| 1 | Gastric adenocarcinoma of fundic gland type:Endoscopicand clinicopathological features显示文摘Gastric adenocarcinoma of fundic gland type(GA-FG) with chief cell differentiation was recently proposed as an extremely rare type of gastric adenocarcinoma. Here, we report 4 cases of GA-FG with chief cell differentiation. Endoscopic features included a submucosal tumor shape or a flat shape, whitish discoloration and dilated vessels on the surface. The tumors were located in the upper or middle third of the stomach. All cases were preoperatively diagnosed as GA-FG by biopsy, and endoscopic submucosal dissection was performed. Resected specimens revealed well-differentiated adenocarcinomas resembling chief cells. Tumor cells were diffusely positive for pepsinogen-Ⅰ, but partially positive for H+/K+-ATPase in scattered locations around the tumor margin. Despite the presence of minimal invasion of the carcinoma into the submucosal layer, which was observed in two cases, neither lymphatic nor venous invasion was detected in any of the cases. | 2 | 早胃的癌症用的程度的诊断灵活光谱成像颜色改进显示文摘 The demarcation line between the cancerous lesion and the surrounding area could be easily recognized with flexible spectral imaging color enhancement (FICE) system compared with conventional white light images. The characteristic f inding of depressed-type early gastric cancer (EGC) in most cases was revealed as reddish lesions distinct from the surrounding yellowish non-cancerous area without magnification. Conventional endoscopic images provide little information regarding depressed lesions located in the tangential line, but FICE produces higher color contrast of such cancers. Histological f indings in depressed area with reddish col- or changes show a high density of glandular structure and an apparently irregular microvessel in intervening parts between crypts, resulting in the higher color con- trast of FICE image between cancer and surrounding area. Some depressed cancers are shown as whitish lesion by conventional endoscopy. FICE also can pro- duce higher color contrast between whitish cancerous lesions and surrounding atrophic mucosa. For nearly flat cancer, FICE can produce an irregular structuralpattern of cancer distinct from that of the surrounding mucosa, leading to a clear demarcation. Most elevated-type EGCs are detected easily as yellowish lesions with clearly contrasting demarcation. In some cases, a partially reddish change is accompanied on the tumor surface similar to depressed type cancer. In addition, the FICE system is quite useful for the detection of minute gastric cancer, even without magnif ication. | 3 | Ghrelin and Helicobacter pylori infection显示文摘Ghrelin is primarily secreted from the stomach and has been implicated in the coordination of eating behavior and weight regulation. Ghrelin also plays an essential role in the mechanism of gastric mucosal defense. Thus, it is important to clarify which diseases primarily infl uence changes in plasma ghrelin concentrations. Helicobacter pylori (H pylori) infection is involved in the pathogenesis of gastritis, gastric and duodenal ulcer, gastric carcinoma, and mucosa-associated lym- phoid tissue lymphoma. H pylori eradication is related to body weight change. Compared, H pylori infected and negative subjects with normal body mass index, plasma ghrelin concentration, gastric ghrelin mRNA, and the number of ghrelin producing cells in gastric mucosa are signifi cantly lower in H pylori infected subjects than in H pylori-negative controls. Plasma ghrelin concentration decreases with the progression of gastric atrophy. Impaired gastric ghrelin production in association with atrophic gastritis induced by H pylori infection accounts for the decrease in plasma ghrelin concentration. However, the ratio of plasma acylated ghrelin to total ghrelin levels is higher in patients with chronic atrophic gastritis than in healthy subjects. This may result from the compensatory increase in plasma active ghrelin concentration in response to gastric atrophy. After H pylori eradication, gastric preproghrelin mRNA expression is increased nearly 4-fold in most cases. However, changes in plasma ghrelin concentrations