1Characterization of focal liver lesions with SonoVue~-enhanced sonography: International multicenter-study in comparison to CT and MRI显示文摘AIM: To evaluate in a multicenter study whether the sonographic characterization of focal liver lesions can be improved using SonoVueüà-enhancement; and to compare this method with computed tomography (CT) and magnetic resonance imaging (MRI). METHODS: One hundred and thirty four patients with one focal liver lesion detected in baseline ultrasound (US) were examined with conventional US, contrastenhanced US (n = 134), contrast-enhanced CT (n = 115) and/or dynamic contrast-enhanced MRI (n = 70). The lesions were classified as malignant, benign or indeterminate and the type of lesion was determined. The f inal diagnosis based on the combined information of all imaging examinations, clinical information and histology (n = 32) was used. Comparisons were made to see whether the addition of contrast-enhanced US led to the improvement of the characterization of doubtful focal liver lesions. RESULTS: In comparison with unenhanced US, SonoVue markedly improves sensitivity and specificity for the characterization (malignant/benign) of focal liver lesions. In comparison with CT and/or dynamic MRI, SonoVueüà -enhanced sonography applied for characterization of focal liver lesions was 30.2% more sensitive in the recognition of malignancy and 16.1% more specific in the exclusion of malignancy and overall 22.9% more accurate. In the subgroup with confirmative histology available (n = 30), sensitivity was 95.5% (CEUS), 72.2% (CT) and 81.8% (MRI), and specif icity was 75.0% (CEUS), 37.5% (CT) and 42.9% (MRI). The sensitivity and specif icity of CEUS for the identif ication of focal nodular hyperplasia (FNH) and hemangiomas was 100% and 87%, resulting in an accuracy of 94.5%. 2Displacement manifold method for type synthesis of lower-mobility parallel mechanisms显示文摘Type synthesis of lower-mobility parallel mechanisms is a hot and frontier topic in international academic and industrial field. Based on the Lie group theory, a displacement manifold synthesis method is proposed. For all the nine kinds of lower-mobility parallel mechanisms, the mechanism displacement manifold, limb displacement manifold and the geometrical conditions which guarantee that the intersection of the limb displacement manifold is the desired mechanism displacement manifold are enumerated. Various limb kinematic chains can be obtained using the product closure of displacement subgroup. Parallel mechanisms can be constructed with these limbs while obeying the geometrical conditions. Hence, all the nine kinds of lower-mobility parallel mechanisms can be synthesized using this method. Since displacement manifold deals with finite motion, the result mechanism of synthesis have full-cycle mobility. 4抗狂犬病病毒中和抗体效价快速荧光灶抑制试验(RFFIT)的建立及应用显示文摘目的建立抗狂犬病病毒中和抗体效价的快速荧光灶抑制试验(RFFIT),验证其与小鼠脑内中和法(MNT)的相关性,并用于马抗狂犬病病毒血清(ERIG)和人狂犬病免疫球蛋白(HRIG)及狂犬病疫苗免疫后血清中和抗体效价的测定。方法待测血清或免疫球蛋白在96孔板中作3倍系列稀释,与能感染80%~95%细胞量的攻击病毒混合后,37℃体外中和1h,接种BSR细胞,培养24h后荧光染色,荧光显微镜下读取结果。用建立的RFFIT法与MNT法同时标定狂犬病免疫球蛋白国家标准品,并检测ERIG和HRIG以及疫苗免疫后血清样品的抗狂犬病病毒中和抗体效价,以比较两种方法的精密度和相关性。结果采用MNT法和RFFIT法检测我国抗狂犬病免疫球蛋白国家标准品的GMT分别为21.4和23.8IU/ml,两种方法的变异系数分别为62.3%和13.3%;两个实验室用RFFIT法测定我国国家标准品的结果分别为21.71和23.81IU/ml。两种方法测定ERIG和HRIG及疫苗免疫后血清中和抗体效价的结果差异无统计学意义,相关系数为0.976,两种方法的检测结果之间呈良好的正相关性。结论RFFIT法可替代MNT法检测抗狂犬病病毒中和抗体效价。吴小红 李加 唐建蓉 Hervé Bourhy 刘景华 曲效民 曹守春 董关木 2009中国生物制品学杂志2009,22,11:19
5Direct shear tests on cemented paste backfill-rock wall and cemented paste backfill-backfill interfaces显示文摘This paper presents the results of the shear strength(frictional strength) of cemented paste backfillcemented paste backfill(CPB-CPB) and cemented paste backfillerock wall(CPB-rock) interfaces. The frictional behaviors of these interfaces were assessed for the short-term curing times(3 d and 7 d) using a direct shear apparatus RDS-200 from GCTS(Geotechnical Consulting & Testing Systems). The shear(friction) tests were performed at three different constant normal stress levels on flat and smooth interfaces. These tests aimed at