1Association of Overlapped and Un-overlapped Comorbidities with COVID-19 Severity and Treatment Outcomes: A Retrospective Cohort Study from Nine Provinces in China显示文摘Objective Several COVID-19 patients have overlapping comorbidities. The independent role of each component contributing to the risk of COVID-19 is unknown, and how some non-cardiometabolic comorbidities affect the risk of COVID-19 remains unclear.Methods A retrospective follow-up design was adopted. A total of 1,160 laboratory-confirmed patients were enrolled from nine provinces in China. Data on comorbidities were obtained from the patients’ medical records. Multivariable logistic regression models were used to estimate the odds ratio(OR) and 95% confidence interval(95% CI) of the associations between comorbidities(cardiometabolic or non-cardiometabolic diseases), clinical severity, and treatment outcomes of COVID-19.Results Overall, 158(13.6%) patients were diagnosed with severe illness and 32(2.7%) had unfavorable outcomes. Hypertension(2.87, 1.30–6.32), type 2 diabetes(T2 DM)(3.57, 2.32–5.49),cardiovascular disease(CVD)(3.78, 1.81–7.89), fatty liver disease(7.53, 1.96–28.96), hyperlipidemia(2.15, 1.26–3.67), other lung diseases(6.00, 3.01–11.96), and electrolyte imbalance(10.40, 3.00–26.10)were independently linked to increased odds of being severely ill. T2 DM(6.07, 2.89–12.75), CVD(8.47,6.03–11.89), and electrolyte imbalance(19.44, 11.47–32.96) were also strong predictors of unfavorable outcomes. Women with comorbidities were more likely to have severe disease on admission(5.46,3.25–9.19), while men with comorbidities were more likely to have unfavorable treatment outcomes(6.58, 1.46–29.64) within two weeks.Conclusion Besides hypertension, diabetes, and CVD, fatty liver disease, hyperlipidemia, other lung diseases, and electrolyte imbalance were independent risk factors for COVID-19 severity and poor treatment outcome. 2Famitinib in combination with concurrent chemoradiotherapy in patients with locoregionally advanced nasopharyngeal carcinoma: a phase 1, open-label, dose-escalation Study显示文摘Background:Famitinib is a tyrosine kinase inhibitor against multiple targets,including vascular endothelial growth factor receptor 2/3,platelet-derived growth factor receptor,and stem cell factor receptor(c-kit).Previous studies have demonstrated anti-tumour activities of famitinib against a wide variety of advanced-stage solid cancers.We aimed to determine the safety and efficacy of famitinib with concurrent chemoradiotherapy(CCRT)in patients with locoregionally advanced nasopharyngeal carcinoma(NPC).We also evaluated the feasibility of contrast-enhanced ultrasound(D-CEUS)as a predictor of early tumour response to famitinib and to correlate functional parameters with clinical efficacy.Methods:The trial was conducted in subjects with stage III or IVa-b NPC using a 3+3 design of escalating fami-tinib doses.Briefly,subjects received 2 weeks of famitinib monotherapy followed by 7 weeks of famitinib plus CCRT.D-CEUS of the neck lymph nodes was performed at day 0,8 and 15 after famitinib was administered before starting concurrent chemoradiotherapy.End points included safety,tolerability and anti-tumour activity.Results:Twenty patients were enrolled(six each for 12.5,16.5 and 20 mg and two for 25 mg).Two patients in the 25 mg cohort developed dose-limiting toxicities,including grade 4 thrombocytopenia and grade 3 hypertension.The most common grade 3/4 adverse events were leukopenia,neutropenia and radiation mucositis.D-CEUS tests showed that more than 60%of patients achieved a perfusion parameter response after 2 weeks taking famitinib alone,and the parameter response was associated with disease improvement.In the famitinib monotherapy stage,three patients(15%)showed partial responses.The complete response rate was 65%at the completion of treatment and 95%3 months after the treatment ended.After a median follow-up of 44 months,the 3-year progression-free survival(PFS)and distant metastasis-free survival were 70%and 75%,respectively.Subjects with a decrease of perfusion parameter response,such as peak intensity decreased at least 30%after 1 week of famitinib treatment,had higher 3-year PFS(90.9%vs.44.4%,95%CI 73.7%-100%vs.11.9%-76.9%,P<0.001)than those with an increase or a reduction of less than 30%.Conclusions:The recommended famitinib dose for phase II trial is 20 mg with CCRT for patients with local advanced NPC.D-CEUS is a reliable and early measure of efficacy for famitinib therapies.Further investigation is required to confirm the effects of famitinib plus chemoradiotherapy.Qiuyan Chen Linquan Tang Na Liu Feng Han Ling Guo Shanshan Guo Jianwei Wang Huai Liu Yanfang Ye Lu Zhang Liting Liu Pan Wang Yingqin Li Qingmei He Xiaoqun Yang Qingnan Tang Yang Li YuJing Liang XueSong Sun Chuanmiao Xie Yunxian Mo Ying Guo Rui Sun Haoyuan Mo Kajia Cao Xiang Guo Musheng Zeng Haiqiang Mai Jun Ma 2018Cancer Communications2018,38,1:6