before and after H pylori cure are not associated with the gastric ghrelin production. | 4 | MK615 inhibits pancreatic cancer cell growth by dual inhibition of Aurora A and B kinases显示文摘AIM:To investigate the anti-neoplastic effect of MK615, an anti-neoplastic compound isolated from Japanese apricot, against human pancreatic cancer cells in vitro. METHODS: Three human pancreatic cancer cell lines PANC-1, PK-1, and PK45H were cultured with MK615 at concentrations of 600, 300, 150, and 0 μg/mL. Growth inhibition was evaluated by cell proliferation assay, and killing activity was determined by lactate dehydrogenase (LDH) assay. Expression of Aurora A and B kinases was detected by real-time polymerase chain reaction (PCR) and Western blotting. Cell cycle stages were evaluated by flow cytometry. RESULTS: The growth inhibitory rates of MK615 at 150, 300, and 600 μg/mL were 2.3% ± 0.9%, 8.9% ± 3.2% and 67.1% ± 8.1% on PANC1 cells, 1.3% ± 0.3%, 8.7% ± 4.1% and 45.7 ± 7.6% on PK1 cells, and 1.2 ± 0.8%, 9.1% ± 2.1% and 52.1% ± 5.5% on PK45H cells, respectively (P <0.05). The percentage cytotoxicities of MK615 at 0, 150, 300, and 600 μg/mL were 19.6% ± 1.3%, 26.7% ± 1.8%, 25.5% ± 0.9% and 26.4% ± 0.9% in PANC1 cells, 19.7% ± 1.3%, 24.7% ± 0.8%, 25.9% ± 0.9% and 29.9% ± 1.1% in PK1 cells, and 28.0% ± 0.9%, 31.2% ± 0.9%, 30.4% ± 1.1% and 35.3 ± 1.0% in PK45H cells, respectively (P < 0.05). Real-time PCR and Western blotting showed that MK615 dually inhibited the expression of Aurora A and B kinases. Cell cycle analysis revealed that MK615 increased the population of cells in G2/M phase. | 6 | Lymphomatoidgastropathy mimicking extranodal NK/T cell lymphoma,nasal type:A case report显示文摘Extranodal natural killer(NK)/T-cell lymphoma,nasal type,exhibits aggressive tumor behavior and carries a poor prognosis.Recently,lymphomatoid gastropathy with NK/T cell infiltration into gastric mucosa has been recognized as a pseudo-malignant disease which regresses without treatment.Because the conventional immunohistochemical criteria of lymphomatoid gastropathy is similar to that of extranodal NK/T-cell lymphoma nasal type,it is difficult to distinguish between the two conditions by histopathological evaluation only.Here,we report a rare case of lymphomatoid gastropathy in a 57-year-old female.Gastroendoscopy on routine check-up revealed elevated reddish lesions < 1 cm in diameter in the gastric fornix and body.Although repeat endoscopies at 1 and 6 mo later revealed no gastric lesions at any locations without any treatments,at 12 mo later gastric lymphomatoid lesions recurred at gastric fornix and body.Histological examination of endoscopic biopsy specimens at 12 mo showed atypical NK cell infiltration with CD3+,CD4-,CD5-,CD7+,CD8-,CD20-,CD30-,CD56+,CD79a-and T-cell-restricted intracellular antigen-1+ into gastric mucosa.After treatment for Helicobacter pylori(H.pylori) eradication,the lesions disappeared in all locations of the gastric fornix and body over the subsequent 12 mo.Here,we report a case of H.pylori-positive lymphomatoid gastropathy with massive NK-cell proliferation,and also review the literature concerning newly identified lymphomatoid gastropathy based on comparison of extra nodal NK/T-cell lymphoma nasal type.In any case,these lesions are evaluated with biopsy specimens,the possibility of this benign entity should be considered,and excessive treatment should be carefully avoided.Close follow-up for this case of lymphomatoid gastropathy is necessary to exclude any underlying malignancy. | Tomohiro Terai Mitsushige Sugimoto Hiroki Uozaki Tetsushi Kitagawa Mana Kinoshita Satoshi Baba Takanori Yamada Satoshi Osawa Ken Sugimoto | 2012 | World Journal of Gastroenterology2012,18,17: | 5 |

