1Natural orifice transluminal endoscopic surgery:Where are we going?显示文摘The foundation for natural orifice transluminal endoscopic surgery(NOTES) is to access the peritoneal and other body cavities through the wall of the alimentary tract via natural orifices,with the goal of performing procedures within the peritoneum and other cavities,without the need to make incisions in the abdominal wall.We have made great progress in the field of NOTES since the publication of the White Paper in 2006.There are still major fundamental goals as outlined by the Society of American Gastrointestinal and Endoscopic Surgeons/American Society for Gastrointestinal Endoscopy joint committee that need to be evaluated and answered before NOTES is ready for widespread clinical use.These include prevention of infection,instrument development,creation of a multitasking platform,and the ability to recognize and treat intraperitoneal complications such as hemorrhage and other physiological adverse events.In response to this need,recent abstracts and papers have focused on the management of intraoperative complications.The next phase is to focus on controlled prospective multicenter clinical trials that compare defined NOTES procedure to standard laparoscopy.The goal is to produce reliable and convincing data for the United States Food and Drug Administration,insurance companies,the physician community and the general public.At the present time,we still have many important milestones that still need to be met.Most investigators agree that a hybrid technique and not a pure NOTES practice should be advocated until devices can meet the current and new challenges in this f ield.Susan H Whang Klaus Thaler 2010World Journal of Gastroenterology2010,16,35:18
2Occult hepatitis B virus infection and blood transfusion显示文摘Transfusion-transmitted infections including hepatitis B virus(HBV) have been a major concern in transfusion medicine. Implementation of HBV nucleic acid testing(NAT) has revealed occult HBV infection(OBI) in blood donors. In the mid-1980 s, hepatitis B core antibody(HBc) testing was introduced to screen blood donors in HBV non-endemic countries to prevent transmission of non-A and non-B hepatitis. That test remains in use for preventing of potential transmission of HBV from hepatitis B surface antigen(HBs Ag)-negative blood donors, even though anti-hepatitis C virus testshave been introduced. Studies of anti-HBc-positive donors have revealed an HBV DNA positivity rate of 0%-15%. As of 2012, 30 countries have implemented HBV NAT. The prevalence of OBI in blood donors was estimated to be 8.55 per 1 million donations, according to a 2008 international survey. OBI is transmissible by blood transfusion. The clinical outcome of occult HBV transmission primarily depends on recipient immune status and the number of HBV DNA copies present in the blood products. The presence of donor anti-HBs reduces the risk of HBV infection by approximately five-fold. The risk of HBV transmission may be lower in endemic areas than in non-endemic areas, because most recipients have already been exposed to HBV. 3Thalidomide effect in endothelial cell of acute radiation proctitis显示文摘AIM: To determine whether thalidomide prevents microvascular injury in acute radiation proctitis in white rats. METHODS: Fourteen female Wistar rats were used: six in the radiation group, six in the thalidomide group, and two in normal controls. The radiation and thalidomide groups were irradiated at the pelvic area using a single 30 Gy exposure. The thalidomide (150 mg/kg) was injected into the peritoneum for 7 d from the day of irradiation. All animals were sacrificed and the rectums were removed on day 8 after irradiation. The microvessels of resected specimens were immunohistochemically stained with thrombomodulin (TM), von Willebrand Factor (vWF), and vascular endothelial growth factor (VEGF). RESULTS: The microscopic scores did not differ significantly between the radiation and thalidomide groups, but both were higher than in the control group. Expression of TM was significantly lower inthe endothelial cells (EC) of the radiation group than in the control and thalidomide groups (P < 0.001). The number of capillaries expressing vWF in the EC was higher in the radiation group (15.3 ± 6.8) than in the control group (3.7 ± 1.7), and the number of capillaries expressing vWF was attenuated by thalidomide (10.8 ± 3.5, P < 0.001). The intensity of VEGF expression in capillaries was greater in the radiation group than in the control group and was also attenuated by thalidomide (P = 0.003). 5Machining characteristics of complex prism pattern on electroplated roll by copper显示文摘The BLU (back light unit) is the core component of the LCD for notebook, mobile-phone, navigation, as well as large sized TV, PID (public information display), etc. In order to enhance optical efficiency of LCD, optical films with the uniform prism patterns have been used for BLU by stacking two films up orthogonally. In this case, light interference-phenomenon occurred such as Morie, wet-out, u-turning, etc. It caused several problems such as low brightness, spots and stripes in LCD. Recently, the high-luminance micro complex prism patterns are actively studied to avoid the light interference-phenomenon and enhance the optical efficiency. In this study, the roll master to manufacture complex micro prism pattern film was machined by using the high precision lathe. The machined patterns on the roll master were 50, 45, 40, 35, 30, 25, 20, 15, 10 and 5 μm in the pitch with 25.0, 22.5, 20.0, 17.5, 15.0, 12.5, 10.0, 7.5, 5.0 and 2.5 μm in the peak height, respectively. The roll was 2 000 mm in length and 320 mm in diameter. The electroplated roll by copper and the natural single crystal diamond tool was used for machining the patterns. The cutting force was measured and analyzed for each cutting condition by using the dynamometer. 