understanding the mobilized shear strength at the CPB-rock and CPB-CPB interfaces during and/or after open stope filling(no exposed face). The applied normal stress levels were varied in a range corresponding to the usually measured in-situ horizontal pressures(longitudinal or transverse) developed within paste-filled stopes(uniaxial compressive strength, s c 150 k Pa). Results show that the mobilized shear strength is higher at the CPB-CPB interface than that at the CPB-rock interface. Also, the perfect elastoplastic behaviors observed for the CPB-rock interfaces were not observed for the CPB-CPB interfaces with low cement content which exhibits a strain-hardening behavior. These results are useful to estimate or validate numerical model for pressures determination in cemented backfill stope at short term. The tests were performed on real backfill and granite. 8Rapid Detection of Filoviruses by Real-time TaqMan Polymerase Chain Reaction Assays显示文摘Ebola virus (EBOV) and Marburg virus (MARV) are causative agents of severe hemorrhagic fever with high mortality rates in humans and non-human primates and there is currently no licensed vaccine or therapeutics. To date, there is no specific laboratory diagnostic test in China, while there is a national need to provide differential diagnosis during outbreaks and for instituting acceptable quarantine procedures. In this study, the TaqMan RT-PCR assays targeting the nucleoprotein genes of the Zaire Ebolavirus (ZEBOV) and MARV were developed and their sensitivities and specificities were investigated. Our results indicated that the assays were able to make reliable diagnosis over a wide range of virus copies from 103 to 109, corresponding to the threshold of a standard RNA transcript. The results showed that there were about 1010 RNA copies per milliliter of virus culture supernatant, equivalent to 10,000 RNA molecules per infectious virion, suggesting the presence of many non-infectious particles. 9Watch and wait approach in rectal cancer:Current controversies and future directions显示文摘According to the main international clinical guidelines,the recommended treatment for locally-advanced rectal cancer is neoadjuvant chemoradiotherapy followed by surgery.However,doubts have been raised about the appropriate definition of clinical complete response(cCR)after neoadjuvant therapy and the role of surgery in patients who achieve a cCR.Surgical resection is associated with significant morbidity and decreased quality of life(QoL),which is especially relevant given the favourable prognosis in this patient subset. Accordingly, therehas been a growing interest in alternative approaches with less morbidity,including the organ-preserving watch and wait strategy, in which surgery isomitted in patients who have achieved a cCR. These patients are managed with aspecific follow-up protocol to ensure adequate cancer control, including the earlyidentification of recurrent disease. However, there are several open questionsabout this strategy, including patient selection, the clinical and radiologicalcriteria to accurately determine cCR, the duration of neoadjuvant treatment, therole of dose intensification (chemotherapy and/or radiotherapy), optimal followupprotocols, and the future perspectives of this approach. 10Developmental pathways associated with cancer metastasis:Notch,Wnt,and Hedgehog显示文摘Master developmental pathways, such as Notch, Wnt, and Hedgehog, are signaling systems that control proliferation, cell death,motility, migration, and stemness. These systems are not only commonly activated in many solid tumors, where they drive or contribute to cancer initiation, but also in primary and metastatic tumor development. The reactivation of developmental pathways in cancer stroma favors the development of cancer stem cells and allows their maintenance, indicating these signaling pathways as particularly attractive targets for efficient anticancer therapies, especially in advanced primary tumors and metastatic cancers. Metastasis is the worst feature of cancer development. This feature results from a cascade of events emerging from the hijacking of epithelial-mesenchymal transition, angiogenesis, migration, and invasion by transforming cells and is associated with poor survival, drug resistance, and tumor relapse. In the present review, we summarize and discuss experimental data suggesting pivotal roles for developmental pathways in cancer development and metastasis, considering the therapeutic potential. 