1湿润烧伤膏治疗慢性糖尿病足溃疡的多中心对照研究显示文摘目的 通过多中心的对照研究,观察、评估湿润烧伤膏(MEBO) 治疗慢性糖尿病足溃疡的疗效及安全性.方法 对2005年1月至2010年1月间,中国和科威特的5个中心治疗的并符合入选标准的128例慢性糖尿病患者(160处溃疡),随机分为MEBO组(实验组,66例,共82处溃疡) 和生理盐水湿润敷料组(对照组,62例患者,共78处溃疡).前瞻性收集患者的人口统计学数据、患者特征以及溃疡创面特点等数据.研究的前12周,每隔2周计算、对比一次患者的溃疡表面积和愈合指数,24周时记录截肢和溃疡发展的情况.结果 两组患者的人口学特征,临床及溃疡特点相似.MEBO组患者在第2周和第3周时分别呈现出愈合指数的明显升高和溃疡表面积的明显下降,这种趋势连续保持了12周(P<0 01).在治疗12周时MEBO组一半以上的溃疡(53 7%)实现了完全愈合(愈合指数=1),与对照组在治疗12周时的25 66%的愈合率相比有非常显著性差异(P=0 0006);治疗12周时,对照组28 2%的患者愈合指数<0 5,而MEBO组无1例患者发生此状况(P=0 0001).治疗24周后,13例患者(10 2%) 因组织坏死行足前段截肢术,其中只有2例患者来自MEBO组(χ2=4 44,P=0 035).此外,每组各有2例患者在治疗24周后出现了新溃疡(P>0 05).实验过程中未见局部用药引发的任何副作用或过敏反应.结论 1)除了具备安全性以外,MEBO在开始治疗2周后就显著提高了各种溃疡的愈合指数,治疗4周后溃疡表面积开始明显缩小,12周时超过50%的溃疡完全愈合,大大缩短了慢性糖尿病足溃疡的愈合过程;2)MEBO组患者治疗24周后的截肢率明显下降.Mahmoud F.Sakr Hossam M.Hamed 陈永翀 李桂茹 张勇 2012中国烧伤创疡杂志2012,24,2:16
2Impact of topical nonsteroidal anti-inflammatory drugs in prevention of macular edema following cataract surgery in diabetic patients显示文摘AIM: To evaluate the efficacy of prophylactic administration of topical non-steroidal anti-inflammatory drugs(NSAIDs) on macular edema following cataract surgery in diabetic patients, and to compare between types of NSAIDs(ketorolac tromethamine 0.4% and nepafenac 0.1%). METHODS: Group 1(control) received artificial tears substitute as a placebo group, group 2(nepafenac) received topical nepafenac 0.1%, and group 3(ketorolac) received topical ketorolac tromethamine 0.4%. Patients were examined postoperatively after completing one week, one month, two months and three months' intervals for evaluating cystoid macular edema(CME) development. The main study outcomes were achieving the best corrected visual acuity(BCVA) and change in the central macular thickness(CMT) measured with optical coherence topography(OCT).RESULTS: Eighty eyes of 76 patients were included in this study. BCVA showed a statistically significant difference at the third month postoperative follow up between the control group and the NSAIDs groups(P=0.04). There was an increase in the CMT in all cases starting from postoperative first week until third month. CMT showed a statistically significant difference between control group and NSAIDs groups from postoperative first month until third month(P=0.008, 0.027, 0.004). There was no statistically significant difference between nepafenac and ketorolac groups in BCVA and OCT CMT. 3Diffusion-weighted magnetic resonance imaging and micro-RNA in the diagnosis of hepatic fibrosis in chronic hepatitis C virus显示文摘BACKGROUND Diffusion-weighted magnetic resonance imaging has shown promise in the detection and quantification of hepatic fibrosis. In addition, the liver has numerous endogenous micro-RNAs(miRs) that play important roles in the regulation of biological processes such as cell proliferation and hepatic fibrosis.AIM To assess diffusion-weighted magnetic resonance imaging and miRs in diagnosing and staging hepatic fibrosis in patients with chronic hepatitis C.METHODS This prospective study included 208 patients and 82 age-and sex-matched controls who underwent diffusion-weighted magnetic resonance imaging of the abdomen, miR profiling, and liver biopsy. Pathological scoring was classified according to the METAVIR scoring system. The apparent diffusion coefficient (ADC) and miR were calculated and correlated with pathological scoring.RESULTS The ADC value decreased significantly with the progression