1Potential beneficial effects of butyrate in intestinal and extraintestinal diseases显示文摘The multiple beneficial effects on human health of the short-chain fatty acid butyrate,synthesized from nonabsorbed carbohydrate by colonic microbiota,are well documented.At the intestinal level,butyrate plays a regulatory role on the transepithelial fluid transport,ameliorates mucosal inflammation and oxidative status,reinforces the epithelial defense barrier,and modulates visceral sensitivity and intestinal motility.In addition,a growing number of studies have stressed the role of butyrate in the prevention and inhibition of colorectal cancer.At the extraintestinal level,butyrate exerts potentially useful effects on many conditions,including hemoglobinopathies,genetic metabolic diseases,hypercholesterolemia,insulin resistance,and ischemic stroke.The mechanisms of action of butyrate are different;many of these are related to its potent regulatory effects on gene expression.These data suggest a wide spectrum of positive effects exerted by butyrate,with a high potential for a therapeutic use in human medicine.Roberto Berni Canani Margherita Di Costanzo Ludovica Leone Monica Pedata Rosaria Meli Antonio Calignano 2011World Journal of Gastroenterology2011,17,12:52
2Nonalcoholic fatty liver disease in patients with inflammatory bowel disease: Beyond the natural history显示文摘BACKGROUND Nonalcoholic fatty liver disease(NAFLD)is a frequently reported condition in patients with inflammatory bowel disease(IBD).Both intestinal inflammation and metabolic factors are believed to contribute to the pathogenesis of IBDassociated NAFLD.AIM To evaluate the prevalence of steatosis and liver fibrosis(LF)in a cohort of IBD patients and the identification of metabolic-and IBD-related risk factors for NAFLD and LF.METHODS IBD patients were consecutively enrolled from December 2016 to January 2018.Demographic,anthropometric and biochemical data were collected so as eating habits.Abdominal ultrasound and transient elastography were performed to evaluate the presence of NAFLD and LF respectively.RESULTS A total of 178 consecutive patients were enrolled and included in the analysis(95 Ulcerative colitis,83 Crohn’s disease).NAFLD was detected by imaging in 72(40.4%)patients.Comparison between patients with and without NAFLD showed no significant differences in terms of IBD severity,disease duration,location/extension,use of IBD-related medications(i.e.,steroids,anti-TNFs,and immunomodulators)and surgery.NAFLD was significantly associated with the presence of metabolic syndrome[MetS;odds ratio(OR):4.13,P=0.001]and obesity defined by body mass index(OR:9.21,P=0.0002).IBD patients with NAFLD showed higher caloric intake and lipid consumption than those without NAFLD,regardless disease activity.At the multivariate analysis,male sex,advanced age and high lipid consumption were independent risk factors for the development of NAFLD.An increased liver stiffness was detected in 21 patients(16%)and the presence of MetS was the only relevant factor associated to LF(OR:3.40,P=0.01).CONCLUSION In this study,we demonstrate that risk factors for NAFLD and LF in the IBD population do not differ from those in the general population.Salvatore Magrì Danilo Paduano Fabio Chicco Arianna Cingolani Cristiana Farris Giovanna Delogu Francesca Tumbarello Mariantonia Lai Alessandro Melis Laura Casula Massimo C Fantini Paolo Usai 2019World Journal of Gastroenterology2019,25,37:8
3Current role of modern radiotherapy techniques in the management of breast cancer显示文摘Breast cancer is the most common type of malignancy in females. Advances in systemic therapies and radiotherapy(RT) provided long survival rates in breast cancer patients. RT has a major role in the management of breast cancer. During the past 15 years several developments took place in the field of imaging and irradiation techniques, intensity modulated RT, hypofractionation and partial-breast irradiation. Currently, improvements in the RT technology allow us a subsequent decrease in the treatment-related complications such as fibrosis and long-term cardiac toxicity while improving the loco-regional control rates and cosmetic results. Thus, it is crucial that modern radiotherapy techniques should be carried out with maximum care and efficiency. Several randomized trials provided evidence for the feasibility of modern radiotherapy techniques in the management of breast cancer. However, the role of modern radiotherapy techniques in the management of breast cancer will continue to be defined by the mature results of randomized trials. 4Prevalence of vacA, cagA and babA2 genes in Cuban Helicobacter pylori isolates显示文摘AIM: To investigate the prevalence of vacuolating cytotoxin (vacA), cytotoxin associated gene A (cagA) and blood adhesion binding antigen (babA2) genotypes of Helicobacter pylori (H pylori) isolates from Cuban dyspeptic patients. METHODS: DNA was extracted from H pylori-positive cultures taken from 130 dyspeptic patients. Genotyping was performed by PCR, using specifi c primers for vacA (s1, s2, m1, m2), cagA and babA2 genes. Endoscopic observations and histological examinations were used to determine patient pathologies. RESULTS: vacA alleles s1 , s2 , m1 and m2 were detected in 96 (73.8%), 34 (26.2%), 75 (57.7%) and 52 isolates (40%), respectively, while the cagA gene was detected in 95 isolates (73.2%). One hundred and seven isolates (82.3%) were babA2-positive. A signif icant correlation was observed between vacAs1m1 and cagA and between vacAs1m1 and babA2 genotypes (P < 0.001 and P < 0.05, respectively) and between babA2 genotype and cagA status (P < 0.05); but, no correlation was observed between vacAs1 and babA2 genotypes. Eighty fi ve (65.4%) and 73 (56.2%) strains were type 1 (vacAs1-cagA-positive) and 'triple- positive' (vacAs1-cagA-babA2-positive), respectively, and their presence was significantly associated with duodenal ulcer (P < 0.01 and P < 0.001, respectively). 