| 2 | Material and regenerative properties of an osteon-mimetic cortical bone-like scaffold显示文摘The objective of this work was to fabricate a rigid,resorbable and osteoconductive scaffold by mimicking the hierarchical structure of the cortical bone.Aligned peptide-functionalize nanofiber microsheets were generated with calcium phosphate(CaP)content similar to that of the natural cortical bone.Next,the CaP-rich fibrous microsheets were wrapped around a microneedle to form a laminated microtube mimicking the structure of an osteon.Then,a set of the osteon-mimetic microtubes were assembled around a solid rod and the assembly was annealed to fuse the microtubes and form a shell.Next,an array of circular microholes were drilled on the outer surface of the shell to generate a cortical bone-like scaffold with an interconnected network of Haversian-and Volkmann-like microcanals.The CaP content,porosity and density of the bone-mimetic microsheets were 240 wt%,8%and 1.9 g/ml,respectively,which were close to that of natural cortical bone.The interconnected network of microcanals in the fused microtubes increased permeability of a model protein in the scaffold.The cortical scaffold induced osteogenesis and vasculogenesis in the absence of bone morphogenetic proteins upon seeding with human mesenchymal stem cells and endothelial colony-forming cells.The localized and timed-release of morphogenetic factors significantly increased the extent of osteogenic and vasculogenic differentiation of human mesenchymal stem cells and endothelial colony-forming cells in the cortical scaffold.The cortical bonemimetic nature of the cellular construct provided balanced rigidity,resorption rate,osteoconductivity and nutrient diffusivity to support vascularization and osteogenesis. | Danial Barati Ozan Karaman Seyedsina Moeinzadeh Safaa Kader Esmaiel Jabbari | 2019 | Regenerative Biomaterials2019,6,2: | 5 |
| 3 | Inoperable esophageal cancer and outcome of palliative care显示文摘AIM:To determine the outcome of esophageal cancer patients referred for palliative care,in Gorgan and Gonbad gastrointestinal clinics,northeast of Iran. METHODS:This cross-sectional study was done on inoperable esophageal cancer cases referred to gastrointestinal clinics in Gorgan and Gonbad city(2005-2006) . Demographic data were collected during the procedure and cases were followed up every one month. Improvement proportion was calculated with 95% confidence interval,to determine the rate of improvement. Survival analysis and Kaplan-Meier methods were used to estimate the duration of palliative care effectiveness. RESULTS:We recruited 39 cases into the study. Squamous cell carcinoma was the most prevalent(92.3%) . The middle third of the esophagus was involved predominantly(51.3%) . Dilation was the most preferred method(89.7%) and stenting was done in 4 cases. Decreasing dysphagia score was not related to palliation method or pathology type of carcinoma. Age of the patients was significantly related to the improvement of dysphagia score. Mean survival time was 137.6 d and median was 103 d. | 4 | Effects of rhubarb(Rheum ribes L.)syrup on dysenteric diarrhea in children:a randomized,double-blind,placebo-controlled trial显示文摘BACKGROUND:Rheum ribes L.is a plant native to China,Iran,Turkey,India,and a few other countries.Antidiarrheal activity is considered to be one of its important properties according to various systems of traditional medicine.An increasing rate of bacterial resistance to antibiotics has led to treatment failure in some cases of shigellosis in children,and underlines a need for safe,efficient and valid options.OBJECTIVE:The purpose of this study is to evaluate the efficacy of R.ribes syrup as a complementary medicine for treatment of shigellosis in children.DESIGN,SETTING,PARTICIPANTS AND INTERVENTIONS:This randomized,doubleblind,placebocontrolled trial started with a group of 150 children aged between 12–72 months with suspected Shigella dysentery.R.ribes syrup or placebo syrup was administered to the intervention and control groups,respectively for 5 days.In addition,the standard antibiotic treatment(ceftriaxone for the first 3 days and cefixime syrup for 2 further days)was administered to both groups.MAIN OUTCOME MEASURES:Body temperature,abdominal pain,need for antipyretics,defecation frequency,stool volume and consistency and microscopic stool examination were recorded as outcome measures.Any observed adverse effects were also recorded.RESULTS:Mean duration of fever and diarrhea in the R.ribes group was significantly lower than that in the placebo group(P=0.016 and 0.001,respectively).In addition,patients in the R.ribes group showed shorter duration of need for antipyretics and shorter duration of abdominal pain(P=0.012 and 0.001,respectively).However,there were no significant differences between the two groups regarding the microscopic stool analyses.Furthermore,no adverse effect was reported.CONCLUSION:R.ribes syrup can be recommended as a complementary treatment for children with Shigella dysentery. | Ali Khiveh Mohammad Hashem Hashempur Mehrdad Shakiba Mohammad Hassan Lotfi Afsaneh Shakeri Seid Kazem Kazemeini Zohre Mousavi Marzie Jabbari Mohammad Kamalinejad Majid Emtiazy | 2017 | Journal of Integrative Medicine2017,15,5: | 3 |

| 6 | Pacemaker implantation in a patient with brugada and sick sinus syndrome显示文摘Brugada syndrome (BrS) is a rare and inherited primary arrhythmic syndrome characterized by ST-segment elevations in the right precordial leads (V1 -V3 ) with an increased risk of sudden cardiac death (SCD). Arrhythmias in BrS are often nocturne, and brady-arrhythmias are often seen in patients with loss-of-function mutations in SCN5A . In this case-report we present a 75-year old woman referred to our outpatient clinic for inherited cardiac diseases for a familial clinical work-up. Since childhood she had suffered from dizziness, absence seizures, and countless Syncope's. In 2004 sick sinus syndrome was suspected and she wastreated with implantation of a pacemaker (PM) at another institution. An inherited cardiac disease was one day suddenly suspected, as the patient had a 61-year old brother who was diagnosed with symptomatic BrS, and treated with an implantable cardioverter defibrillator (ICD) after aborted SCD. A mutation screening revealed a SCN5A [S231CfsX251 (c.692-693delCA )] loss-of-function mutation not previously reported, and as a part of the cascade screening in relatives she was therefore referred to our clinic. In the 7 year period after PM implantation she had experienced no cardiac symptoms, although her electrocardiogram changes now were consistent with a BrS type 1 pattern. A genetic test confirmed that she had the same mutation in SCN5A as her brother. In this case-report we present a loss-of function mutation in SCN5A not previously associated with BrS nor presented in healthy controls. Sinus node dysfunction has previously been documented in patients with symptomatic BrS, which suggests it is not a rare concomitant. The only accepted treatment of BrS is today implantation of an ICD.
