1A study of the efficacy of bacterial biofilm cleanout for gastrointestinal endoscopes显示文摘AIM:To compare the influence and clearance effect of enzymatic and non-enzymatic detergents against Escherichia coli (E. coli) biofilm on the inner surface of gastroscopes.METHODS:Teflon tubes were incubated in a mixture of different detergents and E. coli culture (106 CFU/mL) for 72 h at 15℃,and biofilms on the inner surface of the teflon tubes were analyzed by bacterial count and scanning electron microscopy. To evaluate the clear-ance effect of detergents,after biofilms were formed on the inner surface of Teflon tubes by 72 h lavage with E. coli culture,tubes were lavaged by enzymatic and non-enzymatic detergents at a speed of 250 mL/min,then biofilms on the inner surface were analyzed by bacterial count and scanning electron microscopy.RESULTS:Non-enzymatic detergent had a better inhi-bition function on biofilm formation than enzymatic de-tergent as it reduced bacterial burden by 2.4 log compared with the control samples (P = 0.00). Inhibition function of enzymatic detergent was not significantly different to that of control samples and reduced bac-terial burden by 0.2 log on average (P > 0.05). After lavaging at 250 mL/min for 3 min,no living bacteria were left in the tubes. Scanning electron microscopy observation showed biofi lms became very loose by the high shear force effect. CONCLUSION:Non-enzymatic detergent has a better inhibition effect on biofilm formation at room temperature. 3Expenditure and financial burden for the diagnosis and treatment of colorectal cancer in China:a hospital.based,multicenter,cross-sectional survey显示文摘Background: The increasing prevalence of colorectal cancer(CRC) in China and the paucity of information about relevant expenditure highlight the necessity of better understanding the financial burden and effect of CRC diagnosis and treatment. We performed a survey to quantify the direct medical and non-medical expenditure as well as the resulting financial burden of CRC patients in China.Methods: We conducted a multicenter, cross-sectional survey in 37 tertiary hospitals in 13 provinces across China between 2012 and 2014. Each enrolled patient was interviewed using a structured questionnaire. All expenditure data were inflated to the 2014 Chinese Yuan(CNY; 1 CNY = 0.163 USD). We quantified the overall expenditure and financial burden and by subgroup(hospital type, age at diagnosis, sex, education, occupation, insurance type, household income, clinical stage, pathologic type, and therapeutic regimen). We then performed generalized linear modeling to determine the factors associated with overall expenditure.Results: A total of 2356 patients with a mean age of 57.4 years were included, 57.1 % of whom were men; 13.9% of patients had stage I cancer; and the average previous-year household income was 54,525 CNY.The overall average direct expenditure per patient was estimated to be 67,408 CNY, and the expenditures for stage Ⅰ, Ⅱ, Ⅲ, and Ⅳ disease were 56,099 CNY, 59,952 CNY, 67,292 CNY, and 82,729 CNY, respectively. Non-medical expenditure accounted for 8.3%of the overall expenditure. The 1-year out-of-pocket expenditure of a newly diagnosed patient was 32,649 CNY, which accounted for 59.9% of their previous-year household income and caused 75.0% of families to suffer an unmanageable financial burden. Univariate analysis showed that financial burden and overall expenditure differed in almost all subgroups(P < 0.05), except for sex. Multivariate analysis showed that patients who were treated in specialized hospitals and those who were diagnosed with adenocarcinoma or diagnosed at a later stage were likely to spend more,whereas those with a lower household income and those who underwent surgery spent less(all P < 0.05).Conclusions: For patients in China, direct expenditure for the diagnosis and treatment of CRC seemed catastrophic,and non-medical expenditure was non-ignorable. 