| 1 | Clinical epidemiology and disease burden of nonalcoholic fatty liver disease显示文摘Nonalcoholic fatty liver disease(NAFLD) is defined as the presence of hepatic fat accumulation after the exclusion of other causes of hepatic steatosis, including other causes of liver disease, excessive alcohol consumption, and other conditions that may lead to hepatic steatosis. NAFLD encompasses a broad clinical spectrum ranging from nonalcoholic fatty liver to nonalcoholic steatohepatitis(NASH), advanced fibrosis, cirrhosis, and finally hepatocellular carcinoma(HCC). NAFLD is the most common liver disease in the world and NASH may soon become the most common indication for liver transplantation. Ongoing persistence of obesity with increasing rate of diabetes will increase the prevalence of NAFLD, and as this population ages, many will develop cirrhosis and end-stage liver disease. There has been a general increase in the prevalence of NAFLD, with Asia leading the rise, yet the United States is following closely behind with a rising prevalence from 15% in 2005 to 25% within 5 years. NAFLD is commonly associated with metabolic comorbidities, including obesity, type Ⅱ diabetes, dyslipidemia, and metabolic syndrome. Our understanding of the pathophysiology of NAFLD is constantly evolving. Based on NAFLD subtypes, it has the potential to progress into advanced fibrosis, end-stage liver disease and HCC. The increasing prevalence of NAFLD with advanced fibrosis, is concerning because patients appear toexperience higher liver-related and non-liver-related mortality than the general population. | 2 | 盐岩渗透特性的试验研究及其在深部储气库中的应用显示文摘为给深部盐岩地层(≥1500m)中所建天然气储库的稳定性和密闭性评价提供科学指导,针对取自江苏金坛的不同杂质含量盐岩试样,在荷兰乌德勒支大学HPT实验室开展了在三轴压缩条件下的瞬态法渗透率测试研究,揭示盐岩的变形特征与渗透率演化规律。试验研究表明:在围压20 MPa的常温测试条件下,随着偏应力从0~40MPa递增,损伤后的低含泥盐岩的渗透率从10-16m2逐渐下降至10-21m2以下,而损伤后的高含泥盐岩的渗透率则平均高出前者1~2个量级,推测表明损伤后的盐岩在加载下裂纹闭合、孔隙减小、逐渐压缩密实,进而渗透率快速降低,同时也表明外载压密作用对纯盐岩的裂纹压缩闭合、甚至损伤修复作用更显著。根据试验成果,并利用H.Alkan等[11]建立的盐岩压缩–扩容边界线对位于1500~2000 m深部的盐岩地层中的球形腔体的围岩应力状态分析显示,围岩几乎都位于扩容边界以下的安全区域,由此推知:即使在储气库低压运行条件下,围岩中较难发生扩容而导致密闭性失效事件发生,腔体的密闭性能极好。该研究为深部盐穴储气库的可行性和密闭性评价提供有利支撑。 | 刘伟 MUHAMMAD Nawaz 李银平 SPIERS C J 杨春和 马洪岭 | 2014 | 岩石力学与工程学报2014,33,10: | 14 |
| 3 | Single vs dual(en bloc) kidney transplants from donors ≤ 5 years of age: A single center experience显示文摘AIM: To compare outcomes between single and dual en bloc(EB) kidney transplants(KT) from small pediatric donors. METHODS: Monocentric nonprospective review of KTs from pediatric donors ≤ 5 years of age. Dual EB KT was defined as keeping both donor kidneys attached tothe inferior vena cava and aorta, which were then used as venous and arterial conduits for the subsequent transplant into a single recipient. Donor age was less useful than either donor weight or kidney size in decision-making for kidney utilization as kidneys from donors < 8 kg or kidneys < 6 cm in length were not transplanted. Post-transplant management strategies were standardized in all patients.RESULTS: From 2002-2015, 59 KTs were performed including 34 dual EB and 25 single KTs. Mean age of donors(17 mo vs 38 mo, P < 0.001), mean weight(11.0 kg vs 17.4 kg, P = 0.046) and male donors(50% vs 84%, P = 0.01) were lower in the dual EB compared to the single KT group, respectively. Mean cold ischemia time(21 h), kidney donor profile index(KDPI; 73% vs 62%) and levels of serum creatinine(SCr, 0.37 mg/d L vs 0.49 mg/d L, all P = NS) were comparable in the dual EB and single KT groups, respectively. Actuarial graft and patient survival rates at 5-years follow-up were comparable. There was one case of thrombosis resulting in graft loss in each group. Delayed graft function incidence(12% dual EB vs 20% single KT, P = NS) was slightly lower in dual EB KT recipients. Initial duration of hospital stay(mean 5.4 d vs 5.6 d) and the one-year incidences of acute rejection(6% vs 16%), operative complications(3% vs 4%), and major infection were comparable in the dual EB and single KT groups, respectively(all P = NS). Mean 12 mo SCr and abbreviated MDRD levels were 1.17 mg/d L vs 1.35 