1Pathogenesis and management issues for non-alcoholic fatty liver disease显示文摘Nonalcoholic fatty liver disease (NAFLD) has, although it is a very common disorder, only relatively recently gained broader interest among physicians and scientists. Fatty liver has been documented in up to 10 to 15 percent of normal individuals and 70 to 80 percent of obese individuals. Although the pathophysiology of NAFLD is still subject to intensive research, several players and mechanisms have been suggested based on the substantial evidence. Excessive hepatocyte triglyceride accumulation resulting from insulin resistance is the first step in the proposed 'two hit' model of the pathogenesis of NAFLD. Oxidative stress resulting from mitochondrial fatty acids oxidation, NF-κB-dependent inflammatory cytokine expression and adipocytokines are all considered to be the potential factors causing second hits which lead to hepatocyte injury, inflammation and fibrosis. Although it was initially believed that NAFLD is a completely benign disorder, histologic follow-up studies have showed that fibrosis progression occurs in about a third of patients. A small number of patients with NAFLD eventually ends up with end-stage liver disease and even hepatocellular carcinoma. Although liver biopsy is currently the only way to confirm the NAFLD diagnosis and distinguish between fatty liver alone and NASH, no guidelines or firm recommendations can still be made as for when and in whom it is necessary. Increased physical activity, gradual weight reduction and in selected cases bariatric surgery remain the mainstay of NAFLD therapy. 2Adipokines and proinflammatory cytokines, the key mediators in the pathogenesis of nonalcoholic fatty liver disease显示文摘Nonalcoholic fatty liver disease(NAFLD)is a condition in which excess fat accumulates in the liver of a patient with no history of alcohol abuse or other causes for secondary hepatic steatosis.The pathogenesis of NAFLD and nonalcoholic steatohepatitis(NASH)has not been fully elucidated.The'two-hit'hypothesis is probably a too simplified model to elaborate complex pathogenetic events occurring in patients with NASH.It should be better regarded as a multiple step process,with accumulation of liver fat being the first step,followed by the development of necroinflammation and fibrosis.Adipose tissue,which has emerged as anendocrine organ with a key role in energy homeostasis,is responsive to both central and peripheral metabolic signals and is itself capable of secreting a number of proteins.These adipocyte-specific or enriched proteins,termed adipokines,have been shown to have a variety of local,peripheral,and central effects.In the current review,we explore the role of adipocytokines and proinflammatory cytokines in the pathogenesis of NAFLD.We particularly focus on adiponectin,leptin and ghrelin,with a brief mention of resistin,visfatin and retinol-binding protein 4 among adipokines,and tumor necrosis factor-α,interleukin(IL)-6,IL-1,and briefly IL-18 among proinflammatory cytokines.We update their role in NAFLD,as elucidated in experimental models and clinical practice.Sanja Stojsavljevi? 3Preventing post-endoscopic retrograde cholangiopancreatography pancreatitis: What can be done?显示文摘Post-endoscopic retrograde cholangiopancreatographypancreatitis(PEP) is the most common complication of endoscopic retrograde cholangiopancreatography. The incidence of post-endoscopic retrograde cholangiopancreatography(ERCP) pancreatitis varies substantially and is reported around 1%-10%, although there are some reports with an incidence of around 30%. Usually, PEP is a mild or moderate pancreatitis, but in some instances it can be severe and fatal. Generally, it is defined as the onset of new pancreatictype abdominal pain severe enough to require hospital admission or prolonged hospital stay with levels of serum amylase two to three times greater than normal, occurring 24 h after ERCP. Several methods have been adopted for preventing pancreatitis, such as pharmacological or endoscopic approaches. Regarding medical prevention, only non-steroidal anti-inflammatory drugs, namely diclofenac sodium and indomethacin, are recommended, but there are some other drugs which have some potential benefits in reducing the incidence of post-ERCP pancreatitis. Endoscopic preventive measures include cannulation(wire guided) and pancreatic stenting, while the adoption of the early pre-cut technique is still arguable. 