| 8 | Effect of digital elevation models on monitoring slope displacements in open-pit mine by differential interferometry synthetic aperture radar显示文摘Displacement monitoring in open-pit mines is one of the important tasks for safe management of mining processes.Differential interferometric synthetic aperture radar(DInSAR),mounted on an artificial satellite,has the potential to be a cost-effective method for monitoring surface displacements over extensive areas,such as open-pit mines.DInSAR requires the ground surface elevation data in the process of its analysis as a digital elevation model(DEM).However,since the topography of the ground surface in open-pit mines changes largely due to excavations,measurement errors can occur due to insufficient information on the elevation of mining areas.In this paper,effect of different elevation models on the accuracy of the displacement monitoring results by DInSAR is investigated at a limestone quarry.In addition,validity of the DInSAR results using an appropriate DEM is examined by comparing them with the results obtained by global positioning system(GPS)monitoring conducted for three years at the same limestone quarry.It is found that the uncertainty of DEMs induces large errors in the displacement monitoring results if the baseline length of the satellites between the master and the slave data is longer than a few hundred meters.Comparing the monitoring results of DInSAR and GPS,the root mean square error(RMSE)of the discrepancy between the two sets of results is less than 10 mm if an appropriate DEM,considering the excavation processes,is used.It is proven that DInSAR can be applied for monitoring the displacements of mine slopes with centimeter-level accuracy. | I Nyoman Sudi Parwata Shinichiro Nakashima Norikazu Shimizu Takahiro Osawa | 2020 | Journal of Rock Mechanics and Geotechnical Engineering2020,12,5: | 4 |
| 9 | Gene Polymorphisms and Chemotherapy in Non-small Cell Lung Cancer显示文摘The phamacogenetics is being used to predict whether the selected chemotherapy will be really effective and tolerable to the patient. Irinotecan,oxidized by CYP3A4 to produce inactive compounds,is used for treatment of various cancers including advanced non small cell lung cancer (NSCLC) patients. CYP3A4*16B polymorphism was associated with decreased metabolism of irrinotecan. Irinotecan is also metabolized by carboxylesterase to its principal active metabolite,SN-38,which is subsequently glucuronidated by UGT1As to form the inactive compound SN-38G. UGT1A1*28 and UGT1A1*6 polymorphisms were useful for predicting severe toxicity with NSCLC patients treated with irinotecan-based chemotherapy. Platinum-based compounds (cisplatin,carboplatin) are being used in combination with new cytotoxic drugs such as gemcitabine,paclitaxel,docetaxel,or vinorelbine in the treatment of advanced NSCLC. Cisplatin activity is mediated through the formation of cisplatin-DNA adducts. Gene polymorphisms of DNA repair factors are therefore obvious candidates for determinants of repair capacity and chemotherapy efficacy. ERCC1,XRCC1 and XRCC3 gene polymorphisms were a useful marker for predicting better survival in advanced NSCLC patients treated with platinum-based chemotherapy. XPA and XPD polymorphisms significantly increased response to platinum-based chemotherapy. These DNA repair gene polymorphisms were useful as a predictor of clinical outcome to the platinum-based chemotherapy. EGFR kinase inhibitors induce dramatic clinical responses in NSCLC patients with advanced disease. EGFR gene polymorphism in intron 1 contains a polymorphic single sequence dinucleotide repeat (CA-SSR) showed a statistically significant correlation with the gefitinib response and was appeared to be a useful predictive marker of the development of clinical outcome containing skin rashes with gefitinib treatment. The other polymorphisms of EGFR were also associated with increased EGFR promoter activity. Narrow band imaging increases the detection rate of CIP. Herein, we present a 55-year-old man with symptomatic circumferential inlet patch. He exhibited globus and dysphagia, and esophagogastroduodenoscopy found cir-cumferential CIP, where im-munohistochemistry revealed the existence of pro-ton pumps(H^+, K^+ -ATPase). His throat symptoms were relieved by acid suppressive therapy with pump inhibitors. | 11 | Alpha-fetoprotein before and after pegylated interferon therapy for predicting hepatocellular carcinoma development显示文摘AIM: To investigate factors that accurately predict hepatocellular carcinoma(HCC) development after antiviral therapy in chronic hepatitis C(CHC) patients. METHODS: CHC patients