of fibrosis, from controls(F0) to patients with early fibrosis(F1 and F2) to those with late fibrosis(F3 and F4)(median 1.92, 1.53, and 1.25 × 10^(-3) mm^2/s, respectively)(P = 0.001).The cut-off ADC value used to differentiate patients from controls was 1.83 × 10^(-3) mm^2/s with an area under the curve(AUC) of 0.992. Combining ADC and miR-200 b revealed the highest AUC(0.995) for differentiating patients from controls with an accuracy of 96.9%. The cut-off ADC used to differentiate early fibrosis from late fibrosis was 1.54 × 10^(-3) mm^2/s with an AUC of 0.866. The combination of ADC and miR-200 b revealed the best AUC(0.925) for differentiating early fibrosis from late fibrosis with an accuracy of 80.2%. 4砂土地基中摩擦型能量桩群桩热力学特性研究(英文)显示文摘目的:能量桩在工作状态下的热力学响应十分复杂,同时受到桩顶荷载、桩侧摩擦以及温度等多重因素的影响。当群桩中出现部分能量桩不工作时,将造成上部结构的额外应力与变形。因此,本文重点探讨摩擦型能量桩群桩中部分能量桩在加热制冷作用下的热力学响应,并与单桩的热力学效应进行对比分析。创新点:1.通过建立摩擦型能量桩群桩模型试验,探讨桩侧摩擦对能量桩群桩的影响规律;2.利用能量桩群桩与单桩对比,揭示能量桩群桩与单桩热力响应特性的区别;3.揭示部分能量桩加热制冷作用对能量桩群桩的影响机理。方法:1.建立摩擦型能量桩群桩及单桩的模型试验;2.将能量桩群桩与单桩进行对比,研究能量桩群桩与单桩热力响应特性的区别;3.进行能量桩群桩部分加热制冷试验。结论:1.对于长期工作的能量群桩,可以将其视为一个长宽高与整个群桩相同的热交换体,其表面温度与群桩的平均表面温度一致。2.能量桩单桩在加热过程中,由于桩底受到的限制较大,所以桩顶位移大于桩底位移。3.能量桩单桩在制冷过程中,由于土体及桩体收缩,会出现明显的下沉。4.能量桩群桩桩帽在加热过程中,桩帽的位移与群桩的上半部分长度相关;在本文的试验中,由于群桩上半部分受土的限制较小,因此其位移与桩自由膨胀的位移一样。5.能量桩群桩在制冷期间,群桩的下沉量级要比单桩的大。6.在制冷过程中,能量桩群桩在群桩效应作用下,内部桩的桩底热位移较大。7.能量桩群桩在部分加热的情况下,会出现不均匀沉降,且在加热期间,沉降主要受到不工作桩的牵制影响;而在制冷期间,沉降主要受工作桩的下沉影响。8.摩擦型能量桩的热引起的桩身轴力是与桩侧的土压力大小相关的;由于群桩在群桩效应作用下,桩侧土压力要小于单桩,因此群桩的热引起的桩身轴力要大于单桩。Huai-feng PENG Gang-qiang KONG Han-long LIU Hossam ABUEL-NAGA Yao-hu HAO 2018Journal of Zhejiang University-Science A(Applied Physics & Engineering)2018,19,8:9
5Biochemical changes in phenols,flavonoids,tannins,vitamin E,β-carotene and antioxidant activity during soaking of three white sorghum varieties显示文摘Objective:To investigate the changes in total phenols,flavonoids,tannins,vitamin E, β-carotene and antioxidant activity during soaking of three white sorghum varieties.Methods: The changes in total phenols,total ilavonoids,tannins,phenolic acids compounds,flavonoid components,vitamin E,P-carotene and antioxidant activity during soaking of sorghum grains were determined.Results:Total phenols,total flavonoids,tannins,vitamin E,P-carotene and antioxidant activity in raw sorghum were ranged from 109.21 to 116.70,45.91 to 54.69,1.39 to 21.79 mg/100 g,1.74 to 5.25,0.54 to 1.19 mg/kg and 21.72%to 27.69%and 25.29%to 31.97%,respectively. The above measured compounds were significantly decreased after soaking.p-Hydroxybenzoic acid,vanillic acid,syringic acid and cinnamic acid represent the major phenolic acids in Dorado variety.While ferulic acid,p-coumaric acid,gallic acid and caffeic acid represent the major phenolic acids in Shandaweel-6.On the other hand,protocatechuic acid represents the major phenolic acids in Giza-15.Regarding flavonoids components,Dorado was the highest variety in kampferol and naringenin while Shandaweel-6 was the highest variety in luteolin, apigenin,hypersoid,quercelin and christen.Finally,Giza-15 was the highest variety in catechin. Phenolic acids,flavonoid compounds and antioxidant activities were decreased after soaking. 