4Antimicrobial susceptibility and serotypes of Neisseria meningitidis and Streptococcus pneumoniae in Sri Lanka:Experience from the National Reference Laboratory显示文摘Objective:To determine the antimicrobial susceptibility and serotypes of Neisseria(N.)meningitidis and Streptococcus(S.)pneumoniae in Sri Lankan patients.Methods:We retrospectively analyzed 11 blood culture specimens from suspected patients with invasive meningococcal disease and 26 S.pneumoniae clinical isolates.We tested 6 antimicrobials against N.meningitidis and 12 antimicrobials against S.pneumoniae.Meningococcal serogroup was determined by realtime PCR and Quellung serotyping was used for pneumococcal analysis.Results:N.meningitidis serogroup B was the most common in this study.Intermediate-susceptibility to penicillin was seen in 75.0%(6/8)of strains.Susceptibility to ciprofloxacin,levofloxacin and cotrimoxazole was 62.5%(5/8),62.5%(5/8)and 87.5%(7/8),respectively.Excellent susceptibility was seen in cefotaxime and meropenem.In S.pneumoniae,the most common serotype was 19F in both invasive and non-invasive pneumococcal diseases.The majority of strains showed multidrug resistance.Penicillin non-susceptibility in non-meningeal strains were 13.6%and all meningeal strains were penicillin resistant.Erythromycin was highly resistant in both groups.Amoxicillin showed excellent susceptibility in non-invasive pneumococcal diseases strains.Linezolid,levofloxacin and vancomycin showed 100.0%susceptibility in all pneumococcal isolates.Conclusions:Implementation of vaccines should be considered,especially for children and high-risk populations.This may contribute to reducing pneumococcal and meningococcal invasive disease burden and help prevent emergence of antimicrobial resistant strains.Lilani Karunanayake Veeraraghavan Balaji K.D.N.Gunawardana Rosemol Varghese Vayishnavi Ariram Malka Dassanayake Vaithehi Rajeevan Francis Lakmini Yapa Chamika Herath Nambage Shirani Chandrasiri Priyanka Wimalagunawardhana K.Dhananja Namalie C.T.Hapuarachchi 2022Asian Pacific Journal of Tropical Medicine2022,15,3:0
5Laboratory response to an infant with suspected measles vaccine associated fever and rash in Sri Lanka显示文摘Background:Measles is a highly contagious illness.Sri Lanka(SL)has eliminated the measles in 2019.The coun-try is at risk of importation of measles and there could be vaccine-associated measles like illnesses.Therefore,it is important to investigate patients with fever,rash to differentiate the wild-type from vaccine-type exclud-ing other suspected pathogens to direct infection prevention and control strategies.The objective is to describe the laboratory investigation procedure in an immunocompetent child,developed fever,rash following measles containing vaccine in post-measles eliminated period,SL.Methods:This laboratory based investigation was carried out in National Measles Laboratory,SL.Blood and throat swab were received from a patient with fever,rash,cough and coryza developed at tenth day of receiving the measles containing live-attenuated vaccine.Samples were tested for measles,rubella,and other relevant pathogens according to the laboratory testing algorithm for an immunocompetent child with fever,rash and flu like symptoms.Results:Measles vaccine type A,Edmonston-strain virus was detected after sequencing in throat swab and measles IgM and IgG were positive at sixth-week of illness-onset.In addition,influenza A RNA was detected in throat swab at day-three with detectable parvoB19 IgM in blood sample received at sixth-week of post-onset symptoms.Conclusions:Measles like illness of this immunocompetent child who received measles containing vaccine could be due to measles vaccine-type A or influenza infection.In a measles eliminated,resource-limited setting in SL,there should be a well-defined,testing algorithm to exclude prevalent possible pathogens according to epidemiological and clinical information.Buddhini Samaraweera Malsha G.Thrimavithana Lakmini Gunathilake Prabath Gankanda Janitha Nanayakkara Janaki I.Abeynayake 2023Infectious Medicine2023,2,1:0
6South Asian medicinal plants and chronic kidney disease显示文摘Chronic kidney disease remains as one of the serious health issues in South Asia.The paucity of effective pharmacotherapy targeting the management of chronic kidney disease has led to a search for alternative pharmacologic therapies.The traditional knowledge of medicinal plants plays a key role in the discovery of novel nephroprotective agents.This review aims to present the use of such South Asian ethnomedicinal plants that have sufficient therapeutic potency for the management of kidney diseases.Medicinal plants are rich sources of bioactive compounds that have been reported to exert nephroprotective mechanisms,such as antioxidant,anti-inflammation,diuretic,and immunomodulation.Many South Asian medicinal plants have been detailed in traditional medicinal pharmacopoeias for the management of kidney-related diseases.Some have shown promising effects to address nephropathy in animal models and in vitro research.This information can be beneficial in the development of novel pharmaceutical agents targeting the management of kidney diseases and improvement of quality of life for chronic kidney disease patients by fulfilling the requirements for disease management unmet by modern allopathic medicine.Sachinthi Sandaruwani Amarasiri Anoja Priyadarshani Attanayake Kamani Ayoma Perera Wijewardana Jayatilaka Lakmini Kumari Boralugoda Mudduwa 2020Traditional Medicine Research2020,5,5:0
