| 1 | 经直肠超声引导自动活检枪前列腺穿刺260例分析显示文摘目的 :研究经直肠超声引导自动活检枪前列腺穿刺在诊断早期前列腺癌方面的意义。方法 :患者分为两组 ,第 1组 2 30例为经每年一次 PSA筛选 ,PSA>4μg/ L而进行直肠指检 (DRE)和经直肠超声引导自动活检枪穿刺 (TRU S)加活检的澳大利亚患者 ;第 2组 30例为 DRE有可疑结节而进行 TRUS加活检的国内病例。结果 :经直肠超声引导自动活检枪前列腺穿刺所获标本取材全部优良 ,符合病理诊断的要求。第 1组检出前列腺癌82例 ,其中 T2 a期以内 80例 ,T2 b 2例 ,不典型增生 19例 ;第 2组检出前列腺癌 8例 ,其中 1例属 T2 b,已行根治性前列腺切除术 ,另外 7例至少在 T3期以上。所有患者穿刺后未发生严重并发症。结论 :经直肠超声引导自动活检枪前列腺穿刺是确诊早期前列腺癌的重要的方法 ,而且并发症少 ,值得推广。 | 白强 张华卿 王振声 李青 Keith W Kaye | 2001 | 临床泌尿外科杂志2001，16，9： | 17 |

| 4 | 尿垫试验及调查表在根治性前列腺切除术后尿失禁随访中的应用显示文摘目的 :使用尿垫试验及调查表评价前列腺癌根治术后患者的尿失禁情况 ,观察尿失禁对生活质量的影响 ,研究保留尿道括约肌及患者年龄对术后尿失禁发生率的影响。 方法 :使用尿垫试验及调查表对 16 5例经耻骨后前列腺根治术患者进行连续性的 12~ 14个月的随访调查。患者保持与专职尿失禁护士的电话或直接联系 ,并于术后 12个月到门诊填写调查表、进行尿垫试验。按手术方法将病例分为括约肌修复组 (19例 )和括约肌保留组 (14 6例 ) ,对照观察效果 ;对年龄进行分组 (5 1~ 6 0岁组和 6 1~ 70岁组 )比较累积尿控制率的差异。 结果 :16 5例患者均获随访 :填写调查表并进行尿垫试验 ,7例患者回答使用尿垫而诊断为尿失禁 ,其中 1例有严重尿失禁 ,生活质量受到严重影响 ,其余 6例尿失禁轻 ,生活质量影响轻微 ;尿垫试验中 5例患者尿垫重量增加超过 1g而诊断为尿失禁 ,尿垫试验结果和调查表结果在诊断尿失禁上取得了 98.8%的一致率。按照尿失禁的一般诊断标准 ,本研究中尿道外括约肌保留组随访 12 + 个月时尿控率为 99.3% ,括约肌修复组为 94 .7% ;5 1~ 6 0岁组在刚拔除导尿管时 ,6 0 %的患者有尿失禁 ;6 1~ 70岁组在刚拔除导尿管时 ,82 %的患者有尿失禁 ,但在 3个月以后 ,两组趋于一致。 结论 | 白强 陈方 王益鑫 Glenice Wilson Margaret McCarthy Keith W Kaye | 2004 | 中华男科学杂志2004，10，7： | 5 |

| 6 | Stratification of outcomes for mucinous appendiceal adenocarcinoma with peritoneal metastasis by histological grade显示文摘AIM To investigate the importance of a three-tiered histologic grade on outcomes for patients with mucinous appendiceal adenocarcinoma(MAA).METHODS Two hundred and sixty-five patients with MAA undergoing cytoreductive surgery and hyperthermic intraperitoneal chemotherapy were identified from a prospective database from 2004 through 2014. All pathology was reviewed by our gastrointestinal subspecialty pathologists and histological grade was classified as well-differentiated, moderately differentiated, and poorly differentiated. Survival analysis was performed using Cox proportional hazards regression.RESULTS There were 201(75.8%) well-, 45(16.9%) moderatelyand 19(7.2%) poorly-differentiated tumors. Histological grade significantly stratified the 5-year overall survival(OS), 94%, 71% and 30% respectively(P < 0.001) as well as the 5-year disease-free survival(DFS) 66%, 21% and 0%, respectively(P < 0.001). Independent predictors of DFS included tumor grade(HR = 1.78, 95%CI: 1.21-2.63, P = 0.008), lymph node involvement(HR = 0.33, 95%CI: 0.11-0.98, P < 0.02), previous surgical score(HR = 1.31, 95%CI: 1.1-1.65, P = 0.03) and peritoneal carcinomatosis index(PCI)(HR = 1.05, 95%CI: 1.02-1.08, P = 0.002). Independent predictors of OS include tumor grade(HR = 2.79, 95%CI: 1.26-6.21, P = 0.01), PCI(HR = 1.10, 95%CI: 1.03-1.16, P = 0.002), and complete cytoreduction(HR = 0.32, 95%CI: 0.11-0.92, P = 0.03). Tumor grade and PCI were the only independent predictors of both DFS and OS. Furthermore, histological grade and lymphovascular invasion stratified the risk of lymph node metastasis into a low(6%) and high(40%) risk groups. CONCLUSION Our data demonstrates that moderately differentiated MAA have a clinical behavior and outcome that is distinct from well-and poorly-differentiated MAA. | 8 | Evaluation and treatment of internal impingement of the shoulder in overhead athletes显示文摘One of the most common pathologic processes seen in overhead throwing athletes is posterior shoulderpain resulting from internal impingement. 'Internal impingement' is a term used to describe a constellation of symptoms which result from the greater tuberosity of the humerus and the articular surface of the rotator cuff abutting the posterosuperior glenoid when the shoulder is in an abducted and externally rotated position. The pathophysiology in symptomatic internal impingement is multifactorial,involving physiologic shoulder remodeling,posterior capsular contracture,and scapular dyskinesis. Throwers with internal impingement may complain of shoulder stiffness or the need for a prolonged warm-up,decline in performance,or posterior shoulder pain. On physical examination,patients will demonstrate limited internal rotation and posterior shoulder pain with a posterior impingement test. Common imaging findings include the classic 'Bennett lesion' on radiographs,as well as articular-sided partial rotator cuff tears and concomitant SLAP lesions. Mainstays of treatment include intense non-operative management focusing on rest and stretching protocols focusing on the posterior capsule. Operative management is variable depending on the exact pathology,but largely consists of rotator cuff debridement.