13Water Resource Monitoring Exploiting Sentinel-2 Satellite and Sentinel-2 Satellite Like Time Series;Application in Yangtze River Water Bodies显示文摘As part of the Dragon 4 project,the water extents of Wuchang and Shengjin lakes have been extracted from Sentinel-2 time series,using all exploitable images since the beginning of the acquisitions in 2015.The aim of the study is to assess the capability of the Sentinel-2 constellation and Landsat 8 over the Anhui region,especially the high temporal resolution.A total of 32 dates have been used and 10 Landsat 8 images(Libra)have been added to try to reduce the temporal gaps in the Sentinel-2 acquisitions caused by cloudy conditions.Extractions were done using a SERTIT-ICube automatized routine based on a supervised Maximum Likelihood Classification.These extractions allow to recreate the dynamic of the two lakes and show the drought and wet periods.During the 3 years interval,the surface peaks in July 2016 for both lakes.The lowest level appears at two different dates for each lake;in January 2018 for Wuchang,in February 2017 for Shengjin.Wuchang Lake surface area appears to be more variable than Shengjin Lake,with many local maximum and minimum between the end of 2017 and April 2018.In the case of Wuchang Lake,floating vegetation is a problem for automatic water surface area extraction.The lake is covered by vegetation during long periods of time and the water below can’t be detected by automatic radiometric means.Nevertheless,Sentinel-2 stays a pertinent and powerful tool for hydrological monitoring of lakes confirming the expectation from the remote sensing wetland community before launch.The presence of NIR and SWIR bands induces a strong discrimination between water and other classes,and the systematic acquisitions create dense time series,making analysis more consistent.It makes possible to sensor events occurring over short periods of time.Thanks to this a link can be done between endangered bird species,such as the Siberian Crane and the Lesser White-Fronted Goose and periodically flooded areas.These midterm results illustrated the pertinence and powerful of multi-source optical satellite data for environmental analysis.Julien BRIANT Huber CLAIRE Studer MATHIAS Lei CAO Kunpeng YI Yésou HERV 2020Journal of Geodesy and Geoinformation Science2020,3,4:8

17Usefulness of staging systems and prognostic scores for hepatocellular carcinoma treatments显示文摘Therapeutic management of hepatocellular carcinoma(HCC) is quite complex owing to the underlying cirrhosis and portal vein hypertension. Different scores or classification systems based on liver function and tumoral stages have been published in the recent years. If none of them is currently 'universally' recognized, the Barcelona Clinic Liver Cancer(BCLC) staging system has become the reference classification system in Western countries. Based on a robust treatment algorithm associated with stage stratification, it relies on a high level of evidence. However, BCLC stage B and C HCC include a broad spectrum of tumors but are only matched with a single therapeutic option. Some experts have thus suggested to extend the indications for surgery or for transarterial chemoembolization. In clinical practice, many patients are already treated beyond the scope of recommendations. Additional alternative prognostic scores that could be applied to any therapeutic modality have been recently proposed. They could represent complementary tools to the BCLC staging system and improve the stratification of HCC patients enrolled in clinical trials, as illustrated by the NIACE score. 