2Hepatocellular carcinoma: Therapy and prevention显示文摘Hepatocellular carcinoma (HCC) is one of the most common malignant tumors worldwide. The major etiologies and risk factors for the development of HCC are well defined and some of the multiple steps involved in hepatocarcinogenesis have been elucidated in recent years. Despite these scientific advances and the implementation of measures for the early detection of HCC in patients at risk, patient survival has not improved during the last three decades. This is due to the advanced stage of the disease at the time of clinical presentation and limited therapeutic options. The therapeutic options fall into five main categories: surgical interventions including tumor resection and liver transplantation, percutaneous interventions including ethanol injection and radiofrequency thermal ablation, transarterial interventions including embolization and chemoembolization, radiation therapy and drugs as well as gene and immune therapies. These therapeutic strategies have been evaluated in part in randomized controlled clinical trials that are the basis for therapeutic recommendations. Though surgery, percutaneous and transarterial interventions are effective in patients with limited disease (1-3 lesions, <5 cm in diameter) and compensated underlying liver disease (cirrhosis Child A), at the time of diagnosis more than 80% patients present with multicentric HCC and advanced liver disease or comorbidities that restrict the therapeutic measures to best supportive care. In order to reduce the morbidity and mortality of HCC, early diagnosis and the development of novel systemic therapies for advanced disease, including drugs, gene and immune therapies as well as primary HCC prevention are of paramount importance. Furthermore, secondary HCC prevention after successful therapeutic interventions needs to be improved in order to make an impact on the survival of patients with HCC. New technologies, including gene expression profiling and proteomic analyses, should allow to further elucidate the molecular events underlying HCC development and to identify novel diagnostic markers as well as therapeutic and preventive targets.Hubert E Blum 2005World Journal of Gastroenterology2005,11,47:27

4EGFR and HER2 expression in advanced biliary tract cancer显示文摘AIM:To analyze the pathogenetic role and potential clinical usefulness of the epidermal growth factor receptor(EGFR)and the human epidermal growth factor receptor 2(HER2)in patients with advanced biliary tract cancer(BTC). METHODS:EGFR and HER2 expression was studied in biopsy samples from 124 patients(51%women; median age 64.8 years),with advanced BTC diagnosed between 1997 and 2004.Five micrometers sections of paraffin embedded tissue were examined by standard, FDA approved immunohistochemistry.Tumors with scores of 2+or 3+for HER2 expression on immunochemistry were additionally tested for HER2 gene amplification by fluorescence in situ hybridisation(FISH).RESULTS:34/124 patients(27.4%)had gallbladder cancer,47(37.9%)had intrahepatic BTC and 43(34.7%)had extrahepatic or perihilar BTC.EGFR expression was examined in a subset of 56 samples. EGFR expression was absent in 22/56 tumors(39.3%). Of the remaining samples expression was scored as 1+in 12(21.5%),2+in 13(23.2%)and 3+in 9(16%), respectively.HER2 expression was as follows:score 0 73/124(58.8%),score 1+27/124(21.8%),score 2+ 21/124(17%)and score 3+4/124(3.2%).HER2 gene amplification was present in 6/124,resulting in an overall amplification rate of 5%. 5Interaction of hepatitis C virus envelope glycoprotein E2 with the large extracellular loop of tupaia CD81显示文摘AIM: To further analyze the interaction of tupaia CD81 with hepatitis C virus (HCV) envelope protein E2. METHODS: A tupaia CD81 large extracellular loop (CD81 LEL), which binds to HCV E2 protein, was cloned and expressed as a GST-fusion protein, and interaction of HCV E2 protein with a tupaia CD81 LEL was evaluated by enzyme-linked immunosorbent assay (EIA). RESULTS: Although tupaia and human CD81 LEL differed in 6 amino acid changes, tupaia CD81 LEL was strongly recognized by anti-CD81 antibodies against human CD81 LEL conformation-dependent epitopes. Investigating LEL CD81-E2 interactions by EIA, we demonstrated that binding of tupaia CD81 LEL GST fusion protein to recombinant HCV E2 protein was markedly reduced compared to binding of human CD81 LEL GST fusion protein to recombinant HCV E2 protein. CONCLUSION: These data suggest that the structural differences in-between the tupaia and human CD81 may alter the interaction of the large extracellular loop with HCV envelope glycoprotein E2. 