Current status and progress of pancreatic cancer in China显示文摘Cancer is currently one of the most important public health problems in the world.Pancreatic cancer is a fatal disease with poor prognosis. As in most other countries, the health burden of pancreatic cancer in China is increasing, with annual mortality rates almost equal to incidence rates. The increasing trend of pancreatic cancer incidence is more significant in the rural areas than in the urban areas.Annual diagnoses and deaths of pancreatic cancer in China are now beyond the number of cases in the United States.GLOBOCAN 2012 estimates that cases in China account for 19.45%(65727/337872) of all newly diagnosed pancreatic cancer and 19.27%(63662/330391) of all deaths from pancreatic cancer worldwide. The population's growing socioeconomic status contributes to the rapid increase of China's proportional contribution to global rates. Here,we present an overview of control programs for pancreatic cancer in China focusing on prevention, early diagnosis and treatment. In addition,we describe key epidemiological, demographic, and socioeconomic differences between China and developed countries.Facts including no nationwide screening program for pancreatic cancer, delay in early detection resulting in a late stage at presentation, lack of awareness of pancreatic cancer in the Chinese population, and low investment compared with other cancer types by government have led to backwardness in China's pancreatic cancer diagnosis and treatment. Finally,we suggest measures to improve health outcomes of pancreatic cancer patients in China.Quan-Jun Lin Feng Yang Chen Jin De-Liang Fu 2015World Journal of Gastroenterology2015,21,26:93
A rapid advice guideline for the diagnosis and treatment of 2019 novel coronavirus(2019-nCoV) infected pneumonia(standard version)显示文摘In December 2019, a new type viral pneumonia cases occurred in Wuhan, Hubei Province;and then named '2019 novel coronavirus(2019-nCoV)' by the World Health Organization(WHO) on 12 January 2020. For it is a never been experienced respiratory disease before and with infection ability widely and quickly, it attracted the world’s attention but without treatment and control manual. For the request from frontline clinicians and public health professionals of 2019-nCoV infected pneumonia management, an evidence-based guideline urgently needs to be developed. 7Clinical characteristics of 24 asymptomatic infections with COVID-19 screened among close contacts in Nanjing,China显示文摘Previous studies have showed clinical characteristics of patients with the 2019 novel coronavirus disease(COVID-19)and the evidence of person-to-person transmission.Limited data are available for asymptomatic infections. This study aims to present the clinical characteristics of 24 cases with asymptomatic infection screened from close contacts and to show the transmission potential of asymptomatic COVID-19 virus carriers.Epidemiological investigations were conducted among all close contacts of COVID-19 patients(or suspected patients)in Nanjing,Jiangsu Province,China, from Jan 28 to Feb 9,2020,both in clinic and in community.Asymptomatic carriers were laboratory-confirmed positive for the COVID-19 virus by testing the nucleic acid of the pharyngeal swab samples.Their clinical records, laboratory assessments, and chest CT scans were reviewed. As a result, none of the 24 asymptomatic cases presented any obvious symptoms while nucleic acid screening.Five cases(20.8%)developed symptoms(fever, cough, fatigue, etc.)during hospitalization.Twelve(50.0%)cases showed typical CT images of ground-glass chest and 5(20.8%)presented stripe shadowing in the lungs. The remaining 7(29.2%)cases showed normal CT image and had no symptoms during hospitalization. These 7 cases were younger(median age:14.0 years;P=0.012)than the rest.None of the 24 cases developed severe COVID-19 pneumonia or died. The median communicable period, defined as the interval from the first day of positive nucleic acid tests to the first day of continuous negative tests, was 9.5 days(up to 21 days among the 24 asymptomatic cases).Through epidemiological investigation,we observed a typical asymptomatic transmission to