5Traditional Chinese medicinal herbs combined with epidermal growth factor receptor tyrosine kinase inhibitor for advanced non-small cell lung cancer: a systematic review and meta-analysis显示文摘BACKGROUND: Epidermal growth factor receptor tyrosine kinase inhibitor(EGFR-TKI) targeted treatment has been a standard therapy for advanced non-small cell lung cancer(NSCLC), but it is not tolerated well by all patients. In China, some studies have reported that traditional Chinese medicinal herbs(TCMHs) may increase effi cacy and reduce toxicity when combined with EGFR-TKI, but outside of China few studies of this kind have been attempted. OBJECTIVE: This study is intended to systematically review the existing clinical evidence on TCMHs combined with EGFR-TKI for treatment of advanced NSCLC. SEARCH STRATEGY: PubMed, the Cochrane Library, the Excerpta Medica Database(EMBASE), the China BioMedical Literature(CBM), and the China National Knowledge Infrastructure(CNKI) and web site of the American Society of Clinical Oncology(ASCO), the European Society for Medical Oncology(ESMO), the World Conference of Lung Cancer(WCLC) were searched; the search included all documents published in English or Chinese before October 2013. INCLUSION CRITERIA: We selected randomized controlled trials based on specifi c criteria, the most important of which was that a TCMH plus EGFR-TKI treatment group was compared with an EGFR-TKI control group in patients with advanced NSCLC. DATA EXTRACTION AND ANALYSIS: The modifi ed Jadad scale was used to assess the quality of studies. For each included study, patient characteristics, treatment details, therapeutic approach and clinical outcomes were collected on a standardized form. When disagreements on study inclusion or data extracted from a study emerged, the consensus of all coauthors provided the resolution. The clinical outcome metrics consisted of objective response rate(ORR; complete response + partial response divided by the total number of patients), disease control rate(DCR; complete response + partial response + no change divided by the total number of patients), survival rate, improved or stabilized Karnofsky performance status(KPS), and severe toxicity. RevMan 5.0 software was used for data syntheses and analyses. Risk ratio(RR) and 95% confidence interval(CI) were calculated; if the hypothesis of homogeneity was not rejected(P>0.1, I2<50%), the fi xed-effect model was used to calculate the summary RR and the 95% CI. Otherwise, a random-effect model was used.RESULTS: In this review, 19 studies were included based on the selection criteria. Of them, 13studies were of high quality and 6 studies were of low quality, according to the modifi ed Jadad scale. When the TCMH plus EGFR-TKI treatment groups were compared with the EGFR-TKI control groups the meta-analysis demonstrated a statistically signifi cant higher ORR(RR 1.34; 95% CI 1.15 to 1.57; P=0.000 2), DCR(RR 1.18; 95% CI 1.09 to 1.27; P<0.000 1), one-year survival rate(RR 1.21; 95% CI 1.01 to 1.44; P=0.04), 2-year survival rate(RR 1.91; 95% CI 1.26 to 2.89; P=0.002) and improved or stable KPS(RR 1.38; 95% CI 1.26 to 1.51; P<0.000 01). Severe toxicity for rash was decreased(RR 0.55; 95% CI 0.32 to 0.94; P=0.03), as were nausea and vomiting(RR 0.17; 95% CI 0.04 to 0.72; P=0.02) and diarrhea(RR 0.46; 95% CI 0.24 to 0.89; P=0.02). Sensitivity analysis indicated that fi ndings of the meta-analysis were robust to study quality. 6Lycorine inhibits breast cancer growth and metastasis via inducing apoptosis and blocking Src/FAK-involved pathway显示文摘Breast cancer is the most commonly diagnosed cancer type worldwide among women and more than 90% of patients die from tumor metastasis. Lycorine, a natural alkaloid, has been widely reported possessing potential efficacy against cancer proliferation and metastasis. In our study, the anti-tumor potency on breast cancer was evaluated in vitro and in vivo for the first time. Our results indicated that lycorine inhibited breast cancer cells growth, migration and invasion as well as induced their apoptosis.In in vivo study, lycorine not only suppressed breast tumor growth in xenograft models and inhibited breast tumor metastasis in MDA-MB-231 tail vein model. More importantly, we found lycorine had less toxicity than first-line chemotherapy drug paclitaxel at the same effective dose in vivo. Furthermore, on mechanism, lycorine inhibited tumor cell migration and invasion via blocking the Src/FAK(focal adhesion kinase)-involved pathway. 