| 2 | Clinical characteristics of Helicobacter pylori-negative drugnegative peptic ulcer bleeding显示文摘AIM:To investigate the clinical characteristics and outcomes of idiopathic Helicobacter pylori(H.pylori)-negative and drug-negative]peptic ulcer bleeding(PUB).METHODS:A consecutive series of patients who experienced PUB between 2006 and 2012 was retrospectively analyzed.A total of 232 patients were enrolled in this study.The patients were divided into four groups according to the etiologies of PUB:idiopathic,H.pylori-associated,drug-induced and combined(H.pylori-associated and drug-induced)types.We compared the clinical characteristics and outcomes between the groups.When the silver stain or rapid urease tests were H.pylori-negative,we obtained an additional biopsy specimen by endoscopic re-examination and performed an H.pylori antibody test 6-8 wk after the initial endoscopic examination.For a diagnosis of idiopathic PUB,a negative result of an H.pylori antibody test was confirmed.In all cases,re-bleeding was confirmed by endoscopic examination.For the risk assessment,the Blatchford and the Rockall scores were calculated for all patients.RESULTS:For PUB,the frequency of H.pylori infection was 59.5%(138/232),whereas the frequency of idiopathic cases was 8.6%(20/232).When idiopathic PUB was compared to H.pylori-associated PUB,the idiopathic PUB group showed a higher rate of rebleeding after initial hemostasis during the hospital stay(30%vs 7.4%,P = 0.02).When idiopathic PUB was compared to drug-induced PUB,the patients in the idiopathic PUB group showed a higher rate of rebleeding after initial hemostasis upon admission(30%vs 2.7%,P < 0.01).When drug-induced PUB was compared to H.pylori-associated PUB,the patients in the drug-induced PUB were older(68.49 ± 14.76 years vs 47.83 ± 15.15 years,P< 0.01) and showed a higher proportion of gastric ulcer(77%vs 49%,P < 0.01).However,the Blatchford and the Rockall scores were not significantly different between the two groups.Among the patients who experienced drug-induced PUB,no significant differences were found with respect to clinical characteristics,irrespective of H.pylori infection.CONCLUSION:Idiopathic PUB has unique clinical characteristics such as re-bleeding after initial hemostasis upon admission.Therefore,these patients need to undergo close surveillance upon admission. | Woo Chul Chung Eun Jung Jeon Dae Bum Kim Hea Jung Sung Yeon-Ji Kim Eun Sun Lim Min-ah Kim Jung Hwan Oh | 2015 | World Journal of Gastroenterology2015，21，28： | 8 |
| 3 | Time-dependent effect of combination therapy with erythropoietin and granulocyte colony-stimulating factor in a mouse model of hypoxic-ischemic brain injury显示文摘Erythropoietin (EPO) and granulocyte colonystimulating factor (G-CSF) are likely to play broad roles in the brain. We investigated the effects of combination therapy with EPO and G-CSF in hypoxicischemic brain injury during the acute, subacute, and chronic phases. A total of 79 C57BL/6 mice with hypoxic-ischemic brain injury were randomly assigned acute (days 1–5), subacute (days 11–15) and chronic (days 28–32) groups. All of them were treated with G-CSF (250 μg/kg) and EPO (5 000 U/kg) or saline daily for 5 consecutive days. Behavioral assessments and immunohistochemistry for angiogenesis, neurogenesis, and astrogliosis were performed with an 8-week follow-up. Hypoxia-inducible factor-1 (HIF-1) was also measured by Western blot analysis. The results showed that the combination therapy with EPO and G-CSF in the acute phase significantly improved rotarod performance and forelimb-use symmetry compared to the other groups, while subacute EPO and G-CSF therapy exhibited a modest improvement compared with the chronic saline controls. The acute treatment significantly increased the density of CD31+ (PECAM-1) and α-smooth muscle actin+vessels in the frontal cortex and striatum, increased BrdU+/PSA-NCAM+neurogenesis in the subventricular zone, and decreased astroglial density in the striatum. Furthermore, acute treatment significantly increased the HIF-1 expression in the cytosol and nucleus, whereas chronic treatment did not change the HIF-1 expression, consistent with the behavioral outcomes. | 4 | Effect of esmolol and lidocaine on agitation in awake phase of anesthesia among children:a double-blind,randomized clinical study显示文摘Background:Sevoflurane is widely used to anesthetize children because of its rapid action with minimal irritation of the airways.However,there is a high risk of agitation after emergence from anesthesia.Strabismus surgery,in particular,can trigger agitation because patients have their eyes covered in the postoperative period.The aim of this study was to determine whether or not esmolol and lidocaine could decrease emergence agitation in children.Methods:Eighty-four patients aged 3 to 9 years undergoing strabismus surgery were randomly assigned to a control group(saline only),a group that received intravenous lidocaine 1.5 mg/kg,and a group that received intravenous esmolol 0.5 mg/kg and lidocaine 1.5 mg/kg.Agitation was measured using the objective pain score,Cole 5-point score,and Richmond Agitation Sedation Scale score at the end of surgery,on arrival in the recovery room,and 10 and 30 min after arrival.Results:The group that received the combination of esmolol and lidocaine showed lower OPS and RASS scores than the other two groups when patients awoke from anesthesia(OPS=0(0-4),RASS=-4[(-5)-1])and were transferred to the recovery room(OPS=0(0-8),RASS=-1[(-5)-3])(P<0.05).There was no significant difference in the severity of agitation among the three groups at other time points(P>0.05).Conclusions:When pediatric strabismus surgery is accompanied by sevoflurane anesthesia,an intravenous injection of esmolol and lidocaine could alleviate agitation until arrival in the recovery room. | Jae Young Ji Jin Soo Park Ji Eun Kim Da Hyung Kim Jin Hun Chung Hea Rim Chun Ho Soon Jung Sie Hyeon Yoo | 2019 | Chinese Medical Journal2019，，7： | 3 |
