| 1 | Endobiliary radiofrequency ablation for malignant biliary obstruction显示文摘BACKGROUND: The cornerstone of palliative treatment for inoperable extrahepatic cholangiocarcinoma is the relief of malignant biliary obstruction. The most commonly applied method is endoscopic stenting. However, the procedure can be complicated with stent obstruction. In this respect, endobiliary radiofrequency ablation may serve as an adjunctive tool for prolonging the stent patency. METHODS: Patients who underwent endoscopic retrograde cholangiopancreatography for differential diagnosis and/ or palliative treatment after the diagnosis of inoperable extrahepatic cholangiocarcinoma between March 2011 and January 2012 were analyzed. Those in whom endobiliary radiofrequency ablation and endoscopic stenting was successfully performed were included in the study. Technical details of the procedure, duration of stent patency, length of hospital stay, short-term morbidity and mortality rate were documented. RESULTS: Seventeen patients were analyzed, and 10 patients were included in the study. The morbidity and mortality rate within the first 30 days after the procedure was 20% and 0%, respectively. In 2 patients, mild pancreatitis occurred because of the endobiliary procedure. In 1 patient, endobiliary decompression could not be achieved, and therefore, percutaneous transhepatic biliary drainage was carried out. The median duration of stent patency in 9 patients with successful biliary decompression was 9 months (range 6-15). | 2 | Efficacy of intramuscular diclofenac and fluid replacement in prevention of post-ERCP pancreatitis显示文摘AIM:To assess the efficacy of intramuscular diclofenac and fluid replacement for prevention of post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis.METHODS: A prospective, placebo-controlled study was conducted in 80 patients who underwent ERCP. Patients were randomized to receive parenteral diclofenac at a loading dose of 75 mg followed by the infusion of 5-10 mL/kg per hour isotonic saline over 4 h after the procedure, or the infusion of 500 mL isotonic saline as placebo. Patients were evaluated clinically, and serum amylase levels were measured 4, 8 and 24 h after the procedure.RESULTS: The two groups were matched for age, sex, underlying disease, ERCP findings, and type of treatment. The overall incidence of pancreatitis was 7.5% in the diclofenac group and 17.5% in the placebo group (12.5% in total). There were no significant differences in the incidence of pancreatitis and other variables between the two groups. | 3 | REKF and RUKF for pico satellite attitude estimation in the presence of measurement faults显示文摘When a pico satellite is under normal operational conditions, whether it is extended or unscented, a conventional Kalman filter gives sufficiently good estimation results. However, if the measurements are not reliable because of any kind of malfunctions in the estimation system, the Kalman filter gives inaccurate results and diverges by time. This study compares two different robust Kalman filtering algorithms, robust extended Kalman filter(REKF) and robust unscented Kalman filter(RUKF), for the case of measurement malfunctions. In both filters, by the use of defined variables named as the measurement noise scale factor, the faulty measurements are taken into the consideration with a small weight, and the estimations are corrected without affecting the characteristic of the accurate ones. | 5 | A Novel Method to Improve the Electrochemical Performance of LiMn_2O_4 Cathode Active Material by CaCO_3 Surface Coating显示文摘Spinel LiMn2O4 was synthesized by glycine-nitrate method and coated with CaCO3 in order to enhance the electrochemical performance at room temperature (250C) and 550C. The uncoated and CaCO3-coated LiMn2O4 materials were characterized by X-ray diffraction (XRD), scanning electron microscopy (SEM) and electrochemical tests. XRD and SEM results indicated that CaCO3 particles encapsulated the surface of the LiMn2O4 without causing any structural change. The charge-discharge tests showed that the specific discharge capacity fade of pristine electrode at 25 and 550C were 25.5% and 52%, respectively. However, surface modified cathode shows 7.4% and 29.5% loss compared to initial specific discharge capacity at 70th cycle for 25 and 550C, respectively. | 6 | Effect of Salt Stress on Photosynthetic GasExchange and Chlorophyll FluorescenceParameters in Alhagi pseudalhagi显示文摘Soil salinity is one of the most important environmental factors limiting plant growth and