3Effect of lit-moxa stimulation of Guanyuan(CV 4) acupoint on levels of lactic acid and super-oxide dismutase in skeletal muscle of rats显示文摘OBJECTIVE:To observe the effect of lit-moxa stimulating acupoint Guanyuan(CV 4) on lactic acid and super-oxide dismutase(SOD) in skeletal muscle after exercise exhaustion.METHODS:Twenty-eight adult male Sprague-Dawley rats were randomly divided into normal control group,exhausted control group,exercise group and moxibustion group using exercise training and mild heating with lit-moxa stick as treatment methods.The exhausted control group,moxibustion group and exercise group received an exhaustive swimming after 20 days of intervention.Swimming exhausted times were recorded.Lactic acid and SOD concentration in soleus muscle were detected and compared between every two groups.RESULTS:The swimming exhausted times of the moxibustion group and the exercise group were significantly increased compare to the exhausted control group(P < 0.05).The lactic acid of the exhausted control group was significantly increased comparing with the normal control group(P <0.05),and the lactic acid of the moxibustion group and the exercise group were significantly lower than that of the exhausted control group(P < 0.05).The SOD level of the exhausted control group was significantly decreased comparing with the normal control group(P < 0.05),and the SOD level of the moxibustion group and the exercise group were significantly higher than that of the exhausted control group(both,P < 0.05).There was no statistical difference between the moxibustion group and the exercise group.CONCLUSION:With lit moxa stick,heat stimulating acupoint of Guanyuan(CV 4) decreased the levels of lactic acid and SOD in rat's skeletal muscle.Mo Jie Wu Lili Sun Zhifang Wang Hongbin Ding Na He Long Zhang Lufen Liu Tonghua Li Xiaohong 2015Journal of Traditional Chinese Medicine2015,35,2:6
4Duration-dependent regulation of autophagy by isoflurane exposure in aged rats显示文摘Current evidence suggests a central role for autophagy in many inflammatory brain disorders, including Alzheimer's disease(AD). Furthermore, it is also well accepted that some inhalation anesthetics, such as isoflurane, may cause ADlike neuropathogenesis and resultant postoperative cognitive dysfunction, especially in the elderly population. However, the impact of inhalation anesthetics on autophagic components in the brain remains to be documented. Hence, our objective was to investigate the effects of different durations of isoflurane exposure on hippocampus-dependent learning and hippocampal autophagy in aged rats. Aged Sprague-Dawley rats(20 months old) were randomly exposed to 1.5% isoflurane or 100% oxygen for 1 or 4 h. Animals were then trained in the Morris water maze(4 trials/day for 5 consecutive days). Hippocampal phagophore formation markers, beclin 1 and protein microtubule-associated protein 1 light chain-3B(LC3B), as well as p62, an indicator of autophagic flux, were quantified by western blotting. There was no significant difference in the escape latencies and time spent in the target quadrant, as well as hippocampal expression of beclin 1, LC3 BII, and p62 at 24 h post-anesthesia between the1-h isoflurane-exposed rats and their controls(P >0.05). Four-hour exposure to isoflurane resulted in spatial learning and memory deficits, as evidenced by prolonged escape latencies on days 4 and 5 postanesthesia and less time spent in the target quadrant than sham-exposed animals(P <0.05). These events were accompanied by a decline in hippocampal expression of LC3B-I, LC3B-II, and beclin 1 24 h after isoflurane(P <0.01 and P <0.05). Nevertheless, no significant change in p62 expression was found. Further kinetics study of autophagic changes induced by 4 h of isoflurane showed a transient upregulation of LC3B-I, LC3B-II, and beclin 1 at the end of exposure and a subsequent striking decrease w ithin 12–24 h post-anesthesia(P <0.05). Hippocampal p62 p eaked at 6 h but subsequently resolved. 