7Genotype × environment interaction effects on early fresh storage root yield and related traits in cassava显示文摘Cassava(Manihot esculenta Crantz) is an important root crop worldwide. It exhibits substantial differential genotypic responses to varying environmental conditions, a phenomenon termed genotype × environment interaction(GEI). A significant GEI presents challenges in the selection of superior genotypes. The objective of this study was to examine the effect of genotype,environment and GEI on early fresh storage root yield(FSRY) and related traits in cassava.Accordingly, 12 cassava genotypes were evaluated in a randomised complete block design at three contrasting locations(Jinja, Nakasongola and Namulonge) in Uganda. Trials were harvested nine months after planting and the data collected were analysed using the additive main effects and multiplicative interaction(AMMI) model. The AMMI analysis of variance showed significant variation among genotypes for early FSRY and all other traits assessed.Locations were significantly different for all traits except for cassava brown streak disease root necrosis. The GEI effect was non-significant for early FSRY, but significant for other traits. For early FSRY, 48.5% of the treatment sum of squares was attributable to genotypes, 27.3% to environments, and 24.1% to GEI, indicating a predominance of genotypic variation for this trait.Predominance of genotypic variation was also observed for all the other traits. A majority of the genotypes(67%) had low interaction effects with locations for early FSRY, with Akena, CT2, CT4 and NASE14 being the most stable genotypes for the trait. Significant negative correlation was observed between cassava mosaic disease severity and early FSRY and storage root number,indicating significant negative effects of cassava mosaic disease on early FSRY and stability in cassava. 8Revisiting Emergency Food Reserve Policy and Practiceunder Disaster and Extreme Climate Events显示文摘All food systems will continue to be affected by disasters and extreme climate events. Triggered by recent food crises around the world and climate change concerns,some governments have been trying to develop more robust and resilient food systems. One of the oldest options for many governments is to stockpile emergency food reserves for the purpose of food security and disaster preparedness. In the aftermath of the world food price crises in2007–2008 and 2011, some governments in Asia have been maintaining emergency food reserves to ensure greater supply and price stability. Disasters and extreme climate events help governments to justify emergency food reserves. This research examined emergency food reserve policies in Indonesia, the Philippines, and Malaysia.Emergency food reserves emerged as a practice where the shared objectives of development, disaster risk reduction,and climate change adaptation have been demonstrated by governments. The findings suggest that most governments maintain the strong view that adequate emergency food reserves can buffer national food price shocks and shocks from disasters and climate change, and soften disruptions in trade due to export bans during times of disasters and climate emergencies.Under global climate change scenarios, food security is at risk and volatile(Porter et al. 2014). The expected increase in climate extremes has generated anticipatory actions from governments, including a new push for EFR policy adoption. Triggered by recent disasters and climate change concerns, some governments have been trying to develop more robust and resilient food systems(Fan and Brzeska 2014; Porter et al. 2014). For many countries in Asia, this means the renewed adoption of EFR. Unfortunately, we argue that this is not well understood in climate change adaptation studies as well as contemporary disaster studies.The Association of Southeast Asian Nations(ASEAN)first initiated a Food Security Reserve Agreement in 1979,with the purpose of meeting emergency requirements(ASEAN 1979). Policymakers have been aware of the susceptibility of the region to natural hazards and the possibility of food shortages. But it took 30 years, until soon after the world food crisis in 2007–2008(Hadley and Fan 2010), for the association plus three additional East Asian nations(China, Japan, South Korea) to establish the ASEAN Plus Three Emergency Rice Reserve(APTERR)in 2009, as a mechanism to address potential food shortages in the region in the light of climate and market uncertainties. The final formal agreement was signed in October 2011 in Jakarta(APTERR 2017).The Association of Southeast Asian Nations also established the ASEAN Food Security Information System(AFSIS) that functions as a central information repository for five commodities—rice, maize, soybeans, sugar, and cassava. AFSIS not only monitors and analyzes production,import, export, inventory stock, price, food security ratio,and self-sufficiency ratio for these commodities but also provides data on losses from both floods and droughts in every member state(Lassa et al. 2016). 10Innate immunity–the hallmark of Helicobacter pylori infection in pediatric chronic gastritis显示文摘BACKGROUND Innate immunity was found to be associated with both persistence of Helicobacter pylori(H.pylori)infection and increased risk of gastric cancer.AIM To identify the risk factors associated with H.pylori infection and to establish the role of TLR9 rs352140 in suppressing or promoting inflammation related to this infection in children.METHODS We performed a study of 155 children with digestive symptoms,who were divided into two groups according to the histopathological exam:Group 1–48 children with H.pylori-induced chronic gastritis,and Group 2–control group.RESULTS Rural area and poor living conditions were significantly associated with H.pylori chronic gastritis(P=0.0042/P<0.0001).Both positive immunoglobulin A anti H.pylori and the rapid urease test were significantly associated with H.pylori infection(P<0.0001).Significantly higher values of leukocytes and neutrophils within the peripheral blood were found in children with H.pylori chronic gastritis(P=0.111/P=0.284).We found a significant positive correlation between the variant TT genotype of TLR9 rs352140 polymorphism and both leucocytes and neutrophils(P=0.0225/P=0.0292).CONCLUSION Variant TT genotype carriers of the TLR9 rs352140 gene polymorphism might have a more severe degree of inflammation.Lorena Elena Meliț Cristina Oana Mărginean Maria Oana Săsăran Simona Mocan Dana Valentina Ghiga Alina Bogliş Carmen Duicu 2021World Journal of Clinical Cases2021,9,23:2
