| 1 | Adenomyomatosis of the gallbladder in childhood: A systematic review of the literature and an additional case report显示文摘AIM: To investigate the diagnostic and therapeutic assessment in children with adenomyomatosis of the gallbladder(AMG).METHODS: AMG is a degenerative disease characterized by a proliferation of the mucosal epithelium which deeply invaginates and extends into the thickened muscular layer of the gallbladder, causing intramural diverticula. Although AMG is found in up to 5% of cholecystectomy specimens in adult populations, this condition in childhood is extremely uncommon. Authors provide a detailed systematic review of the pediatric literature according to PRISMA guidelines, focusing on diagnostic and therapeutic assessment. An additional case of AMG is also presented. RESULTS:Five studies were finally enclosed, encompassing 5 children with AMG. Analysis was extended to our additional 11-year-old patient, who presented diffuse AMG and pancreatic acinar metaplasia of the gallbladder mucosa and was successfully managed with laparoscopic cholecystectomy. Mean age at presentation was 7.2 years. Unspecific abdominal pain was the commonest symptom. Abdominal ultrasound was performed on all patients, with a diagnostic accuracy of 100%. Five patients underwent cholecystectomy, and at follow-up were asymptomatic. In the remaining patient, completely asymptomatic at diagnosis, a conservative approach with monthly monitoring via ultrasonography was undertaken. CONCLUSION: Considering the remote but possible degeneration leading to cancer and the feasibility of laparoscopic cholecystectomy even in small children, evidence suggests that elective laparoscopic cholecystectomy represent the treatment of choice. | 2 | Inflammatory fibroid polyps in children:A new case report and a systematic review of the pediatric literature显示文摘AIM: To study that inflammatory fibroid polyps(IFPs) in children are extremely uncommon tumors that may occur throughout the gastrointestinal tract. METHODS: A systematic review of the pediatric literature and a report of a new case of IFP is also pres-ented. The Pub Med database was searched for original studies on pediatric IFPs since 1960, according to 'Preferred reporting items for systematic reviews and meta-analyses' guidelines for systematic reviews. RESULTS: Five studies were finally enclosed, encompassing 6 children with IFPs(mean age 64 mo). Tumors were located in the stomach(2 patients), in the small bowel(2 patients), in the rectum(1 patient) and in the colon(1 patient). Open surgery was performed in all patients and complete excision of the mass was achieved in all cases. All patients are alive and free of symptom. Authors described a further case of a 3-year-old boy with a large duodenal IFP, in whom the tumor was removed by 'en block resection'. The presence of IFP throughout the gastrointestinal tract and its variable clinical appearances make it difficult to diagnose. An accurate pre-operative assessment is fundamental in order to differentiate IFP from other more aggressive gastrointestinal tumor, enabling unnecessary demolitive surgery. | 3 | Endoscopic treatment of periampullary duodenal duplication cysts in children: Four case reports and review of the literature显示文摘BACKGROUND Duodenal duplications are rare congenital anomalies of the gastrointestinal tract.As the periampullary variant is much rarer,literature is scant and only few authors have reported their experience in diagnosis and treatment,particularly with operative endoscopy.CASE SUMARY To report our experience with the endoscopic treatment in a series of children with periampullary duodenal duplication cysts,focusing on the importance of obtaining an accurate preoperative anatomic assessment of the malformations.The pediatric periampullary duodenal duplication cyst literature is reviewed.We conducted a systematic review according to the PRISMA guidelines.The PubMed database was searched for original studies on“duodenal duplication”,“periampullary duplication”or“endoscopic management”published since 1990,involving patients younger than 18 years of age.Eligible study designs were case report,case series and reviews.We analyzed the data and reported the results in table and text.Fifteen eligible articles met the inclusion criteria with 16 patients,and analysis was extended to our additional 4 cases.Median age at diagnosis was 13.5 years.Endoscopic treatment was performed in 10(50%)patients,with only 2 registered complications.CONCLUSION Periampullary duodenal duplication cysts in pediatric patients are very rare.Our experience suggests that an accurate preoperative assessment is critical.In the presence of sludge or stones inside the duplication,endoscopic retrograde cholangio-pancreatography is mandatory to demonstrate a communication with the biliary tree.Endoscopic treatment resulted in a safe,minimally invasive and effective treatment.In periampullary duodenal duplication cyst endoscopically treated children,long-term follow-up is still necessary considering the potential malignant transformation at the duplication site. | Anna Lavinia Bulotta Maria Vittoria Stern Dario Moneghini Filippo Parolini Maria Pia Bondioni Guido Missale Giovanni Boroni Daniele Alberti | 2021 | World Journal of Gastrointestinal Endoscopy2021，13，10： | 1 |

