1Chronic Liver Failure-Sequential Organ Failure Assessment is better than the Asia-Pacific Association for the Study of Liver criteria for defining acute-on-chronic liver failure and predicting outcome显示文摘AIM:To compare the utility of the Chronic Liver FailureSequential Organ Failure Assessment(CLIF-SOFA)and Asia-Pacific Association for the Study of Liver(APASL)definitions of acute-on-chronic liver failure(ACLF)in predicting short-term prognosis of patients with ACLF.METHODS:Consecutive patients of cirrhosis with acute decompensation were prospectively included.They were grouped into ACLF and no ACLF groups as per CLIF-SOFA and APASL criteria.Patients were followed up for 3 mo from inclusion or mortality whichever was earlier.Mortality at 28-d and 90-d was compared between no ACLF and ACLF groups as per both criteria.Mortality was also compared between different grades of ACLF as per CLIF-SOFA criteria.Prognostic scores like CLIF-SOFA,Acute Physiology and ChronicHealth Evaluation(APACHE)-Ⅱ,Child-Pugh and Model for End-Stage Liver Disease(MELD)scores were evaluated for their ability to predict 28-d mortality using area under receiver operating curves(AUROC).RESULTS:Of 50 patients,38 had ACLF as per CLIFSOFA and 19 as per APASL criteria.Males(86%)were predominant,alcoholic liver disease(68%)was the most common etiology of cirrhosis,sepsis(66%)was the most common cause of acute decompensation while infection(66%)was the most common precipitant of acute decompensation.The 28-d mortality in no ACLF and ACLF groups was 8.3%and 47.4%(P=0.018)as per CLIF-SOFA and 39%and 37%(P=0.895)as per APASL criteria.The 28-d mortality in patients with no ACLF(n=12),ACLF grade 1(n=11),ACLF grade 2(n=14)and ACLF grade 3(n=13)as per CLIF-SOFA criteria was 8.3%,18.2%,42.9%and76.9%(χ2 for trend,P=0.002)and 90-d mortality was 16.7%,27.3%,78.6%and 100%(χ2 for trend,P<0.0001)respectively.Patients with prior decompensation had similar 28-d and 90-d mortality(39.3%and 53.6%)as patients without prior decompensation(36.4%and 63.6%)(P=NS).AUROCs for 28-d mortality were 0.795,0.787,0.739 and 0.710 for CLIF-SOFA,APACHE-Ⅱ,Child-Pugh and MELD scores respectively.On multivariate analysis of these scores,CLIF-SOFA was the only significant independent predictor of mortality with an odds ratio 1.538(95%CI:1.078-2.194).CONCLUSION:CLIF-SOFA criteria is better than APASL criteria to classify patients into ACLF based on their prognosis.CLIF-SOFA score is the best predictor of short-term mortality.Radha K Dhiman Swastik Agrawal Tarana Gupta Ajay Duseja Yogesh Chawla 2014World Journal of Gastroenterology2014,20,40:26

5Aceclofenac-induced hepatotoxicity:An ameliorative effect of Terminalia bellirica fruit and ellagic acid显示文摘BACKGROUND Aceclofenac(ACF),a widely used nonsteroidal anti-inflammatory drug,has been associated with a number of severe cases of clinical hepatotoxicity.Terminalia bellirica,an evergreen tree,is known to have several ethnomedicinal uses including antioxidant and hepatoprotective effects.Hence T.bellirica fruit extracts and its phytoconstituent ellagic acid(EA)are expected to provide protection against oxidative stress and liver damage produced by long-term use of ACF.AIM To evaluate the antioxidant and hepatoprotective activities of T.bellirica fruit extracts and EA against ACF-induced toxicity in albino Wistar rats.METHODS The in vitro antioxidant activities of T.bellirica fruit ethyl acetate and aqueous extracts were measured by metal ion chelation and nitric oxide radical scavenging assays.The in vivo antioxidant and hepatoprotective effects of T.bellirica extracts(200 mg/kg)and EA(40 mg/kg)in ACF-induced hepatotoxic rats were assessed in serum and liver tissue after oral administration for 21 d.Silymarin(40 mg/kg)was used as a standard control.Oxidative stress markers in the blood(ferric reducing ability of plasma and lipid peroxidation inhibition)and liver tissues(superoxide dismutase,catalase and malondialdehyde)were analyzed using standard protocols.Liver function markers such as alkaline phosphatase,glutamic pyruvic transaminase,glutamic oxaloacetic transaminase,lactate dehydrogenase,γ-glutamyl transferase,creatinine,total protein,and uric acid were evaluated in rat serum.RESULTS The T.bellirica fruit ethyl acetate