7Synthesis and Characterization of Phosphated Konjac Glucomannan Hydrogels显示文摘Konjac glucomannan (KGM) was crosslinked with sodium tripolyphosphate (STPP) to synthesize hydrogels. The crosslinking reaction was confirmed by FT-IR. 9Transcriptional Regulation of miR528 by OsSPL9 Orchestrates Antiviral Response in Rice显示文摘Many microRNAs (miRNAs) are critical regulators of plant antiviral defense.However,little is known about how these miRNAs respond to virus invasion at the transcriptional level.We previously show that defense against Rice stripe virus (RSV) invasion entailed a reduction of miR528 accumulation in rice,alleviating miR528-mediated degradation of L-Ascorbate Oxidase (AO) mRNA and bolstering the antiviral activity of AO.Here we show that the miR528-A0 defense module is regulated by the transcription factor SPL9.SPL9 displayed high-affinity binding to specific motifs within the promoter region of miR528 and activated the expression of miR528 gene in vivo.Loss-of-function mutations in SPL9 caused a significant reduction in miR528 accumulation but a substantial increase of AO mRNA,resulting in enhanced plant resistance to RSV.Conversely,transgenic overexpression of SPL9 stimulated the expression of miR528 gene,hence lowering the level of AO mRNA and compromising rice defense against RSV.Importantly,gain in RSV susceptibility did not occur when SPL9 was overexpressed in mir528 loss-of-function mutants,or in transgenic rice expressing a miR528-resistant AO.Taken together,the finding of SPL9-mediated transcriptional activation of miR528 expression adds a new regulatory layer to the miR528-A0 antiviral defense pathway.Shengze Yao Zhirui Yang Rongxin Yang Yu Huang Ge Guo Xiangyue Kong Ying Lan Tong Zhou He Wang Wenming Wang Xiaofeng Cao Jianguo Wu Yi Li 2019Molecular Plant2019,12,8:10
10Incidence and risk factors of tuberculosis among the elderly population in China:a prospective cohort study显示文摘Background:China is facing challenges of the shifting presentation of tuberculosis(TB)from younger to elderly due to an ageing population,longer life expectancy and reactivation disease.However,the burden of elderly TB and influence factors are not yet clear.To fill the gap,we generated a cohort study to measure the magnitude of TB incidence and associated factors among the elderly population aged 65 years and above in China.Methods:In this cohort established in 2013 through a prevalence survey conducted in selected sites,a total of 34076 elderlies without TB were enrolled into two-year follow-up.We used both active and passive case findings to find out all TB patients among them.The person-year(PY)incidence rates for both bacteriologically positive TB and active TB were calculated.Cox proportional regression model was performed to test effect of risk factors,and the population attributable fraction(PAF)of each risk factor contributing to incident TB among elderlies was calculated.Results:Over the two-year follow-up period,a total of 215 incident active TB were identified,62 of which were bacteriologically positive.The incidence rates for active TB and bacteriologically positive TB were 481.8 per 100000 PY(95%CI:417.4–546.2 per 100000 PY)and 138.9 per 100000 PY(95%CI:104.4–173.5 per 100000 PY),respectively.Incident cases detected by active case finding were significantly higher(P<0.001).Male,non-Han nationality,previously treated TB,ex/current smoker and body mass index(BMI)<18.5 presented as independent predictors for developing TB disease.For developing bacteriologically positive TB,the biggest contribution was from self-reported ex or current smoker(18.06%).And,for developing active TB,the biggest contribution was from non-Han nationality(35.40%),followed by male(26.80%)and age at 75 years and above(10.85%).Conclusions:Ageing population in China had a high TB incidence rate and risk to develop TB disease,implying that National TB Program(NTP)needs to prioritize for elderly.Active case finding should be applied capture more active TB cases among this particular population,especially for male,non-Han nationality,and those with identified risk factors.Jun Cheng Yan-Ni Sun Can-You Zhang Yan-Ling Yu Li-Hong Tang Hong Peng Ying Peng Yu-Xia Yao Shuang-Yi Hou Jian-Wei Li Jin-Ming Zhao Lan Xia Lin Xu Yin-Yin Xia Fei Zhao Li-Xia Wang Hui Zhang 2020Infectious Diseases of Poverty2020,9,1:10

