| 2 | Achalasia: A review of clinical diagnosis, epidemiology,treatment and outcomes显示文摘Achalasia is a neurodegenerative motility disorder of the oesophagus resulting in deranged oesophageal peristalsis and loss of lower oesophageal sphincter function.Historically,annual achalasia incidence rates were believed to be low,approximately 0.5-1.2 per 100000.More recent reports suggest that annual incidence rates have risen to 1.6 per 100000 in some populations.The aetiology of achalasia is still unclear but is likely to be multi-factorial.Suggested causes include environmental or viral exposures resulting in inflammation of the oesophageal myenteric plexus,which elicits an autoimmune response.Risk of achalasia may be elevated in a sub-group of genetically susceptible people.Improvement in the diagnosis of achalasia,through the introduction of high resolution manometry with pressure topography plotting,has resulted in the development of a novel classification system for achalasia.This classification system can evaluate patient prognosis and predict responsiveness to treatment.There is currently much debate over whether pneumatic dilatation is a superior method compared to the Heller’s myotomy procedure in the treatment of achalasia.A recent com-parative study found equal efficacy,suggesting that patient preference and local expertise should guide the choice.Although achalasia is a relatively rare condition,it carries a risk of complications,including aspiration pneumonia and oesophageal cancer.The risk of both squamous cell carcinoma and adenocarcinoma of the oesophagus is believed to be significantly increased in patients with achalasia,however the absolute excess risk is small.Therefore,it is currently unknown whether a surveillance programme in achalasia patients would be effective or cost-effective. | Orla M O'Neill Brian T Johnston Helen G Coleman | 2013 | World Journal of Gastroenterology2013,19,35: | 26 |
| 3 | Current trends in management of hepatitis B virus reactivation in the biologic therapy era显示文摘Hepatitis B virus (HBV) reactivation represents an emerging cause of liver disease in patients undergoing treatment with biologic agents. In particular, the risk ofHBV reactivation is heightened by the use monoclonalantibodies, such as rituximab (anti-CD20) and alemtuzumab (anti-CD52) that cause profound and longlasting immunosuppression. Emerging data indicatethat HBV reactivation could also develop following theuse of other biologic agents, such as tumor necrosis factor (TNF)-α inhibitors. When HBV reactivation is di-agnosed, it is mandatory to suspend biologic treatmentand start antiviral agents immediately. However, preemptive antiviral therapy prior to monoclonal antibodyadministration is crucial in preventing HBV reactivationand its clinical consequences. Several lines of evidencehave shown that risk of HBV reactivation is greatlyreduced by the identifi cation of high-risk patients andthe use of prophylactic antiviral therapy. | 4 | Cytoplasmic dynein:a key player in neurodegenerative and neurodevelopmental diseases显示文摘Cytoplasmic dynein is the most important molecular motor driving the movement of a wide range of cargoes towards the minus ends of microtubules.As a molecular motor protein,dynein performs a variety of basic cellular functions including organelle transport and centrosome assembly.In the nervous system,dynein has been demonstrated to be responsible for axonal retrograde transport.Many studies have revealed direct or indirect evidence of dynein in neurodegenerative diseases such as amyotrophic lateral sclerosis,Charcot-Marie-Tooth disease,Alzheimer’s disease,Parkinson’s disease and Huntington’s disease.Among them,a number of mutant proteins involved in various neurodegenerative diseases interact with dynein.Axonal transport disruption is presented as a common feature occurring in neurodegenerative diseases.Dynein heavy chain mutant mice also show features of neurodegenerative diseases.Moreover,defects of dynein-dependent processes such as autophagy or clearance of aggregation-prone proteins are found in most of these diseases.Lines of evidence have also shown that dynein is associated with neurodevelopmental diseases.In this review,we focus on dynein involvement in different neurological diseases and discuss potential underlying mechanisms. | CHEN Xiang-Jun XU Huan COOPER Helen M LIU Yaobo | 2014 | Science China(Life Sciences)2014,57,4: | 8 |

| 9 | Apple dwarfing rootstocks exhibit an imbalance in carbohydrate allocation and reduced cell growth and metabolism显示文摘Apple dwarfing rootstocks cause earlier shoot termination and reduced root and shoot mass.To identify physiological factors responsible for rootstock-induced growth restriction,we compared vascular-enriched gene expression between two dwarfing rootstocks(‘M27’and‘M9’)and the vigorous rootstock‘M793’using RNA sequencing and quantitative reverse transcriptase PCR.Differentially expressed genes common to both dwarfing rootstocks belonged to five main biological processes:(1)primary metabolism,(2)cell wall synthesis and modification,(3)secondary metabolism,(4)hormone signalling and response and(5)redox homeostasis.Genes promoting the biosynthesis of amino acids,lipids and cell walls were downregulated in dwarfing rootstocks,whereas genes promoting the breakdown of these compounds were upregulated.The only exception to this trend was the upregulation