6Neurotherapeutic potential of erythropoietin after ischemic injury of the central nervous system显示文摘Erythropoietin (EPO) is one of the most successful biopharmaceuticals in history and is used for treating anemia of different origins. However, it became clear that EPO could also work in a neuroprotective, antiapoptotic, antioxidative, angiogenetic and neurotropic way. It causes stimulation of cells to delay cell apoptosis, especially in the central nervous system. In rodent models of focal cerebral ischemia, EPO showed an impressive reduction of infarct size by 30% and improvement of neurobehavioral outcome by nearly 40%. A large animal model dealing with ischemia and reperfusion of the spinal cord showed that EPO could reduce the risk of spinal cord injury significantly. In addition, some clinical studies tested whether EPO works in real live clinical settings. One of the most promising studies showed the innocuousness and improvements in follow-up, outcome scales and in infarct size, of EPO-use in humans suffering from ischemic stroke. Another study ended unfortunately in a negative outcome and an increased overall death rate in the EPO group. The most possible reason was the involvement of patients undergoing simultaneously systemic thrombolysis with recombinant tissue plasminogen activator. An experimental study on rats demonstrated that administration of EPO might exacerbate tissue plasminogen activator-induced brain hemorrhage without reducing the ischemic brain damage. This case shows clearly how useful animal models can be to check negative side effects of a treatment before going into clinical trials. Other groups looked in human trials at the effects of EPO on the outcome after ischemic stroke, relation to circulating endothelial progerdtor cells, aneurysmal subarachnoid hemorrhage, traumatic brain injury, hemoglobin transfusion thresholds and elective first-time coronary artery bypass surgery. Most of the results were pos-让ive, but are based mostly on small group sizes. However, some of the most neglected facts when focusing on experimental setups of ischemia of the central nervous system are issues like age and comorbidities. 11Hepatitis C virus infection and apoptosis显示文摘Apoptosis is central for the control and elimination of viral infections. In chronic hepatitis C virus (HCV) infection, enhanced hepatocyte apoptosis and upregulation of the death inducing ligands CD95/Fas, TRAIL and TNFα occur. Nevertheless, HCV infection persists in the majority of patients. The impact of apoptosis in chronic HCV infection is not well understood. It may be harmful by triggering liver fibrosis, or essential in interferon (IFN) induced HCV elimination. For virtually all HCV proteins, pro- and anti-apoptotic effects have been described, especially for the core and NS5A protein. To date, it is not known which HCV protein affects apoptosis in vivo and whether the infectious virions act pro- or anti- apoptotic. With the availability of an infectious tissue culture system, we now can address pathophysiologically relevant issues. 12Role of environmental pollution in irritable bowel syndrome显示文摘Irritable bowel syndrome(IBS),with the prevalence of 10%-20 % of the population has become an emerging problem worldwide. IBS is a functional gastrointestinal(GI) disorder characterized by abdominal pain or discomfort and altered bowel habits. The etiology of IBS contains genetic,psychological,and immunological factors,and has not been fully elucidated; of note,recent studies also point at environmental pollution and its role in the development of functional GI diseases. In this review we focus on several environmental factors,such as bacterial contamination,air pollution,radiation and even stress as potential triggers of IBS. We discuss associated disturbances in homeostasis,such as changes in intestinal microbiome and related pathophysiological mechanisms. 