5Role of Ki-67 as a prognostic factor in gastrointestinal stromal tumors显示文摘AIM:To investigate primarily the prognostic value of Ki-67,as well as other parameters,in gastrointestinal stromal tumors(GISTs).METHODS:Ki-67,c-KIT,platelet-derived growth factor receptor-alpha(PDGFRα),smooth muscle actin(SMA),CD34,S100 were stained for immunohistochemistry which was performed on formalin-fixed,paraffinembeded sections on representative block from each case.Proliferation index counted by Ki-67 antibody was calculated as a number of positive nuclear reaction over 100 cells.Immunoreactivity for c-KIT and PDGFRα was evaluated semiquantitatively(weak,intermediate,strong) and for c-KIT type of reactivity was analyzed(cytoplasmic,membrane and 'dot-like' staining).Immunoreactivity for SMA,CD34 and S100 were was evaluated as positive or negative antigen expression.Pathologic parameters investigated in this study included tumor size,cell type(pure spindle,pured epitheloid mixed spindle and epitheloid),mitotic count,hemorrhage,necrosis,mucosal ulceration.Clinical data included age,gender,primary tumor location and spread of disease.χ 2 test and Student's t-test were used for comparisons of baseline characteristics.The Cox's proportional hazard model was used for univariable and multivariable analyses.Survival rates were calculated by Kaplan-Meier method and statistical significance was determined by the log-rank test.RESULTS:According to the stage of disease,there were 36 patients with localized disease,29 patients with initially localized disease but with its recurrence in the period of follow up,and finally,35 patients had metastatic disease from the very beginning of disease.Tumor originated most commonly in the stomach(41%),small intestine was the second most common location(36%).The mean size of primary tumors was 6.5 cm.The mean duration of follow-up was 60 mo.Multiple parameters were analyzed for their effect on overall survival,but no one reached statistical significance(P = 0.06).Analysis of time to progression/relapse in initially localized disease(univariate analysis),tumor size,mitotic count,Ki-67 and type of d-KIT distribution(cytoplasmic vs membrane/'dot-like') showed statistically significant correlation.In multivariate analysis in the group of patients with localized disease,there were only 2 parameters that have impact on relapse,Ki-67 and SMA(P < 0.0001 and P < 0.034,respectively).Furthermore,Ki-67 was analyzed in localized diseasevs localized with recurrence and metastatic disease.It was shown that there is a strict difference between these 2 groups of patients(median value was 2.5 for localized disease vs 10.0 for recurrent/metastatic disease,P < 0.0001).It was also shown that the cut-off value which is still statistically significant in terms of relapse on the level of 6%.The curves for survival on that cut-off level are significantly different(P < 0.04,Cox F).CONCLUSION:Ki-67 presents a significant prognostic factor for GIST recurrence which could be of great importance in evaluating malignant potential of disease.Borislav Belev Iva Bri Juraj Prejac Zrna Antunac Golubi Damir Vrbanec Jadranka Bozikov Ivan Aleri Marko Boban Jasminka Jaki Razumovi 2013World Journal of Gastroenterology2013,19,4:14
6Endoclips vs large or small-volume epinephrine in peptic ulcer recurrent bleeding显示文摘AIM:To compare the recurrent bleeding after endoscopic injection of different epinephrine volumes with hemoclips in patients with bleeding peptic ulcer.METHODS:Between January 2005 and December 2009,150 patients with gastric or duodenal bleeding ulcer with major stigmata of hemorrhage and nonbleeding visible vessel in an ulcer bed(Forrest Ⅱa) were included in the study.Patients were randomized to receive a small-volume epinephrine group(15 to 25 mL injection group;Group 1,n = 50),a large-volume epinephrine group(30 to 40 mL injection group;Group 2,n = 50) and a hemoclip group(Group 3,n = 50).The rate of recurrent bleeding,as the primary outcome,was compared between the groups of patients included in the study.Secondary outcomes compared between the groups were primary hemostasis rate,permanent hemostasis,need for emergency surgery,30 d mortality,bleeding-related deaths,length of hospital stay and transfusion requirements.RESULTS:Initial hemostasis was obtained in all patients.The rate of early recurrent bleeding was 30%(15/50) in the small-volume epinephrine group(Group 1) and 16%(8/50) in the large-volume epinephrine group(Group 2)(P = 0.09).The rate of recurrent bleeding was 4%(2/50) in the hemoclip group(Group 3);the difference was statistically significant with regard to patients treated with either small-volume or large-volume epinephrine solution(P = 0.0005 and P = 0.045,respectively).Duration of hospital stay was significantly shorter among patients treated with hemoclips than among patients treated with epinephrine whereas there were no differences in transfusion requirement or even 30 d mortality between the groups.CONCLUSION:Endoclip is superior to both small and large volume injection of epinephrine in the prevention of recurrent bleeding in patients with peptic ulcer.Neven Ljubicic Ivan Budimir Alen Biscanin Marko Nikolic Vladimir Supanc Davor Hrabar Tajana Pavic 2012World Journal of Gastroenterology2012,18,18:11
