1Ankylosing spondylitis: A state of the art factual backbone显示文摘Ankylosing spondylitis(AS) is a chronic inflammatory disease that affects 1% of the general population. As one of the most severe types of spondyloarthropathy, AS affects the spinal vertebrae and sacroiliac joints, causing debilitating pain and loss of mobility. The goal of this review is to provide an overview of AS, from the pathophysiological changes that occur as the disease progresses, to genetic factors that are involved with its onset. 2Percutaneous nephrolithotomy for staghorn calculi: Troubleshooting and managing complications显示文摘Staghorn calculi comprise a unique subset of complex kidney stone disease. Percutaneousnephrolithotomy (PCNL) is the gold standard treatment for staghorn stones. Despitecontinuous refinements to the technique and instrumentation of PCNL, these stones remaina troublesome challenge for endourologists and are associated with a higher rate of perioperativecomplications than that for non-staghorn stones. Common and notable intraoperativecomplications include bleeding, renal collecting system injury, injury of visceral organs, pulmonarycomplications, thromboembolic complications, extrarenal stone migration, andmisplacement of the nephrostomy tube. Postoperative complications include infection and urosepsis,bleeding, persistent nephrocutaneous urine leakage, infundibular stenosis, and death.In this review, we report recommendations regarding troubleshooting measures that can beused to identify and characterize these complications. 3Design and performance analysis of position-based impedance control for an electrohydrostatic actuation system显示文摘Electrohydrostatic actuator(EHA) is a type of power-by-wire actuator that is widely implemented in the aerospace industry for flight control, landing gears, thrust reversers, thrust vector control, and space robots. This paper presents the development and evaluation of positionbased impedance control(PBIC) for an EHA. Impedance control provides the actuator with compliance and facilitates the interaction with the environment. Most impedance control applications utilize electrical or valve-controlled hydraulic actuators, whereas this work realizes impedance control via a compact and efficient EHA. The structures of the EHA and PBIC are firstly introduced. A mathematical model of the actuation system is established, and values of its coefficients are identified by particle swarm optimization. This model facilitates the development of a position controller and the selection of target impedance parameters. A nonlinear proportional-integral position controller is developed for the EHA to achieve the accurate positioning requirement of PBIC. The controller compensates for the adverse effect of stiction, and a position accuracy of 0.08 mm is attained.Various experimental results are presented to verify the applicability of PBIC to the EHA. 4Primary hepatic solitary fibrous tumor with histologically benign and malignant areas显示文摘Extrapleural solitary fibrous tumor(SFT) is an uncommon mesenchymal neoplasm,presenting most commonly in the intrathoracic sites but which has been reported at numerous extrathoracic locations. The majority of intra-thoracic SFTs are benign,but 10%-15% behave aggressively. We report a case of primary hepatic SFT with histologically benign and malignant areas. A 65-year-old man underwent an abdominal CT scan following a cerebrovascular accident,which demonstrated a sharply demarcated large liver mass with a heterogenous enhancing area and occupying most of the left lobe of the liver. Histological examination following a hemihepatectomy showed an SFT with morphological patterns ranging from benign to malignant areas,including pleomorphism,increased cellularity,herringbone pattern,necrosis and a raised mitotic count. On review of the literature,only an occasional case report with malignant areas in a hepatic SFT was identified. 5Impulse magnetic stimulation facilitates synaptic regeneration in rats following sciatic nerve injury显示文摘The current studies describing magnetic stimulation for treatment of nervous system diseases mainly focus on transcranial magnetic stimulation and rarely focus on spinal cord magnetic stimula-tion.Spinal cord magnetic stimulation has been confirmed to promote neural plasticity after injuries of spinal cord,brain and peripheral nerve.To evaluate the effects of impulse magnetic stimulation of the spinal cord on peripheral nerve regneration,we compressed a 3 mm segment located in the middle third of the hip using a sterilized artery forceps to induce ischemia.Then,all animals un-derwent impulse magnetic stimulation of the lumbar portion of spinal crod and spinal nerve roots daily for 1 month.Electron microscopy results showed that in and below the injuryed segment,the inflammation and demyelination of neural tissue were alleviated,apoptotic cells were reduced,and injured Schwann cells and myelin fibers were repaired.These findings suggest that high-frequency impulse magnetic stimulation of spinal cord and corresponding spinal nerve roots promotes synaptic regeneration following sciatic nerve injury.Sergey A.Zhivolupov Miroslav M.Odinak Nariman A.Rashidov Ludmila S.Onischenko Igor N.Samartsev Anton A.Jurin 2012Neural Regeneration Research2012,7,17:6
