1Efficacy of neoadjuvant therapy and surgical rescue for locally advanced hepatoblastomas:10 year single-center experience and literature review显示文摘AIM:To report our experience with long-term outcomes after multimodal management therapy.METHODS:An observational retrospective study was performed containing seven patients with hepatoblastoma(Hbl)treated in our institution,a tertiary referral center,from 2003 to 2011.Demographic,preoperative,surgical,and outcome variables were collected.A survival analysis and a review of the current literature related to combination neoadjuvant chemotherapy and surgical resection on Hbl were performed.RESULTS:The median age at surgery was 14.4 mo,with a male to female ratio of 4:3.Pretext staging at diagnosis was as follows:stageⅠ,4 cases;stageⅡ,2 patients;and stageⅢ,1 case.Mean pretreatment tumor volume was 735 cm3.Five out of seven patients received neoadjuvant chemotherapy according to SIOPEL-3 or SIOPEL-6 protocols.Tumor volume and alphafetoprotein levels significantly dropped after neoadjuvant therapy.Surgical procedures performed included hemihepatectomies,segmentectomies and atypical resection.All patients received chemotherapy after surgery.Median postoperative hospital stay was 8 d.All patients were alive and disease-free after a median follow-up period of 23 mo.With regards to the literature review,seventeen articles were found that were related to our search.CONCLUSION:Our series shows how multimodal management of Hbl,exhaustive control and a meticulous surgical approach leads to almost 100%complete resection with optimal postoperative results.Dolores Ayllon Teran Oscar Gómez Beltran Rubén Ciria Bru Elena Mateos González María José Pea Rosa Antonio Luque Molina Pedro López Cillero Javier Briceo Delgado 2014World Journal of Gastroenterology2014,20,29:12
2TT virus infection in patients with chronic hepatitis B and response of TTV to lamivudine显示文摘AIM: To investigate the responses of TT virus (TTV) and hepatitis B virus (HBV) to a long-term lamivudine therapy.METHODS: Sixteen patients infected with both TTV and HBV were treated with lamivudine 100 mg daily for 30 months. Blood samples were drawn at the beginning of the therapy and subsequently at month 3, 6, 9, 12 and 30.Serum TTV was quantified by real time PCR and serum HBV was detected by hybridization assay and nested polymerase chain reaction.RESULTS: TTV infection was detected in 100 % of HBV-infected patients. Loss of serum TTV DNA after one year of treatment occurred in 1/16 (6 %) patients. At the end of therapy, TTV DNA was positive in 94 % of them. 3Applications of finite element simulation in orthopedic and trauma surgery显示文摘Research in different areas of orthopedic and trauma surgery requires a methodology that allows both a more economic approach and the ability to reproduce different situations in an easy way. Simulation models have been introduced recently in bioengineering and could become an essential tool in the study of any physiological unity, regardless of its complexity. The main problem in modeling with finite elements simulation is to achieve an accurate reproduction of the anatomy and a perfect correlation of the different structures, in any region of the human body. Authors have developed a mixed technique, joining the use of a three-dimensional laser scanner Roland Picza captured together with computed tomography(CT) and 3D CT images, to achieve a perfect reproduction of the anatomy. Finite element(FE) simulation lets us know the biomechanical changes that take place after hipprostheses or osteosynthesis implantation and biological responses of bone to biomechanical changes. The simulation models are able to predict changes in bone stress distribution around the implant, so allowing preventing future pathologies. The development of a FE model of lumbar spine is another interesting application of the simulation. The model allows research on the lumbar spine, not only in physiological conditions but also simulating different load conditions, to assess the impact on biomechanics. Different degrees of disc degeneration can also be simulated to determine the impact on adjacent anatomical elements. Finally, FE models may be useful to test different fixation systems, i.e., pedicular screws, interbody devices or rigid fixations compared with the dynamic ones. We have also developed models of lumbar spine and hip joint to predict the occurrence of osteoporotic fractures, based on densitometric determinations and specific biomechanical models, including approaches from damage and fracture mechanics. 6Generation of 84-fs pulses from a mode-locked Tm:CNNGG disordered garnet crystal laser显示文摘A mode-locked laser based on a Tm:CNNGG disordered crystal as an active medium and a single-walled carbon nanotube saturable absorber is demonstrated, operating at a central wavelength of 2018 nm. 8GSTM3, but not IZUMO1, is a cryotolerance marker of boar sperm显示文摘Background: Cryopreservation is currently the most efficient method for long-term preservation of mammalian gametes and is extensively used in swine artificial insemination(AI) centres. However, it is well-known that cryopreservation procedures induce changes in the water phase in both intra and extracellular compartments,which alter the content and localisation of several proteins and ends up curtailing the structural integrity of functional sperm(i.e., cryoinjuries). Alterations and deficiencies of sperm-oocyte binding proteins during gamete recognition are one of the causes of reproductive failure both in vitro and in vivo. In this sense, characterisation of cryopreservation effects upon oocyte-binding proteins of sperm, such as IZUMO1 and GSTM3, is essential when assessing the impact of this technique in swine reproduction.Results: Cryopreservation was found to induce changes in the localisation of IZUMO1 and GSTM3 in boar sperm.However, the relative content of both proteins was not altered after thawing. Furthermore, whereas IZUMO1 content was found not to be related to the cryotolerance of boar sperm, GSTM3 content was observed to be higher in poor(PFE) than in good(GFE) freezability ejaculates in both pre-frozen(1.00 INT·mm^2± 0.14 INT·mm^2 vs.0.72 INT·mm^2± 0.15 INT·mm^2;P < 0.05) and post-thawed(0.96 INT·mm^2± 0.20 INT·mm^2 vs. 70 INT·mm^2± 0.19 INT·mm^2;P < 0.05) samples. Moreover, GSTM3 levels were found to be higher in those spermatozoa that exhibited low mitochondrial activity, high reactive oxygen species(ROS) production, and high membrane lipid disorder postthaw(P < 0.05).Conclusions: The difference in GSTM3 content between GFE and PFE, together with this protein having been found to be related to poor sperm quality post-thaw, suggests that it could be used as a cryotolerance marker of boar spermatozoa. 