| 2 | Large bowel injuries during gynecological laparoscopy显示文摘Laparoscopy is one of the most frequently preferred surgical options in gynecological surgery and has advantages over laparotomy, including smaller surgical scars, faster recovery, less pain and earlier return of bowel functions. Generally, it is also accepted as safe and effective and patients tolerate it well. However, it is still an intra-abdominal procedure and has the similar potential risks of laparotomy, including injury of a vital structure, bleeding and infection. Besides the wellknown risks of open surgery, laparoscopy also has its own unique risks related to abdominal access methods,pneumoperitoneum created to provide adequate operative space and the energy modalities used during the procedures. Bowel, bladder or major blood vessel injuries and passage of gas into the intravascular space may result from laparoscopic surgical technique. In addition, the risks of aspiration, respiratory dysfunction and cardiovascular dysfunction increase during laparoscopy. Large bowel injuries during laparoscopy are serious complications because 50% of bowel injuries and60% of visceral injuries are undiagnosed at the time of primary surgery. A missed or delayed diagnosis increases the risk of bowel perforation and consequently sepsis and even death. | 3 | Genome-wide MicroRNA Expression Profiles in COPD: Early Predictors for Cancer Development显示文摘Chronic obstructive pulmonary disease(COPD) significantly increases the risk of developing cancer. Biomarker studies frequently follow a case-control set-up in which patients diagnosed with a disease are compared to controls. Longitudinal cohort studies such as the COPD-centered German COPD and SYstemic consequences-COmorbidities NETwork(COSYCONET) study provide the patient and biomaterial base for discovering predictive molecular markers. We asked whether microRNA(miRNA) profiles in blood collected from COPD patients prior to a tumor diagnosis could support an early diagnosis of tumor development independent of the tumor type. From 2741 participants of COSYCONET diagnosed with COPD, we selected 534 individuals including 33 patients who developed cancer during the follow-up period of 54 months and 501 patients who did not develop cancer, but had similar age, gender and smoking history. Genome-wide miRNA profiles were generated and evaluated using machine learning techniques. For patients developing cancer we identified nine miRNAs with significantly decreased abundance(two-tailed unpaired t-test adjusted for multiple testing P < 0.05), including members of the miR-320 family. The identified mi RNAs regulate different cancer-related pathways including the MAPK pathway(P = 2.3 x 10^(-5)). We also observed the impact of confounding factors on the generated miRNA profiles, underlining the value of our matched analysis. For selected miRNAs, qRT-PCR analysis was applied to validate the results. | 4 | Potential triggering factors of acute liver failure as a first manifestation of autoimmune hepatitis-a single center experience of 52 adult patients显示文摘AIM To investigate potential triggering factors leading to acute liver failure(ALF) as the initial presentation of autoimmune hepatitis(AIH).METHODS A total of 565 patients treated at our Department between 2005 and 2017 for histologically-proven AIH were retrospectively analyzed. However, 52 patients(9.2%) fulfilled the criteria for ALF defined by the 'American Association for the Study of the Liver(AASLD)'. According to this definition, patients with 'acute-on-chronic' or 'acute-on-cirrhosis' liver failure were excluded. Following parameters with focus on potential triggering factors were evaluated: Patients' demographics, causation of liver failure, laboratory data(liver enzymes, MELD-score, autoimmune markers, virus serology), liver histology, immunosuppressive regime, and finally, outcome of our patients.RESULTS The majority of patients with ALF were female(84.6%) and mean age was 43.6 ± 14.9 years. Interestingly, none of the patients with ALF was positive for antiliver kidney microsomal antibody(LKM). We could identify potential triggering factors in 26/52(50.0%) of previously healthy patients presenting ALF as their first manifestation of AIH. These were drug-induced ALF(57.7%), virus-induced ALF(30.8%), and preceding surgery in general anesthesia(11.5%), respectively. Unfortunately, 6 out of 52 patients(11.5%) did not survive ALF and 3 patients(5.7%) underwent liver transplantation(LT). Comparing data of survivors and patients with non-recovery following treatment, MELDscore(P < 0.001), age(P < 0.05), creatinine(P < 0.01), and finally, ALT-values(P < 0.05) reached statistical significance. CONCLUSION Drugs, viral infections, and previous surgery may trigger ALF as the initial presentation of AIH. | 5 | Circulating extracellular RNAs,myocardial remodeling,and heart failure in patients with acute coronary syndrome显示文摘Background:Given high on-treatment mortality in heart failure(HF),identifying molecular pathways that underlie adverse cardiac remodeling may offer novel biomarkers and therapeutic avenues.Circulating extracellular RNAs(ex-RNAs)regulate important biological processes and are emerging as biomarkers of disease,but less is known about their role in the acute setting,particularly in the setting of HF.Methods:We examined the ex-RNA profiles of 296 acute coronary syndrome(ACS)survivors enrolled in the Transitions,Risks,and Actions in Coronary Events Center for Outcomes Research and Education Cohort.We measured 374 ex-RNAs selected a priori,based on previous findings from a large population study.We employed a two-step,mechanism-driven approach to identify ex-RNAs associated with echocardiographic phenotypes(left ventricular[LV]ejection fraction,LV mass,LV end-diastolic volume,left atrial[LA]dimension,and LA volume index)then tested relations of these ex-RNAs with prevalent HF(N=31,10.5%).We performed further bioinformatics