7Antibiotic Resistance Genes in the Human-Impacted Environment:A One Health Perspective显示文摘Antibiotic resistance and its environmental component are gaining more attention as part of combating the growing healthcare crisis. The One Health framework, promulgated by many global health agencies, recognizes that antimicrobial resistance is a truly inter-domain problem in which human health, animal agriculture, and the environment are the core and interrelated components.This prospectus presents the status and issues relevant to the environmental component of antibiotic resistance, namely, the needs for advancing surveillance methodology: the environmental reservoirs and sources of resistance, namely, urban wastewater treatment plants, aquaculture production systems, soil receiving manure and biosolid, and the atmosphere which includes longer range dispersal.Recently, much work has been done describing antibiotic resistance genes in various environments;now quantitative, mechanistic,and hypothesis-driven studies are needed to identify practices that reduce real risks and maintain the effectiveness of our current antibiotics as long as possible. Advanced deployable detection methods for antibiotic resistance in diverse environmental samples are needed in order to provide the surveillance information to identify risks and define barriers that can reduce risks. Also needed are practices that reduce antibiotic use and thereby reduce selection for resistance, as well as practices that limit the dispersal of or destroy antibiotic-resistant bacteria or their resistance genes that are feasible for these varied environmental domains.James M. 8Overview and developments in noninvasive diagnosis of nonalcoholic fatty liver disease显示文摘High prevalence of non-alcoholic fatty liver disease (NAFLD) and very diverse outcomes that are related to disease form and severity at presentation have made the search for noninvasive diagnostic tools in NAFLD one of the areas with most intense development in hepatology today.Various methods have been investigated in the recent years,including imaging methods like ultrasound and magnetic resonance imaging,different forms of liver stiffness measurement,various biomarkers of necroinflammatory processes (acute phase reactants,cytokines,markers of apoptosis),hyaluronic acid and other biomarkers of liver fibrosis.Multicomponent tests,scoring systems and diagnostic panels were also developed with the purposes of differentiating non-alcoholic steatohepatitis from simple steatosis or discriminating between various fibrosis stages.In all of the cases,performance of noninvasive methods was compared with liver biopsy,which is still considered to be a gold standard in diagnosis,but is by itself far from a perfect comparative measure.We present here the overview of the published data on various noninvasive diagnostic tools,some of which appear to be very promising,and we address as well some of still unresolved issues in this interesting field.Neven Bari Ivan Leroti Lea Smiri-Duvnjak Vedran Tomai Marko Duvnjak 2012World Journal of Gastroenterology2012,18,30:7

10Application of a novel porous tantalum implant in rabbit anterior lumbar spine fusion model:in vitro and in vivo experiments显示文摘Background Some porous materials have been developed to enhance biologic fusion of the implants to bone in spine fusion surgeries.However,there are several inherent limitations.In this study,a novel biomedical porous tantalum was applied to in vitro and in vivo experiments to test its biocompatibility and osteocompatibility.Methods Bone marrow-derived mesenchymal stem cells (BMSCs) were cultured on porous tantalum implant.Scanning electron microscope (SEM) and Cell Counting Kit-8 assay were used to evaluate the cell toxicity and biocompatibility.Twenty-four rabbits were performed discectomy only (control group),discectomy with autologous bone implanted (autograft group),and discectomy with porous tantalum implanted (tantalum group) at 