| 1 | 考虑职业倦怠的护士夜班指派问题显示文摘护士排班问题旨在满足一系列如法律法规、患者需求等约束条件的前提下,制定出在特定时间段内最优的护士值班表。职业倦怠是指服务于助人行业的人们因工作时间过长、工作量过大、工作强度过高所经历的一种疲惫不堪的状态。护士经常经历疾病和死亡,不可避免地加班和上夜班,所以较容易产生职业倦怠。主要研究考虑职业倦怠的护士排班问题。前期研究表明夜班对护士的职业倦怠有较显著的影响,应用马尔科夫决策过程建立了考虑职业倦怠的护士夜班指派模型。该模型在考虑患者需求的前提下在特定时间段内为一组不同倦怠水平的护士指派夜班。利用夜班指派模型输出的最佳指派方案进行了仿真,并与几种常用的不考虑倦怠因素的指派方案进行了比较分析,最后对最优指派策略进行了敏感度分析,验证了考虑职业倦怠的护士夜班指派模型的实用价值与意义。 | 朱嫣然 耿娜 Andrea Matta | 2018 | 工业工程与管理2018,23,4: | 9 |

| 4 | In vitro effect of amoxicillin and clarithromycin on the 3' region of cagA gene in Helicobacter pylori isolates显示文摘AIM:To evaluate the in vitro effect of amoxicillin and clarithromycin on the cag pathogenicity island(cag PAI).METHODS:One hundred and forty-nine clinical isolates of Helicobacter pylori(H.pylori)cultured from gastric biopsies from 206 Colombian patients with dyspeptic symptoms from a high-risk area for gastric cancer were included as study material.Antimicrobial susceptibility was determined by the agar dilution method.Resistant isolates at baseline and in amoxicillin and clarithromycin serial dilutions were subjected to genotyping(cagA,vacA alleles s and m),Glu-Pro-Ile-Tyr-Ala(EPIYA)polymerase chain reaction and random amplified polymorphic DNA(RAPD).Images of the RAPD amplicons were analyzed by Gel-Pro Analyzer 4.5program.Cluster analyses was done using SPSS 15.0statistical package,where each of the fingerprint bands were denoted as variables.Dendrograms were designed by following Ward’s clustering method and the estimation of distances between each pair of H.pylori isolates was calculated with the squared Euclidean distance.RESULTS:Resistance rates were 4%for amoxicillin and 2.7%for clarithromycin with 2%double resistances.Genotyping evidenced a high prevalence of the genotype cagA-positive/vacA s1m1.The 3’region of cagA gene was successfully amplified in 92.3%(12/13)of the baseline resistant isolates and in 60%(36/60)of the resistant isolates growing in antibiotic dilutions.Upon observing the distribution of the number of EPIYA repetitions in each dilution with respect to baseline isolates,it was found that in 61.5%(8/13)of the baseline isolates,a change in the number of EPIYA repetitions lowered antibiotic pressure.The gain and loss of EPIYA motifs resulted in a diversity of H.pylori subclones after bacterial adjustment to changing conditions product of antibiotic pressure.RAPD PCR evidenced the close clonal relationship between baseline isolates and isolates growing in antibiotic dilutions.CONCLUSION:Antibiotic pressure does not induce loss of the cag pathogenicity island,but it can leadin most cases-to genetic rearrangements within the3’region cagA of the founding bacteria that can affect the level of tyrosine phosphorylation impacting on its cellular effects and lead to divergence of cagA-positive subclones. | Javier Andrés Bustamante-Rengifo Andrés Januer Matta Alvaro Pazos Luis Eduardo Bravo | 2013 | World Journal of Gastroenterology2013,19,36: | 5 |
| 5 | Punctual mutations in 23S rRNA gene of clarithromycinresistant Helicobacter pylori in Colombian populations显示文摘AIM To characterize punctual mutations in 23S rRNA gene of clarithromycin-resistant Helicobacter pylori(H. pylori) and determine their association with therapeutic failure.METHODS PCR products of 23S rRNA gene V domain of 74 H. pylori isolates; 34 resistant to clarithromycin(29 from a low-risk gastric cancer(GC) population: TumacoColombia, and 5 from a high-risk population: TuquerresColombia) and 40 from a susceptible population(28 from Tumaco and 12 from Túquerres) were sequenced using capillary electrophoresis. The concordance between mutations of V domain 23S rRNA gene of H. pylori and therapeutic failure was determined using the Kappa coefficient and Mc Nemar's test was performed to determine the relationship between H. pylori mutationsand clarithromycin resistance.RESULTS23S rRNA gene from H. pylori was amplified in 56/74 isolates, of which 25 were resistant to clarithromycin(20 from Tumaco and 5 from Túquerres, respectively). In 17 resistant isolates(13 from Tumaco and 4 from Túquerres) the following mutations were found: A1593 T1, A1653 G2, C1770 T, C1954 T1, and G1827 C in isolates from Tumaco, and A2144 G from Túquerres. The mutations T2183 C, A2144 G and C2196 T in H. pylori isolates resistant to clarithromycin from Colombia are reported for the first time. No association between the H. pylori mutations and in vitro clarithromycin resistance was found. | 6 | Predictors of recurrent bile duct stone after clearance by endoscopic retrograde cholangiopancreatography:A case-control study显示文摘Background:Recurrent common bile duct(CBD)stone is a long-term sequalae among patients who un-derwent endoscopic retrograde cholangiopancreatography(ERCP)with CBD stone extraction.Data regard-ing risk factors for recurrent CBD stone are scarce.We aimed to identify predictors of recurrent CBD stone.Methods:We performed a retrospective case-controlled study from