7Acaricidal activity of different extracts from Syzygium cumini L.Skeels(Pomposia) against Tetranychus urticae Koch显示文摘Objective:To investigate the acaricidal activity of different extracts from Syzygium cumini(S.cumini)(Pomposia)againsst Tetranyckus urticae Koch(71 urticae)and the biochemical changes in antioxidants enzymes.Methods:Six extracts of S.cumini(Pomposia)at concentrations of 75,150 and 300μg/mL were used to control T.urticae(Koch).Results:The ethanol extract showed the most efficient acaricidal activity agent against T.urticae(98.5%)followed by hexane extract(94.0%),ether and ethyl acetate extract(90.0%).The LC_(50)values of the promising extract-were 85.0,101.0,102.0 and 98.0μ/mL,respectively.The activities of enzymes including ascorbate peroxidase(APX),peroxidase(POD),superoxide dismutase(SOD)and catalase(CAT)in susceptible mites were increased.The activities of all antioxidant enzymes reach the maximum value in mites at LC_(50)with ethanol and ethyl acetate extracts,respectively.Conclusions:The extract of S.cumini has acaricidal acivity against T.urticae,and the ethanol extract is the most efficient.Abd El-Moneim MR Afify Hossam S El-Beltagi Sayed A Fayed Emad A Shalaby 2011Asian Pacific Journal of Tropical Biomedicine2011,1,5:5
8多中心随机对照试验:再生复原技术治疗各种开放性创面的疗效评价显示文摘目的评估再生疗法(MEBT/MEBO)治疗各种急慢性开放性创面的安全性和有效性。方法将2007年1月至2012年1月期间因各种开放性创伤就诊于3家不同医院的276例患者(376处创面)随机分组,接受MEBT/MEBO治疗(1组:患者141例,创面190处)或生理盐水湿敷治疗(2组:患者135例,创面186处)。前瞻性收集人口统计学、诱发疾病、合并症和创面特征等数据;每隔2周计算和对比刨面面积和愈合指数,持续12周;记录治疗后24周的二次截肢情况。结果开放性创伤包括慢性褥疮(202处)、糖尿病足溃疡(119处)和各种其它创面(55处)。受试患者中171例(63%)为男性,105例(37%)为女性;年龄介于5~102岁,平均年龄63岁±14.5岁。两组患者在人口统计学、临床特点、生化检查和创面特征方面接近。MEBT/MEBO治疗组在治疗2周时,其愈合指数显著升高、创面面积显著缩小;在12周时,MEBT/MEBO组中66.3%(126/190)的创面实现了完全愈合(HI=1),而生理盐水湿敷组仅22.6%(42/186)的创面完全愈合(,=71.4,P=0.000)。治疗12周时,MEBT/MEBO组无l例患者的愈合指数〈0.5;而生理盐水湿敷组38.7%(72/186)的患者愈合指数〈0.5X=165,P=0.000)。两组患者均无不良反应或过敏反应发生。治疗后24周,与生理盐水湿敷组相比,MEBT/MEBO治疗组的二次截肢率明显降低(3.3%:19.0%)(X。=7.52,P=0.006)。结论MEBT/MEBO不仅操作简便、使用安全,还能显著促进开放性创面的愈合,治疗2周就可显著提高创面愈合指数,12周时几乎2/3的创面可完全愈合,24周时糖尿病足溃疡患者中发生二次截肢的患者明显减少。Mahmoud F.Sakr 王乡宁 付子俊 Hossam Hamed Hesham El-Torky 2013中国烧伤创疡杂志2013,25,3:4
9Response of primiparous and multiparous buffaloes to yeast culture supplementation during early and mid-lactation显示文摘Strains of live Saccharomyces cerevisiae yeast have exhibited probiotic effects in ruminants. This study investigated the effects of the dietary yeast supplement, S. cerevisiae(Yea-Sacc^(1026)), on primiparous(PP)and multiparous(MP) Egyptian buffaloes in early to mid-lactation. Lactating buffaloes were fed either a basal total mixed ration(TMR, control; 4 PP and 8 MP) or the basal TMR plus 10 g Yea-Sacc^(1026) per buffalo cow per day(yeast; 4 PP and 8 MP). The feeds were given from 15 days prepartum to 180 days postpartum. Feed intake, body weight, and milk yields(MY) were recorded, and milk and blood samples were collected for analyses. Feces were collected from days 45 to 47 during early lactation and from days 90 to92 during mid-lactation to determine apparent digestibility of dry matter(DM), organic matter(OM),crude protein(CP) and crude fiber(CF). Energy corrected milk yield(ECM), feed conversion, and energy and nitrogen conversion efficiency were calculated. Yeast treated MP buffaloes consumed more DM(P < 0.041) and CP than the untreated control group. Apparent digestibility of DM and OM were significantly greater at mid-lactation for treated versus control group(P = 0.001). Crude fiber digestibility was greater in MP than in PP buffaloes(P = 0.049), and yeast supplemented MP cows had a greater CF digestibility than control MP buffaloes at mid-lactation(P = 0.010). Total blood