6Improvement of machining quality of copper-plated roll mold by controlling temperature variation显示文摘Micro prism film used in LCD industry can be manufactured by roll to roll method with copper-plated roll mold. As copper-plated roll mold is getting larger, pitch error is getting severer. The pitch error drops the quality of micro prism film. The main cause of the pitch error was investigated during machining large roll mold whose machined length was 1 200 mm. The temperature of machining system was elevated during machining roll mold, and this elevation induced thermal expansion of the system. The temperature variation around the roll mold also made thermal expansion of the roll mold. The amount of thermal expansion had strong relationship to the amount of pitch error. Therefore, the roll mold was machined after warming-up of machining system and precise temperature controller around copper-plated roll mold was installed, which minimized the temperature variation. 7Acute idiopathic pancreatitis in pregnancy:A case study显示文摘Acute pancreatitis during pregnancy is a rare event,and can be associated with high maternal mortality and fetal loss.Gallstone disease is thought to be the most common causative factor of acute pancreatitis,but,in many cases,the cause remains unclear.We report a case of a 36-year-old woman at 35 wk of gestation,who presented with severe pain confined to the upper abdomen and radiating to the back.The patient was diagnosed with acute idiopathic pancreatitis,which was managed conservatively;she recovered within several days and then delivered a healthy baby.Therefore it is important to consider acute pancreatitis when a pregnant woman presents with upper abdominal pain,nausea and vomiting in order to improve fetal and maternal outcomes for patients with acute pancreatitis.Ji Yeon Kim Sung Hoon Jung Hyung Wook Choi Dong Jin Song Cheol Yoon Jeong Dong Hyun Lee Il Soon Whang 2014World Journal of Gastroenterology2014,20,43:4
8Comparison of CT and MRI for presurgical characterization of paraaortic lymph nodes in patients with pancreatico-biliary carcinoma显示文摘AIM:To determine the accuracy of computed tomography (CT) and magnetic resonance (MR) for presurgical characterization of paraaortic lymph nodes in patients with pancreatico-biliary carcinoma. METHODS:Two radiologists independently evaluated CT and MR imaging of 31 patients who had undergone lymphadenectomy (9 metastatic and 22 non-metastatic paraaortic nodes). Receiver operating characteristic (ROC) curve analysis was performed using a five point scale to compare CT with MRI. To re-define the morphologic features of metastatic nodes, we evaluated CT scans from 70 patients with 23 metastatic paraaortic nodes and 47 non-metastatic ones. The short axis diameter, ratio of the short to long axis, shape, and presence of necrosis were compared between metastatic and non-metastatic nodes by independent samples t-test and Fisher's exact test. P < 0.05 was considered statistically significant. RESULTS:The mean area under the ROC curve for CT (0.732 and 0.646, respectively) was slightly higher than that for MRI (0.725 and 0.598, respectively) without statistical significance (P = 0.940 and 0.716,respectively). The short axis diameter of the metastatic lymph nodes (mean = 9.2 mm) was significantly larger than that of non-metastatic ones (mean = 5.17 mm, P < 0.05). Metastatic nodes had more irregular margins (44.4%) and central necrosis (22.2%) than non-metastatic ones (9% and 0%, respectively), with statistical significance (P < 0.05). CONCLUSION:The accuracy of CT scan for the characterization of paraaortic nodes is not different from that of MRI. 10Paraaortic lymph node metastasis in patients with intra-abdominal malignancies:CT vs PET显示文摘AIM:To compare the diagnostic accuracy of computed tomography(CT)and positron emission tomography (PET)for the preoperative detection of paraaortic lymph node(PAN)metastasis in patients with intraabdominal malignancies. METHODS:Sixty-six patients with intra-abdominal malignancies who underwent both CT and PET before lymphadenectomy were included in this study. Histopathologically,13 patients had metastatic PAN, while 53 had non-metastatic PAN.The CT criteria for metastasis were:short diameter of>8 mm,lobular or irregular shape,and/or combined ancillary findings, including necrosis,conglomeration,vessel encasement, and infiltration.The PET criterion was positive fluorodeoxyglucose(FDG)uptake.The sensitivity, specificity,positive predictive value(PPV),negative predictive value(NPV),and accuracy of both modalities were compared with the pathologic findings,and the false positive and false negative cases with both CT and PET were analyzed.RESULTS:The sensitivity,specificity,PPV,NPV,and accuracy of CT were 61.5%,84.9%,50%,90%and 80.3%,respectively.For PET,the percentages were 46.2%,100%,100%,88.3%,and 89.4%.Additionally, there were 8 false positive CT cases(8/53,15.1%) and zero false positive PET cases.Of the 13 metastatic PANs,there were 5 false negative CT scans(38.5%) and 7(53.9%)false negative PET scans. 15Plasmonic bacteria on a nanoporous mirror via hydrodynamic trapping for rapid identification of waterborne pathogens显示文摘A rapid,precise method for identifying waterborne pathogens is critically needed for effective disinfection and better treatment.However,conventional methods,such as culture-based counting,generally suffer from slow detection times and low sensitivities.Here,we developed a rapid detection method for tracing waterborne pathogens by an innovative optofluidic platform,a plasmonic bacteria on a nanoporous mirror,that allows effective hydrodynamic cell trapping,enrichment of pathogens,and optical signal amplifications.We designed and simulated the integrated optofluidic platform to maximize the enrichment of the bacteria and to align bacteria on the nanopores and plasmonic mirror via hydrodynamic cell trapping.Gold nanoparticles are self-assembled to form antenna arrays on the surface of bacteria,such as Escherichia coli and Pseudomonas aeruginosa,by replacing citrate with hydroxylamine hydrochloride in order to amplify the signal of the plasmonic optical array.Owing to the synergistic contributions of focused light via the nanopore geometry,self-assembled nanoplasmonic optical antennas on the surface of bacteria,and plasmonic mirror,we obtain a sensitivity of detecting E.coli as low as 102 cells/ml via surface-enhanced Raman spectroscopy.We believe that our label-free strategy via an integrated optofluidic platform will pave the way for the rapid,precise identification of various pathogens.Keumrai Whang Jong-Hwan Lee Yonghee Shin Wooju Lee Young Wan Kim Dongchoul Kim Luke P.Lee Taewook Kang 2018Light(Science & Applications)2018,7,1:2