extract exhibited superior metal ion chelating and nitric oxide radical scavenging abilities during in vitro antioxidant assays as compared to aqueous extracts.Oral administration of ACF in rats(15 mg/kg)for 21 d produced oxidative stress and adversely affected liver function suggesting liver injury.Treatment with extracts(ethyl acetate and aqueous),EA and silymarin accounted for a significant reduction in the adverse effects of ACF on oxidative stress and liver function markers in serum and hepatic tissue in rats.Histopathological evaluation of the liver indicated that the extracts and EA significantly decreased the degree of liver damage.The in vivo efficacy of EA was higher than T.bellirica fruit extracts.Of these extracts,ethyl acetate extract revealed comparatively better antioxidant and hepatoprotective activity.CONCLUSION Ellagic acid and T.bellirica fruit extracts exhibited considerable hepatoprotective and antioxidant activities in long-term ACF-treated rats.Ashutosh Gupta Abhay K Pandey 2020World Journal of Hepatology2020,12,11:6
6Gastric remnant twist in the immediate post-operative period following laparoscopic sleeve gastrectomy显示文摘Twist of stomach remnant post sleeve gastrectomy is a rare entity and difficult to diagnose pre-operatively. We are reporting a case of gastric volvulus post laparoscopic sleeve gastrectomy, which was managed conservatively. A 38-year-old lady with a body mass index of 54 underwent laparoscopic sleeve gastrectomy. Sleeve gastrectomy was performed over a 32 French bougie using Endo-GIA tri-stapler. On post-operative day 1, patient had nausea and non-bilious vomiting. An upper gastrointestinal gastrografin study on postoperative days 1 and 2 revealed collection of contrast in the fundic area of stomach with poor flow distally, and she vomited gastrograffin immediately post procedure. With the suspicion of a stricture in the mid stomach as the cause, the patient was taken back for a exploratory laparoscopy and intra-operative endoscopy. We found a twist in the gastric tube which was too tight for the endoscope to pass through. This was managed conservatively with a long stent to keep the gastric tube straight and patent. The stent was discontinued in 7 d and the patient did well. In laparoscopic sleeve gastrectomy the stomach is converted into a tube and is devoid of its supports. If the staples fired are not aligned appropriately, it can predispose this stomach tube to undergo torsion along its long axis. Such a twist can be avoided by properly aligning the staples and by placing tacking sutures to the omentum and new stomach tube. 7CT patterns of nodal disease in pediatric chest tuberculosis显示文摘AIM:To highlight various patterns of nodal involvement and post treatment changes in pediatric chest tuberculosis based on contrast enhanced computed tomography(CECT) scans of chest.METHODS:This was a retrospective study consisting of 91 patients aged less than 17 years,who attended Paediatrics OPD of All India Institute of Medical Sciences with clinically diagnosed tuberculosis or with chest radiographs suggestive of chest tuberculosis.These patients had an initial chest radiograph as well as CECT of the chest and follow up imaging after 6 mo,and in some cases 9 mo,of completion of anti-tubercular treatment(ATT).CECT of these patients was reviewed for the location and extent of nodal involvement along with determination of site,size,enhancement pattern and calcification.RESULTS:Enlargement of mediastinal or hilar lymph nodes was found in 88/91 patients(96.7%),with the most common locations being paratracheal(84.1%),and subcarinal(76.1%).The most common pattern of enhancement was found to be inhomogenous.The nodes were conglomerate in 56.8% and discrete in 43.2%.In addition,perinodal fat was obscured in 84.1% of patients.In the post-treatment scan,there was 87.4% reduction in the size of the nodes.All nodes post-treatment were discrete and homogenous with perinodal fat present.Calcification was found both pre-and post-treatment,but there was an increase in incidence after treatment(41.7%).There was hence a reduction in size,change in enhancement pattern,and appearance of perinodal fat with treatment.CONCLUSION:Tubercular nodes have varied appearance and enhancement pattern.Conglomeration and obscuration of perinodal fat suggest activity.In residual nodes decision to continue ATT requires clinical correlation.Amar Mukund Rashmi Khurana Ashu S Bhalla Arun K Gupta Sushil K Kabra 2011World Journal of Radiology2011,3,1:5

