际叶盘通常一定程度地偏离设计值,其动力学特性通常因此而发生较大改变。以某型航空压气机高保真叶盘模型为例,采用失谐叶盘减缩建模方法,对不同阶次激励下的随机失谐叶盘响应特性进行了统计分析。研究表明:叶盘最大响应随着随机失谐值的增大呈先急剧上升后下降并趋于稳定趋势,表现'阀值'效应。此外,将失谐作为设计参数,着重研究了常见的主动失谐叶盘的响应特性,可见主动失谐叶盘相较于同等失谐程度的随机失谐叶盘具有更小的响应幅值。最后,分析了主动失谐叶盘对随机失谐的鲁棒性。段勇亮 臧朝平 Petrov E.P. 2015航空发动机2015,41,6:4
15EBSD表征变形后退火Al7075/石墨烯纳米片/碳纳米管复合材料显示文摘研究变形后退火对碳纳米管(CNTs)和石墨烯纳米片(GNPs)增强热轧Al7075基复合材料显微组织演变的影响。对搅拌铸造样品进行多道次热轧,然后在450℃下退火4 h。采用SEM、EDS和EBSD技术观察材料的显微组织演变。EBSD数据显示,添加体积分数(下同)为0.87%的GNPs和CNTs可明显抑制再结晶的发生。当添加0.96%的CNTs时,再结晶受到部分抑制。而当添加0.92%的GNPs时,明显加快了粒子激发形核(PSN)机制导致的再结晶。在有增强相存在的情况下,再结晶晶粒的体积分数与PSN的产生有显著的关系。主要织构的强度和类型都取决于增强相的种类。Siavash IMANIAN GHAZANLOU Baitallah EGHBALI Roumen PETROV 2021Transactions of Nonferrous Metals Society of China2021,31,8:4
16Sol-gel synthesis, surface and magnetic properties of nanocrystalline SmAlO_3显示文摘Nanocrystals of single-phase samarium monoaluminate (SmAlO3) were synthesized and studied for the first time. Malic acid was employed as a new complexing agent in the sol-gel process. The material was characterized by X-ray diffraction (XRD), scanning electron microscopy (SEM), energy-dispersive X-ray analysis (EDS), specific surface area and density. The lowest size of the nanoparticles was about 50–60 nm and some of them formed agglomerates. The binding energies of core-level electrons of Sm (3d, 4d, 4f, 5p, 5s), Al (2s, 2p) and O (1s, 2s) in nanocrystalline SmAlO3 were obtained by X-ray photoelectron spectroscopy (XPS) and found shifted in respect to the correspond- ing values for the same elements. The temperature dependence of the AC magnetic susceptibility of the substance was determined between 2.5 and 250 K. The experimental molar magnetic susceptibility was compared with the theoretical one of Sm3+ ions. 17Dynamic failure of dry and fully saturated limestone samples based on incubation time concept显示文摘This paper outlines the results of experimental study of the dynamic rock failure based on the comparison of dry and saturated limestone samples obtained during the dynamic compression and split tests. The tests were performed using the Kolsky method and its modifications for dynamic splitting. The mechanical data(e.g. strength, time and energy characteristics) of this material at high strain rates are obtained. It is shown that these characteristics are sensitive to the strain rate. A unified interpretation of these rate effects, based on the structuraletemporal approach, is hereby presented. It is demonstrated that the temporal dependence of the dynamic compressive and split tensile strengths of dry and saturated limestone samples can be predicted by the incubation time criterion. Previously discovered possibilities to optimize(minimize) the energy input for the failure process is discussed in connection with industrial rock failure processes. It is shown that the optimal energy input value associated with critical load, which is required to initialize failure in the rock media, strongly depends on the incubation time and the impact duration. The optimal load shapes, which minimize the momentum for a single failure impact, are demonstrated. Through this