| 1 | 【原创论文】不育夫妇中男方的代谢综合征与前列腺异常的情况显示文摘目前尚无年轻男性不育症患者代谢综合征与前列腺相关的症状和体征之间关系的系统研究。我们研究了171对不育夫妇中的男性(36.5±8.3岁),代谢综合征是根据国家胆固醇教育计划成人治疗专题小组第三次报告确定。所有人接受了激素(包括总睾酮和胰岛素)、精液f包括白细胞介素8,精浆中白细胞介素8(SIL-8))、阴囊及经直肠超声评估。因为我们此前已有大样本队列研究了不育男性代谢综合征和阴囊检测指标之间的相关性,这里我们重点研究代谢综合征与经直肠前列腺B超资料的相关性。前列腺相关症状采用美国国立卫生研究院评估慢性前列腺炎症状指数(NIH-CPSI)和国际前列腺症状评分(iPSS)。22位受试者符合代谢综合征的诊断标准。经年龄调整后的有序多项Logistic回归模型分析显示:胰岛素水平增加是代谢综合征的一个组分(Wald=29.5,P〈0.0001)并与TT负相关(调整后的r=0.359,P〈O.0001)。未观察到代谢综合征和NIH.CPSI或IPSS评分之间有相关性。经年龄、睾酮、胰岛素校正后有序多项Logistic回归模型显示:代谢综合征组分数目的增加与正常精子形态负相关(Wald=5.59,P〈0.02)、与前列腺炎症的分子标志物血清sIL-8水平正相关(Wald=4.32,P〈0.05),与前列腺总体积和移行带体积(Wald=17.6和12.5,P〈0.0001)、动脉收缩期峰值流速(Wald=9.57,P=0.002)、质地不均匀性(HR=8711.05—3.33],P〈0.05)、钙化灶大小(Wald=3.11,P〈0.05)正相关,但不与精囊体积或功能指标正相关。总之,不育夫妇中的男性代谢综合征与前列腺增大、生化指标(sIL8)和前列腺炎症的超声指标呈正相关,而与前列腺相关症状无正相关,表明代谢综合征可能是良性前列腺增生症的早期亚临床表现的触发点。 | Francesco Lotti Giovanni Corona Linda Vignozzi Matteo Rossi Elisa Maseroli Sarah Cipriani Mauro Gacci Gianni Forti Mario Maggi | 2014 | Asian Journal of Andrology2014，16，2： | 6 |
| 2 | 前列腺增大与性功能障碍的临床相关性显示文摘前列腺直肠指检可以为了解前列腺的生长状态和发现可疑的外周结节提供有用的信息。本实验的目的在于研究经直肠指检发现前列腺增大与性功能障碍(SD)的临床和生化相关性。对一组2379例患者进行回顾性分析。样本(n=1823;平均年龄为54.7±11.4岁)选自无明显前列腺疾病的人群。对几个指标进行了研究。经过校正后,经直肠指检发现的前列腺增大与代谢终合症(HR=1.346[1.129.1.759];P=0.030)、二型糖尿病(HR=I.489[1.120.1.980];P=0.006)、低密度脂蛋白胆固醇增高(〉100mgdl^-1;HR=1.354[1.018—1.801];P=0.037)及平均血压增高(HR=1.017n.007.1.027]每增加1mmHg;P=0.001)的高发病存在相关性。因此,前列腺增大也与动脉性勃起功能障碍及其他的男科疾病的高发病相关,如精索静脉曲张和早泄。在前列腺增大的人群中PSA水平显著高于非前列腺增大的人群(HR=3-318[2.304;4.799]每增加1单位PSA;P〈0.0001)。根据不同标准,动脉性ED也与PSA水平升高相关。结论:我们的研究结果表明对于ED患者,进行前列腺大小的检查,包括临床(DRE)和生化(PSA)检查是必要的,以此能对患者的性功能障碍和代谢及心血管背景有全面的了解。 | Giovanni Corona Mauro Gacci Elisa Maseroli Giulia Rastrelli Linda Vignozzi Alessandra Sforza Gianni Forti Edoardo Mannucci Mario Maggi | 2014 | Asian Journal of Andrology2014，16，5： | 1 |

| 5 | Contemporary techniques and outcomes of surgery for locally advanced renal cell carcinoma with focus on inferior vena cava thrombectomy:The value of a multidisciplinary team显示文摘Objective:To report the outcomes of surgery for a contemporary series of patients with locally advanced non-metastatic renal cell carcinoma(RCC)treated at a referral academic centre,focusing on technical nuances and on the value of a multidisciplinary team.Methods:We queried our prospective institutional database to identify patients undergoing surgical treatment for locally advanced(cT3-T4 N0-1 M0)renal masses suspected of RCC at our centre between January 2017 and December 2020.Results:Overall,32 patients were included in the analytic cohort.Of these,12(37.5%)tumours were staged as cT3a,8(25.0%)as cT3b,5(15.6%)as cT3c,and 7(21.9%)as cT4;6(18.8%)patients had preoperative evidence of lymph node involvement.Nine(28.1%)patients underwent nephron-sparing surgery while 23(71.9%)received radical nephrectomy.A template-based lymphadenectomy was performed in 12 cases,with evidence of disease in 3(25.0%)at definitive histopathological analysis.Four cases of RCC with level IV inferior vena cava thrombosis were successfully treated using liver transplant techniques without the need for extracorporeal circulation.While intraoperative complications were recorded in 3(9.4%)patients,no postoperative major complications(Clavien-Dindo3)were observed.At histopathological analysis,2(6.2%)patients who underwent partial nephrectomy harboured oncocytoma,while the most common malignant histotype was clear cell RCC(62.5%),with a median Leibovich score of 6(interquartile range 5e7).Conclusion:Locally advanced RCC is a complex and heterogenous disease posing several challenges to surgical teams.Our experience confirms that provided careful patient selection,surgery in experienced hands can achieve favourable perioperative,oncological,and functional outcomes. | Riccardo Campi Paolo Barzaghi Alessio Pecoraro Maria Lucia Gallo Damiano Stracci Alberto Mariotti Saverio Giancane Simone Agostini Vincenzo Li Marzi Arcangelo Sebastianelli Pietro Spatafora Mauro Gacci Graziano Vignolini Francesco Sessa Paolo Muiesan Sergio Serni | 2022 | Asian Journal of Urology2022，9，3： | 1 |