7Jiangtang Xiaoke granule attenuates glucose metabolism disorder via regulating endoplasmic reticulum stress in the liver of type 2 diabetes mellitus mice显示文摘OBJECTIVE:To observe the effect of Jiangtang Xiaoke(JTXK) granule on endoplasmic reticulum(ER)stress in high fat diet(HFD)-induced type 2 diabetes mellitus(T2 DM) KK-Ay mice.METHODS:KK-Ay mice were fed with HFD to induce the T2 DM model,while normal control C57 BL/6 J mice were given standard feed.Fasting blood glucose(FBG) in all mice was measured weekly and oral glucose tolerance tests(OGTTs) were performed at 4 and 10 weeks after start of treatment to determine glucose metabolism.Serum fasting insulin(FINS) and insulin sensitivity index(ISI) were measured to determine insulin sensitivity.m RNA expressions of eukaryotic initiation factor-2 alpha(e IF2α),glucose regulated protein 78(GRP78),activating transcription factor 4(ATF4),and C/EBP homology protein(CHOP) were assessed by reverse transcription polymerase chain reaction and the protein expressions of p-e IF2α,GRP78,and CHOP were assessed by Western blotting.RESULTS:JTXK granule significantly reduced FBG and free fatty acid levels and improved OGTT at the120 min of the 10-week treatment in T2 DM KK-Ay mice.FINS and Hb Alc levels were reduced and insulin sensitivities were increased in KK-Ay diabetic mice,which were improved with the treatment of JTXK granule,especially at the 7 and 3.5 g/kg doses.JTXK granule at the 3.5 g/kg dose was most effective in reducing both gene and protein expressions of e IF2α,GRP78,and CHOP.CONCLUSION:ER stress response is increased in T2 DM KK-Ay mice.Treatment with JTXK granule attenuates glucose disorders,improves insulin sensitivity,and reduces serum FFA in T2 DM KK-Ay mice.The mechanisms may be attributed to regulation of the signaling ER stress pathway via decreasing e IF2α phosphorylation and suppressing e IF2α-ATF4-CHOP activation.Zhang Yi Mo Fangfang Zhang Dongwei Gao Sihua Zhao Dandan Yu Na Mu Qianqian Zuo Jiacheng Ma Yue 2018Journal of Traditional Chinese Medicine2018,38,4:3
8Establishment of hepatic and neural differentiation platforms of Wilson’s disease specific induced pluripotent stem cells显示文摘The combination of disease-specific human induced pluripotent stem cells(iPSC)and directed cell differentiation offers an ideal platform for modeling and studying many inherited human diseases.Wilson’s disease(WD)is a monogenic disorder of toxic copper accumulation caused by pathologic mutations of the ATP7B gene.WD affects multiple organs with primary manifestations in the liver and central nervous system(CNS).In order to better investigate the cellular pathogenesis of WD and to develop novel therapies against various WD syndromes,we sought to establish a comprehensive platform to differentiate WD patient iPSC into both hepatic and neural lineages.Here we report the generation of patient iPSC bearing a Caucasian population hotspot mutation of ATP7B.Combining with directed cell differentiation strategies,we successfully differentiated WD iPSC into hepatocyte-like cells,neural stem cells and neurons.Gene expression analysis and cDNA sequencing confirmed the expression of the mutant ATP7B gene in all differentiated cells.Hence we established a platform for studying both hepatic and neural abnormalities of WD,which may provide a new tool for tissue-specific disease modeling and drug screening in the future.Fei Yi Jing Qu Mo Li Keiichiro Suzuki Na Young Kim Guang-Hui Liu Juan Carlos Izpisua Belmonte 2012Protein & Cell2012,3,11:3
9Oxidized mitochondrial DNA sensing by STING signaling promotes the antitumor effect of an irradiated immunogenic cancer cell vaccine显示文摘Exposure to ionizing radiation,a physical treatment that inactivates live tumor cells,has been extensively applied to enhance the antitumor responses induced by cancer cell vaccines in both animal research and human clinical trials.However,the mechanisms by which irradiated cells function as immunogenic tumor vaccines and induce effective antitumor responses have not been fully explored.Here,we demonstrate that oxidized mitochondrial DNA(mtDNA)and stimulator of interferon genes(STING)signaling play a key roles in the enhanced antitumor effect achieved with an irradiated tumor cell vaccine.Elevations in ROS and oxidized mtDNA 8-OHG content could be induced in irradiated tumor cells.Oxidized mtDNA derived from irradiated tumor cells gained access to the cytosol of dendritic cells(DCs).Oxidized mtDNA,as a DAMP or adjuvant,activated the STING-TBK1-IRF3-IFN-β pathway in DCs,which subsequently cross-presented irradiated tumor cell-derived antigens to CD8^(+)T cells and elicited antitumor immunity.The results of our study provide insight into the mechanism by which an irradiated cell vaccine mediates antitumor immunity,which may have implications for new strategies to improve the efficacy of irradiated vaccines.Chunju Fang Fei Mo Li Liu Jing Du Min Luo Ke Men Feifei Na Wei Wang Hanshuo Yang Xiawei Wei 2021Cellular & Molecular Immunology2021,18,9:2