lipids decreased after yeast supplementation(P= 0.029). Milk yields, ECM, fat and protein yields increased for yeast treated MP buffaloes(P < 0.039). The study concluded that the response to yeast supplementation in buffalo cows is parity dependent. Multiparous buffaloes respond to yeast supplementation with an increased DM intake and CF digestibility without significant weight gains, allowing a greater ECM yield with less fat mobilization. Supplementing buffaloes with yeast culture may increase milk production in early lactation and results in a more persistent milk production during mid-lactation. 10Biochemical changes in grape rootstocks resulted from humic acid treatments in relation to nematode infection显示文摘Objective:To investigate the effect of humic acid on nematode infected,resistant and susceptible grapes in relation to lipid peroxidation and antioxidant mechanisms on selected biochemical parameters known as proactive substances.Methods:The grape rootstocks,superior,superior/ freedom and freedom were reacted differently to Meloidogyne incognita and Rotylenchulus reniformis according to rootstock progenitor.Two weeks after inoculation,two commercial products of humic acid were applied at the rate of(2,4 mL or grams/plant) as soil drench.After 4 months,nematode soil populations were extracted and counted.A subsample of roots from each plant was stained and gall numbers,embedded stages per root were calculated,final population, nematode build up(Pf/Pi),average of eggs/eggmass were estimated.Subsamples of fresh root of each treatment were chemically analyzed.Results:Freedom reduced significantly the nematode criteria and build up.Humic acid granules appeared to be more suppressive to nematode build up on superior and the higher dose on superior/freedom than liquid treatments.On freedom,all treatments reduced significantly the nematode build up regardless to the material nature.The higher dose was more effective than the lower one.As a result of humic acid applications,the malondialdehyde(MDA) and H_2O_2 contents were significandy reduced after humic acid treatments while the antioxidant compounds glutathione(GSH),ascorbic acid(ASA) and total phenol contents were significandy increased when compared with check.Antioxidant defense enzymes ascorbate peroxidase(AFX),superoxide dismutase(SOD),catalase(CAT) and polyphenol oxidase(PPO) showed significant increase in their specific activities in treated plants compared with nematode treated check.Conclnsions:Humic acid treatments improve we yield of grape by increasing the contents of antioxidant compounds and the specific activities of antioxidant enzymes.Hosny H Kesba Hossam S El-Beltagi 2012Asian Pacific Journal of Tropical Biomedicine2012,2,4:4
11Celecoxib-induced cholestatic liver failure requiring orthotopic liver transplantation显示文摘Selective cyclooxygenase-2 (COX-2) inhibitors are widely used due to their efficacy and good safety profile. However, recent case reports have described varying degrees of liver injuries associated with the use of COX-2 inhibitors. We report the case of a patient who developed acute cholestatic hepatitis progressing to hepatic failure requiring liver transplantation, following a 3-d course of celecoxib for treatment of generalized muscle aches and pains. 12正念冥想对血液透析患者感知压力和生活质量的影响显示文摘目的本研究旨在探讨正念冥想对接受血液透析的终末期肾病患者特质正念、感知压力、情绪调节和生活质量的影响。