| 1 | Laparoscopic Partial Nephrectomy With Segmental Renal Artery Clamping:Technique and Clinical Outcomes显示文摘Background:Warm ischemic injury is one of the most important factors affecting renal function in partial nephrectomy(PN).The technique of segmental renal artery clamping emerges as an alternative to conventional renal artery clamping for renal hilar control.Objective:To evaluate the feasibility and efficiency of laparoscopic PN(LPN) with segmental renal artery clamping in comparison with the conventional technique.Design,setting,and participants:A total of 75 patients underwent LPN from June 2007 to November 2009.All patients had T1a or T1b tumor in one kidney and a normal contralateral kidney.Thirty-seven patients underwent surgeries with main renal artery clamping,and 38 underwent surgeries with segmental artery clamping.Intervention:All procedures were performed by the same laparoscopic surgeon.Measurements:Blood loss,operation time,warm ischemia(WI) time,and complications affected renal function before and after operation were recorded.Results and limitations:All LPNs were completed without conversion to open surgery or nephrectomy.The novel technique slightly increased WI time(p < 0.001) and intraoperative blood loss(p = 0.006),while it provided better postoperative affected renal function(p < 0.001) compared with the conventional technique.The total complication rate was 12%.Among the 38 cases where segmental renal artery clamping was performed,7 had to convert to the conventional method.Tumor size and location influenced the number of clamped segmental arteries.Long-term postoperative renal function is still awaited.Conclusions:LPN with segmental artery clamping is safe and feasible in clinical practice.It minimizes the intraoperative WI injury and improves early postoperative affected renal function compared with main renal artery clamping.(C) 2010 European Association of Urology.Published by Elsevier B.V.All rights reserved. | Shao,P.F. | 2 | Two regimes of Atlantic multidecadal oscillation: cross-basin dependent or Atlantic-intrinsic显示文摘The Atlantic Multidedal Oscillation(AMO) is a prominent mode of sea surface temperature variability in the Atlantic and incurs significant global influence. Most coupled models failed to reproduce the observed 50–80-year AMO, but were overwhelmed by a 10–30-year AMO. Here we show that the 50–80-year AMO and 10–30-year AMO represent two different AMO regimes. The key differences are:(1) the 50–80-year AMO involves transport of warm and saline Atlantic water into the Greenland-Iceland-Norwegian(GIN)Seas prior to reaching its maximum positive phase, while such a transport is weak for the 10–30-year AMO;(2) the zonality of atmospheric variability associated with the 50–80 year AMO favors the transport of warm and saline water into the GIN Seas;(3) the disappearance of Pacific variability weakens the zonality of atmospheric variability and the transport of warm and saline water into the GIN Seas, leading to the weakening of the 50–80-year AMO. In contrast, the 10–30-year AMO does not show dependence on the variability in Pacific and in the GIN Seas and may be an Atlantic-intrinsic mode. | 3 | Comparative mitochondrial genome analysis of Varunidae and its phylogenetic implications显示文摘Complete mitochondrial genomes(mitogenomes)can indicate phylogenetic relationships,as well as useful information for gene rearrangement mechanisms and molecular evolution.Currently,the phylogenetic location of the genus Varuna(Brachyura:Varunidae)has not been well resolved mainly because of limited representatives(only two extant species).Here,we determined a new mitogenome of this genus(Varuna litterata)and added the published mitogenomes to reconstruct the phylogeny of Varunidae.The 16368-bp mitogenome contains the entire set of 37 genes and a putative control region.The characteristics of this newly sequenced mitogenome were described and compared with the other 15 Varunidae mitogenomes.All 16 analyzed mitogenomes have identical gene order and similar molecular features.The sliding window and genetic distance analyses demonstrate highly variable nucleotide diversity,with comparatively low variability of COI and COII,and high variability of ND6.The nonsynonymous/synonymous substitution rates(dN/dS ratio)analysis shows that all 13 PCGs are under purifying selection and ATP8 gene evolves under the least selective pressure.Twelve tRNA genes,two rRNAs,one PCG,and the putative control region are found to be rearranged with respect to the pancrustacean ground pattern gene order.Tandem duplication/random loss model is adopted to explain the large-scale gene rearrangement events occurring in Varunidae mitogenomes.Phylogenetic analyses show that all Varunidae species are placed into one group,and form a sister clade with Macrophthalmidae.Nevertheless,the phylogenetic relationships within Varunidae are not completely consistent based on the two different datasets used in this study.These findings will contribute to a better understanding of gene rearrangement and molecular evolution in Varunidae mitogenomes,as well as provide insights into the phylogenetic studies of Brachyura. | Ying Zhang Li Gong Xinting Lu Zengliang Miao Lihua Jiang Bingjian Liu Liqin Liu Pengfei Li Xu Zhang Zhenming Lü | 2022 | Acta Oceanologica Sinica2022，41，6： | 2 |

| 10 | Efficacy of Qifu Lizhong enema prescription(芪附理中灌肠方)on intestinal mucosal tight junction function modulation of ulcerative colitis rat model显示文摘OBJECTIVE:To investigate the efficacy and mechanism of Qifu Lizhong enema prescription(芪附理中灌肠方,QFLZ)on intervening ulcerative colitis(UC)rat model with TCM spleen and kidney Yang insufficiency syndrome METHODS:Seventy-two male Sprague-Dawley rats were randomly assigned to six groups:normal model,mesalazine,and QFLZ high,medium,and low dose groups,each with 12 rats.After 3 d of adaptation feeding,all groups except the normal group were induced using rhubarb decoction in combination with trinitrobenzene sulfonic acid(TNBS)/55%ethanol to establish a UC rat model.Following successful modeling,the normal and model groups received daily saline enema,while the Chinese medicine and Western medicine groups received daily QFLZ and Mesalazine enema for 2 weeks respectively.The disease activity index score,hematoxylin and eosin staining,immunohistochemistry,and Western blotting were used to determine the expression of claudin 1,claudin 2,zonula occludens-1protein(ZO-1),and F-actin proteins in each rat colon tissue following treatment.RESULTS:QFLZ significantly alleviated the structural disorganization in the form of epithelial glands in the intestinal mucosa of rats with UC and retarded the progression of the disease.The intestinal mucosal epithelial cells of UC rats showed decreased expression of claudin 1,ZO-1,F-actin(P<0.05),claudin 2 appeared elevated(P<0.05),which resulted in impaired TJ.Treatment with QFLZ resulted in elevated expression of claudin 1(P<0.05),ZO-1(P<0.05)and F-actin(P<0.05)and decreased expression of claudin 2(P<0.05),which allowed for repair of the intestinal mucosal TJ,which in turn served as a treatment for UC.CONCLUSIONS:The mechanism of repairing TJ function and repairing the intestinal mucosal barrier by QFLZ may be associated with up-regulation of claudin 1,ZO-1,and F-actin levels,and down-regulation of claudin 2 expression level. | CHENG Jing Lü Shangbin ZHANG Yi HU Shuangyuan WEI Pengfei ZHOU Wenhua SHI Junfeng | 2023 | Journal of Traditional Chinese Medicine2023，43，2： | 1 |
