| 1 | Portal vein thrombosis:Insight into physiopathology,diagnosis,and treatment显示文摘Portal vein thrombosis (PVT) is a relatively common complication in patients with liver cirrhosis, but might also occur in absence of an overt liver disease. Several causes, either local or systemic, might play an important role in PVT pathogenesis. Frequently, more than one risk factor could be identified; however, occasionally no single factor is discernable. Clinical examination, laboratory investigations, and imaging are helpful to provide a quick diagnosis, as prompt treatment might greatly affect a patient's outcome. In this review, we analyze the physiopathological mechanisms of PVT development, together with the hemodynamic and functional alterations related to this condition. Moreover, we describe the principal factors most frequently involved in PVT development and the recent knowledge concerning diagnostic and therapeutic procedures. | 3 | Transarterial chemoembolization using 40 μm drug eluting beads for hepatocellular carcinoma显示文摘AIM To assess the safety and efficacy of transarterial chemoembolization(TACE) of hepatocellular carcinoma(HCC) using a new generation of 40 μm drug eluting beads in patients not eligible for curative treatment.METHODS Drug eluting bead TACE(DEB-TACE) using a new generation of microspheres(embozene tandem,40 μm) preloaded with 100 mg of doxorubicin was performed on 48 early or intermediate HCC patients with compensatedcirrhosis.Response to therapy was assessed with Response Evaluation Criteria in Solid Tumors(RECIST) and modified RECIST(m RECIST) guidelines applied to computed tomography or magnetic resonance imaging.Eleven out of the 48 treated patients treated progressed on to receive liver orthotopic transplantation(OLT).This allowed for histological analysis on the treated explanted nodules.RESULTS DEB-TACE with 40 μm showed a good safety profile without major complications or 30-d mortality.The objective response rate of treated tumors was 72.6% and 26.7% according to m RECIST and RECIST respectively.Histological examination in 11 patients assigned to OLT showed a necrosis degree > 90% in 78.6% of cases.The overall time to progression was 13 mo(11-21).CONCLUSION DEB-TACE with 40 μm particles is an effective treatment for the treatment of HCC in early-intermediate patients(Barcelona Clinic Liver Cancer stage A/B) with a good safety profile and good results in term of objective response rate and necrosis. | Giorgio Greco Tommaso Cascella Antonio Facciorusso Roberto Nani Rodolfo Lanocita Carlo Morosi Marta Vaiani Giuseppina Calareso Francesca G Greco Antonio Ragnanese Marco A Bongini Alfonso V Marchianò Vincenzo Mazzaferro Carlo Spreafico | 2017 | World Journal of Radiology2017,9,5: | 3 |
| 4 | Hepatitis delta virus: From infection to new therapeutic strategies显示文摘The hepatitis delta virus(HDV)is a small RNA virus that encodes a single protein and which requires the hepatitis B virus(HBV)-encoded hepatitis B surface antigen(HBsAg)for its assembly and transmission.HBV/HDV co-infections exist worldwide and show a higher prevalence among selected groups of HBV-infected populations,specifically intravenous drug users,practitioners of high-risk sexual behaviours,and patients with cirrhosis and hepatocellular carcinoma.The chronic form of HDV-related hepatitis is usually severe and rapidly progressive.Patterns of the viral infection itself,including the status of co-infection or super-infection,virus genotypes(both for HBV and HDV),and persistence of the virus’replication,influence the outcome of the accompanying and manifested liver disease.Unfortunately,disease severity is burdened by the lack of an effective cure for either virus type.For decades,the main treatment option has been interferon,administered as mono-therapy or in combination with nucleos(t)ide analogues.While its efficacy has been reported for different doses,durations and courses,only a minority of patients achieve a sustained response,which is the foundation of eventual improvement in related liver fibrosis.The need for an efficient therapeutic alternative remains.Research efforts towards this end have led to new treatment options that target specific steps in the HDV life cycle;the most promising among these are myrcludex B,which inhibits virus entry into hepatocytes,lonafarnib,which inhibits farnesylation of the viral-encoded LHDAg large hepatitis D antigen,and REP-2139,which interferes with HBsAg release and assembly. | Grazia A Niro Arianna Ferro Francesca Cicerchia Isabella Brascugli Marilena Durazzo | 2021 | World Journal of Gastroenterology2021,27,24: | 2 |
| 5 | Hazard and population vulnerability analysis: a step towards landslide risk assessment显示文摘In this paper, an attempt to analyse landslide hazard and vulnerability in the municipality of Pahuatlán, Puebla, Mexico, is presented. In order to estimate landslide hazard, the susceptibility,magnitude(area-velocity ratio) and landslide frequency of the area of interest were produced based on information derived from a geomorphological landslide inventory; the latter was generated by using very high resolution satellite stereo pairs along with information derived from other sources(Google Earth,aerial photographs and historical information).Estimations of landslide susceptibility were determined by combining four statistical techniques:(i) logistic regression,(ii) quadratic discriminant analysis,(iii) linear discriminant analysis, and(iv)neuronal networks. A Digital Elevation Model(DEM)of 10 m spatial resolution was used to extract the slope angle, aspect, curvature, elevation and relief.These factors, in addition to land cover, lithology anddistance to faults, were used as explanatory variables for the susceptibility models. Additionally, a Poisson model was used to estimate landslide temporal frequency, at the same time as landslide magnitude was obtained by using the relationship between landslide area and the velocity of movements. Then,due to the complexity of evaluating it, vulnerability of population was analysed by applying the Spatial Approach to Vulnerability Assessment(SAVE) model which considered levels of exposure, sensitivity and lack of resilience. Results were expressed on maps on which different spatial patterns of levels of landslide hazard and vulnerability were found for the inhabited areas.