mg/d L and 72.5 m L/min per 1.73 m^2 vs 60.5 m L/min per 1.73 m^2(both P = NS) in the dual EB and single KT groups, respectively. | 5 | Autoregressive neural networks with exogenous variables for indoor temperature prediction in buildings显示文摘Thermal models of buildings are helpful to forecast their energy use and to enhance the control of their mechanical systems.However,these models are building-specific and require a tedious,error-prone and time-consuming development effort relying on skilled building energy modelers.Compared to white-box and gray-box models,data-driven(black-box)models require less development time and a minimal amount of information about the building characteristics.In this paper,autoregressive neural network models are compared to gray-box and black-box linear models to simulate indoor temperatures.These models are trained,validated and compared to actual experimental data obtained for an existing commercial building in Montreal(QC,Canada)equipped with roof top units for air conditioning.Results show that neural networks mimic more accurately the thermal behavior of the building when limited information is available,compared to gray-box and black-box linear models.The gray-box model does not perform adequately due to its under-parameterized nature,while the linear models cannot capture non-linear phenomena such as radiative heat transfer and occupancy.Therefore,the neural network models outperform the alternative models in the presented application,reaching a coefficient of determination R2 up to 0.824 and a root mean square error down to 1.11℃,including the error propagation over time for a 1-week period with a 5-minute time-step.When considering a 50-hour time horizon,the best neural networks reach a much lower root mean square error of around 0.6℃,which is suitable for applications such as model predictive control. | Benoit Delcroix Jérôme Le Ny Michel Bernier Muhammad Azam Bingrui Qu Jean-Simon Venne | 2021 | Building Simulation2021,14,1: | 3 |
| 6 | Interferon-β induced in female genital epithelium by HIV-1 glycoprotein 120 via Toll-like-receptor 2 pathway acts to protect the mucosal barrier显示文摘More than 40%of HIV infections occur via female reproductive tract(FRT)through heterosexual transmission.Epithelial cells that line the female genital mucosa are the first line of defense against HIV-1 and other sexually transmitted pathogens.These sentient cells recognize and respond to external stimuli by induction of a range of carefully balanced innate immune responses.Previously,we have shown that in response to HIV-1 gp120,the genital epithelial cells(GECs)from upper reproductive tract induce an inflammatory response that may facilitate HIV-1 translocation and infection.In this study,we report that the endometrial and endocervical GECs simultaneously induce biologically active interferon-β(IFNβ)antiviral responses following exposure to HIV-1 that act to protect the epithelial tight junction barrier.The innate antiviral response was directly induced by HIV-1 envelope glycoprotein gp120 and addition of gp120 neutralizing antibody inhibited IFNβproduction.Interferon-βwas induced by gp120 in upper GECs through Toll-like receptor 2 signaling and required presence of heparan sulfate on epithelial cell surface.The induction of IFNβwas dependent upon activation of transcription factor IRF3(interferon regulatory factor 3).The IFNβwas biologically active,had a protective effect on epithelial tight junction barrier and was able to inhibit HIV-1 infection in TZM-bl indicator cells and HIV-1 replication in T cells.This is the first report that recognition of HIV-1 by upper GECs leads to induction of innate antiviral pathways.This could explain the overall low infectivity of HIV-1 in the FRT and could be exploited for HIV-1 prophylaxis. | Aisha Nazli Sara Dizzell Muhammad Atif Zahoor Victor H Ferreira Jessica Kafka Matthew William Woods Michel Ouellet Ali A Ashkar Michel J Tremblay Dawn ME Bowdish Charu Kaushic | 2019 | Cellular & Molecular Immunology2019,16,2: | 2 |