14Two-phase air-water flows: Scale effects in physical modeling显示文摘Physical modeling represents probably the oldest design tool in hydraulic engineering together with analytical approaches. In free surface flows, the similitude based upon a Froude similarity allows for a correct representation of the dominant forces, namely gravity and inertia. As a result fluid flow properties such as the capillary forces and the viscous forces might be incorrectly reproduced, affecting the air entrainment and transport capacity of a high-speed model flow. Small physical models operating under a Froude similitude systematically underestimate the air entrainment rate and air-water interfacial properties. To limit scale effects, minimal values of Reynolds or Weber number have to be respected. The present article summarizes the physical background of such limitations and their combination in terms of the Morton number. Based upon a literature review, the existing limits are presented and discussed, resulting in a series of more conservative recommendations in terms of air concentration scaling. 16建立广州生物库进行慢性重大疾病研究概述显示文摘目的报告《建立广州生物库进行慢性重大疾病研究》过去十年的研究进展和今后十年的研究展望。方法选择≥50岁的30 518名有广州市户籍的居民,收集其个人背景资料、职业接触、居住和生活环境与习惯、营养摄入及个人与家族史等详细资料,并作体格检查,血生化检验、尿柯铁宁及心电图、X线胸片与肺功能检测等,同时储存其白细胞与血浆在-196℃液态氮库内使其永生化;定期重复体检随访及电话随访并建立亚队列进行基因、生活方式与职业环境等因素对慢性重大疾病影响的研究。计数资料采用χ2检验,计量资料符合正态分布的采用t检验,不符合正态分布的采用秩和检验,以双侧P<0.05为差异有统计学意义。结果 (1)建立液态氮生物库、条码识别技术与Fico11-Pague分离细胞室及分子流行病学研究室的管理系统;(2)分三个阶段收集30 518名研究对象建立生物库中老年队列;(3)基线资料分析表明,'三高'凸显,男女血压高值分别为24.0%和17.8%,高血压患病率分别为45.3%和41.3%,χ2值为39.89,P<0.001;血糖高值和糖尿病患者男性分别为11.1%和12.0%,略高于女性的9.9%和12.6%,χ2值分别为9.00和2.04,P=0.191和0.293,差异无统计学意义;而高脂血症更为明显,检出率则女性明显高于男性;(4)中央型肥胖、超重和全身性肥胖以及代谢综合征也呈现女性高于男性;短期记忆受损则男性为13.9%,稍高于女性的12.7%,χ2值为8.05,P=0.005;(5)发现不良的生活方式如吸烟、二手烟、饮酒、缺乏体力活动、不良睡眠状态及打鼾对健康的影响,经历不同社会经济环境(生命历程)成长发育对健康的影响,研究已发表论文80多篇;(6)第一次重复体检和电话随访已于2012年12月结束,除606名研究对象因户口迁出或身份证有误无法确定外,成功随访率达98.0%,已知1 801例死亡,随访资料仍在处理和分析中。结论既往十年《广州生物库中老年队列研究》项目已建立标准化的研究平台及完成第一次重复体检与电话随访,未来十年将继续重复体检为主的纵向随访及建立亚队列进行基因、职业与环境因素及生活方式对慢性重大疾病影响的研究,不断向深度和广度发展。江朝强 林大庆 郑家强 刘斌 张维森 靳雅丽 徐琳 Kin Bong Hubert Lam Graham Neil Thomas Peymane Adap 朱彤 梁卓伟 2013中国预防医学杂志2013,14,11:6
17次氯酸钙饼剂在饮用水消毒中应用显示文摘针对我国村镇供水工程对消毒技术的需求,采用消毒效果可靠、设备管理简单的次氯酸钙消毒。次氯酸钙消毒由次氯酸钙复配饼剂以及自动化加药器构成。针对次氯酸钙消毒进行了实验室模拟水厂试验,试验结果表明在管道流量为9m3/h时,投加药量为0.3~0.5L/h即可达到国家饮用水卫生标准对出厂水消毒剂余氯要求的限值0.3mg/L,且有效氯浓度稳定、衰减慢。在模拟水厂试验的基础上,将次氯酸钙消毒设施投入水厂实际应用,以廊坊广阳区万庄南水厂为例,探讨次氯酸钙消毒设施的实际运行效果以及最佳运行工况。实际应用表明次氯酸钙消毒效果可靠,设备简单方便,无需设备维护,运行费用低,吨水处理成本约0.012 5元。因此,次氯酸钙饼状消毒剂配备专用自动化加药器在村镇供水工程中具有较大的推广价值。李连香 孙瑞刚 王智勇 田克军 王桂娟 Gordon Dong Hubert Huang 2012中国农村水利水电2012,,11:5

19Physical modelling and scale effects of air-water flows on stepped spillways显示文摘During the last three decades, the introduction of new construction materials (e.g. RCC (Roller Compacted Concrete), strengthened gabions) has increased the interest for stepped channels and spillways. However stepped chute hydraulics is not simple, because of different flow regimes and importantly because of very-strong interactions between entrained air and turbu- lence. In this study, new air-water flow measurements were conducted in two large-size stepped chute facilities with two step heights in each facility to study experimental distortion caused by scale effects and the soundness of result extrapolation to pro- totypes. Experimental data included distributions of air concentration, air-water flow velocity, bubble frequency, bubble chord length and air-water flow turbulence intensity. For a Froude similitude, the results implied that scale effects were observed in both facilities, although the geometric scaling ratio was only Lr=2 in each case. The selection of the criterion for scale effects is a critical issue. For example, major differences (i.e. scale effects) were observed in terms of bubble chord sizes and turbulence levels al- though little scale effects were seen in terms of void fraction and velocity distributions.