3 levels:L3–L4,L4–L5,and L5–L6 in random order.All the 24 rabbits were randomly sacrificed at the different post-operative times (2,4,6,and 12 months;n = 6 at each time point).Histologic examination and micro-computed tomography scans were done to evaluate the fusion process.Comparison of fusion index scores between groups was analyzed using one-way analysis of variance.Other comparisons of numerical variables between groups were made by Student t test.Results All rabbits survived and recovered without any symptoms of nerve injury.Radiographic fusion index scores at 12 months post-operatively between autograft and tantalum groups showed no significant difference (2.89 ± 0.32 vs.2.83 ± 0.38, F= 244.60, P = 0.709).Cell Counting Kit-8 assay showed no significant difference of absorbance values between the leaching liquor group and control group (1.25 ± 0.06 vs.1.23 ± 0.04, t = –0.644, P = 0.545),which indicated the BMSC proliferation without toxicity.SEM images showed that these cells had irregular shapes with long spindles adhered to the surface of tantalum implant.No implant degradation,wear debris,or osteolysis was observed.Histologic results showed solid fusion in the porous tantalum and autologous bone implanted intervertebral spaces.Conclusion This novel porous tantalum implant showed a good biocompatibility and osteocompatibility,which could be a valid biomaterial for interbody fusion cages.Ming Lu Song Xu Zi-Xiong Lei Dong LU Wei Cao Marko Huttula Chang-He Hou Shao-Hua Du Wei Chen Shuang-Wu Dai Hao-Miao Li Da-Di Jin 2019Chinese Medical Journal2019,,1:7
11Role of gut microbiota in cardiovascular diseases显示文摘The human gut is colonized by a community of microbiota,primarily bacteria,that exist in a symbiotic relationship with the host.Intestinal microbiota-host interactions play a critical role in the regulation of human physiology.Deleterious changes to the composition of gut microbiota,referred to as gut dysbiosis,has been linked to the development and progression of numerous diseases,including cardiovascular disease(CVD).Imbalances in host-microbial interaction impair homeostatic mechanisms that regulate health and can activate multiple pathways leading to CVD risk factor progression.Most CVD risk factors,including aging,obesity,dietary patterns,and a sedentary lifestyle,have been shown to induce gut dysbiosis.Dysbiosis is associated with intestinal inflammation and reduced integrity of the gut barrier,which in turn increases circulating levels of bacterial structural components and microbial metabolites,including trimethylamine-N-oxide and short-chain fatty acids,that may facilitate the development of CVD.This article reviews the normal function and composition of the gut microbiome,mechanisms leading to the leaky gut syndrome,its mechanistic link to CVD and potential novel therapeutic approaches aimed towards restoring gut microbiome and CVD prevention.As CVD is the leading cause of deaths globally,investigating the gut microbiota as a locus of intervention presents a novel and clinically relevant avenue for future research.Marko Novakovic Amit Rout Thomas Kingsley Robert Kirchoff Amteshwar Singh Vipin Verma Ravi Kant Rahul Chaudhary 2020World Journal of Cardiology2020,12,4:6

14Raman lasing and soliton mode-locking in lithium niobate microresonators显示文摘The recent advancement in lithium-niobite-on-insulator(LNOI)technology is opening up new opportunities in optoelectronics,as devices with better performance,lower power consumption and a smaller footprint can be realised due to the high optical confinement in the structures.The LNOI platform offers both largeχ(2)andχ(3)nonlinearities along with the power of dispersion engineering,enabling brand new nonlinear photonic devices and applications for the next generation of integrated photonic circuits.However,Raman scattering and its interaction with other nonlinear processes have not been extensively studied in dispersion-engineered LNOI nanodevices.In this work,we characterise the Raman radiation spectra in a monolithic lithium niobate(LN)microresonator via selective excitation of Raman-active phonon modes.The dominant mode for the Raman oscillation is observed in the backward direction for a continuous-wave pump threshold power of 20mW with a high differential quantum efficiency of 46%.We explore the effects of Raman scattering on Kerr optical frequency comb generation.We achieve mode-locked states in an X-cut LNOI chip through sufficient suppression of the Raman effect via cavity geometry control.Our analysis of the Raman effect provides guidance for the development of future chip-based photonic devices on the LNOI platform.Mengjie Yu Yoshitomo Okawachi Rebecca Cheng Cheng Wang Mian Zhang Alexander L.Gaeta Marko Loncar 2020Light(Science & Applications)2020,9,1:5