9Transarterial chemoembolization for liver metastases from solid pseudopapillary epithelial neoplasm of pancreas: A case report显示文摘Solid pseudo-papillary epithelial neoplasm(SPEN) is a rare epithelial tumor of pancreas with a low malignant potential occurs most commonly in young females. We report a case of 40 years old woman presented withextensive liver metastasis from SPEN of pancreatic body for which she was operated four years ago. Due to the extensive nature of metastatic disease she was offered Transarterial chemoembolisation(TACE) using gemcitabine as chemotherapeutic agent. Short term follow up after a month of TACE with multiphase computed tomography showed > 90% resolution in the viable tumor with significant clinical improvement. TACE ensures targeted delivery of chemotherapeutic drugs in higher doses with least systemic toxicity and is more effective and safe than systemic chemotherapy. 10Acute kidney injury and hepatorenal syndrome in cirrhosis显示文摘Acute kidney injury(AKI)in cirrhosis,including hepatorenal syndrome(HRS),is a common and serious complication in cirrhotic patients,leading to significant morbidity and mortality.AKI is separated into two categories,non-HRS AKI and HRS-AKI.The most recent definition and diagnostic criteria of AKI in cirrhosis and HRS have helped diagnose and prognosticate the disease.The pathophysiology behind non-HRS-AKI and HRS is more complicated than once theorized and involves more processes than just splanchnic vasodilation.The common biomarkers clinicians use to assess kidney injury have significant limitations in cirrhosis patients;novel biomarkers being studied have shown promise but require further studies in clinical settings and animal models.The overall management of non-HRS AKI and HRS-AKI requires a systematic approach.Although pharmacological treatments have shown mortality benefit,the ideal HRS treatment option is liver transplantation with or without simultaneous kidney transplantation.Further research is required to optimize pharmacologic and nonpharmacologic approaches to treatment.This article reviews the current guidelines and recommendations of AKI in cirrhosis.Kapil Gupta Abhishek Bhurwal Cindy Law Scott Ventre Carlos D Minacapelli Savan Kabaria You Li Christopher Tait Carolyn Catalano Vinod K Rustgi 2021World Journal of Gastroenterology2021,27,26:5
11Association of NFKB1 gene polymorphism (rs28362491) with levels of inflammatory biomarkers and susceptibility to diabetic nephropathy in Asian Indians显示文摘AIM To investigate the association of NFKB1 gene-94 ATTG insertion/deletion(rs28362491) polymorphism with inflammatory markers and risk of diabetic nephropathy in Asian Indians.METHODS A total of 300 subjects were recruited(100 each), normoglycemic,(NG); type 2 diabetes mellitus(T2DM) without any complications(DM) and T2 DM with diabetic nephropathy [DM-chronic renal disease(CRD)]. Analysis was carried out by polymerase chain reaction-restriction fragment length polymorphism and ELISA. Pearson's correlation, analysis of variance and logistic regression wereused for statistical analysis.RESULTS The allelic frequencies of-94 ATTG insertion/deletion were 0.655/0.345(NG), 0.62/0.38(DM) and 0.775/0.225(DM-CRD). 12Hepatoprotective effect of Solatium xanthocarpum fruit extract against CCl_4 induced acute liver toxicity in experimental animals显示文摘Objective:To investigate the hepatoprotective potential of Solonum xanthocarpum(Solanaceae) (S.xanthocarpum) in experimental rats to validate its traditional claim.Methods:50%ethanolic fruit extract of S.xanthocarpum(SXE,100.200 or 400 mg/kg hods weight) was administered daily for 14davs in experimental animals.Liver injury was induced chemically,by CCl4 administration (1 mL/kg i.p.).The hepatoprotective activity was assessed using various biochemical parameters like aspartate aminotransferase(AST),alanine aminotransferase(ALT).Serum alkaline phosphatise (SALP) and total bilirubin.Meanwhile,in vivo antioxidant activities as lipid peroxidation(LPO), reduced glutathione(GSH).superoxide dismutase(SOD) and calalase(CAT) were screened along with histopathological studies.Results:Obtained results demonstrated that the treatment with SXE significantly(P<0.05- <0.001) and dose-dcpendcntly prevented chemically induced increase in serum levels of hepatic enzymes.Furthermore.SXE significantly(up to P<0.001) reduced the lipid peroxidation in the liver tissue and restored activities of defence antioxidant enzymes GSH,SOU and catalasc towards normal levels.Histopathology of the liver tissue showed that SXE attenuated the hepatocellular necrosis and led to reduction of inflammatory cells inflltration. 