productivity in many regions in the world. Salt stress directly affects plant photosynthesis, which is an important physiological process to limit plant growth and crop yield. But the effects of salt stress on the mechanism of the photosynthesis is still not clear. In this experiment, the salt tolerant plant Alhagi pseudalhagi was selected as the experiment material, and the salt sensitive plant Vigna radiata as the control, to explore the effects of salt stress on photosynthetic parameters of A. pseudalhagi. Plants were grown in a greenhouse,cultured with 1/2 Hoagland nutrient solution, treated by 0(control), 50, 100 or 200 s of mmol/L of NaCl solution for 12 d. Then,the chlorophyll contents, gas exchange parameters and chlorophyll a fluorescence in each treatment were measured. The results showed that under the salt stress simulated by 50 mmol/L NaCl, the net photosynthetic rate(Pn) and stomatal conductance(gs)of A. pseudalhagi were significantly increased compared with the control, while intercellular CO_2 concentration(Ci) was significantly decreased, Fv'/Fm', Fv/Fm and Fv/Fo were increased over time, but had no significant differences with the control ФPSⅡ,ETR and qP were significantly increased, and NPQ was significantly decreased. Under the salt stress simulated by 100 and 200mmol/L NaCl, Fv'/Fm', Fv/Fm, Fv/Fo, ФPSⅡ and qP of A. pseudalhagi were gradually decreased over time. | 7 | Comparison of one and three initial monthly intravitreal ranibizumab injection in patients with macular edema secondary to branch retinal vein occlusion显示文摘AIM: To compare three initial monthly intravitreal ranibizumab(IVR) injections followed by pro re nata(PRN) dosing with one initial monthly IVR injections followed by PRN dosing for macular edema(ME) secondary to branch retinal vein occlusion(BRVO).METHODS: Forty-two eyes of 42 patients who had IVR injections for BRVO were retrospectively studied. Eighteen eyes received 1 initial IVR injection(1+PRN group) and 24 eyes received 3 monthly IVR injections(3+PRN). At 1, 3, 6 and 12mo; spectral-domain optical coherence tomography(SD-OCT) was performed. Central macular thickness(CMT), the integrity of the external limiting membrane(ELM), the presence of subretinal fluid, cyst size, the presence of inner segment/outer segment(IS/OS) defect were determined.RESULTS: At baseline the mean CMT was 521.3±153.2 μm in the 3+PRN group while it was 438.1±162.4 μm in 1+PRN group. At the final visit, mean CMT was 278.3±87.8 μm in the 3+PRN group and 285.2±74.2 μm in the 1+PRN group(P=0.079). The changes in CMT over the entire study period were also comparable in both groups(243±160 μm in the 3+PRN group, and 152.9±175.3 μm in the 1+PRN group; P=0.090). At baseline, best-corrected visual acuity(BCVA) was 0.92±0.60 logarithm of the minimal angle of resolution(logMAR) in the 3+PRN group, while it was 0.72±0.46 logMAR in the 1+PRN group. Final BCVA was 0.42±0.55 logMAR in the 3+PRN group and 0.38±0.50 logMAR in the 1+PRN group(P=0.979). Additionally, the BCVA changes from baseline to final visit were not significantly different(-0.50±0.45 logMAR in the 3+PRN group, and-0.33±0.39 logMAR in the 1+PRN group; P=0.255).CONCLUSION: No significant differences in the anatomical or functional results are found between 3+PRN and 1+PRN regimens in the patients receiving ranibizumab for ME secondary to BRVO. | 9 | Neuroprotective effects of daidzein on focal cerebral ischemia injury in rats显示文摘Daidzein, a plant extract, has antioxidant activity. It is hypothesized, in this study, that daidzein exhibits neuroprotective effects on cerebral ischemia. Rat models of middle cerebral artery occlusion were intraperitoneally administered daidzein. Biochemical and immunohistochemical tests showed that superoxide dismutase and nuclear respiratory factor 1 expression levels in the brain tissue decreased after ischemia and they increased obviously after daidzein administration; malondialdehyde level and apoptosis-related cysteine peptidase caspase-3 and caspase-9 immunoreactivity in the brain tissue increased after ischemia and they decreased obviously after daidzein administration. Hematoxylin-eosin staining and luxol fast blue staining results showed that intraperitoneal administration of daidzein markedly alleviated neuronal damage in the ischemic brain tissue. The effect of erbium doping on the electrochemical behavior of LiFePO4 was studied in this paper. The samples were characterized by X-ray powder diffraction and scanning electron microscopy and the electrochemical properties were