15Epidemiological Investigation of Risk Factors of the Pregnant Women with Early Spontaneous Abortion in Beijing显示文摘Objective: To determine the risk factors of the pregnant women with early spontaneous abortion in Beijing. Methods: A total of 34,417 cases of pregnant women were participated in the survey from January 2000 to December 2013. A questionnaire was informed to each woman. The content of questionnaire includes four parts: general condition, obstetrical history, past history and family history, and living environment and habits. The mental condition was evaluated with Self-Rating Anxiety Scale(SAS) and Self-Rating Depression Scale(SDS). Results: A total of 32,296 questionnaires were collected. The spontaneous abortion rate in the total sample was 3.0%. There was no significant difference between the normal pregnancy group and spontaneous abortion group in terms of general condition, obstetrical and past history(P>0.05). Significant differences between the two groups were found in terms of decoration during pregnancy, keeping pets, near mobile communication base station within 100 m around the residence, drinking during pregnancy, having a cold during pregnancy and SAS(P<0.05). Having a cold during pregnancy, decoration during pregnancy, near mobile communication base station within 100 m around the residence, keeping pets and high SAS were determined the independent risk factors of spontaneous abortion by Logistic regression analysis. The independent role of each component contributing to the risk of COVID-19 is unknown, and how some non-cardiometabolic comorbidities affect the risk of COVID-19 remains unclear.Methods A retrospective follow-up design was adopted. A total of 1,160 laboratory-confirmed patients were enrolled from nine provinces in China. Data on comorbidities were obtained from the patients’ medical records. Multivariable logistic regression models were used to estimate the odds ratio(OR) and 95% confidence interval(95% CI) of the associations between comorbidities(cardiometabolic or non-cardiometabolic diseases), clinical severity, and treatment outcomes of COVID-19.Results Overall, 158(13.6%) patients were diagnosed with severe illness and 32(2.7%) had unfavorable outcomes. Hypertension(2.87, 1.30–6.32), type 2 diabetes(T2 DM)(3.57, 2.32–5.49),cardiovascular disease(CVD)(3.78, 1.81–7.89), fatty liver disease(7.53, 1.96–28.96), hyperlipidemia(2.15, 1.26–3.67), other lung diseases(6.00, 3.01–11.96), and electrolyte imbalance(10.40, 3.00–26.10)were independently linked to increased odds of being severely ill. T2 DM(6.07, 2.89–12.75), CVD(8.47,6.03–11.89), and electrolyte imbalance(19.44, 11.47–32.96) were also strong predictors of unfavorable outcomes. Women with comorbidities were more likely to have severe disease on admission(5.46,3.25–9.19), while men with comorbidities were more likely to have unfavorable treatment outcomes(6.58, 1.46–29.64) within two weeks.Conclusion Besides hypertension, diabetes, and CVD, fatty liver disease, hyperlipidemia, other lung diseases, and electrolyte imbalance were independent risk factors for COVID-19 severity and poor treatment outcome. 17Medical expenditure for esophageal cancer in China:a 10-year multicenter retrospective survey(2002-2011)显示文摘Background: Esophageal cancer is associated with substantial disease burden in China, and data on the economic burden are fundamental for setting priorities in cancer interventions. The medical expenditure for the diagnosis and treatment of esophageal cancer in China has not been fully quantified. This study aimed to examine the medical expenditure of Chinese patients with esophageal cancer and the associated trends.Methods: From 2012 to 2014, a hospital-based multicenter retrospective survey was conducted in 37 hospitals in 13 provinces/municipalities across China as a part of the Cancer Screening Program of Urban China. For each esophageal cancer patient diagnosed between 2002 and 2011, clinical information and expense data were extracted by using structured questionnaires. All expense data were reported in Chinese Yuan(CNY; 1 CNY = 0.155 USD) based on the2011 value and inflated using the year-specific health care consumer price index for China.Results: A total of 14,967 esophageal cancer patients were included in the analysis. It was estimated that the overall average expenditure per patient was 38,666 CNY, and an average annual increase of 6.27% was observed from 2002(25,111 CNY) to 2011(46,124 CNY). The average expenditures were 34,460 CNY for stage Ⅰ,39,302 CNY for stage Ⅱ,40,353 CNY for stage Ⅲ, and 37,432 CNY for stage IV diseases(P < 0.01). 