of starch synthesis genes in dwarfing rootstocks.Non-structural carbohydrate analysis demonstrated that starch concentrations in‘M9’roots,stems and grafted‘Royal Gala’(‘RG’)scions were double that of equivalent tissues from‘RG’homo-grafted trees(‘RG’/‘RG’).Fructose and glucose concentrations were much lower in all three tissues of the‘RG’/‘M9’trees.Together,these data indicate that dwarfing rootstocks are in a state of sugar depletion and reduced cellular activity despite having large starch reserves.Another significant finding was the over-accumulation of flavonoids and the downregulation of auxin influx transporters MdAUX1 and MdLAX2 in dwarfing rootstocks.We propose that both factors reduce polar auxin transport.The results of this study contribute novel information about the physiological state of dwarfing rootstocks. | Toshi M Foster Peter A McAtee Chethi N Waite Helen L Boldingh Tony K McGhie | 2017 | Horticulture Research2017,4,1: | 4 |
| 10 | Total hepatectomy and liver transplantation as a two-stage procedure for fulminant hepatic failure: A safe procedure in exceptional circumstances显示文摘AIM: To evaluate the outcomes of two-stage liver transplant at a single institution, between 1993 and March 2015.METHODS: We reviewed our institutional experience with emergency hepatectomy followed by transplantation for fulminant liver failure over a twenty-year period. A retrospective review of a prospectively maintained liver transplant database was undertaken at a national liver transplant centre. Demographic data, clinical presentation, preoperative investigations, cardiocirculatory parameters, operative and postoperative data were recorded.RESULTS: In the study period, six two-stage liver transplants were undertaken. Indications for transplantation included acute paracetamol poisoning(n = 3), fulminant hepatitis A(n = 1), trauma(n = 1) and exertional heat stroke(n = 1). Anhepatic time ranged from 330 to 2640 min. All patients demonstrated systemic inflammatory response syndrome in the first post-operative week and the incidence of sepsis was high at 50%. There was one mortality, secondary to cardiac arrest 12 h following re-perfusion. Two patients required re-transplantation secondary to arterial thrombosis. | 12 | Diagnostic accuracy of tests for Helicobacter pylori in an Alaska Native population显示文摘AIM:To evaluate the accuracy of two non-invasivetests in a population of Alaska Native persons. Highrates of Helico bacter pylori(H. pylori) infection,H. pylori treatment failure,and gastric cancer in this population necessitate documentation of infection status atmultiple time points over a patient's life.METHODS:In 280 patients undergoing endoscopy,H. pylori was diagnosed by culture,histology,rapidurease test,13C urea breath test(UBT) ,and immuno-globulin G antibodies to H. pylori in serum. The performances of 13C-UBT and antibody test were compared to a gold standard defined by a positive H. pylori testby culture or,in case of a negative culture result,bypositive histology and a positive rapid urease test.RESULTS:The sensitivity and specificity of the 13C-UBT were 93% and 88%,respectively,relative to thegold standard. The antibody test had an equivalents ensitivity of 93% with a reduced specificity of 68%.The false positive results for the antibody test were as-sociated with previous treatment for an H. pylori infection [relative risk(RR) = 2.8]. | 15 | Defining strawberry shape uniformity using 3D imaging and genetic mapping显示文摘Strawberry shape uniformity is a complex trait,influenced by multiple genetic and environmental components.To complicate matters further,the phenotypic assessment of strawberry uniformity is confounded by the difficulty of quantifying geometric parameters‘by eye’and variation between assessors.An in-depth genetic analysis of strawberry uniformity has not been undertaken to date,due to the lack of accurate and objective data.Nonetheless,uniformity remains one of the most important fruit quality selection criteria for the development of a new variety.In this study,a 3D-imaging approach was developed to characterise berry shape uniformity.We show that circularity of the maximum circumference had the closest predictive relationship with the manual uniformity score.Combining five or six automated metrics provided the best predictive model,indicating that human assessment of uniformity is highly complex.Furthermore,visual assessment of strawberry fruit quality in a multi-parental QTL mapping population has allowed the identification of genetic components controlling uniformity.A“regular shape”QTL was identified and found to be associated with three uniformity metrics.The QTL was present across a wide array of germplasm,indicating a potential candidate for marker-assisted breeding,while the potential to implement genomic selection is explored.A greater understanding of berry uniformity has been achieved through the study of the relative impact of automated metrics on human perceived uniformity.Furthermore,the comprehensive definition of strawberry shape uniformity using 3D imaging tools has allowed precision phenotyping,which has improved the accuracy of trait quantification and unlocked the ability to accurately select for uniform berries. | Bo Li Helen M.Cockerton Abigail WJohnson Amanda Karlström Eleftheria Stavridou Greg Deakin Richard J.Harrison | 2020 | Horticulture Research2020,7,1: | 2 |