investigation, a possible approach to reduce the specific energy required for rock cutting by means of high-frequency vibrations is also discussed.Yuri V. Petrov Ivan V. 18An aortoduodenal fistula as a complication of immunoglobulin G4-related disease显示文摘Most primary aortoduodenal fistulas occur in the presence of an aortic aneurysm,which can be part of immunoglobulin G4(IgG4)-related sclerosing disease.We present a case who underwent endovascular grafting of an aortoduodenal fistula associated with a high serum IgG4 level.A 56-year-old male underwent urgent endovascular reconstruction of an aortoduodenal fistula.The patient received antibiotics and other supportive therapy,and the postoperative course was uneventful,however,elevated levels of serum IgG,IgG4 and C-reactive protein were noted,which normalized after the introduction of steroid therapy.Control computed tomography angiography showed no endoleaks.The primary aortoduodenal fistula may have been associated with IgG4-related sclerosing disease as a possible complication of IgG4-related inflammatory aortic aneurysm.Endovascular grafting of a primary aortoduodenal fistula is an effective and minimally invasive alternative to standard surgical repair.Momir Sarac Ivan Marjanovic Mihailo Bezmarevic Uros Zoranovic Stanko Petrovic Miodrag Mihajlovic 2012World Journal of Gastroenterology2012,18,42:3
19微藻油脂制备生物基多元醇理化性能分析与结构表征显示文摘为了实现有效高值利用非食用、不占耕地的绿色可再生油脂资源,该研究以微藻油脂为原料,采用环氧化-开环法和氢甲酰化-加氢还原法制备了2种生物基多元醇,通过主要理化指标碘值、酸值、环氧基含量、羟值的测定,重要理论理化指标理论双键含量、理论环氧基含量、理论羟值、转化率和选择性的计算,结合气质联用、凝胶渗透色谱、红外光谱、核磁共振等仪器分析手段,对原料和产物的理化性能和结构进行分析与表征。结果表明,微藻油脂的主要脂肪酸是油酸,质量分数为91.10%,碘值为88.46 g/100 g,理论双键含量0.34 mol/100 g,环氧-开环法得到的多元醇碘值降至0.62 g/100 g,羟值为150.35 mg/g,原料转化率和目标产物选择性分别为99.30%和86.74%,结构中引入仲羟基,通过及时除去环氧反应过程中生成的水、开环反应选择适宜的醇用量可提升产品质量。氢甲酰化-加氢还原法得到的多元醇碘值降至2.12 g/100 g,羟值为166.29 mg/g,原料转化率和目标产物选择性分别为97.60%和95.83%,结构中引入伯羟基,该法为非均相反应,催化剂过滤可除,副反应少,溶剂廉价可回收,产品易分离且质量容易控制。2种方法的原料利用率都很高,目标产物选择性好,产品性能可通过简单易行的理化指标测定和理论计算方法进行有效评价,并可利用仪器分析手段快速监测反应进程、保证产品质量、查找质量问题、拓宽应用范围,具有很好的经济性和工业应用前景,研究结果为油脂为原料的生物基多元醇深入研究和工业应用提供了科学可靠的数据基础和技术支持。杨晓琴 喻艳华 谢东 Petrovic Zoran S 洪健 万显美 2020农业工程学报2020,36,7:3
20Structured laser beams: toward 2-μm femtosecond laser vortices显示文摘Structured ultrashort-pulse laser beams, and in particular eigenmodes of the paraxial Helmholtz equation, are currently extensively studied for novel potential applications in various fields, e.g., laser plasma acceleration,attosecond science, and fine micromachining. To extend these prospects further, in the present work we push forward the advancement of such femtosecond structured laser sources into the 2-μm spectral region. Ultrashortpulse Hermite– and Laguerre–Gaussian laser modes both with a pulse duration around 100 fs are successfully produced from a compact solid-state laser in combination with a simple single-cylindrical-lens converter. The negligible beam astigmatism, the broad optical spectra, and the almost chirp-free pulses emphasize the high reliability of this laser source. This work, as a proof of principle study, paves the way toward few-cycle pulse generation of optical vortices at 2 μm.