the cohabiting family members, which even caused severe COVID-19 pneumonia.Overall, the asymptomatic carriers identified from close contacts were prone to be mildly ill during hospitalization. However, the communicable period could be up to three weeks and the communicated patients could develop severe illness. These results highlighted the importance of close contact tracing and longitudinally surveillance via virus nucleic acid tests.Further isolation recommendation and continuous nucleic acid tests may also be recommended to the patients discharged.Zhiliang Hu Ci Song Chuanjun Xu Guangfu Jin Yaling Chen Xin Xu Hongxia Ma Wei Chen Yuan Lin Yishan Zheng Jianming Wang Zhibin Hu Yongxiang Yi Hongbing Shen 2020Science China(Life Sciences)2020,63,5:77
Study of liver cirrhosis over ten consecutive years in Southern China显示文摘AIM: To investigate the etiology and complications of liver cirrhosis(LC) in Southern China. METHODS: In this retrospective, cross-sectional study, we identified cases of liver cirrhosis admitted between January 2001 to December 2010 and reviewed the medical records. Patient demographics, etiologies and complications were collected, and etiological changes were illustrated by consecutive years and within twotime periods(2001-2005 and 2006-2010). All results were expressed as the mean ± SD or as a percentage. The χ2 test or Student's t-test was used to analyze the differences in age, gender, and etiological distribution, and one-way analysis of variance was applied to estimate the trends in etiological changes. We analyzed the relationship between the etiologies and complications using unconditioned logistic regression, and the risk of upper gastrointestinal bleeding(UGIB) and hepatocellular carcinoma(HCC) in the major etiological groups was evaluated as ORs. A P value less than 0.05 was considered significant. Statistical computation was performed using SPSS 17.0 software.RESULTS: In this study, we identified 6719(83.16%) male patients and 1361(16.84%) female patients. The average age of all of the patients was 50.5 years at the time of diagnosis. The distribution of etiological agents was as follows: viral hepatitis, 80.62% [hepatitis B virus(HBV) 77.22%, hepatitis C virus(HCV) 2.80%,(HBV + HCV) 0.58%]; alcohol, 5.68%; mixed etiology, 4.95%; cryptogenic, 2.93%; and autoimmune hepatitis, 2.03%; whereas the other included etiologies accounted for less than 4% of the total. Infantile hepatitis syndrome LC patients were the youngest(2.5 years of age), followed by the metabolic LC group(27.2 years of age). Viral hepatitis, alcohol, and mixed etiology were more prevalent in the male group, whereas autoimmune diseases, cryptogenic cirrhosis, and metabolic diseases were more prevalent in the female group. When comparing the etiological distribution in 2001-2005 with that in 2006-2010, the proportion of viral hepatitis decreased from 84.7% to 78.3%(P < 0.001), and the proportion of HBV-induced LC also decreased from 81.9% to 74.6%(P < 0.001). The incidence of mixed etiology, cryptogenic cirrhosis, and autoimmune diseases increased by 3.1%(P < 0.001), 0.5%(P = 0.158), and 1.3%(P < 0.001), respectively. Alcohol-induced LC remained relatively steady over the 10-year period. The ORs of the development of UGIB between HBV and other major etiologies were as follows: HCV, 1.07; alcohol, 1.89; autoimmune, 0.90; mixed etiology, 0.83; and cryptogenic, 1.76. The ORs of the occurrence of HCC between HBV and other major etiologies were as follows: HCV, 0.54; alcohol, 0.16; autoimmune, 0.05; mixed etiology, 0.58; and cryptogenic, 0.60.CONCLUSION: The major etiology of liver cirrhosis in Southern China is viral hepatitis. However, the proportions of viral hepatitis and HBV are gradually decreasing. Alcoholic LC patients exhibit a greater risk of experiencing UGIB, and HBV LC patients may have a greater risk of HCC.Xing Wang Shang-Xiong Lin Jin Tao Xiu-Qing Wei Yuan-Ting Liu Yu-Ming Chen Bin Wu 2014World Journal of Gastroenterology2014,20,37:72