January 2010 to December 2019.In-clusion criteria included patients who had recurrent CBD stone at least 6 months after the index ERCP,in which complete stone extraction was performed and normal cholangiogram was obtained.Overall,457 patients were included.Forty-two patients(9.2%)had recurrent CBD stone,and 415 patients(90.8%)did not have recurrent CBD stone.Results:In univariate analysis,male sex[odds ratio(OR)=0.49,P=0.033]was a protective factor,while endoscopic stone extraction by basket vs.balloon(OR=2.55,P=0.005),older age(OR=1.03,P=0.003),number of CBD stones(OR=1.99,P=0.037),size of CBD stone(OR=4.06,P=0.003)and mechanical lithotripsy(OR=9.22,P=0.004)were risk factors for recurrent CBD stone.In multivariate logistic regression analysis,mechanical lithotripsy[OR=9.73,95%confidence interval(CI):1.69-55.89,P=0.010],basket clearance vs.combined basket and balloon(OR=18.25,95%CI:1.05-318.35,P=0.046)and older age(OR=1.02,95%CI:1.00-1.05,P=0.023)were risk factors,and male sex(OR=0.39,95%CI:0.19-0.81,P=0.012)was a protective factor. | Wisam Sbeit Anas Kadah Matta Simaan Amir Shahin Tawfik Khoury | 2022 | Hepatobiliary & Pancreatic Diseases International2022,21,1: | 5 |
| 7 | Starch Digestibility and Physicochemical and Cooking Properties of Irradiated Rice Grains显示文摘This study evaluated the starch digestibility, physicochemical properties and cooking characteristics of polished rice(varieties IRGA417 and IAC202) subjected to the doses of 0(as the control), 1, 2 and 5 k Gy of gamma radiation. The highest dose decreased the apparent amylose content, peak viscosity, water absorption and volume expansion. Irradiation increased the solid loss by 119% and 187% for IAC202 and IRGA417, respectively, when comparing the higher dose with the control. For IAC202, irradiation decreased the rapidly digestible starch and increased the slowly digestible starch(SDS) and resistant starch(RS). IRGA417 showed an elevation of SDS and a reduction in RS. | 8 | Safety aware fuzzy longitudinal controller for automated vehicles显示文摘Vehicle automation holds significant opportunities in the efforts for safer road traffic.Even lower levels of automation like adaptive cruise control(ACC)systems have great potential.Apart from safety benefits,ACC systems could improve the efficiency of traffic networks.However,contemporary ACC systems are designed to be comfortable,but not guaranteed to be safe.Moreover,according to recent literature studies,they have been found to be conservative and string unstable,with traffic oscillations being amplified when traveling downstream.Due to those issues,severe problems can arise in the traffic flow and the efficiency of road traffic networks may deteriorate.Automating the vehicles’longitudinal movement through ADAS systems offers new opportunities in improving the safety conditions on the roads by designing more consistent controllers.In the present paper,a fuzzy controller is developed for automated longitudinal vehicle control.The controller is validated through simulation and real-world data of ACC systems that are currently available in the market.Using fuzzy surrogate safety metrics,the controller becomes aware of the safety level of any situation and can react properly.Moreover,the driving behavior is efficient for the traffic flow,comfortable and string stable,dampening traffic shockwaves that travel upstream.Results prove the robustness of the proposed solution. | Konstantinos Mattas George Botzoris Basil Papadopoulos | 2021 | Journal of Traffic and Transportation Engineering(English Edition)2021,8,4: | 2 |
| 9 | Genomic variability of Helicobacter pylori isolates of gastric regions from two Colombian populations显示文摘AIM To compare the genomic variability and the multiple colonization of Helicobacter pylori(H. pylori) in patients with chronic gastritis from two Colombian populations with contrast in the risk of developing gastric cancer(GC): Túquerres-Nari?o(High risk) and Tumaco-Nari?o(Low risk).METHODS Four hundred and nine patients from both genders with dyspeptic symptoms were studied. Seventy-two patients were included in whom H. pylori was isolated from three anatomic regions of the gastric mucosa,(31/206) of the high risk population of GC(Túquerres) and(41/203) of the low risk population of GC(Tumaco). The isolates were genotyped by PCR-RAPD. Genetic diversity between the isolates was evaluated by conglomerates analysis and multiple correspondence analyses. RESULTS The proportion of virulent genotypes of H. pylori was 99% in Túquerres and 94% in Tumaco. The coefficient of similarity of Nei-Li showed greater genetic diversityamong isolates of Túquerres(0.13) than those of Tumaco(0.07). After adjusting by age, gender and type of gastritis, the multiple colonization was 1.7 times more frequent in Túquerres than in Tumaco(P = 0.05).CONCLUSION In Túquerres, high risk of GC there was a greater probability of multiple colonization by H. pylori.