方法2021年1—5月,在科威特亚赫拉医院透析中心共纳入74例终末期肾病患者,随机分为试验组(n=37)和对照组(n=37)。实验组在血液透析期间进行正念冥想,每次30 min,每周3次,持续5周;对照组进行闭目放松练习,时间与频次与试验组相同。使用正念觉知量表(Mindful Attention Awareness Scale,MAAS)、感知压力量表(Perceived Stress Scale,PSS)、情绪调节问卷(Emotion Regulation Questionnaire,ERQ)和肾病生活质量问卷(Kidney Disease Quality of Life,KDQOL-36),在基线(T0)、干预中期(T1)和干预结束时(T2)对两组进行测量。该研究已在临床试验中心注册(NCT05176730)。结果重复测量的方差分析结果显示,与对照组相比,试验组感知压力降低,特质正念、情绪调节和与肾疾病相关生活质量得到改善,且随着时间的推移,试验组感知压力更低,特质正念、情绪调节及生活质量改善更明显。结论本研究结果为卫生政策制定者和医院管理者提供了一个可能的工具,有望作为一种缓解压力和提高生活质量的方法应用于血液透析患者。此研究还需在多种环境中重复验证并进行后续评估。Hossam Alhawatmeh Sami Alshammari Jehad A.Rababah 2022International Journal of Nursing Sciences2022,9,2:3
13Transfer matrix method for determination of the natural vibration characteristics of elastically coupled launch vehicle boosters显示文摘The analysis of natural vibration characteristics has become one of important steps of the manufacture and dynamicdesignintheaerospaceindustry.Thispaperpresents a new scenario called virtual cutting in the context of the transfer matrix method of linear multibody systems closedlooptopologyforcomputingthefreevibrationcharacteristics of elastically coupled fl xible launch vehicle boosters. In this approach, the coupled system is idealized as a triple-beam system-like structure coupled by linear translational springs,where a non-uniform free-free Euler–Bernoulli beam is used.A large thrust-to-weight ratio leads to large axial accelerations that result in an axial inertia load distribution from nose to tail. Consequently, it causes the development of significan compressive forces along the length of the launch vehicle.Therefore, it is important to take into account this effect in the transverse vibration model. This scenario does not need the global dynamics equations of a system, and it has high computational efficien y and low memory requirements. 15Efficacy and safety of botulinum toxin in treatment of anismus: A systematic review显示文摘AIM: To evaluate the efficacy and safety of botulinum toxin type A(BTX-A) in the management of patients with anismus. METHODS: An organized search of published literature was conducted using electronic databases including: Pub Med/MEDLINE, and Cochrane Central Register of Control ed Trials, also an internet-based search using 'Google Scholar' service was conducted. Both comparative and observational studies were included. We excluded irrelevant articles, editorials, case reports, reviews, and meta-analyses. The studies that followed the patients less than 6 mo were excluded. Variables collected were demographic data of the patients, technique of BTX-A injection and number of sessions, short-term and longterm clinical improvement, post-injection changes in electromyography(EMG), defecography, manometry, and balloon expulsion test, and complications recorded after BTX-A injection.RESULTS: Seven studies comprising 189 patients were included in the review. The median age of the patients was 41.2 years and female-to-male ratio was 1.3:1. The median dose of BTX-A injected per procedure was 100 IU(range, 20-100 IU). Lateral injection was done in five trails and combined lateral and posterior injections in two trials. Three studies used endorectal ultrasonographyguided technique, one study used EMG-guided technique,whereas the remaining three studies used manual palpation with the index finger. The median percentage of patients who reported initial improvement of symptoms