who received pegylated interferon and ribavirin were enrolled in this cohort study that investigated the ability of alpha-fetoprotein(AFP) to predict HCC development after interferon(IFN) therapy. RESULTS: Of 1255 patients enrolled, 665 developed sustained virological response(SVR) during mean follow-up period of 5.4 years. HCC was occurred in 89 patients, and 20 SVR patients were included. Proportional hazard models showed that HCC occurred in SVR patients showing AFP ≥ 5 ng/m L before therapy and in non-SVR patients showing AFP ≥ 5 ng/m L before and 1 year after therapy besides older age, and low platelet counts. SVR patients showing AFP ≥ 5 ng/m L before therapy and no decrease in AFP to < 5 ng/m L 1 year after therapy had significantly higher HCC incidence than non-SVR patients showing AFP ≥ 5 ng/m L before therapy and decreased AFP(P = 0.043). AFP ≥ 5 ng/m L before therapy was significantly associated with low platelet counts and high values of alanine aminotransferase(ALT) in stepwise logistic regression analysis. | 13 | Inverted Meckel's diverticulum preoperatively diagnosed using double-balloon enteroscopy显示文摘An inverted Meckel's diverticulum is a rare gastrointestinal congenital anomaly that is difficult to diagnose prior to surgery and presents with anemia, abdominal pain, or intussusception. Here, we report the case of 57-year-old men with an inverted Meckel's diverticulum, who was preoperatively diagnosed using doubleballoon enteroscopy. He had repeatedly experienced epigastric pain for 2 mo. Ultrasonography and computed tomography showed intestinal wall thickening in the pelvis. Double-balloon enteroscopy via the anal route was performed for further examination, which demonstrated an approximately 8-cm, sausage-shaped, submucosal tumor located approximately 80 cm proximal to the ileocecal valve. A small depressed erosion was observed at the tip of this lesion. Forceps biopsy revealed heterotopic gastric mucosa. Thus, the patient was diagnosed with an inverted Meckel's diverticulum, and single-incision laparoscopic surgery was performed. This case suggests that an inverted Meckel's diverticulum should be considered as a differential diagnosis for a submucosal tumor in the ileum. | 18 | Biomass and net primary productivity of mangrove communities along the Oligohaline zone of Sundarbans, Bangladesh显示文摘Background:The article presents the first estimates of biomass and productivity for mangrove forests along the Oligohaline zone of the Sundarbans Reserve Forest(SRF),Bangladesh.This study was conducted overone year from March 2016 to April 2017.Stand structure,above and below-ground biomass changes,and litterfall production were measured within a 2100 m^2 sample plot.Methods:All trees in the study plots were numbered and height(H) and diameter at breast height(DBH) were measured.Tree height(H) and DBH for each tree were measured in March 2016 and 2017.We apply the above and belowground biomass equation for estimating the biomass of the mangrove tree species(Chave et al.Oecologia145:87-99,2005; Komiyama et al.J Trop Ecol 21:471-477,2005).Litterfall was collected using 1-mm mesh litter traps with collection area of 0.42 m^2.Net Primary Production(NPP) was estimated by the summation method of Ogawa Primary productivity of Japanese forests:productivity of terrestrial communities,JIBP synthesis(1977) and Matsuura and Kajimoto Carbon dynamics of terrestrial ecosystem:Systems approach to global environment(2013).Results:Heritiera fomes has maintained its dominance of the stand and also suffered the highest tree mortality(2.4%) in the suppressed crown class.The total above-ground biomass(AGB) and below-ground biomass(BGB) of the studied stand was 154.8 and 84.2 Mg ha^(-1),respectively.Among the total biomass of the trees,64.8% was allocated to AGB and 35.2% to BGB.In case of species-wise contribution of biomass allocation,Avicennia officinalis showed the highest score and Aglaia cucullata the lowest.Mean annual total litterfall was 10.1 Mg ha^(-1)·yr^(-1),with the maximum litterfall in winter or dry season and late summer or rainy season.The mean AGB increment and above-ground net primary productivity(AGNPP) were 7.1 and 172 