| 1 | Hepatitis B virus infection, diabetes mellitus, and their synergism for cholangiocarcinoma development: A casecontrol study in Korea显示文摘AIM: To identify possible risk factors and their synergism for cholangiocarcinoma development.METHODS: A hospital-based, case-control study in which we included 276 cholangiocarcinoma patients [193 extrahepatic cholangiocarcinoma(ECC) and 83 intrahepatic cholangiocarcinoma(ICC)], diagnosed at a training hospital in Korea between 2007 and 2013, and 552 healthy controls matched 2:1 for age, sex, and date of diagnosis. Risk factors for cholangiocarcinoma and possible synergism between those factors were evaluated using conditional logistic regression and synergism index, respectively.RESULTS:There was an association between cholangiocarcinoma and hepatitis B virus(HBV) infection, diabetes mellitus(DM), cholecystolithiasis, choledocholithiasis, and hepatolithiasis, with the adjusted odds ratios(AORs) of 4.1, 2.6, 1.7, 12.4, and 39.9, respectively. Synergistic interaction on the additive model was investigated between HBV infection and DM(AOR = 12.2; 95%CI: 1.9-80.1). In the subgroup analyses, cholecystolithiasis, choledocholithiasis, hepatolithiasis, and DM were significant risk factors for ECC(AOR = 2.0, 18.1, 14.9, and 2.0, respectively), whereas choledocholithiasis, hepatolithiasis, HBV infection, and DM were risk factors for ICC(AOR = 8.6, 157.4, 5.3 and 4.9, respectively). Synergistic interaction was also observed between HBV infection and DM(OR = 22.7; 95%CI: 2.4-214.1). However, there was no synergistic interaction between other significant risk factors for cholangiocarcinoma.CONCLUSION: In this Korean study, HBV infection and DM were found to exert independent and synergistic effects on the risk for cholangiocarcinoma, including ICC. | 2 | Endoscopic and clinicopathologic characteristics of early gastric cancer with high microsatellite instability显示文摘AIM:To investigate endoscopic and clinicopathologic characteristics of early gastric cancer (EGC) according to microsatellite instability phenotype.METHODS:Data were retrospectively collected from a single tertiary referral center.Of 981 EGC patients surgically treated between December 2003 and October 2007,73 consecutive EGC patients with two or more microsatellite instability (MSI) mutation [high MSI (MSI-H)] and 146 consecutive EGC patients with one or no MSI mutation (non-MSI-H) were selected.The endoscopic and clinicopathologic features were compared between the MSI-H and non-MSI-H EGC groups.RESULTS:In terms of endoscopic characteristics,MSI-H EGCs more frequently presented with elevated pattern (OR 4.38,95% CI:2.40-8.01,P < 0.001),moderateto-severe atrophy in the surrounding mucosa (OR 1.91,95% CI:1.05-3.47,P = 0.033),antral location (OR 3.99,95% CI:2.12-7.52,P < 0.001) and synchronous lesions,compared to non-MSI-H EGCs (OR 2.65,95% CI:1.16-6.07,P = 0.021).Other significant clinicopathologic characteristics of MSI-H EGC included predominance of female sex (OR 2.77,95% CI:1.53-4.99,P < 0.001),older age (> 70 years) (OR 3.30,95% CI:1.57-6.92,P = 0.002),better histologic differentiation (OR 2.35,95% CI:1.27-4.34,P = 0.007),intestinal type by Lauren classification (OR 2.34,95% CI:1.15-4.76,P = 0.019),absence of a signet ring cell component (OR 2.44,95% CI:1.02-5.86,P = 0.046),presence of mucinous component (OR 5.06,95% CI:1.27-20.17,P = 0.022),moderate-to-severe lymphoid stromal reaction (OR 3.95,95% CI:1.59-9.80,P = 0.003),and co-existing underlying adenoma (OR 2.66,95% CI:1.43-4.95,P = 0.002).CONCLUSION:MSI-H EGC is associated with unique endoscopic and clinicopathologic characteristics including frequent presentation in protruded type,co-existing underlying adenoma,and synchronous lesions. | Jaehoon Jahng Young Hoon Youn Kwang Hyun Kim Junghwan Yu Yong Chan Lee Woo Jin Hyung Sung Hoon Noh Hyunki Kim Hoguen Kim Hyojin Park Sang In Lee | 2012 | World Journal of Gastroenterology2012，18，27： | 6 |
| 3 | Risk factors for posterior synechiae of the iris after 23-gauge phacovitrectomy显示文摘AIM:To identify risk factors for the development of posterior synechiae of the iris(PSI) after 23-gauge phacovitrectomy.METHODS:A retrospective chart review was performed in consecutive Asian patients treated with 23-gauge phacovitrectomy with a 3-piece intraocular lens(IOL) or a single-piece 4 haptics IOL.RESULTS:A total of 263 eyes from 242 patients were included in the study. Postoperative PSI was identified in16(6.1%) eyes. In multivariate analysis, C3F8 gas tamponade, oil tamponade, and long operation time were significantly associated with PSI formation. There was no difference in the incidence of PSI between the groups using two different types of IOL(P =0.779).·CONLUSION: C3F8 gas or oil tamponade and long operation time increased the incidence of PSI after 23-gauge phacovitrectomy. | 4 | Effect of ladle outlet geometry on internal porosity in gravity casting automotive brackets: An experimental investigation显示文摘This study investigated whether the pouring ladle's outlet shape could reduce the porosity of aluminum alloy casting products and whether it could be applied to mass production of automotive brackets based on computer simulations and experiment.The filling behaviors of the melt were simulated to compare the flow characteristics of the molten metal poured from the conventional ladle and the proposed ladle.Results show that for the conventional ladle,the pouring metal is V-shaped,while it is relatively circular and poured at a more constant rate in the proposed ladle.CT scans reveal that the proposed ladle reduces the porosity of the cast product.The proposed ladle resulted in an average of 30 fewer pores,a 299.56 mm3 smaller average total pore volume,and a 0.041% lower difference in average porosity.It is concludes that the proposed ladle can be used in the gravity casting process.By changing the ladle outlet shape can reduce the casting failure rate of massproduced automobile bracket. | Jaehoon Choi Hoyoung Hwang Shinill Kang | 2020 | China Foundry2020，17，1： | 2 |