9META-R:A software to analyze data from multi-environment plant breeding trials显示文摘META-R(multi-environment trial analysis in R)is a suite of R scripts linked by a graphical user interface(GUI)designed in Java language.The objective of META-R is to accurately analyze multi-environment plant breeding trials(METs)by fitting mixed and fixed linear models from experimental designs such as the randomized complete block design(RCBD)and the alpha-lattice/lattice designs.META-R simultaneously estimates the best linear and unbiased estimators(BLUEs)and the best linear and unbiased predictors(BLUPs).Additionally,it computes the variance-covariance parameters,as well as some statistical and genetic parameters such as the least significant difference(LSD)at 5%significance,the coefficient of variation in percentage(CV),the genetic variance,and the broad-sense heritability.These parameters are very important in the selection of top performing genotypes in plant breeding.META-R also computes the phenotypic and genetic correlations among environments and between traits,as well as their statistical significance.The genetic correlations between environments or traits can be visualized in a biplot graph or a tree diagram(dendrogram).Genetic correlations are very important for identifying environments with similar behavior or making indirect selection and identifying the most highly associated traits.META-R performs multi-environment analyses by using the residual maximum likelihood(REML)method;these analyses can be done by environment,across environments by grouping factors(stress conditions,nitrogen content,etc.)and across environments;the analyses across environments can be done with a pre-defined degree of heritability.Gregorio Alvarado Francisco M.Rodríguez Angela Pacheco Juan Burgueño JoséCrossa Mateo Vargas Paulino Pérez-Rodríguez Marco A.Lopez-Cruz 2020The Crop Journal2020,8,5:4
10Total hepatectomy and liver transplantation as a two-stage procedure for fulminant hepatic failure: A safe procedure in exceptional circumstances显示文摘AIM: To evaluate the outcomes of two-stage liver transplant at a single institution, between 1993 and March 2015.METHODS: We reviewed our institutional experience with emergency hepatectomy followed by transplantation for fulminant liver failure over a twenty-year period. A retrospective review of a prospectively maintained liver transplant database was undertaken at a national liver transplant centre. Demographic data, clinical presentation, preoperative investigations, cardiocirculatory parameters, operative and postoperative data were recorded.RESULTS: In the study period, six two-stage liver transplants were undertaken. Indications for transplantation included acute paracetamol poisoning(n = 3), fulminant hepatitis A(n = 1), trauma(n = 1) and exertional heat stroke(n = 1). Anhepatic time ranged from 330 to 2640 min. All patients demonstrated systemic inflammatory response syndrome in the first post-operative week and the incidence of sepsis was high at 50%. There was one mortality, secondary to cardiac arrest 12 h following re-perfusion. Two patients required re-transplantation secondary to arterial thrombosis. 11Dissecting Maize Productivity:Ideotypes Associated with Grain Yield under Drought Stress and Well-watered Conditions显示文摘To increase maize (Zea mays L.) yields in drought-prone environments and offset predicted maize yield losses under future climates,the development of improved breeding pipelines using a multi-disciplinary approach is essential.Elucidating key growth processes will provide opportunities to improve drought breeding progress through the identification of key phenotypic traits,ideotypes,and donors.In this study,we tested a large set of tropical and subtropical maize inbreds and single cross hybrids under reproductive stage drought stress and well-watered conditions.Patterns of biomass production,senescence,and plant water status were measured throughout the crop cycle.Under drought stress,early biomass production prior to anthesis was important for inbred yield,while delayed senescence was important for hybrid yield.Under well-watered conditions,the ability to maintain a high biomass throughout the growing cycle was crucial for inbred yield,while a stay-green pattern was important for hybrid yield.While new quantitative phenotyping tools such as spectral reflectance (Normalized Difference Vegetation Index,NDVI) allowed for the characterization of growth and senescence patterns as well as yield,qualitative measurements of canopy senescence were also found to be associated with grain yield.Jill E.Cairns Ciro Sanchez Mateo Vargas Raziel Ordoez Jose Luis Araus 2012Journal of Integrative Plant Biology2012,54,12:3
12Structured laser beams: toward 2-μm femtosecond laser vortices显示文摘Structured ultrashort-pulse laser beams, and in particular eigenmodes of the paraxial Helmholtz equation, are currently extensively studied for novel potential applications in various fields, e.g., laser plasma acceleration,attosecond science, and fine micromachining. To extend these prospects further, in the present work we push forward the advancement of such femtosecond structured laser sources into the 2-μm spectral region. Ultrashortpulse Hermite– and Laguerre–Gaussian laser modes both with a pulse duration around 100 fs are successfully produced from a compact solid-state laser in combination with a simple single-cylindrical-lens converter. The negligible beam astigmatism, the broad optical spectra, and the almost chirp-free pulses emphasize the high reliability of this laser source. This work, as a proof of principle study, paves the way toward few-cycle pulse generation of optical vortices at 2 μm.