Effects of different resuscitation fluid on severe acute pancreatitis显示文摘AIM: To compare effects of different resuscitation fluid on microcirculation, inflammation, intestinal barrier and clinical results in severe acute pancreatitis (SAP). METHODS: One hundred and twenty patients with SAP were enrolled at the Pancreatic Disease Institute between January 2007 and March 2010. The patients were randomly treated with normal saline (NS group), combination of normal saline and hydroxyethyl starch (HES) (SH group), combination of normal saline, hydroxyethyl starch and glutamine (SHG group) in resuscitation. The ratio of normal saline to HES in the SH and SHG groups was 3:1. The glutamine (20% glutamine dipeptide, 100 mL/d) was supplemented into the resuscitation liquid in the SHG group. Complications and outcomes including respiratory and abdominal infection, sepsis, abdominal hemorrhage, intra-abdominal hypertension, abdominal compartment syndrome (ACS), renal failure, acute respiratory distress syndrome (ARDS), multiple organ dysfunction syndrome (MODS), operation intervention, length of intensive care unit stay, length of hospital stay, and mortality at 60 d were compared. Moreover, blood oxygen saturation (SpO 2 ), gastric intramucosal pH value (pHi), intra-abdominal pressure (IAP), inflammation cytokines, urine lactulose/mannitol (L/M) ratio, and serum endotoxin were investigated to evaluate the inflammatory reaction and gut barrier. RESULTS: Compared to the NS group, patients in the SH and SHG groups accessed the endpoint more quickly (3.9 ± 0.23 d and 4.1 ± 0.21 d vs 5.8 ± 0.25 d, P < 0.05) with less fluid volume (67.26 ± 28.53 mL/kg/d, 61.79 ± 27.61 mL/kg per day vs 85.23 ± 21.27 mL/kg per day, P < 0.05). Compared to the NS group, incidence of renal dysfunction, ARDS, MODS and ACS in the SH and SHG groups was obviously lower. Furthermore, incidence of respiratory and abdominal infection was significantly decreased in the SH and SHG groups, while no significant difference in sepsis was seen. Moreover, less operation time was needed in the SH and SHG group than the NS group, but the difference was not significant. The mortality did not differ significantly among these groups. Blood SpO 2 and gastric mucosal pHi in the SH and SHG groups increased more quickly than in the NS group, while IAP was significantly decreased in the SH and SHG group. Moreover, the serum tumor necrosis factor-α, interleukin-8 and C-reactive protein levels in the SH and SHG groups were obviously lower than in the NS group at each time point. Furthermore, urine L/M ratio and serum endotoxin were significantly lower in the SH group and further decreased in the SHG group.CONCLUSION: Results indicated that combination of normal saline, HES and glutamine are more efficient in resuscitation of SAP by relieving inflammation and sustaining the intestinal barrier.Gang Zhao Jun-Gang Zhang He-Shui Wu Jin Tao Qi Qin Shi-Chang Deng Yang Liu Lin Liu Bo Wang Kui Tian Xiang Li Shuai Zhu Chun-You Wang 2013World Journal of Gastroenterology2013,19,13:56

Thermal regimes and degradation modes of permafrost along the Qinghai-Tibet Highway显示文摘Permafrost on the Qinghai-Tibet Plateau (QTP) is widespread, thin, and thermally unstable. Under a warming climate during the past few decades, it has been degrading extensively with generally rising ground temperatures, the deepening of the maximum summer thaw, and with lessening of the winter frost penetration. The permafrost has degraded downward, upward and laterally. Permafrost has thinned or, in some areas, has totally disappeared. The modes of permafrost degradation have great significance in geocryology, in cold regions engineering and in cold regions environmental management. Permafrost in the interior of the QTP is well represented along the Qing-hai-Tibet Highway (QTH), which crosses the Plateau through north to south and traverses 560 km of permafrost-impacted ground. Horizontally, the degradation of permafrost occurs more visibly in the sporadic permafrost zone in the vicinity of the lower limit of permafrost (LLP), along the margins of taliks, and around permafrost islands. Downward degradation develops when the maximum depth of seasonal thaw exceeds the maximum depth of seasonal frost, and it generally results in the formation of a layered talik disconnecting the permafrost from the seasonal frost layer. The downward degrada- tion is divided