13Diffusion weighted imaging in gynecological malignancies-present and future显示文摘The management of gynaecological malignancies has undergone a significant change in recent years with our improved understanding of cancer biogenetics, development of new treatment regimens and enhanced screening. Due to the rapid blooming of newer methods and techniques in gynaecology, surgery and oncology the scope and the role of imaging has also widened. Functional imaging in the form of diffusion weighted imaging(DWI) has been recently found to be very useful in assessing various tumours. Its ability to identify changes in the molecular level has dramatically changed the diagnostic approach of radiologists which was solely based on morphological criteria. It can improve the diagnostic accuracy of conventional magnetic resonance imaging, lend a hand in assessing tumour response to treatment regimens and detect tumour recurrence with better spatial resolution, negative radiation and diagnostic accuracy compared to positron emission tomography scan. The ability to quantify the diffusion has also lead to potential prediction of tumour aggressiveness and grade which directly correlate with the patient prognosis and management. Hence, it has become imperative for a radiologist to understand the concepts of DWI and its present and evolving role. 14Concurrent removal of intravitreal lens fragments after phacoemulsification with pars plana vitrectomy prevents development of retinal detachment显示文摘AIM: To evaluate the outcomes of 'concurrent vitrectomy' to retrieve dislocated lens fragment during phacoemulsification.METHODS: In a retrospective, observational case series, data of patients who underwent 'concurrent' pars plana vitrectomy(PPV) for dislocated lens fragments between the period 2000 and 2008 were reviewed. Data collected included patient demographics, pre-operative visual acuity, intra-operative occurrence of retinal breaks, duration of follow up, post-operative intraocular pressure, final best-corrected visual acuity(BCVA),presence of cystoid macular edema(CME) and occurrence of rhegmatogenous retinal detachment(RRD).· RESULTS: A total of 58 eyes of 58 patients were included in the study. At 12 mo the mean postoperative BCVA was log MAR 0.17(20/30) with a range of log MAR0 to 0.69(20/20 to 20/100), with 96.6%(56/58) of patients showing post- operative improvement in visual acuity(P =0.005). 16Plasma cell leukemia-one in a million: A case report显示文摘BACKGROUND Plasma cell leukemia(PCL) is diagnosed by the presence of an absolute plasma cell count of > 2 × 109/L or 20% plasma cells in the peripheral blood. Because the incidence of PCL is relatively low, our case report study presents a rare opportunity to describe the clinical and pathological characteristics of this leukemia, as well as different modalities of treatment and outcomes of primary PCL(pPCL).CASE SUMMARY A 56-year-old male with a history of hypertension complained of pain in the left flank area which started four months prior to admission. On admission, his vital signs were stable, and physical examination was completely benign. Laboratory evaluation showed hemoglobin of 5.1 g/dL, white blood cell count of 6.6 cells per cubic millimeter with 16% atypical lymphocytes, and platelet count of 51000 per microliter. Peripheral smear showed more than 10%-15% of plasma cells(Figure1), and flow cytometry of peripheral blood confirmed PCL with 24% plasma cells CD138+. Bone marrow biopsy demonstrated 80% plasma cells(38+, 138+, 117+,10-, 19-, 20-, 56-) with 90% cellularity. The Oncology team was consulted, and VCD therapy was started. After completing therapy at 1, 4, 8, and 11 d, the patient was discharged home. The patient was being considered for a bone marrow transplant evaluation within two months of discharge.CONCLUSION PCL is a rare and aggressive form of leukemia with a poor prognosis. 