1心率作为冠心病与左室收缩障碍患者的预后危险因素(BEAUTIFUL):一项随机、对照试验的一个亚组分析显示文摘背景BEAUTIFUL研究对降心率药物伊伐布雷定的发病率与病死率收益进行了评估。BEAUTIFUL试验中的安慰剂组是由稳定型冠心病与左室收缩功能障碍的大型患者队列组成。我们对该安慰剂组进行了亚组分析,旨在检验这样一种假设,即基线静息心率升高是心血管疾病发病与死亡的标志。 方法采用Cox比例风险模型,对心率≥70次/min患者(2693例)以及〈70次/min患者(2745例)进行对比,分析基线静息心率与心血管结局之间的关系。将心率作为一个连续变量,并采用更为精细的心率分类进行其他分析。 结果在对基线特征进行校正后,心率≥70次/min患者的心血管疾病死亡(34%,P=0.0041)、心力衰竭入院(53%,P=0.0001)、心肌梗死入院(46%,P=0.0066)及冠状动脉血运重建(38%,P=0.037)风险升高。心率每升高5次/min,患者的心血管疾病死亡(8%,P=0.0005)、心力衰竭入院(16%.P〈0.0001)、心肌梗死入院(7%,P=0.052)及冠状动脉血运重建(8%,P=0.034)风险亦升高。对心率精细分组的分析表明,心率〉70次/min患者的病死率与心力衰竭发生率持续升高,但这种相关性在冠状动脉结局方面并不显著。对于心力衰竭结局而言,静息心率对早期事件的预测价值较晚期事件的预测价值更强。 结论在冠心病和左室收缩功能障碍的患者中,心率升高(≥70次/min)意味着心血管疾病结局风险升高,同时对心力衰竭相关性结局与冠状动脉事件相关性结局有着不同的影响。Kim Fox Ian Ford P Gabriel Steg Michal Tendera Michele Robertson Roberto Ferrari 罗亮(译) 2008世界临床医学2008,2,6:5
2Impact of Helicobacter pylori on the healing process of the gastric barrier显示文摘AIM To determine the impact of selected well defined Helicobacter pylori(H. pylori) antigens on gastric barrier cell turnover.METHODS In this study,using two cellular models of gastric epithelial cells and fibroblasts,we have focused on exploring the effects of well defined H. pylori soluble components such as glycine acid extract antigenic complex(GE),subunit A of urease(Ure A),cytotoxin associated gene A protein(Cag A) and lipopolysaccharide(LPS) on cell turnover by comparing the wound healing capacity of the cells in terms of their proliferative and metabolic activity as well as cell cycle distribution. Toxic effects of H. pylori components have been assessed in an association with damage to cell nuclei and inhibition of signal transducer and activator of transcription 3(STAT3) phosphorylation. RESULTS We showed that H. pylori GE,Cag A and Ure A promoted regeneration of epithelial cells and fibroblasts,which is necessary for effective tissue healing. However,in vivo increased proliferative activity of these cells may constitute an increased risk of gastric neoplasia. In contrast,H. pylori LPS showed a dose-dependent influence on the process of wound healing. At a low concentration(1 ng/m L) H. pylori LPS accelerated of healing epithelial cells,which was linked to significantly enhanced cell proliferation and MTT reduction as well as lack of alterations in cell cycle and downregulation of epidermal growth factor(EGF) production as well as cell nuclei destruction. By comparison,H. pylori LPS at a high concentration(25 ng/m L) inhibited the process of wound repair,which was related to diminished proliferative activity of the cells,cell cycle arrest,destruction of cell nuclei and downregulation of the EGF/STAT3 signalling pathway.CONCLUSION In vivo H. pylori LPS driven effects might lead to the maintenance of chronic inflammatory response and pathological disorders on the level of the gastric mucosal barrier.Eliza Mnich Magdalena Kowalewicz-Kulbat Paulina Sicińska Krzysztof Hinc Michal Obuchowski Adrian Gajewski Anthony P Moran Magdalena Chmiela 2016World Journal of Gastroenterology2016,22,33:4
3Sensitivity analysis of parameters and contrasting performance of ground filtering algorithms with UAV photogrammetry-based and LiDAR point clouds显示文摘Most ground filtering algorithms are primarily designed for airborne LiDAR point cloud processing and their successful use in identifying ground points from photogrammetric point clouds remains questionable.We compared six ground filtering algorithms implemented in Metashape,ArcGIS,CloudCompare,LAStools,and PDAL.We used UAV photogrammetry-based(acquired under leaf-off conditions)and airborne LiDAR(leaf-on)point clouds of the same area to:(i)compare accuracy of generated DTMs;(ii)evaluate the effect of vegetation density and terrain slope on filtering accuracy;and(iii)assess which algorithm parameters have the greatest effect on the filtering accuracy.Our results show that the performance of filtering algorithms was affected by the point cloud type,terrain slope and vegetation cover.The results were generally better for LiDAR(RMSE 0.13–0.19 m)than for photogrammetric(RMSE 0.19–0.23 m)point clouds.The behavior in varying vegetation and terrain conditions was consistent for LiDAR point clouds.However,when applied on photogrammetric point clouds,the algorithms’behavior was inconsistent,especially in areas of steep slope(except for the Progressive Triangulated Irregular Network in LAStools).Parameters related to the selection of the initial minimum elevation ground points were the most influential in all algorithms and point clouds.Petr Klápště Michal Fogl Vojtěch Barták Kateřina Gdulová Rudolf Urban Vítězslav Moudrý 2020International Journal of Digital Earth2020,13,12:2
