| 1 | Diabetic cardiomyopathy:Pathophysiology,diagnostic evaluation and management显示文摘Diabetes affects every organ in the body and cardiovascular disease accounts for two-thirds of the mortality in the diabetic population.Diabetes-related heart disease occurs in the form of coronary artery disease(CAD),cardiac autonomic neuropathy or diabetic cardiomyopathy(DbCM).The prevalence of cardiac failure is high in the diabetic population and DbCM is a common but underestimated cause of heart failure in diabetes.The pathogenesis of diabetic cardiomyopathy is yet to be clearly defined.Hyperglycemia,dyslipidemia and inflammation are thought to play key roles in the generation of reactive oxygen or nitrogen species which are in turn implicated.The myocardial interstitium undergoes alterations resulting in abnormal contractile function noted in DbCM.In the early stages of the disease diastolic dysfunction is the only abnormality,but systolic dysfunction supervenes in the later stages with impaired left ventricular ejection fraction.Transmitral Doppler echocardiography is usually used to assess diastolic dysfunction,but tissue Doppler Imaging and Cardiac Magnetic Resonance Imaging are being increasingly used recently for early detection of DbCM.The management of DbCM involves improvement in lifestyle,control of glucose and lipid abnormalities,and treatment of hypertension and CAD,if present.The role of vasoactive drugs and antioxidants is being explored.This review discusses the pathophysiology,diagnostic evaluation and management options of DbCM. | Joseph M Pappachan George I Varughese Rajagopalan Sriraman Ganesan Arunagirinathan | 2013 | World Journal of Diabetes2013,4,5: | 57 |

DM poses about four times higher risk for stroke. Cardiometabolic risk factors including obesity, hypertension, and dyslipidaemia often co-exist in patients with DM that add on to stroke risk. Because of the strong association between DM and other stroke risk factors, physicians and diabetologists managing patients should have thorough understanding of these risk factors and management. Although treatment with antiretroviral therapy has extended the life expectancy of people with HIV, liver disease induced by hepatitis B virus(HBV) and hepatitis C virus(HCV) causes significant numbers of non-acquired immune deficiency syndrome(AIDS)-related deaths in coinfected patients. In recent years, new insights into the mechanisms of accelerated fibrosis and liver disease progression in HIV/HCV and HIV/HBV co-infections have been reported. It is worth to understand the mechanism or role of stem cells in the treatment of heart failure. Bone marrow derived stem cells are commonly practiced with an aim to improve the function of the heart. The majority of studies have been conducted with acute myocardial infarction and a few has been investigated with the use of stem cells for treating chronic or dilated cardiomyopathy. Heterogeneity in the treated group using stem cells has greatly emerged. Ever increasing demand for any alternative made is of at most priority for cardiomyopathy. Stem cells are of top priority with the current impact that has generated among physicians. | 9 | Liver as a target of human immunodeficiency virus infection显示文摘Liver injury is a characteristic feature of human immunodeficiency virus(HIV) infection, which is the second most common cause of mortality in HIV-infected patients. Now it is recognized that liver plays a key role in HIV infection pathogenesis. Antiretroviral therapy(ART), which suppresses HIV infection in permissive immune cells, is less effective in hepatocytes, thereby making these cells a silent reservoir of HIV infection. In addition to direct hepatotoxic effects of HIV, certain ART treatment modalities provide hepatotoxic effects. The exact mechanisms of HIV-triggered chronic hepatitis progression are not elucidated, but the liver is adversely affected by HIV-infection and liver cells are prominently involved in HIV-elicited injury. These effects are potentiated by second hits like alcohol. | 14 | An innovative approach to minimize excess sludge production in sewage treatment using integrated bioreactors显示文摘The present investigation deals with an application of integrated sequential oxic and anoxic bioreactor(SOABR) and fluidized immobilized cell carbon oxidation(FICCO) reactor for the treatment of domestic wastewater with minimum sludge generation. The performance of integrated SOABR-FICCO system was evaluated on treating the domestic wastewater at hydraulic retention time(HRT) of 3 hr and 6 hr for 120 days at organic loading rate(OLR)of 191 ± 31 mg/(L·hr). The influent wastewater was characterized by chemical oxygen demand(COD) 573 ± 93 mg/L; biochemical oxygen demand(BOD5) 197 ± 35 mg/L and total suspended solids(TSS) 450 ± 136 mg/L. The integrated SOABR-FICCO reactors have established a significant removal of COD by 94% ± 1%, BOD5 by 95% ± 0.6% and TSS by 95% ± 4% with treated domestic wastewater characteristics COD 33 ± 5 mg/L; BOD59 ± 0.8 mg/L and TSS 17 ± 9 mg/L under continuous mode of operation for 120 days. The mass of dry sludge generated from SOABR-FICCO system was 22.9 g/m^3. The sludge volume index of sludge formed in the SOABR reactor was 32 mL/g and in FICCO reactor it was 46 mL/g. The sludge formed in SOABR and FICCO reactor was characterized by TGA, DSC and SEM analysis. | 16 | Prolonged feeding with guanidinoacetate, a methyl group consumer, exacerbates ethanol-induced liver injury显示文摘AIM To investigate the hypothesis that exposure to guanidinoacetate(GAA, a potent methyl-group consumer) either alone or combined with ethanol intake for a prolonged period of time would cause more advanced liver pathology thus identifying methylation defects as the initiator and stimulator for progressive liver damage.METHODS Adult male Wistar rats were fed the control or ethanolLieber De Carli diet in the absence or presence of GAA supplementation. At the end of 6 wk of the feeding regimen, various biochemical and histological analyses were conducted. RESULTS Contrary to our expectations, we observed that GAA treatment alone resulted in a histologically normal liver without evidence of hepatosteatosis despite persistence of some abnormal biochemical parameters. This protection could result from the generation of creatine from the ingested GAA. Ethanol treatment for 6 wk exhibited changes in liver methionine metabolism and persistence of histological and biochemical defects as reported before. Further, when the rats were fed the GAA-supplemented ethanol diet, similar histological and biochemical changes as observed after 2 wk of combined treatment, including inflammation, macroand micro-vesicular steatosis and a marked decrease in the methylation index were noted. In addition, rats on the combined treatment exhibited increased liver toxicity and even early fibrotic changes in a subset of animals in this group. | 20 | Controlling the solid-state luminescence of gold(I) N-heterocyclic carbene complexes through changes in the structure of molecular aggregates显示文摘Thermally stable, solid-state luminescent organic materials are highly desired for the development of practical applications.Herein we synthesized new gold(I) complexes with N-heterocyclic carbene ligands, which have the ability to form strong metalorganic bond. Consequently, their thermochemical stability is enhanced at temperatures around 300 °C. Precise design of the molecular structure of the ligands, with a focus on ensuring low steric hindrance around Au atoms in order to limit disturbances to Au/Au interactions, provided a complex with a densely packed crystal with a shorter intermolecular Au–Au distance(3.17 ?)than the typical distance. In the solid state, this complex exhibited strong aurophilic interactions, which generated intense phosphorescence even in air at room temperature(quantum yield=16%) in spite of absence of any phosphorescence in solution.This behavior is characteristic for solid-state luminescence referred to as aggregation-controlled emission. Furthermore, the gold(I) complex displays capacity for mechano-and vapo-chromism—that is, the ability to change color reversibly in response to the application of external stimuli.