10Berberine Induces Cell Apoptosis through Cytochrome C/Apoptotic Protease-Activating Factor 1/Caspase-3 and Apoptosis Inducing Factor Pathway in Mouse Insulinoma Cells显示文摘Objective:To investigate apoptotic effects of berberine,a significant alkaloids component existing in Rhizoma coptidis,and its possible acting mechanism in insulinoma cells.Methods:Different concentrations of berberine were used to treat mouse insulinoma(MIN6)cells for various period of time.The viability and apoptosis of the cells were analyzed using methylthiazolyldiphenvl-tetrazolium bromide assay,flow cytometry and enzyme-linked immuno sorbent assay.Changes in the relating pro-and anti-apoptosis proteins were detected by western-blotting.Results:The half-maximal inhibitory concentration(IC50)of berberine was 5.7μmol/L on MIN6 cells viability for 16 h.Berberine caused a 20%reduction(P<0.05)in cell number after only 4-h incubation;which reached 50%after 24 h(P<0.01).Berberine treatment for 16 h significantly increased the level of DNA fragmentation.The flow cytometry showed the apoptotic rate increased 2.9-and 4.6-fold after treating with berberine(5μmol/L)for 8 and 16 h,while 3-and 8.7-fold after 10μmol/L treatment for 8 and 16 h(P<0.01).Berberine treatment dramatically elevated the expression ratio of Bax to Bcl-2.Meanwhile,berberine notably increased the apoptosis-inducing factors and cytochrome C transforming from the mitochondria to the cytoplasm.Apoptotic protease-activating factor 1(Apaf-1)was subsequently activated after cytochrome C release.Furthermore,caspase-3 and poly adenosine diphosphate-ribose polymerase were also activated to trigger apoptosis cascade.Conclusion:High concentration(5 and 10μmol/L)of berberine could induce the apoptosis of MIN6 cells through cytochrome C/Apaf-1/caspase-3 and apoptosis inducing factor(AIF)pathway.FANG Xin MIAO Xiao-liang LIU Jun-li ZHANG Dong-wei WANG Min ZHAO Dan-dan MU Qian-qian YU Na MO Fang-fang YIN Hong-ping GAO Si-hua 2019Chinese Journal of Integrative Medicine2019,25,11:2
11Reduction in Intrahepatic cccDNA and Integration of HBV in Chronic Hepatitis B Patients with a Functional Cure显示文摘Background and Aims:Functional cure(FC)is characterized by the clearance of the hepatitis B surface antigen from the serum of patients with chronic hepatitis B(CHB).However,the level of intrahepatic covalently closed circular DNA(cccDNA)and hepatitis B virus(HBV)integration remains unclear.We conducted this study to determine them and reveal their value in the treatment of CHB.Methods:There were two sessions to elucidate the changes in intrahepatic cccDNA and HBV integration after antiviral therapy.In the first session,116 patients were enrolled and divided into FC,non-functional cure(NFC),and CHB groups,including 48 patients with functionally cured CHB,27 with CHB without functional cure after antiviral treatment,and 41 with treatment-naïve CHB.Patients were tested for both intrahepatic cccDNA and other viral markers.All patients in the FC group were followed up for at least 24 weeks to observe relapse.In the second session,another ten patients were included for in-depth whole-genome sequencing to analyze HBV integration.Results:Thirteen patients in the FC group were negative for intrahepatic cccDNA.Intrahepatic cccDNA was much higher in the CHB group compared with the FC group.Seven patients had HBsAg seroreversion,including two with virological relapse.Integration of HBV was detected in one(33.3%)functionally cured patients and in seven(100%)with CHB.28.0%of the HBV breakpoints were assigned in the 1,500 nt to 1,900 nt range of the HBV genome.Conclusions:After achieving an FC,the rate of intrahepatic cccDNA and HBV integration was significantly reduced in patients with CHB.For those patients who cleared intrahepatic cccDNA,the chances of developing virological relapse were even lower.Weiqiang Gan Na Gao Lin Gu Zhishuo Mo Xiuqing Pang Ziying Lei Zhiliang Gao 2023Journal of Clinical and Translational Hepatology2023,11,2:2
