| 1 | Probiotic supplementation decreases intestinal transit time: Meta-analysis of randomized controlled trials显示文摘AIM: To determine the efficacy of probiotic supplementation on intestinal transit time (ITT) and to identify factors that influence these outcomes. METHODS: A systematic review of randomized controlled trials (RCTs) of probiotic supplementation that measured ITT in adults was conducted by searching MEDLINE and EMBASE using relevant combinations. Main search limits included RCTs of probiotic supplementation in healthy or constipated adults that measured ITT. Study quality was assessed using the Jadad scale. A random effects meta-analysis was performed with standardized mean difference (SMD) of ITT between probiotic and control groups as the primary outcome. Meta-regression and subgroup analyses were conducted to examine the impact of moderator variables on ITT SMD. RESULTS: A total of 11 clinical trials with 13 treatment effects representing 464 subjects were included in this analysis. Probiotic supplementation was associated with decreased ITT in relation to controls, with an SMD of 0.40 (95%CI: 0.20-0.59, P < 0.001). Constipation (r 2 = 39%, P = 0.01), higher mean age (r 2 = 27%, P = 0.03), and higher percentage of female subjects (r 2 = 23%, P < 0.05) were predictive of decreased ITT with probiotics in meta-regression. Subgroup analyses demonstrated statistically greater reductions in ITT with probiotics in subjects with vs without constipation and in older vs younger subjects [both SMD: 0.59 (95%CI: 0.39-0.79) vs 0.17 (95%CI: -0.08-0.42), P = 0.01]. Medium to large treatment effects were identified with Bifidobacterium Lactis (B. lactis ) HN019 (SMD: 0.72, 95%CI: 0.27-1.18, P < 0.01) and B. lactis DN-173 010 (SMD: 0.54, 95%CI: 0.15-0.94, P < 0.01) while other single strains and combination products yielded small treatment effects. | 2 | Real-time three-dimensional myocardial contrast echocardiography in assessment of myocardial perfusion defects显示文摘Background Both real-time three-dimensional echocardi ography (RT3DE) and myocardial contrast echocardiography (MCE) are novel imaging techniques. The purpose of this study was to confirm the feasibility and accuracy of RT3DE combined with MCE for quantitative evaluation of myocardial perfusion defects. Methods Thirteen dogs underwent ligation of the left anterior descending artery (LAD, n=6) or distal branch of the left circumflex artery (LCX, n=7) under general anaesthesia. Three to four ml of a perfluoropropane (C 3F 8) microbubble contrast agent was injected intravenously to assess the resulting myocardial perfusion defects with a commercially available Philips SONOS-7500 ultrasound system. After removal of the dog hearts, Evans blue dye was injected via the left and righ t coronary arteries to stain the myocardium at risk. In vitro anatomic measurements of myocardial mass after removal of the animals’ hearts were used as control s. Results Left ventricular (LV) mass determined by RT3DE ranged 36.7-68.9 g [mean, (54.6±9.6) g] before coronary artery ligation, and correlated highly (r=0.99) with in vitro measurement of LV mass [range, 38.9-71.1 g; mean, (55.6±9.3) g]. There was no significant difference between RT3DE and in vitro measurements of LV mass [range, 36.7-68.9 g; mean, (51.3±12.5) g. Or range, 38.9-71. 1 g; mean, (53.7±12.3) g, respectively] and under-perfused mass [range, 0-21.4 g; mean, (12.0±6.9) g. Or range, 0-19.8 g; mean, (10.8±6.3) g, respectively] after th e LAD ligation (P>0.05). Likewise, no significant difference was present between RT3DE and in vitro measurements of LV mass [range, 50.1-65.4 g; mean, (57.5±5.9 ) g. Or range, 51.5-65.8 g; mean, (57.3±6.4) g, respectively] and under-perfused m ass [range, 0-25.6 g; mean, (13.3±9.6) g. Or range, 0-22.7 g; mean, (12.8±8.1 ) g, respectively] after the LCX ligation (P>0.05). For all the animals with coronary ligation, LV mass measured by RT3DE ranged 35.9-68.6 g [mean, (54.8±10.0) g] a nd there was no significant difference between RT3DE and in vitro measurements of LV mass and under-perfused mass (P>0.05, r=0.99). Further, the under-perfused mass derived from RT3DE [range, 0-25.6 g; mean, (12.7±8.2) g] correlate d strongly with the in vitro measurements [range, 0-22.7 g; mean, (11.9±7.2) g] ( r=0.96). | 3 | Incidence of brain metastasis in patients with esophageal carcinoma显示文摘AIM:To determine the incidence of brain metastasis in a contemporary group of patients with carcinoma of the esophagus.METHODS:Retrospective analysis of 53 patients with esophageal carcinoma