1Management of pancreatic fluid collections: A comprehensive review of the literature显示文摘Pancreatic fluid collections(PFCs) are a frequent complication of pancreatitis. It is important to classify PFCs to guide management. The revised Atlanta criteria classifies PFCs as acute or chronic, with chronic fluid collections subdivided into pseudocysts and walled-off pancreatic necrosis(WOPN). Establishing adequate nutritional support is an essential step in the management of PFCs. Early attempts at oral feeding can be trialed in patients with mild pancreatitis. Enteral feeding should be implemented in patients with moderate to severe pancreatitis. Jejunal feeding remains the preferred route of enteral nutrition. Symptomatic PFCs require drainage; options include surgical, percutaneous, or endoscopic approaches. With the advent of newer and more advanced endoscopic tools and expertise, and an associated reduction in health care costs, minimally invasive endoscopic drainage has become the preferable approach. An endoscopic ultrasonography-guided approach using a seldinger technique is the preferred endoscopic approach. Both plastic stents and metal stents are efficacious and safe; however, metal stents may offer an advantage, especially in infected pseudocysts and in WOPN. Direct endoscopic necrosectomy is often required in WOPN. Lumen apposing metal stents that allow for direct endoscopic necrosectomy and debridement through the stent lumen are preferred in these patients. Endoscopic retrograde cholangio pancreatography with pancreatic duct(PD) exploration should be performed concurrent to PFC drainage. PD disruption is associated with an increased severity of pancreatitis, an increased risk of recurrent attacks of pancreatitis and long-term complications, and a decreased rate of PFC resolution after drainage. 2Randomised controlled trial of triclosan coated vs uncoated sutures in primary hip and knee arthroplasty显示文摘BACKGROUND Triclosan-coated vicryl plus suture(Ethicon, Inc.) was developed to reduce microbial colonisation during surgical procedures. However, its effect on wound healing and surgical site infections remain unclear after hip and knee arthroplasty surgery.AIM To determine the effect of triclosan-coated sutures(TCS) vs non-coated sutures on wound healing, following primary hip and knee arthroplasties.METHODS A single-centred, double-blind randomised controlled trial(RCT) was undertaken. We randomly allocated patients to receive either the triclosan-coated sutures(TCS vicryl plus) or non-coated sutures(NCS vicryl) during the closure of unilateral primary hip and knee arthroplasties. We utilised the ASEPSIS wound scoring system to evaluate wound healing for the first 6 weeks post-operatively.RESULTS One hundred and fifty patients undergoing primary total hip or knee arthroplasty over a one-year period were included. Eighty-one were randomised to the TCS group and 69 to the NCS group. Despite no statistically significant difference in the ASEPSIS scores among the study groups(P = 0.75), sensitivity analysis using the Mann Whitney test(P = 0.036) as well as assessment of the wound complications at 6 weeks follow up, demonstrated significantly higher wound complication rates in the TCS group(8 vs 1, P = 0.03).CONCLUSION No clear advantage was demonstrated for using the TCS. 3Structural and Tectonic Evolution of El-Faiyum Depression,North Western Desert,Egypt Based on Analysis of Landsat ETM+,and SRTM Data显示文摘False color Landsat ETM+ (bands 7, 4, 2 in RGB) and SRTM images of the El-Faiyum depression, Egypt, highlights major NE-SW faults and other lineaments trending NW-SE and N-S. Airborne magnetic data reveal some E-W subsurface faults which are not recognized on Landsat ETM+ and SRTM images. Ratio images (5/7, 3/1, 4/3) and (5/7, 5/1, 4) were used for lithological discrimination of different rock types. The El-Faiyum area is occupied by sedimentary rocks of Middle Eocene, Upper Eocene, Oligocene, Lower Miocene, Pliocene, Pleistocene and Holocene. Understanding the structural regime of El-Faiyum depression in the Western Desert of Egypt is significant in relation to the neotectonics of the Nile Delta and to the development of new communities west of the delta. The area is covered by Late Cretaceous, Middle to Late Eocene deposits, Oligocene deposits (comprising fluviatile sediments and basalt sheets) and Pliocene and Quaternary alluvium and sand deposits. New geological and structural maps were prepared at a scale of 1 : 100 000 using Landsat ETM+ and SRTM images coupled with extensive field work. The major structural trends are E-W, ENE, NS and NW-the same trends observed elsewhere in North Africa from NW Sudan to NE Egypt including north and central Sinai. El-Faiyum depression is a structurally-controlled tectonic basin, marked by northeast-striking faults along its northern margin. These faults are parallel to similar faults of the Syrian arc fold belt, which includes a group of related faults, folds, push-up structures, and basins. Qarun Lake is located in the northern part of El-Faiyum depression, at a right-step between two strands of the NE striking fault system. We have suggested elsewhere that El-Faiyum depression is a pull-apart basin related to extension between these mismatched strands of strike-slip faults. The E-W structures are probably related to the relative motion between south Europe and north Egypt and closure of the Neotethys. The continuation of these faults in the present area is only detected from magnetic and seismic studies. The question that needs to be raised is why these old structural features have been reactivated in recent times. 5Early detection of hepatocellular carcinoma co-occurring with hepatitis C virus infection: A mathematical model显示文摘AIM: To develop a mathematical model for the early detection of hepatocellular carcinoma(HCC) with a panel of serum proteins in combination with α-fetoprotein(AFP).METHODS: Serum levels of interleukin(IL)-8, soluble intercellular adhesion molecule-1(s ICAM-1), soluble tumor necrosis factor receptor Ⅱ(s TNF-RⅡ), proteasome, and β-catenin were measured in 479 subjects categorized into four groups:(1) HCC concurrent with hepatitis C virus(HCV) infection(n = 192);(2) HCV related liver cirrhosis(LC)(n = 96);(3) Chronic hepatitis C(CHC)(n = 96); and(4) Healthy controls(n = 95). The R package and different modules for binary and multi-class classifiers based on generalized linear models were used to model the data. Predictive power was used to evaluate the performance of the model. Receiver operating characteristic curve analysis over pairs of groups was used to identify the best cutoffs differentiating the different groups. RESULTS: We revealed mathematical models, based on a binary classifier, made up of a unique panel of serum proteins that improved the individual performance of AFP in discriminating HCC patients from patients with chronic liver disease either with or without cirrhosis. We discriminated the HCC group from the cirrhotic liver group using a mathematical model(-11.3 + 7.38 × Prot + 0.00108 × s ICAM + 0.2574 ×β-catenin + 0.01597 × AFP) with a cutoff of 0.6552, which achieved 98.8% specificity and 89.1% sensitivity. For the discrimination of the HCC group from the CHC group, we used a mathematical model [-10.40 + 1.416 × proteasome + 0.002024 × IL + 0.004096 × s ICAM-1 +(4.251 × 10-4) × s TNF + 0.02567 ×β-catenin + 0.02442 × AFP] with a cutoff 0.744 and achieved 96.8% specificity and 89.7% sensitivity. Additionally, we derived an algorithm, based on a binary classifier, for resolving the multi-class classification problem by using three successive mathematical model predictions of liver disease status.