investigated by the charge-discharge test. An initial discharge capacity of 149 mAh·g-1 was achieved for the LiEr0.02Fe0.98PO4/C composite cathode with a rate of 0.1 C. The electronic conductivity of Er doped LiFePO4/C was measured as 10-2 S·cm-1. The results indicated that erbium doping did not destroy the lattice structure of LiFePO4 and enlarge the lattice volume. | 14 | Lung parenchymal changes in patients with ankylosing spondylitis显示文摘AIM: To assess lung parenchymal changes in ankylosing spondylitis (AS) using high resolution computed tomography (HRCT). METHODS: We included 78 AS patients whose average age was 33.87 (18-56) years with a ratio of 53 males to 25 females who were followed up for 3.88 (1-22) years on average. neumonia and tuberculosis were excluded. In a detailed examination of lung HRCT findings, we investigated the presence of parenchymal micronodules,parenchymal bands, subpleural bands, interlobular and intralobular septal thickening, irregularity of interfaces,ground glass opacity, consolidation, mosaic pattern,bronchial wall thickening, bronchial dilatation, tracheal dilatation, pleural thickening, emphysema, thoracic cage asymmetry, honeycomb appearance, structural distortion, apical fibrosis and other additional findings.RESULTS: In detailed HRCT evaluations, lung parenchymal changes were found in 46 (59%) of all patients. We found parenchymal bands in 21 (27%) cases, interlobular septal thickening in 9 (12%), emphysema in 9 (12%), apical fibrosis in 8 (10%), ground-glass opacities in 7 (9%), parenchymal micronodules in 5 (6%), irregularity in interfaces in 3 (4%), bronchial dilatation in 3 (4%), mosaic pattern in 2 (3%), pleural thickening in 2 (3%), consolidation in 1 (1%), bronchial wall thick ening in 1 (1%) and a subpleural band in 1 (1%) case. Furthermore, we detected subsegmental atelectasis in 2 patients and a cavitary lesion in 1 patient. CONCLUSION: Our study had the highest number of AS cases of all previous studies in evaluating lung paren chymal changes. | 15 | The predictive value of triglyceride-glucose index for in-hospital and one-year mortality in elderly non-diabetic patients with ST-segment elevation myocardial infarction显示文摘BACKGROUND Triglyceride-glucose(TyG)index,a reliable marker of insulin resistance,was associated with cardiovascular morbidity and mortality.This study aimed to evaluate the predictive value of TyG index for mortality in elderly non-diabetic pa-tients with ST-segment elevation myocardial infarction(STEMI).METHODS In total,430 non-diabetic patients aged over 65 years with STEMI were consecutively included.The TyG index was calculated by using the following formula:TyG index=ln(fasting triglyceride×fasting glucose/2).The 5-item modified frailty index score was utilized to determine comorbidities.Patients were divided into three groups based on TyG index tertiles.RESULTS Patients included into high TyG index tertile were male and had higher body mass index,glucose,triglyceride,low-density lipoprotein cholesterol,5-item modified frailty index and GRACE risk score;and had low systolic blood pressure,estim-ated glomerular filtration rate and left ventricular ejection fraction.A one-unit increase in TyG index was associated with 3.03 ex-tra cases per 1000 person-day for in-hospital mortality and 0.29 extra cases per 1000 person-day for long-term mortality.There was a non-linear relationship between TyG index and the risk of mortality with an increased risk above 8.5 for TyG index.Kaplan-Meier survival curves revealed that patients in high TyG index tertile had higher in-hospital and long-term mortality rates than those in low TyG index tertile.CONCLUSIONS This was the first study to demonstrate that the TyG index could predict in-hospital and long-term mortality in elderly non-diabetic STEMI patients. | FaysalŞaylık TufanÇınar Murat Selçuk İbrahim Halil Tanboğa | 2022 | Journal of Geriatric Cardiology2022,19,8: | 3 |
