| 5 | Microbial markers in colorectal cancer detection and/or prognosis显示文摘Colorectal cancer(CRC) is the second leading cause of cancer worldwide. CRC is still associated with a poor prognosis among patients with advanced disease. On the contrary, due to its slow progression from detectable precancerous lesions, the prognosis for patients with early stages of CRC is encouraging. While most robust methods are invasive and costly, actual patient-friendly screening methods for CRC suffer of lack of sensitivity and specificity. Therefore, the development of sensitive, non-invasive and cost-effective methods for CRC detection and prognosis are necessary for increasing the chances of a cure. Beyond its beneficial functions for the host, increasing evidence suggests that the intestinal microbiota is a key factor associated with carcinogenesis. Many clinical studies have reported a disruption in the gut microbiota balance and an alteration in the faecal metabolome of CRC patients, suggesting the potential use of a microbialbased test as a non-invasive diagnostic and/or prognostic tool for CRC screening. This review aims to discuss the microbial signatures associated with CRC known to date, including dysbiosis and faecal metabolome alterations, andthe potential use of microbial variation markers for noninvasive early diagnosis and/or prognostic assessment of CRC and advanced adenomas. Dielectric diagnostic methods deduce moisture in the solid insulation from dielectric properties like polarisation depolarisation currents and dissipation factor vs. frequency. A new instrument 'Dirana' combines time domain (PDC) and frequency domain (FDS) measurements and thus substantially shortens the measurement duration. New software was developed which bases on a new data pool,measured at new and aged pressboard samples with various moisture contents and oil impregnation. Its analysis algorithm compares measurements from a transformer to modelled dielectric responses,obtained from the so-called XY-model. To avoid overestimation of moisture for aged transformers,the analysis algorithm compensates for the influence of conductive aging by-products. Moisture especially increases the losses in the low frequency range of the dielectric response of pressboard. Thus,data on the left-hand side of the area dominated by interfacial polarization (insulation geometry) are required for reliable moisture determination. | 10 | Palliative chemotherapy for gastroesophageal cancer in old and very old patients: A retrospective cohort study at the National Center for Tumor Diseases, Heidelberg显示文摘AIM:To investigate the outcome of palliative chemotherapy in old patients with gastroesophageal cancer at the National Center for Tumor Diseases,Heidelberg.METHODS:Using a prospectively generated database,we retrospectively analyzed 55 patients≥70years under palliative chemotherapy for advanced gastroesophageal cancer at the outpatient clinic of the National Center for Tumor Diseases Heidelberg,Germany between January 2006 and December2013.Further requirements for inclusion were(1)histologically proven diagnosis of gastroesophageal cancer;(2)advanced(metastatic or inoperable)disease;and(3)no history of radiation or radiochemotherapy.The clinical information included Eastern Cooperative Oncology Group performance status(ECOG PS),presence and site of metastases at diagnosis,date of previous surgery and perioperative chemotherapy,start and stop date of first-line treatment,toxicities and consecutive dosage reductions of first-line treatment,response to first-line therapy,date of progression,usage of second-line therapies and date and cause of death.Survival times[progression-free survival(PFS),overall survival(OS)and residual survival(RS)]were calculated.Toxicity and safety were examined.Prognostic factors including ECOG PS,age and previousperioperative treatment were analyzed.RESULTS:Median age of our cohort was 76 years.86%of patients received a combination of two cytotoxic drugs.76 percent of patients had an oxaliplatin-based first-line therapy with the oxaliplatin and 5-fluorouracil regimen being the predominantely chosen regimen(69%).Drug modifications due to toxicity were necessary in 56%of patients,and 11%of patients stopped treatment due to toxicities.Survival times of our cohort are in good accordance with the major phaseⅢtrials that included mostly younger patients:PFS and OS were 5.8 and 9.5 mo,respectively.Survival differed significantly between patient groups with low(≤1)and high(≥2)ECOG PS(12.7 mo vs 3.8 mo,P<0.001).Very old patients(≥75 years)did not show a worse outcome in terms of survival.Patients receiving secondline treatment(51%)had a significantly longer RS than patients with best supportive care(6.8 vs 1.4 mo,P=0.001).Initial ECOG PS was a strong prognostic factor for PFS,OS and RS.CONCLUSION:Old patients with non-curable gastroesophageal cancer should be offered chemotherapy,and ECOG PS is a tool for balancing benefit and harm upfront.Second-line treatment is reasonable. | Anne Katrin Berger Stefanie Zschaebitz Christine Komander Dirk Jger Georg Martin Haag | 2015 | World Journal of Gastroenterology2015,21,16: | 6 |

| 19 | Effect of process parameters on additive-free electrospinning of regenerated silk fibroin nonwovens显示文摘Silk fibroin is a biomaterial with multiple beneficial properties for use in regenerative medicine and tissue engineering.When dissolving and processing the reconstituted silk fibroin solution by electrospinning,the arrangement and size of fibers can be manifold varied and according fiber diameters reduced to the nanometer range.Such nonwovens show high porosity as well as potential biocompatibility.Usually,electrospinning of most biomaterials demands for the application of additives,which enable stable electrospinning by adjusting viscosity,and are intended to evaporate during processing or to be washed out afterwards.However,the use of such additives increases costs and has to be taken into account in terms of biological risks when used for biomedical applications.In this study,we explored the possibilities of additive-free electrospinning of pure fibroin nonwovens and tried to optimize process parameters to enable stable processing.We used natural silk derived from the mulberry silkworm Bombyx mori.After degumming,the silk fibroin was dissolved and the viscosity of the spinning solution was controlled by partial evaporation of the initial solving agent.This way,we were able to completely avoid the use of additives and manufacture nonwovens,which potentially offer higher biocompatibility and reduced immunogenicity.Temperature and relative humidity during electrospinning were systematically varied(25–35°C,25–30%RH).In a second step,the nonwovens optionally underwent methanol treatment to initiate beta-sheet formation in order to increase structural integrity and strength.Comprehensive surface analysis on the different nonwovens was performed using scanning electron microscopy and supplemented by additional mechanical testing.Cytotoxicity was evaluated using BrdU-assay,XTT-assay,LDH-assay and live-dead staining.Our findings were,that an increase of temperature and relative humidity led to unequal fiber diameters and defective nonwovens.Resistance to penetration decreased accordingly.The most uniform fiber diameters of 998±63 nm were obtained at 30°C and 25%relative humidity,also showing the highest value for resistance to penetration(0.20 N).The according pure fibroin nonwoven also showed no signs of cytotoxicity.However,while the biological response showed statistical evidence,the material characteristics showed no statistically significant correlation to changes of the ambient conditions within the investigated ranges.We suggest that further experiments should explore additional ranges for temperature and humidity and further focus on the repeatability of material properties in dependency of suitable process windows. | Alexander Kopp Ralf Smeets Martin Gosau Nadja Kröger Sandra Fuest Marius Köpf Magnus Kruse Judith Krieger Rico Rutkowski Anders Henningsen Simon Burg | 2020 | Bioactive Materials2020,5,2: | 3 |