18Expression of p53, p16 and COX-2 in pancreatic cancer with tissue microarray显示文摘BACKGROUND: Pancreatic cancer development and progression is driven by the accumulation of genetic changes. In this study we constructed tissue microarray containing specimens from pancreatic cancer, adjacent non-cancer tissue and normal tissue to survey the expression of p53, p16 and cyclooxyganase-2 (COX-2). METHODS: Tissue microarray containing 337 specimens from different stages of pancreatic cancer, adjacent noncancer tissue and normal tissues was constructed, and the expression of p53, p16 and COX-2 was assayed by immunohistochemistry to consecutive formalin-fixed tissue microarray sections. RESULTS: The expression of p53, p16 and COX-2 was significantly higher in tumorous tissues than in non-tumorons ones. A significant relationship was observed between p53 and COX-2, or p16 and COX-2. But no obvious correlation was seen between p53 and p16 expressions. Logistic regression analysis showed p53 and COX-2 as dependent predictors in pancreatic carcinogenesis, and a reciprocal relationship to neoplastic progression between p53 and COX-2. 19Promoting Bone Mesenchymal Stem Cells and Inhibiting Bacterial Adhesion of Acid-Etched Nanostructured Titanium by Ultraviolet Functionalization显示文摘Titanium(Ti) and its alloys are used extensively in orthopedic implants because of their excellent biocompatibility,mechanical properties and corrosion resistance. However,titanium-based implant materials face many severe complications,such as implant loosening due to poor osseointegration and bacterial infections,which may lead to implant failure. Hence,preparing a biomaterial surface,which enhances the interactions with host cells and inhibits bacterial adhesion,may be an optimal strategy to reduce the incidence of implant failure. This study aims to improve osseointegration and confer antibacterial properties on Ti through a combination of two surface modifications including nanostructuring generated by acid etching and ultraviolet(UV) light treatment.Our results showed that without UV treatment,the acid etching treatment of Ti surface was effective at both improving the adhesion of bone mesenchymal stem cells(BMSCs) and increasing bacterial adhesion. A further UV treatment of the acid-etched surface however,not only significantly improved the cell adhesion but also inhibited bacterial adhesion. The acid-etched nanostructured titanium with UV treatment also showed a significant enhancement on cell proliferation,alkaline phosphatase(ALP) activity and mineralization. 20Validity of the Chinese Version of the Patient-Generated Subjective Global Assessment (PG-SGA) in Lung Cancer Patients显示文摘The prevalence of malnutrition is high among oncology patients in China. Although the Patient- Generated Subjective Global Assessment (PG-SGA) is widely recognized as an important nutritional assessment tool, it has not been validated for Chinese cancer patients. The purpose of this study was to conduct an analysis of nutrition screening and assessment data to validate the Chinese version of the PG-SGA in Chinese patients with lung cancer. This was an observational, cross-sectional study of 2,000 consecutive adult patients with lung cancer treated at several tertiary hospitals in China. Anthropometric and patient descriptive data were collected. The PG-SGA generated a score for the nutritional risk and a global rating for the nutritional status. The internal consistency, external consistency, and construct validity were evaluated, and a known-groups comparison and principal component analyses were conducted to evaluate the reliability and validity of the PG-SGA. The known-groups comparison demonstrated the ability of the PG-SGA to differentiate lung cancer patients of different nutritional and body mass index (BMI) groups. A confirmatory factor analysis supported the original four-factor structure of the PG-SGA. In terms of the internal and external consistency, the PG-SGA demonstrated intra-class correlation coefficients (ICC) up to 0.987. Alternative form validity was also demonstrated between Box 4 of the PG-SGA and the Karnofsky Performance Status score, with excellent external consistency (r = 0.972, P < 0.001). The construct validity was supported, and selected questionnaire dimensions were evident in the principal component analysis. Significant Spearman correlations (P < 0.001) were demonstrated. The Chinese version of the PG-SGA demonstrated significant reliability and sufficient exploratory properties to support its validity. It seems to be a valid tool that can be used to access the nutritional status of Chinese lung cancer patients.