who received radiotherapy as a component of treatment between 1998 and 2007,including patient and tumor characteristics,and subsequent diagnosis of brain metastasis.The association between the histological type of esophageal cancer and the incidence of brain metastasis was assessed using Fisher's exact test.RESULTS:Forty-four of the fifty-three patients in this study had adenocarcinoma and nine had squamous cell carcinoma,ranging from stage ⅡA-ⅣB.Primary treatment was surgery with neoadjuvant chemoradiotherapy (trimodality therapy) in 19% of patients;chemoradiotherapy in 42%;and surgery and adjuvant radiotherapy in 7%.Twenty-five percent of patients inthis study received palliative radiotherapy.The overall incidence of brain metastasis in this cohort was 13%.Adenocarcinoma was the primary tumor histology in all of the patients who developed brain metastasis,representing an incidence of 16% in this subgroup.No patients with squamous cell carcinoma received trimodality therapy.The association between histology and brain metastasis was not statistically significant.CONCLUSION:The incidence of brain metastasis in this contemporary cohort of patients with esophageal carcinoma is higher than previously reported and was confined to those with adenocarcinoma. | Ron S Smith Robert C Miller | 2011 | World Journal of Gastroenterology2011,17,19: | 6 |

| 6 | Variations in zonal fruit starch concentrations of apples – a developmental phenomenon or an indication of ripening?显示文摘Patterns of starch hydrolysis in stem,equatorial,and calyx zones of‘Honeycrisp’and‘Empire’apples(Malus sylvestris(L.)Mill var.domestica(Borkh.)Mansf.)during maturation and ripening,and in‘Gala’apples in response to propylene or 1-methylcyclopropene(1-MCP)treatments after harvest,were studied.Differences in zonal starch concentrations were found for‘Empire’and‘Gala’fruits,but not for‘Honeycrisp’.During maturation and ripening of‘Empire’,the concentration of starch was highest in the calyx end and lowest in the stem region.Differences in rates of starch hydrolysis among zones were not detected.‘Honeycrisp’and‘Empire’had the highest concentration of sorbitol in the calyx region,whereas it was highest in the stem-end region in‘Gala’.The distribution differences of glucose,fructose,and sucrose were similar in all three cultivars;higher fructose and glucose concentrations in the stem region,and higher sucrose concentrations in the calyx end of the fruit.Postharvest treatment of‘Gala’with propylene did not affect the internal ethylene concentration of the fruit but 1-MCP markedly inhibited it.Starch concentrations were highest in the calyx end but gradients of starch among zones were not changed by postharvest treatment.The rate of hydrolysis was slowed by 1-MCP treatment,but was unaffected by propylene.Postharvest treatments influenced sorbitol,glucose,and fructose concentrations.Patterns of starch concentration among the zones did not confirm differences in ripening,but reflected its uneven distribution throughout the fruit during development.Therefore,measured differences in zonal starch are most likely related to starch accumulation during fruit development,rather than differences in rates of starch degradation during ripening. | Franziska C Doerflinger William B Miller Jacqueline F Nock Christopher B Watkins | 2015 | Horticulture Research2015,2,1: | 3 |

Meta-regression and subgroup analyses examined the impact of moderator variables on SMD of ITT.RESULTS: A total of 15 clinical trials with 17 treatment effects representing 675 subjects were included in this analysis. Probiotic supplementation was moderately efficacious in decreasing ITT compared to control, with an SMD of 0.38(95%CI: 0.23-0.53, P < 0.001). Subgroup analyses demonstrated statistically greater reductions in ITT with probiotics in subjects with vs without constipation(SMD: 0.57 vs 0.22, P < 0.01) and in studies with high vs low study quality(SMD: 0.45 vs 0.00, P = 0.01). Constipation(R^2 = 38%, P < 0.01), higher study quality(R^2 = 31%, P = 0.01), older age(R^2 = 27%, P = 0.02), higher percentage of female subjects(R^2 = 26%, P = 0.02), and fewer probiotic strains(R^2 = 20%, P < 0.05) were predictive of decreased ITT with probiotics in meta-regression. Medium to large treatment effects were identified with B. lactis HN019(SMD: 0.67, P < 0.001) and B. lactis DN-173 010(SMD: 0.54, P < 0.01) while other probiotic strains yielded negligible reductions in ITT relative to control.CONCLUSION: Probiotic supplementation is moderately efficacious for reducing ITT in adults.