into four stages: 1) initial degradation, 2) accelerated degradation, 3) layered talik and 4) finally the conversion of permafrost to seasonally frozen ground (SFG). The upward degradation occurs when the geothermal gradient in permafrost drops to less than the geothermal gradients in the underlying thawed soil layers. Three types of permafrost temperature curves (stable, degrading, and phase-changing transitory permafrost) illustrate these modes. Although strong differentiations in local conditions and permafrost types exist, the various combinations of the three degradation modes will ultimately transform permafrost into SFG. Along the QTH, the downward degradation has been proceeding at annual rates of 6 to 25 cm, upward degradation at 12 to 30 cm, and lateral degradation in the sporadic permafrost zone at 62 to 94 cm during the last quarter century. These rates exceed the 4 cm per year for the past 20 years reported for the discontinuous permafrost zone in subarctic Alaska, the 3 to 7 cm per year reported in Mongolia, and that of the thaw-stable permafrost in subarctic Yakutia and Arctic Alaska.JIN Huijun ZHAO Lin WANG Shaoling JIN Rui 2006Science China Earth Sciences2006,49,11:50
A new principle of synthetic cascade utilization of chemical energy and physical energy显示文摘We propose a new principle of the cascade utilization of both chemical energy and physical energy in energy systems with the integration of chemical processes and thermal cycles. Particularly, a general equation of energy levels of substance, Gibbs free energy of chemical reaction and physical energy is explicitly founded. On the basis of this equation, a chemical-looping combustion and an indirect combustion are investigated. Furthermore, a mechanism of energy release, with the combination of decreasing the energy level of Gibbs free energy and upgrading the energy level of low or middle- temperature thermal energy, is clarified. The promising results obtained here establish a theoretical basis for the further investigation of multi-function systems in which energy and the environment are compatible, and create a new approach to improve the performance of traditional thermal cycles.JIN Hongguang1, HONG Hui1,2, WANG Baoqun1,2, HAN Wei1,2 & LIN Rumou1,2 1. 15Further Information of the Associated Li Deposits in the No.6 Coal Seam at Jungar Coalfield, Inner Mongolia, Northern China显示文摘Total 138 coal samples and 14 parting samples were taken from the No. 6 Seam of the Jungar Coalfield, Inner Mongolia. These samples were analysed by optical microscopy, sequential chemical extraction procedure (SCEP), inductively coupled plasma mass spectrometry (ICP-MS), X-ray powder diffraction (XRD), and scanning electron microscope in conjunction with an energy-dispersive X-ray spectrometer (SEM-EDX) analysis. The results indicate that the Li contents have reached the industrial grade of the coal associated Li deposit, and the total Li reserves have reached 2406600 tons, that is, 5157000 tons Li2O in the No. 6 seam in the Jungar Coalfield. The sequential chemical extraction procedure results suggest that the Li concentration is mainly related to inorganic matter. The minerals in the coals consist of kaolinite, boehmite, chlorite-group mineral, quartz, calcite, pyrite, siderite and amorphous clay material. Some Li could be absorbed by clay minerals in the Li-bearing coal seam. The chlorite phase?could be?most likely the host for a part of Li. The Yinshan Oldland should be the most possible source of Li of the coal.SUN Yuzhuang ZHAO Cunliang LI Yanheng WANG Jinxi ZHANG Jianya JIN Zhe LIN Mingyue Wolfgang KALKREUTH 2013Acta Geologica Sinica(English Edition)2013,87,4:33

