| 1 | 肛瘘填料与纤维蛋白胶对肛门直肠瘘的闭合效果比较显示文摘PURPOSE:Long-term closure rates of anorectal fistulas using fibrin glue have been disappointing,possibly because of the liquid consistency of the glue.A suturable bioprosthetic plug(Surgisis,Cook Surgical,Inc.)was fashioned to close the primary opening of fistula tracts.A prospective cohort study was performed to compare fibrin glue vs.the anal fistula plug.METHODS:Patients with high transsphincteric fistulas,or deeper,were prospectively enrolled.Patients with Crohn’s disease or superficial fistulas were excluded.Age,gender,number and type of fistula tracts,and previous fistula surgeries were compared between groups.Under general anesthesia and in prone jackknife position,the tract was irrigated with hydrogen peroxide.Fistula tracts were occluded by fibrin glue vs.closure of the primary opening using a Surgisisanal fistula plug.RESULTS:Twenty-five patients were prospectively enrolled.Ten patients underwent fibrin glue closure,and 15 used a fistula plug.Patient’s age,gender,fistula tract characteristics,and number of previous closure attempts was similar in both groups.In the fibrin glue group,six patients(60 percent)had persistence of one or more fistulas at three months,compared with two patients(13 percent)in the plug group(P < 0.05,Fisher exact test).CONCLUSIONS:Closure of the primary opening of a fistula tract using a suturable biologic anal fistula plug is an effective method of treating anorectal fistulas.The method seems to be more reliable than fibrin glue closure.The greater efficacy of the fistula plug may be the result of the ability to suture the plug in the primary opening,therefore,closing the primary opening more effectively.Further prospective,long-term studies are warranted. | Johnson E.K. | 14 | Evaluation of genetic diversity in short duration cotton(Gossypium hirsutum L.)显示文摘Background: Cotton(Gossypium hirsutum L.) is an important fiber crop in Bangladesh. Genetic diversity among the genotypes of a germplasm has a great importance for cotton breeding. An experiment was carried out at the experimental field of Cotton Research, Training and Seed Multiplication Farm, Sreepur, Gazipur during the cropping season of 2015-2016 with 100 genotypes to evaluate genetic diversity of cotton genotypes for short duration using field performance.Results: The genotypes under study were grouped into ten clusters through multivariate analysis using GENSTAT-5.Cluster III contained maximum number of genotypes(16) while cluster X contained the least number of genotypes(7).The inter cluster distances were larger than intra cluster distances in all cases suggesting wider genetic diversity among the genotypes of different clusters. The maximum and minimum inter cluster distances were observed between clusters II and V(10.78) and clusters VIII and IX(3.30), respectively. The results indicated diverse and close relationship among the genotypes of those clusters. Earliness index, single boll weight and days to boll opening showed the higher contribution to the genetic divergence among 19 characters.Conclusion: Based on the results of genetic diversity and earliness index, the genotypes from cluster Ⅱ could be used as parent in hybridization program for the development of short duration cotton variety. | AKTER T. | 16 | 吉西他滨-卡铂方案后续贯性应用紫杉醇-卡铂方案作为晚期卵巢癌一线治疗方案:一项Ⅱ期研究显示文摘Objective. To determine the feasibility and efficacy of sequential gemcitabine- carboplatin followed by paclitaxel- carboplatin in the first- line treatment of advanced epit-helial ovarian cancer, with the response rate as the primary endpoint. Methods. After primary laparotomy, 56 patients with FIGO Stages III- IV disease were given 4 cycles of gemcitabine 1000 mg/m2 d1,8 and carboplatin AUC5 (44 patients) or AUC6 (12 patients) d1 q3wk followed by 4 cycles of paclitaxel 175 mg/m2 d1 and carboplatin AUC5/6 q3wk. Of the tumors, 43 were serous, 6 clear cell, 4 endometrioid, and 3 anaplastic type. Thirty- seven (66.1% ) of the patients were suboptimally debulked. Results. Forty-seven patients were evaluable for response by CA- 125 criteria, and 46 (98% ) responded. Thirty patients (after gemcitabine- carboplatin) and 24 (after paclitaxel- carboplatin) were evaluable for response by CT (RECIST criteria), respectively. After the four gemcitabine- carboplatin cycles, the objective response rate was 83% (6 CR, 19 PR). Following completion of the whole sequential regimen, 7 patients showed a CR and 15 a PR, respectively, giving a response rate of 92% . The median progression- free survival was 12.8 months after a median follow- up of 19 months (range 7- 35 months)- .The median overall survival has not been reached yet. The main toxicity was neutropenia as 139/221 (62.9% ) of the gemcitabine- carboplatin cycles and 92/181 (50.8% ) of the paclitaxel- carboplatin cycles, respectively, were associated with Grades 3- 4 neutropenia. Neutropenia was reported as a serious adverse event in 5.7% of the cycles, and GCSF support was needed in 18.4% of the cycles. Only the gemcitabine- carboplatin cycles were associated with a marked thrombocytopenia (32.1% Grades 3- 4). Of the other side effects, marked allergy occurred in 14/52 (27% ) exposed to paclitaxel. A total of 14 patients discontinued the treatment prematurely: 3 due to lack of efficacy, 1 due to protocol violation, and 10 due to toxicity (4 allergic reactions to paclitaxel, 3 complicated neutropenias, 1 fever, and 2 unspecified toxicities). The mean relative dose intensities were: gemcitabine 84.0% , paclitaxel 85.4% , and carboplatin 96.5% . Of the gemci-tabine- carboplatin cycles and paclitaxel- carboplatin cycles, 32% and 38% were delayed, respectively. Gemcitabine d8 dose had to be omitted in 8% of the cycles. Conclusion. The sequential regimen of gemcitabine- carboplatin followed by paclitaxel- carboplatin is feasible in chemotherapy- naive ovarian cancer.