6Carbamazepine solubility enhancement in tandem with swellable polymer osmotic pump tablet: A promising approach for extended delivery of poorly water-soluble drugs显示文摘Elementary osmotic pump(EOP)is a unique extended release(ER)drug delivery system based on the principle of osmosis.It has the ability to minimize the amount of the drug,accumulation and fluctuation in drug level during chronic uses.Carbamazepine(CBZ),a poorly water-soluble antiepileptic drug,has serious side effects on overdoses and chronic uses.The aim of the present study was to design a new EOP tablet of CBZ containing a solubility enhancers and swellable polymer to reduce its side effects and enhance the patient compliance.Firstly,a combination of solubilizing carriers was selected to improve the dissolution of the slightly soluble drug.Then,designing the new EOP tablet and investigating the effect of different variables of core and coat formulations on drug release behavior by single parameter optimization and by Taguchi orthogonal design with analysis of variance(ANOVA),respectively.The results showed that CBZ solubility was successfully enhanced by a minimum amount of combined polyvinyl pyrrolidone(PVP K30)and sodium lauryl sulfate(SLS).The plasticizer amount and molecular weight(MW)together with the osmotic agent amount directly affect the release rate whereas the swellable polymer amount and viscosity together with the semi-permeable membrane(SPM)thickness inversely influence the release rate.In addition,the tendency of following zero order kinetics was mainly affected by the coat components rather than those of the core.Further,orifice size does not have any significant effect on the release behavior within the range of 0.1 mm to 0.8 mm.In this study we report the successful formulation of CBZ-EOP tablets,which were similar to the marketed product Tegretol CR 200 and able to satisfy the USP criterion limits and to deliver about 80%of CBZ at a rate of approximately zero order for up to 12 h.Hadjira Rabti Jumah Masoud Mohammed Salmani Eltayeb Sulimen Elamin Narimane Lammari Jie Zhang Qineng Ping 2014Asian Journal of Pharmaceutical Sciences2014,9,3:4
7Uncertainty quantification of stability and damage detection parameters of coupled hydrodynamic-ground motion in concrete gravity dams显示文摘In this paper, models of the global system of the Koyna dam have been created using ABAQUS software considering the dam-reservoir-fbundation interaction. Non coupled models and the coupled models were compared regarding the horizontal displacement of the dam crest and the differential settlement of the dam base in clay foundation. Meta models were constructed and uncertainty quantification process was adopted by the support of Sobol's sensitivity in dices considering five un certain parameters by exploiting Box-Behnken experimental method. The non coupled models results determined overestimated predicted stability and damage detection in the dam. The rational effects of the reservoir height were very sensitive in the variation of the horizontal displacement of the dam crest with a small interaction effect with the beta viscous damping coefficient of the clay fbundation. The modulus of elasticity of the clay foundation was the decisive parameter regarding the variation of the differential settlement of the dam base. The XFEM approach has been used for damage detection in relation with both minimum and maximum values of each uncertain parameter. 9Effect of periocular injection of celecoxib and propranolol on ocular level of vascular endothelial growth factor in a diabetic mouse model显示文摘AIM: To investigate the effects of periocular injection of propranolol and celecoxib on ocular levels of vascular endothelial growth factor(VEGF) in a diabetic mouse model.METHODS: Forty 4-6wk BALB-C male mice weighing20-25 g were used. The study groups included: non-diabetic control(group 1), diabetic control(group 2),diabetic propranolol(group 3), and diabetic celecoxib(group 4). After induction of type 1 diabetes by streptozotocin, propranolol(10 μg) and celecoxib(200 μg dissolved in carboxymethylcellulose 0.5%) were injected periocularly. The ocular level of VEGF was measured in all the study groups using enzyme-linked immuno sorbent assay(ELISA) method. RESULTS: Ocular VEGF level was significantly increased(1.25 fold) in the diabetic control group when compared to the non-diabetic group one week after induction with streptozotocin(P =0.002). Both periocular propranolol and celecoxib significantly reduced ocular VEGF levels(P =0.047 and P <0.001, respectively). The effect was more pronounced with celecoxib.