| 7 | Role of preoperative tracheobronchoscopy in newborns with esophageal atresia:A review显示文摘Preoperative tracheobronchoscopy(TBS) in the diag-nostic assessment of newborns affected by esophageal atresia(EA) was described in 1981. Nevertheless, the value of the procedure is actually much debated; only a few studies have clearly explored the advantages of TBS and this procedure is not yet routinely included inthe diagnostic and therapeutic assessment in many in-ternational pediatric surgery settings. Routine preoper-ative TBS is a safe procedure that enables the accurate examination of the tracheobronchial tree, the visualiza-tion of tracheoesophageal fistula and the diagnosis of tracheomalacia or associated respiratory anomalies. When a distal fistula is found, its occlusion with a Fog-arty balloon catheter improves mechanical ventilation and facilitates surgical repair. This review provides a detailed overview on the use of TBS in newborns with EA, focusing on technical aspects, anesthesiological management, indications and limits. | 8 | Nutritional and physical improvements in older adults through the DOREMI remote coaching approach:a real-world study显示文摘Background Malnutrition(excess or defect)and sedentariness act as an accelerator in the older people frailty process.A systemic solution has been developed to engage older people in a healthier lifestyle using serious games and food monitoring.The study aimed to evaluate protocol influence on variables related to unhealthy behaviors improving dietary habits through a remote nutritional coaching approach and stimulating the population to increase physical activity through Exergames.Methods Thirty-two subjects(25 Treatments and 7 Controls,aging 65-80 years),of which 15(11 Treatments and 4 Controls)living in the UK(ACCORD and ExtraCare Villages placed in Shenley Wood(Milton Keynes),St.Crispin(Northampton),and Showell Court(Wolverhampton))and 17(14 Treatments and 3 Controls)in Italy(Genoa,Liguria),were recruited and characterized in terms of nutritional status,physical,somatometric,hemodynamic and biochemical measurements,and body composition.Participants were stimulated to adopt the Mediterranean dietary pattern,by a food diary diet-app,and perform regular physical activity,by the Exergame app,for three months.At the end of the trial,users underwent the same test battery.Data were tested for normality of distribution by the Shapiro-Wilk test.Comparisons between groups were performed at baseline by unpaired Student’s t-test for continuous variables,chi-square test,or Fisher’s exact test for categorical variables.Analysis of Variance(ANOVA)for repeated measures was used to analyze the significance of changes over time between groups.Results At the end of the trial,significant reductions of systolic(15 mmHg,P=0.001),diastolic(5 mmHg,P=0.025),mean(10 mmHg,P=0.001)blood pressure,and rate-pressure product(RPP)(1,105 mmHg∗bpm,P=0.017)values were observed in DOREMI users.A trend of improvement of physical performance by the short physical performance battery(SPPB)was observed for balance and walk subtests.A significant decrease(0.91 kg,P=0.043)in Body Mass Index(BMI)was observed in overweight subjects(BMI>25 kg/m 2)after DOREMI intervention in the entire population.The Mini Nutritional Assessment(MNA)score(1,P=0.004)significantly increased after intervention,while waist measure(3 cm,P<0.001)significantly decreased in the DOREMI users.A reduction in glycated hemoglobin(Hb)was registered(0.20%,P=0.018)in the DOREMI UK users.Conclusions Improvement of healthy behavior by technological tools,providing feedback between user and remote coach and increasing user’s motivation,appears potentially effective.This information and communica-tion technologies(ICT)approach offers an innovative solution to stimulate healthy eating and lifestyle behaviors,supporting clinicians in patient management. | Federico Vozzi Filippo Palumbo Erina Ferro Karl Kreiner Franca Giugni Rachel Dutton Shirley Hall Daniele Musian Marina Parolini Patrizia Riso Oberdan Parodi | 2022 | Intelligent Medicine2022，2，4： | 0 |