Guidelines on multidisciplinary approaches for the prevention and management of diabetic foot disease(2020 edition)显示文摘In recent years,as living standards have continued to improve, the number of diabetes patients in China, along with the incidence of complications associated with the disease, has been increasing.Among these complications, diabetic foot disease is one of the main causes of disability and death in diabetic patients.Due to the differences in economy, culture, religion and level of medical care available across different regions, preventive and treatment methods and curative results for diabetic foot vary greatly. In multidisciplinary models built around diabetic foot, the timely assessment and diagnosis of wounds and appropriate methods of prevention and treatment with internal and external surgery are key to clinical practice for this pathology. In 2019,under the leadership of the Jiangsu Medical Association and Chinese Diabetes Society, the writing group for the Guidelines on multidisciplinary approaches for the prevention and management of diabetic foot disease(2020 edition)was established with the participation of scholars from the specialist areas of endocrinology, burn injury, vascular surgery, orthopedics, foot and ankle surgery and cardiology.Drawing lessons from diabetic foot guidelines from other countries, this guide analyses clinical practices for diabetic foot, queries the theoretical basis and grades and gives recommendations based on the characteristics of the pathology in China. This paper begins with assessments and diagnoses of diabetic foot, then describes treatments for diabetic foot in detail, and ends with protections for high-risk feet and the prevention of ulcers. This manuscript covers the disciplines of internal medicine, surgical, nursing and rehabilitation and describes a total of 50 recommendations that we hope will provide procedures and protocols for clinicians dealing with diabetic foot.Aiping Wang Guozhong Lv Xingbo Cheng Xianghua Ma Wei Wang Jianchao Gui JiHu Meng Lu Guoping Chu Jin’an Chen Hao Zhang Yiqiu Jiang Yuedong Chen Wengbo Yang Lin Jiang Houfa Geng Rendong Zheng Yihui Li Wei Feng Boey Johnson Wenjuan Wang Dalong Zhu Yin Hu 2020Burns & Trauma2020,8,1:34
Photovoltaic and Solar Power Forecasting for Smart Grid Energy Management显示文摘Due to the challenge of climate and energy crisis, renewable energy generation including solar generation has experienced significant growth.Increasingly high penetration level of photovoltaic(PV)generation arises in smart grid.Solar power is intermittent and variable,as the solar source at the ground level is highly dependent on cloud cover variability, atmospheric aerosol levels, and other atmosphere parameters. The inherent variability of large-scale solar generation introduces significant challenges to smart grid energy management.Accurate forecasting of solar power/irradiance is critical to secure economic operation of the smart grid. This paper provides a comprehensive review of the theoretical forecasting methodologies for both solar resource and PV power.Applications of solar forecasting in energy management of smart grid are also investigated in detail. Can Wan Jian Zhao Yonghua Song Zhao Xu Jin Lin Zechun Hu 2015CSEE Journal of Power and Energy Systems2015,1,4:29
The Yellow Sea green tides were dominated by one species, Ulva (Enteromorpha) prolifera, from 2007 to 2011显示文摘Green tides have occurred in the Yellow Sea successively from 2007 to 2011. Genetic analysis of the 5-year green-tide-forming algae needs to be performed to determine the source of the biomass and understand the mechanism of the green tide blooms. In this study, free-floating green algae were collected at different sites in the Yellow Sea in 2010 and 2011. Data on 182 free-floating samples and 155 attached Ulva samples from previous studies on the Yellow Sea green tides from 2007 to 2009 were also taken into consideration. Morphology observation and molecular phylogenetic analyses indicated that the Yellow Sea green tides were dominated by a single species, Ulva prolifera, from 2007 to 2011. Considering that at least five Ulva species inhabit the north coast of China, the unialgal composition of the green tides implied that (1) there may be some special physiology and propagation pathways of U. prolifera for its rapid expansion, (2) the mechanisms of the Yellow Sea green tide formation were similar for the last five years, and (3) the intra-species genetic variation and population structure of U. prolifera need to be studied to determine the exact origin of the bloom-forming biomass.ZHAO Jin JIANG Peng LIU ZhengYi WEI Wei LIN HanZhi LI FuChao WANG JinFeng QIN Song 2013Chinese Science Bulletin2013,58,19:27
