| 6 | Pediatric Nd:YAG laser capsulotomy in the operating room: review of 87 cases显示文摘AIM: To report a large series of children having Nd:YAG laser capsulotomy in the operating room using the lateral decubitus position. METHODS: Medical records of children who underwent Nd:YAG laser capsulotomy in the operating room at Ann & Robert H. Lurie Children's Hospital of Chicago between September 2008 and April 2017 were reviewed. Induction of general anesthesia and intubation was performed in the supine position after which the patient was placed in lateral decubitus position. The Nd:YAG laser capsulotomy was performed using a standard protocol. At the completion of the procedure, the patient was turned back into the supine position and extubated. RESULTS: This study included 87 eyes of 60 patients. Patient's age ranged from 1 to 18 y(mean 6.4±4.1 y). In most cases(84/87, 97%), the procedure was performed under general anesthesia. In all cases, good focus on the membrane was achieved, and the procedure was performed successfully. There were no intraoperative ocular or anesthesia-related complications. | 7 | Impact of a simulation-based induction programme in gastroscopy on trainee outcomes and learning curves显示文摘BACKGROUND Pre-clinical simulation-based training(SBT)in endoscopy has been shown to augment trainee performance in the short-term,but longer-term data are lacking.AIM To assess the impact of a two-day gastroscopy induction course combining theory and SBT(Structured PRogramme of INduction and Training–SPRINT)on trainee outcomes over a 16-mo period.METHODS This prospective case-control study compared outcomes between novice SPRINT attendees and controls matched from a United Kingdom training database.Study outcomes comprised:(1)Unassisted D2 intubation rates;(2)Procedural discomfort scores;(3)Sedation practice;(4)Time to 200 procedures;and(5)Time to certification.RESULTS Total 15 cases and 24 controls were included,with mean procedure counts of 10 and 3(P=0.739)pre-SPRINT.Post-SPRINT,no significant differences between the groups were detected in long-term D2 intubation rates(P=0.332)or discomfort scores(P=0.090).However,the cases had a significantly higher rate of unsedated procedures than controls post-SPRINT(58%vs 44%,P=0.018),which was maintained over the subsequent 200 procedures.Cases tended to perform procedures at a greater frequency than controls in the post-SPRINT period(median:16.2 vs 13.8 per mo,P=0.051),resulting in a significantly greater proportion of cases achieving gastroscopy certification by the end of follow up(75% vs 36%,P=0.017).CONCLUSION In this pilot study,attendees of the SPRINT cohort tended to perform more procedures and achieved gastroscopy certification earlier than controls.These data support the role for wider evaluation of pre-clinical induction involving SBT. | Keith Siau James Hodson Peter Neville Jeff Turner Amanda Beale Susi Green Aravinth Murugananthan Paul Dunckley Neil D Hawkes | 2020 | World Journal of Gastrointestinal Endoscopy2020，12，3： | 2 |

| 12 | A randomized,fellow eye,comparison of keratometry,aberrometry,tear film,axial length and the anterior chamber depth after eye rubbing in non-keratoconic eyes显示文摘Background:To investigate the effect of eye rubbing on keratometry(K),aberrometry,tear film break-up-time(TFBUT)and anterior chamber depth(ACD).Methods:Volunteers without any corneal pathology or dry eyes were randomised to rubbing in one eye and the fellow-eye was control.Eye rubbing was performed for 2 min.Primary outcomes studied were anterior and posterior K changes.Secondary outcomes were changes in TFBUT,axial length(AL)&ACD,K changes in various zones,asphericity and aberrometry.Pre and post rubbing K,aberrometry,ACD and TFBUT were assessed in a predetermined sequence.The relationship of the above parameters to axial length(AL)was also assessed.Astigmatism was analysed using vector analysis.Results:Pre versus post rubbing,anterior flatter K further flattened(42.51±1.52 D vs.42.36±1.53 D,p=0.003)and the changes to J0 vector in central cornea(−0.16±0.26 D vs.-0.27±0.33 D,p=0.038)suggested change to against-the-rule(ATR)astigmatism.There was significant change in Z^(+2)_(2) polynomial following rubbing.We found a positive correlation between axial length and change in posterior K(r=0.335,p=0.020).The TFBUT reduced following eye rubbing(15.3 s vs.13.9 s,p=0.0001).There was a positive correlation between AL and increase in ACD post rubbing(r=0.300,p=0.038).There was a positive correlation between ACD and change in mean posterior K(r=0.305,p=0.035).Conclusions:In healthy eyes,following eye rubbing,there is a significant change in TFBUT and central anterior K changes towards ATR astigmatism.Longer eyes had more changes in posterior K and ACD.Whereas,eyes with deeper ACD showed more steepness of posterior K. | Jordan V.Chervenkoff Elizabeth Hawkes Gabriela Ortiz Deborah Horney Mayank A.Nanavaty | 2017 | Eye and Vision2017，4，1： | 2 |