18Evaluation of Doppler-ultrasonography in the diagnosis of transjugular intrahepatic portosystemic shunt dysfunction: A prospective study显示文摘AIM To prospectively evaluate the performance of Dopplerultrasonography(US) for the detection of transjugular intrahepatic portosystemic shunt(TIPS) dysfunction within a multicenter cohort of cirrhotic patients.METHODS This study was conducted in 10 french teaching hospitals. After TIPS insertion, angiography and liver Doppler-US were carried out every six months to detect dysfunction(defined by a portosystemic gradient ≥ 12 mm Hg and/or a stent stenosis ≥ 50%). The association between ultrasonographic signs and dysfunction was studied by logistic random-effects models, and the diagnostic performance of each Doppler criterion was estimated by the bootstrap method. This study was approved by the ethics committee of Tours.RESULTS Two hundred and eighteen pairs of examinations performed on 87 cirrhotic patients were analyzed. Variables significantly associated with dysfunction were: The speed of flow in the portal vein(P = 0.008), the reversal of flow in the right(P = 0.038) and left(P = 0.049) portal branch, the loss of modulation of portal flow by the right atrium(P = 0.0005), ascites(P = 0.001) and the overall impression of the operator(P = 0.0001). The diagnostic performances of these variables were low; sensitivity was < 58% and negative predictive value was < 73%. Therefore, dysfunction cannot be ruled out from Doppler-US. CONCLUSION The performance of Doppler-US for the detection of TIPS dysfunction is poor compared to angiography. 19High-resolution multimodal flexible coherent Raman endoscope显示文摘Coherent Raman scattering microscopy is a fast,label-free,and chemically specific imaging technique that shows high potential for future in vivo optical histology.However,the imaging depth in tissues is limited to the sub-millimeter range because of absorption and scattering.Realization of coherent Raman imaging using a fiber endoscope system is a crucial step towards imaging deep inside living tissues and providing information that is inaccessible with current microscopy tools.Until now,the development of coherent Raman endoscopy has been hampered by several issues,mainly related to the fiber delivery of the excitation pulses and signal collection.Here,we present a flexible,compact,coherent Raman,and multimodal nonlinear endoscope(4.2mm outer diameter,71mm rigid length)based on a resonantly scanned hollow-core Kagomé-lattice double-clad fiber.The fiber design enables distortion-less,background-free delivery of femtosecond excitation pulses and back-collection of nonlinear signals through the same fiber.Sub-micrometer spatial resolution over a large field of view is obtained by combination of a miniature objective lens with a silica microsphere lens inserted into the fiber core.We demonstrate high-resolution,high-contrast coherent anti-Stokes Raman scattering,and second harmonic generation endoscopic imaging of biological tissues over a field of view of 320μm at a rate of 0.8 frames per second.These results pave the way for intraoperative label-free imaging applied to real-time histopathology diagnosis and surgery guidance.Alberto Lombardini Vasyl Mytskaniuk Siddharth Sivankutty Esben Ravn Andresen Xueqin Chen Jérôme Wenger Marc Fabert Nicolas Joly Frédéric Louradour Alexandre Kudlinski Hervé Rigneault 2018Light(Science & Applications)2018,7,1:6
20Recommendations for the use of chemoembolization in patients with hepatocellular carcinoma: Usefulness of scoring system?显示文摘Several hepatocellular carcinoma(HCC) staging systems have been established, and a variety of countryspecific treatment strategies are also proposed. The barcelona- clinic liver cancer(BCLC) system is the most widely used in Europe. The Hong Kong liverCancer is a new prognostic staging system; it might become the reference system in Asia. Transarterial chemoembolization(TACE) is the most widely used treatment for HCC worldwide; but it showed a benefit only for intermediate stage HCC(BCLC B), and there is still no consensus concerning treatment methods and treatment strategies.