17Cell-type specificity of β-actin expression and its clinicopathological correlation in gastric adenocarcinoma显示文摘AIM:To investigate cell type specific distribution ofβ-actin expression in gastric adenocarcinoma and its correlation with clinicopathological parameters.METHODS:β-actin is a housekeeping gene,frequently used as loading control,but,differentially expresses in cancer.In gastric cancer,an overall increased expression ofβ-actin has been reported using tissue disruptive techniques.At present,no histological data is available to indicate its cell type-specific expression and distribution pattern.In the present study,we analyzedβ-actin expression and distribution in paired normal and tumor tissue samples of gastric adenocarcinoma patients using immunohistochemistry(IHC),a tissue non-disruptive technique as well as tissue disruptive techniques like reverse transcriptase-polymerase chain reaction(RT-PCR)and western blotting.Correlation ofβ-actin level with clinicopathological parameters was done using univariate analysis.RESULTS:The results of this study showed significant overexpression,at both mRNA and protein level in tumor tissues as confirmed by RT-PCR(1.47±0.13 vs2.36±0.16;P<0.001)and western blotting(1.92±0.26 vs 2.88±0.32;P<0.01).IHC revealed thatβ-actin expression is majorly distributed between epithelial and inflammatory cells of the tissues.Inflammatory cells showed a significantly higher expression compared to epithelial cells in normal(2.46±0.13 vs 5.92±0.23,P<0.001),as well as,in tumor tissues(2.79±0.24 vs6.71±0.14,P<0.001).Further,comparison of immunostaining between normal and tumor tissues revealed that both epithelial and inflammatory cells overexpressβ-actin in tumor tissues,however,significant difference was observed only in inflammatory cells(5.92±0.23vs 6.71±0.14,P<0.01).Moreover,combined expression in epithelial and inflammatory cells also showed significant increase(4.19±0.15 vs 4.75±0.14,P<0.05)in tumor tissues.In addition,univariate analysis showed a positive correlation ofβ-actin level of inflammatory cells with tumor grade(P<0.05)while epithelial cells exhibited negative correlation(P>0.05).CONCLUSION:In gastric cancer,β-actin showed an overall higher expression predominantly contributed by inflammatory or tumor infiltrating immune cells of the tissue microenvironment and correlates with tumor grade.Shafqat A Khan Monica Tyagi Ajit K Sharma Savio G Barreto Bhawna Sirohi Mukta Ramadwar Shailesh V Shrikhande Sanjay Gupta 2014World Journal of Gastroenterology2014,20,34:2
18Negundoside,an irridiod glycoside from leaves of Vitex negundo,protects human liver cells against calcium-mediated toxicity induced by carbon tetrachloride显示文摘AIM:To evaluate the protective effect of 2'-p-hydroxy benzoylmussaenosidic acid [negundoside(NG) ,against carbon tetrachloride(CCl4) -induced toxicity in HuH-7 cells. METHODS:CCl4 is a well characterized hepatotoxin,and inducer of cytochrome P450 2E1(CYP2E1) -mediated oxidative stress. In addition,lipid peroxidation and accumulation of intracellular calcium are important steps in the pathway involved in CCl4 toxicity. Liver cells(HuH-7) were treated with CCl4,and the mechanism of the cytoprotective effect of NG was assessed. Silymarin,a known hepatoprotective drug,was used as control. RESULTS:NG protected HuH-7 cells against CCl4 toxicity and loss of viability without modulating CYP2E1 activity. Prevention of CCl4 toxicity was associated with a reduction in oxidative damage as reflected by decreased generation of reactive oxygen species(ROS) ,a decrease in lipid peroxidation and accumulation of intracellular Ca2+ levels and maintenance of intracellular glutathione homeostasis. Decreased mitochondrial membranepotential(MMP) ,induction of caspases mediated DNA fragmentation and cell cycle arrest,as a result of CCl4 treatment,were also blocked by NG. The protection afforded by NG seemed to be mediated by activation of cyclic adenosine monophosphate(cAMP) synthesis and inhibition of phospholipases(cPLA2) .
