1Clinical characteristics of 24 asymptomatic infections with COVID-19 screened among close contacts in Nanjing,China显示文摘Previous studies have showed clinical characteristics of patients with the 2019 novel coronavirus disease(COVID-19)and the evidence of person-to-person transmission.Limited data are available for asymptomatic infections.This study aims to present the clinical characteristics of 24 cases with asymptomatic infection screened from close contacts and to show the transmission potential of asymptomatic COVID-19 virus carriers.Epidemiological investigations were conducted among all close contacts of COVID-19 patients(or suspected patients)in Nanjing,Jiangsu Province,China,from Jan 28 to Feb 9,2020,both in clinic and in community.Asymptomatic carriers were laboratory-confirmed positive for the COVID-19 virus by testing the nucleic acid of the pharyngeal swab samples.Their clinical records,laboratory assessments,and chest CT scans were reviewed.As a result,none of the 24 asymptomatic cases presented any obvious symptoms while nucleic acid screening.Five cases(20.8%)developed symptoms(fever,cough,fatigue,etc.)during hospitalization.Twelve(50.0%)cases showed typical CT images of ground-glass chest and 5(20.8%)presented stripe shadowing in the lungs.The remaining 7(29.2%)cases showed normal CT image and had no symptoms during hospitalization.These 7 cases were younger(median age:14.0 years;P=0.012)than the rest.None of the 24 cases developed severe COVID-19 pneumonia or died.The median communicable period,defined as the interval from the first day of positive nucleic acid tests to the first day of continuous negative tests,was 9.5 days(up to 21 days among the 24 asymptomatic cases).Through epidemiological investigation,we observed a typical asymptomatic transmission to the cohabiting family members,which even caused severe COVID-19 pneumonia.Overall,the asymptomatic carriers identified from close contacts were prone to be mildly ill during hospitalization.However,the communicable period could be up to three weeks and the communicated patients could develop severe illness.These results highlighted the importance of close contact tracing and longitudinally surveillance via virus nucleic acid tests.Further isolation recommendation and continuous nucleic acid tests may also be recommended to the patients discharged.Zhiliang Hu Ci Song Chuanjun Xu Guangfu Jin Yaling Chen Xin Xu Hongxia Ma Wei Chen Yuan Lin Yishan Zheng Jianming Wang Zhibin Hu Yongxiang Yi Hongbing Shen 2020Science China(Life Sciences)2020,63,5:77
2COVID-19: Challenges to GIS with Big Data显示文摘The outbreak of the 2019 novel coronavirus disease(COVID-19)has caused more than 100,000 people infected and thousands of deaths.Currently,the number of infections and deaths is still increasing rapidly.COVID-19 seriously threatens human health,production,life,social functioning and international relations.In the fight against COVID-19,Geographic Information Systems(GIS)and big data technologies have played an important role in many aspects,including the rapid aggregation of multi-source big data,rapid visualization of epidemic information,spatial tracking of confirmed cases,prediction of regional transmission,spatial segmentation of the epidemic risk and prevention level,balancing and management of the supply and demand of material resources,and socialemotional guidance and panic elimination,which provided solid spatial information support for decision-making,measures formulation,and effectiveness assessment of COVID-19 prevention and control.GIS has developed and matured relatively quickly and has a complete technological route for data preparation,platform construction,model construction,and map production.However,for the struggle against the widespread epidemic,the main challenge is finding strategies to adjust traditional technical methods and improve speed and accuracy of information provision for social management.At the data level,in the era of big data,data no longer come mainly from the government but are gathered from more diverse enterprises.As a result,the use of GIS faces difficulties in data acquisition and the integration of heterogeneous data,which requires governments,businesses,and academic institutions to jointly promote the formulation of relevant policies.At the technical level,spatial analysis methods for big data are in the ascendancy.Currently and for a long time in the future,the development of GIS should be strengthened to form a data-driven system for rapid knowledge acquisition,which signifies ts that GIS should be used to reinforce the social operation parameterization of models and methods,especially when providing support for social management.Chenghu Zhou Fenzhen Su Tao Pei An Zhang Yunyan Du Bin Luo Zhidong Cao Juanle Wang Wen Yuan Yunqiang Zhu Ci Song Jie Chen Jun Xu Fujia Li Ting Ma Lili Jiang Fengqin Yan Jiawei Yi Yunfeng Hu Yilan Liao Han Xiao 2020Geography and Sustainability2020,1,1:13