| 16 | Effects of moisture damage on asphalt mixtures显示文摘The reduction in the ability of bitumen to bond with the aggregate surface due to the infiltration of moisture has been recognised for years, and this deterioration phenomenon is called moisture damage. In general, the loss of bonding between bitumen and aggregate shortens the service life of the top layer of the pavement. Many investigations have been conducted to understand the mechanisms of moisture damage due to the loss of bonding strength between bitumen and aggregate and to find ways to improve and strengthen the bond to mitigate the effect of moisture. This paper reviews the extensive literature on the loss of bitumen-aggregate bonding strength due to moisture damage in asphalt mixtures.The general description of the theories and mechanisms that explain the effect of the thermodynamic, chemical, physical and mechanical characteristics of the bitumen and aggregate on the bonding phenomenon are discussed in this paper. In addition, the causes of and contributing factors to moisture damage and methods to improve the bond between bitumen and aggregates are also discussed. Moreover, a description of the test methods that can be used to evaluate moisture damage in poorly bonded and compacted mixtures are also presented. Special attention is given to a well-known method, known as the pulloff test, which has been successfully used to evaluate aggregate-binder bond strength, both for laboratory and in-situ tests. This includes the test methods, the factors that affect the bonding strength results and their correlation with other test method. | 17 | Oncogenic fingerprint of epidermal growth factor receptor pathway and emerging epidermal growth factor receptor blockade resistance in colorectal cancer显示文摘Epidermal growth factor receptor(EGFR) has been an attractive target for treatment of epithelial cancers, including colorectal cancer(CRC). Evidence from clinical trials indicates that cetuximab and panitumumab(antiEGFR monoclonal antibodies) have clinical activity in patients with metastatic CRC. The discovery of intrinsic EGFR blockade resistance in Kirsten RAS(KRAS)-mutant patients led to the restriction of anti-EGFR antibodies to KRAS wild-type patients by Food and Drug Administration and European Medicine Agency. Studies have since focused on the evaluation of biomarkers to identify appropriate patient populations that may benefit from EGFR blockade. Accumulating evidence suggests that patients with mutations in EGFR downstream signaling pathways including KRAS, BRAF, PIK3CA and PTEN could be intrinsically resistant to EGFR blockade. Recent whole genome studies also suggest that dynamic alterations in signaling pathways downstream of EGFR leads to distinct oncogenic signatures and subclones which might have some impact on emerging resistance in KRAS wild-type patients. | 20 | Phase I study on the safety and preliminary efficacy of allogeneic mesenchymal stem cells in hypoxic-ischemic encephalopathy显示文摘BACKGROUND Hypoxic-ischemic encephalopathy(HIE)is a leading cause of morbidity and mortality in the adult as well as in the neonate,with limited options for treatment and significant dysfunctionality.AIM To investigate the safety and preliminary efficacy of allogeneic mesenchymal stem cells(MSCs)in HIE patients.METHODS Patients who had HIE for at least 6 mo along with significant dysfunction and disability were included.All patients were given Wharton’s jelly-derived MSCs at 1×106/kg intrathecally,intravenously,and intramuscularly twice a month for two months.The therapeutic effects and prognostic implications of MSCs were evaluated by multiple follow-ups.Functional independence measure(FIM),modified Ashworth,and Karnofsky scales were used to assess any side effects,neurological and cognitive functions,and overall outcomes.RESULTS The 8 subjects included in the study had a mean age of 33.25±10.18 years.Mean HIE exposure and mean post-HIE durations were 45.63±10.18 and 19.67±29.04 mo,respectively.Mean FIM score was 18.38±1.06,mean modified Ashworth score was 43.5±4.63,and mean Karnofsky score was 20.For the first 24 h,5 of the patients experienced a subfebrile state,accompanied by mild headaches due to intrathecally administration and muscle pain because of intramuscularly administration.Neurological and functional examinations,laboratory tests,electroencephalography,and magnetic resonance imaging were performed to assess safety of treatment.Mean FIM score increased by 20.88±3.31 in the first month(P=0.027)and by 31.38±14.69 in 12 mo(P=0.012).The rate of patients with an FIM score of 126 increased from 14.58%to 16.57%in the first month and 24.90%in 12 mo.CONCLUSION Multiple triple-route Wharton’s jelly-derived MSC administrations were found to be safe for HIE patients,indicating neurological and functional improvement.Based on the findings obtained here,further randomized and placebo research could be performed. | Serdar Kabataş ErdinçCivelek Necati Kaplan Eyüp Can Savrunlu Gülseli Berivan Sezen Mourat Chasan Halil Can Ali Genç Yener Akyuva Osman Boyalı Furkan Diren Erdal Karaoz | 2021 | World Journal of Experimental Medicine2021,11,2: | 2 |