15Cellulose structure and lignin distribution in normal and compression wood of the Maidenhair tree(Ginkgo biloba L.)显示文摘We studied in detail the mean micro fibril angle and the width of cellulose crystals from the pith to the bark of a 15-year-old Maidenhair tree(Ginkgo biloba L.). The orientation of cellulose micro fibrils with respect to the cell axis and the width and length of cellulose crystallites were determined using Xray diffraction. Raman microscopy was used to compare the lignin distribution in the cell wall of normal/opposite and compression wood, which was found near the pith. Ginkgo biloba showed a relatively large mean micro fibril angle,varying between 19° and 39° in the S2 layer, and the average width of cellulose crystallites was 3.1–3.2 nm. Mild compression wood without any intercellular spaces or helical cavities was observed near the pith. Slit-like bordered pit openings and a heavily lignified S2 L layer con firmed the presence of compression wood. Ginkgo biloba showed typical features present in the juvenile wood of conifers. The micro fibril angle remained large over the 14 annual rings. The entire stem disc,with a diameter of 18 cm, was considered to consist of juvenile wood. 16The influence of physiobiomechanical parameters,technical aspects of shooting,and psychophysiological factors on biathlon performance:A review显示文摘The biathlon, an Olympic sporting discipline that combines cross-country skiing with rifle marksmanship, entails considerable physiological demands, as well as fine motor control while shooting after intense exercise and under mental pressure. Although much of our knowledge about cross-country skiing is probably also applicable to the biathlon, carrying the rifle and shooting under stress make this discipline somewhat unique. The present review summarizes and examines the scientific literature related to biathlon performance, with a focus on physiological and biomechanical factors and shooting technique, as well as psychophysiological aspects of shooting performance. 17The fundamental performance of FAST with 19-beam receiver at L band显示文摘The Five-hundred-meter Aperture Spherical radio Telescope(FAST) has passed national acceptance and finished one pilot cycle of ‘Shared-Risk’ observations. It will start formal operation soon. In this context, this paper describes testing results of key fundamental parameters for FAST, aiming to provide basic support for observation and data reduction of FAST for scientific researchers. The 19-beam receiver covering 1.05–1.45 GHz was utilized for most of these observations. The fluctuation in electronic gain of the system is better than 1% over 3.5 hours, enabling enough stability for observations. Pointing accuracy,aperture efficiency and system temperature are three key parameters for FAST. The measured standard deviation of pointing accuracy is 7.9′′, which satisfies the initial design of FAST. When zenith angle is less than 26.4°, the aperture efficiency and system temperature around 1.4 GHz are ~0.63 and less than 24 K for central beam, respectively. The sensitivity and stability of the 19-beam backend are confirmed to satisfy expectation by spectral HI observations toward NGC 672 and polarization observations toward 3 C 286. 20Genetic polymorphisms in non-alcoholic fatty liver disease:Clues to pathogenesis and disease progression显示文摘The spectrum of non-alcoholic fatty liver disease(NAFLD) ranges from simple steatosis through steatohepatitis to advanced f ibrosis and cirrhosis.Although the reason why only a minority of patients develop progressive forms of disease still remains largely unclear,recent research has identified genetic factors as a possible basis for this variation in disease presentation.Most of the studies have been focused on f inding associations between advanced disease forms and selected single nucleotide polymorphisms in genes encoding various proteins involved in disease pathogenesis.Although there are many limitations regarding the study design and interpretation of published data,further carefully planned studies together with implementation of new genetic technologies will likely bring new insights into disease pathogenesis and potential benefits to the management of patients with NAFLD.Marko Duvnjak Neven Bari Vedran Tomai Ivan Leroti 2009World Journal of Gastroenterology2009,15,48:4