3Factors related to improved American Spinal Injury Association grade of acute traumatic spinal cord injury显示文摘BACKGROUND Acute traumatic spinal cord injury(ATSCI)usually results in disability,yet data on contemporary national trends of ATSCI incidence are limited.AIM To provide a systematic and basic theoretical basis for improving the treatment of acute spinal cord injury.METHODS Data from the Peking University Third Hospital Inpatient Sample databases were analyzed.A total of 304 patients with ATSCI were included from 2012 to 2017.The epidemiological data,treatment,complications and clinical outcomes of these patients were reviewed.RESULTS Of the 304 patients,257(84.5%)were male,and 75%of the patients were 55 years old or younger.135 patients had improved follow-up American Spinal Injury Association(ASIA)grades(44.4%).Only 14 patients with ASIA grade A improved.A statistically significant difference in prognosis between patients who underwent surgery within 72 h and those who underwent surgery after 72 h was observed(P<0.05).Surgery within 72 h resulted in better prognosis.The Steroid group and the Non-Steroid group showed a significant difference in outcome among patients with ASIA grades A and B(P<0.05).Patients with pneumonia had a poorer prognosis than patients without pneumonia(P<0.05).Surgery within 72 h resulted in better prognosis.CONCLUSION This study found that there was no significant difference in hospitalization time and prognosis between the Steroid group and the Non-Steroid group,but the patients with severe spinal cord injury(ASIA grades A and B)who underwent surgery combined with steroid therapy had a better prognosis than those who underwent surgery alone.The disastrous consequences of ATSCI and lack of consensus on the management strategy are obvious.Further improvements in treatment planns are needed in order to obtain more reliable functional outcomes.Ci Tian Yang Lv Shu Li Dai-Dai Wang Yi Bai Fang Zhou Qing-Bian Ma 2020World Journal of Clinical Cases2020,8,20:5
4Cardiac arrhythmias and cardiac arrest related to mushroom poisoning: A case report显示文摘BACKGROUND Mushroom exposure is a global health issue.The manifestations of mushroom poisoning (MP) may vary.Some species have been reported as rhabdomyolytic,hallucinogenic,or gastrointestinal poisons.Critical or even fatal MPs are mostly attributable to Amanita phalloides,with the development of severe liver or renal failure.Myocardial injury and even cases mimicking ST-segment elevation myocardial infarction (STEMI) have been previously reported,while cardiac arrhythmia or cardiac arrest is not commonly seen.CASE SUMMARY We report a 68-year-old woman with MP who suffered from delirium,seizure,long QT syndrome on electrocardiogram (ECG),severe cardiac arrhythmias of multiple origins,and cardiac arrest.She was intubated and put on blood perfusion.Her kidney and liver functions were intact;creatine kinase-MB was mildly elevated,and then fell within normal range during her hospital stay.We sent the mushrooms she left for translation elongation factor subunit 1α,ribosomal RNA gene sequence,and internal transcribed spacer sequence analyses.There were four kinds of mushrooms identified,two of which were found to be toxic.CONCLUSION This is the first time that we found cardiac toxicity caused by Panaeolus subbalteatus and Conocybe lactea,which were believed to be toxic to the liver,kidney,and brain.We suggest that intensive monitoring and ECG follow-up are essential to diagnose prolonged QT interval and different forms of tachycardia in MP patients,even without the development of severe liver or renal failure.The mechanisms need to be further investigated and clarified based on animal experiments and molecular signal pathways.Shu Li Qing-Bian Ma Ci Tian Hong-Xia Ge Yang Liang Zhi-Guo Guo Cheng-Duo Zhang Bei Yao Jia-Ning Geng Fran Riley 2019World Journal of Clinical Cases2019,7,16:4
5Early T-cell precursor leukemia:a subtype of high risk childhood acute lymphoblastic leukemia显示文摘Acute lymphoblastic leukemia includes T-cell acute lymphoblastic leukemia(T-ALL)and B-cell acute lymphoblastic leukemia(B-ALL).In children,T-ALL usually has a worse prognosis than B-ALL,although childhood T-ALL prognoses have improved remarkably.The varying outcomes among T-ALL cases suggest that an unrecognized biological heterogeneity may contribute to chemo-resistance.Deep exploration of T-lymphocyte development in recent years has found a subgroup of patients with a phenotype that resembles early T-cell precursor,which confers a much poorer prognosis than any other form of T-ALL.This novel subtype of T-ALL was called early T-cell precursor acute lymphoblastic leukemia(ETP-ALL).Flow cytometry data from T-ALL patients enrolled in Shanghai Children’s Medical Center between July 2002 and October 2010 were assessed according to Dr.Campana’s protocol.Among total 89 T-ALL cases,74 cases had enough immunophenotype data available to differentiate between ETP(CD1a^(-),CD8^(-),CD5^(dim),at least one marker of stem cell or myeloid lineage)and non-ETP.From these 74 subjects,12 ETP-ALL cases(16.2%)were identified.The event-free survival(EFS)rate at 66.8 months was 11.1%±10.1% for ETP-ALL and 57.6%±5.6% for non-ETP-ALL(P=0.003).The overall survival rates were 13.3%±11.0% for ETP-ALL and 64.7%±6.3% for non-ETP-ALL(P=0.002).Our findings demonstrate that early T-cell precursor leukemia is a very high-risk subtype of acute lymphoblastic leukemia with poor prognosis.Meilin Ma Xiang Wang Jingyan Tang Huiliang Xue Jing Chen Ci Pan Hua Jiang Shuhong Shen 2012Frontiers of Medicine2012,6,4:2