was 77.4%(range 37.5%-86.7%), this percentage declined to a median of 46%(range 25%-100%) at 4 mo after injection of BTX-A. Rates of improvement evaluated by balloon expulsion test, EMG, and defecography ranged between(37.5%-80%),(54%-86.7%), and(25%-86.6%), respectively. Fourteen(7.4%) patients developed complications after injection of BTX-A. Complication rates across the studies ranged from 0% to 22.6%. CONCLUSION: Initial satisfactory improvement of symptoms after BTX-A injection remarkably deteriorated after 3 mo of the procedure. However, repeated injection may provide better sustained results with no additional morbidities. 16Parameters Affecting Energy Consumption for Producing High Carbon Ferromanganese in a Closed Submerged Arc Furnace显示文摘The power consumption is considered to be the most important factor affecting the production cost of ferromanganese alloy.Different parameters affecting the energy consumption for industrial production of high carbon ferromanganese HCFeMn were investigated in a closed submerged arc furnace.The analysis of industrial data revealed that the most energy-consumed factors were the direct reduction by solid carbon,Boudouard reaction,metal and slag formation,and decomposition of fluxing materials(limestone and dolomite).To reduce the energy consumption and minimize the energy losses in the production process of HCFeMn,it was recommended to use Mn blend with minimum Mn to Fe ratio of 6and lower SiO2content or higher basicity.The added coke must be adjusted according to the material balance to prevent the over-coke and to minimize the highly endothermic'Boudouard reaction'.In addition,it was recommended to work at basic slags with the ratio of(CaO+MgO)to SiO2equal to 1.0-1.2instead of much higher slag basicity.Furthermore,the mass losses had to be minimized through adjusting the handling and charging process and to take care of all metal produced.Azza Ahmed Hossam Halfa Mohamed K.El-Fawakhry Hoda El-Faramawy Mamdouh Eissa 2014Journal of Iron and Steel Research(International)2014,21,7:3

19Exploring the role of molecular biomarkers as a potential weapon against gastric cancer: a review of the literature显示文摘Gastric cancer(GC) is a global health problem and a major cause of cancer-related death with high recurrence rates ranging from 25% to 40% for GC patients staging Ⅱ-Ⅳ. Unfortunately, while the majority of GC patients usually present with advanced tumor stage; there is still limited evidence-based therapeutic options. Current approach to GC management consists mainly of; endoscopy followed by, gastrectomy and chemotherapy or chemo-radiotherapy. Recent studies in GC have confirmed that it is a heterogeneous disease. Many molecular characterization studies have been performed in GC. Recent discoveries of the molecular pathways underlying the disease have opened the door to more personalized treatment and better predictable outcome. The identification of molecular markers is a useful tool for clinical managementin GC patients, assisting in diagnosis, evaluation of response to treatment and development of novel therapeutic modalities. While chemotherapeutic agents have certain physiological effects on the tumor cells, the prediction of the response is different from one type of tumor to the other. The specificity of molecular biomarkers is a principal feature driving their application in anticancer therapies.