Mg·ha^(-1)·yr^(-1),respectively.Total net primary productivity(NPP) was estimated to be 21.0 Mg·ha^(-1)·yr^(-1) over the observed period.The results in the Sundarbans mangrove forests exhibited that mangrove communities with similar height and diameter produced different biomass production with the different basal area.The present analysis revealed that the root biomass was large enough and the mean ratio of above-/below-ground biomass was estimated to be 1.84.Conclusions:Mangrove communities growing at the oligohaline zone of the Sundarbans,Bangladesh showed high biomass and net primary production indicating their ecological and conservation significance that may be considered in future decision making process for the area as well as in understanding the role of Sundarbans mangrove forest on mitigating the effect of global warming. | Md. | 19 | A regression model-based method for indoor positioning with compound location fingerprints显示文摘This paper proposed and evaluated an estimation method for indoor positioning.The method combines location fingerprinting and dead reckoning differently from the conventional combinations.It uses compound location fingerprints,which are composed of radio fingerprints at multiple points of time,that is,at multiple positions,and displacements between them estimated by dead reckoning.To avoid errors accumulated from dead reckoning,the method uses short-range dead reckoning.The method was evaluated using 16 Bluetooth beacons installed in a student room with the dimensions of 11×5 m with furniture inside.The Received Signal Strength Indicator(RSSI)values of the beacons were collected at 30 measuring points,which were points at the intersections on a 1×1 m grid with no obstacles.A compound location fingerprint is composed of RSSI vectors at two points and a displacement vector between them.Random Forests(RF)was used to build regression models to estimate positions from location fingerprints.The root mean square error of position estimation was 0.87 m using 16 Bluetooth beacons.This error is lower than that received with a single-point baseline model,where a feature vector is composed of only RSSI values at one location.The results suggest that the proposed method is effective for indoor positioning. | Tomofumi Takayama Takeshi Umezawa Nobuyoshi Komuro Noritaka Osawa | 2019 | Geo-Spatial Information Science2019,22,2: | 2 |
| 20 | Tuberous sclerosis patient with neuroendocrine carcinoma of the esophagogastric junction:A case report显示文摘BACKGROUND Tuberous sclerosis complex(TSC)is a rare inherited disease with non-cancerous tumor growths in the skin,brain,kidneys,heart,and lungs.The co-occurrence of neuroendocrine neoplasm(NEN)with TSC is even rarer.There have been few reports on the relationship between TSC and neuroendocrine tumors(NETs),and fewer on the relationship between TSC and neuroendocrine carcinoma(NEC),a subtype of NEN.This is the first reported case of NEC occurring at the esophagogastric junction in a patient with TSC.CASE SUMMARY A 46-year-old woman visiting our hospital for the treatment of TSC was admitted to the emergency department with tarry stools and dizziness.Computed tomography scans revealed thickness of the gastric cardia,multiple metastatic lesions of the liver,and enlarged lymph nodes near the lesser curvature of the stomach.Esophagogastroduodenoscopy revealed a type 3 tumor located from the esophagogastric junction to the fundus,and the pathological diagnosis by biopsy was NEC.The patient was treated with seven courses of cisplatin+irinotecan,followed by eight courses of ramucirumab+nab-paclitaxel,one course of nivolumab,and two courses of S-1+oxaliplatin.Twenty-three months after the first treatment,the patient died because of disease progression and deterioration of the general condition.CONCLUSION This case of NEC occurring in a patient with TSC indicates a difference in the occurrence of NETs and NECs. | Natsuki Ishida Takahiro Miyazu Satoshi Tamura Satoshi Suzuki Shinya Tani Mihoko Yamade Moriya Iwaizumi Satoshi Osawa Yasushi Hamaya Kazuya Shinmura Haruhiko Sugimura Katsutoshi Miura Takahisa Furuta Ken Sugimoto | 2020 | World Journal of Gastroenterology2020,26,45: | 2 |