analysis of microRNA(miRNAs)predicted targets’genes ontology categories and molecular pathways.Results:We identified 44 ex-RNAs associated with at least one echocardiographic phenotype associated with HF.Of these 44 exRNAs,miR-29-3p,miR-584-5p,and miR-1247-5p were also associated with prevalent HF.The three microRNAs were implicated in the regulation p53 and transforming growth factor-βsignaling pathways and predicted to be involved in cardiac fibrosis and cell death;miRNA predicted targets were enriched in gene ontology categories including several involving the extracellular matrix and cellular differentiation.Conclusions:Among ACS survivors,we observed that miR-29-3p,miR-584-5p,and miR-1247-5p were associated with both echocardiographic markers of cardiac remodeling and prevalent HF.Relevance for Patients:miR-29c-3p,miR-584-5p,and miR-1247-5p were associated with echocardiographic phenotypes and prevalent HF and are potential biomarkers for adverse cardiac remodeling in HF. | Khanh-Van Tran Kahraman Tanriverdi Gerard P.Aurigemma Darleen Lessard Mayank Sardana Matthew Parker Amir Shaikh Matthew Gottbrecht Zachary Milstone Selim Tanriverdi Olga Vitseva John F.Keaney Catarina I.Kiefe David D.McManus Jane E.Freedman | 2019 | Journal of Clinical & Translational Research2019,5,1: | 7 |
| 6 | Oxidative stress and lipid peroxidation products:effect of pinealectomy or exogenous melatonin injections on biomarkers of tissue damage during acute pancreatitis显示文摘BACKGROUND:Melatonin (N-acetyl-5-methoxytripta-mine) is a free radical scavenger and a strong antioxidant,secreted by the pineal gland.In this study,we evaluated the effects of decreasing and increasing serum melatonin levels on malonyldialdehyde (MDA),superoxide dismutase (SOD),and reduced glutathione (GSH) levels in pancreatic tissue from rats with experimental acute pancreatitis.METHODS:Experimental acute pancreatitis was induced in three groups of Wistar albino rats (10 animals per group) by pancreatic ductal ligation.The first group had only acute pancreatitis and served as the control.Surgical pinealectomy was added to acute pancreatitis in the second group,removing the source of endogenous melatonin (low melatonin levels group).The third group was given 0.1 ml daily intraperitoneal injections of 20 mg/ml melatonin solution for one week (high melatonin levels group).The effects of melatonin levels were evaluated by comparison of the levels of MDA,SOD,and GS in pancreatic tissue.RESULT:We found that intraperitoneal melatonin injections decreased the levels of MDA and increased the levels of SOD and GSH in pancreatic tissue.CONCLUSION:Exogenous melatonin has a preventive effect on lipid peroxidation and oxidative damage in acute pancreatitis. | Cavit l Kahraman Dinler O■uz Hasdemir Oktay Büyüka■ik Güler Bu■dayci | 2010 | Hepatobiliary & Pancreatic Diseases International2010,9,1: | 7 |

| 8 | Rock characterization while drilling and application of roof bolter drilling data for evaluation of ground conditions显示文摘Despite recent advances in mine health and safety, roof collapse and instabilities are still the leading causes of injury and fatality in underground mining operations. Improving safety and optimum design of ground support requires good and reliable ground characterization. While many geophysical methods have been developed for ground characterizations, their accuracy is insufficient for customized ground support design of underground workings. The actual measurements on the samples of the roof and wall strata from the exploration boring are reliable but the related holes are far apart, thus unsuitable for design purposes. The best source of information could be the geological back mapping of the roof and walls, but this is disruptive to mining operations, and provided information is only from rock surface.Interpretation of the data obtained from roof bolt drilling can offer a good and reliable source of information that can be used for ground characterization and ground support design and evaluations. This paper offers a brief review of the mine roof characterization methods, followed by introduction and discussion of the roof characterization methods by instrumented roof bolters. | 12 | Umbilical cord derived mesenchymal stem cell implantation in retinitis pigmentosa: a 6-month follow-up results of a phase 3 trial显示文摘AIM:To investigate the efficacy and the safety of umbilical cord derived mesenchymal stem cell(UC-MSC)implantation in patients with retinitis pigmentosa(RP).METHODS:This prospective,single-center,phase 3 clinical study enrolled 124 eyes of 82 RP patients.The patients received 5 million UC-MSCs to the suprachoroidal area with a surgical procedure.Patients were evaluated on the 1st day,1st,and 6th months postoperatively.Best corrected visual acuity(BCVA),anterior segment and fundus examinations,color photography,optical coherence tomography(OCT),and visual field(VF)tests were carried out at each visit.Fundus fluorescein angiography(FFA)and multifocal electroretinography(mfERG)recordings were performed at the end of the 6th month.Ocular and systemic adverse events of the surgical procedure were also noted.RESULTS:All of the 82 patients completed the 6-month follow-up period.None of them had any serious systemic or ocular complications.There were statistically significant improvements in BCVA and VF during the study(all P<0.05).The amplitudes of the P1 waves in the central areas showed significant improvements in mfERG recordings.There were also significant increases in implicit times of P1 waves in the central areas.CONCLUSION:Suprachoroidal administration of UC-MSCs has beneficial effect on BCVA,VF,and mfERG measurements during the 6-month follow-up period.Cell mediated therapy based on the secretion of growth factors(GFs)seems to be an effective and safe option for degenerative retinal diseases. | Neslihan Sinim Kahraman Ayse Oner | 2020 | International Journal of Ophthalmology(English edition)2020,13,9: | 2 |
